There is no prognostic model that is reliable and practical for patients who have received curative liver resection (CLR) for hepatocellular carcinoma (HCC). This study aimed to establish and validate a Surgery-Specific Cancer of the Liver Italian Program (SSCLIP) scoring system for those patients.668 eligible patients who underwent CLR for HCC from five separate tertiary hospitals were selected. The SSCLIP was constructed from a training cohort by adding independent predictors that were identified by Cox proportional hazards regression analyses to the original Cancer of the Liver Italian Program (CLIP). The prognostic performance of the SSCLIP at 12 and 36-months was compared with data from existing models. The patient survival distributions at different risk levels of the SSCLIP were also assessed.Four independent predictors were added to construct the SSCLIP, including age (HR = 1.075, 95%CI: 1.019-1.135, P = 0.009), albumin (HR = 0.804, 95%CI: 0.681-0.950, P = 0.011), prothrombin time activity (HR = 0.856, 95%CI: 0.751-0.975, P = 0.020) and microvascular invasion (HR = 19.852, 95%CI: 2.203-178.917, P = 0.008). In both training and validation cohorts, 12-month and 36-month prognostic performance of the SSCLIP were significantly better than those of the original CLIP, model of end-stage liver disease-based CLIP, Okuda and Child-Turcotte-Pugh score (all P < 0.05). The stratification of risk levels of the SSCLIP showed an enhanced ability to differentiate patients with different outcomes.A novel SSCLIP to predict survival of HCC patients who received CLR based on objective parameters may provide a refined, useful prognosis algorithm.

Introduction: A potential manner to lower the morbidity with the hypertensive disoreders of pregancy is to explore the time of day of delivery. Objective: To compare composite neonatal adverse outcomes among term women with hypertensive disorders. Methods: This population-based cohort study used the U.S. vital statistics dataset from 2013 to 2017. Time of delivery was categorized into three shifts. The primary outcome was composite neonatal adverse outcome.. Results: Compared to neonates delivered at the first shift, the risk of composite neonatal adverse outcome was higher at the third shift (aRR = 1.19, 95% CI = 1.13–1.25). Conclusion: the risk of composite neonatal adverse outcome is higher if the delivery occurs at the third (23:00–7:00) shift.

Objective Our primary objective was to determine whether biophysical profiles (BPP) performed on the antepartum unit result in changes in clinical decision making. Study Design A retrospective cohort chart review was performed among women who had a BPP during hospital admission. BPP status was categorized as normal (8/8 points) and abnormal (6/8 or less points). The primary outcome, clinical decision making, was the need for prolonged external fetal monitoring (defined as > 2 hours) or decision to proceed with delivery. Secondary outcomes included mode of delivery, indicated preterm delivery, birth weight, 5-minute Apgar's score

In this paper, a hybrid wide drain polycrystalline silicon (poly-Si) fin-like thin-film transistor (FinTFT) for radio frequency (RF) applications is fabricated. A high ION/IOFF current ratio (>108) and favorable gate control (SS = 198 mV/dec) are observed, and a high cutoff frequency (fτ = 9.55 GHz @ VDS = 1 V and 12.45 GHz @ VDS = 2 V) is achieved. The hybrid wide drain poly-Si FinTFT developed in this paper exhibits excellent gate controllability and good high-frequency characteristics as compared with the conventional poly-Si TFT. The effect of the extended length of wide drain on the high-frequency characteristics of poly-Si FinTFT is investigated and analyzed by using the extracted small-signal parameters. Increasing the extended length not only can improve the high-frequency performance which degrade due to additional fringing capacitance in 3-D multigate structure but also can increase the breakdown voltage. This result suggest the possible use of the poly-Si TFT technology for RF integrated circuits and show a great potential to the future system on panel or system on glass.

Objective: To estimate the prevalence of hypertension and antihypertensive medication recommended among U.S. reproductive-aged women according to the 2017 American College of Cardiology/American Heart Association (ACC/AHA) guideline as compared with the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation and Treatment of High Blood Pressure (JNC7) guideline and to identify factors associated with newly classified hypertensive women. Methods: We analyzed data from the National Health and Nutrition Examination Survey 2005–2014. Hypertension was defined using blood pressure measurements and/or self-reported antihypertensive medication use. Multivariable Poisson regression models with robust error variance were conducted. Results: Among 4,575 (weighted n ​= ​40,194,602) non-pregnant women aged 20–44 years, the prevalence of hypertension was 16.5% using the 2017 guideline, and 7.8% based on the JNC7 guideline. Following the 2017 guideline, 8.6% would be recommended for antihypertensive medication, similar to 8.5% according to JNC7 guideline. Following the 2017 guideline, women with older age (35–44 years), obesity, and diabetes were more likely, while Hispanic women were less likely, to be newly classified as hypertensive. Conclusion: Among reproductive-aged women, compared to JNC7 guidelines, the prevalence of hypertension increased by 112% following the 2017 guideline, but the percentage of women recommended for antihypertensive medication was similar.

IntroductionLittle is known about health-related quality of life (HRQOL) among people with multiple chronic conditions. We examined the association between the number of chronic conditions and self-reported HRQOL outcomes among adults in the United States.MethodsWe used data from the Behavioral Risk Factor Surveillance System (BRFSS) in 2007 (n = 430,912) to compare 4 HRQOL measures for people with any of 8 chronic conditions. We also assessed the frequency of self-reported physical and mental distress and the number of days activity was limited because of chronic conditions. We estimated prevalence and adjusted odds ratios (AORs) and 95% confidence intervals (CIs) by using survey logistic regression analyses.ResultsPeople with 3 or more chronic conditions had the highest risk of reporting fair or poor health compared with respondents with no chronic conditions (AOR, 8.7; 95% CI, 8.0-9.4). People with cardiovascular conditions or diabetes had higher risk of reporting poor HRQOL outcomes than those with other chronic conditions. The odds ratios for frequent physical distress were consistently higher than those for frequent mental distress and frequent activity limitations for all conditions.ConclusionStrategies that help clinicians to manage their patients’ chronic conditions may contribute to improved HRQOL among adults. Our findings may help to inform these strategies.
