13. A Case of the Double Cancer Which Was Difficult to Distinguish Intrahepatic Cholangiocarcinoma From Multiple Hepatocellular Carcinomas

Abstract

A man in his 60s was introduced for a further examination of chronic hepatitis C virus infection, and two nodules were detected in the liver by abdominal ultrasonography (US). Serum alpha-fetoprotein was elevated (56 ng/ml). The nodules were identified in various imaging examinations (abdominal US, contrast-enhanced US, contrast-enhanced CT and Gd-EOB-DTPA MRI) in segment 8 of the liver, both of which showed a typical contrast-enhanced pattern of hepatocellular carcinoma (HCC). Preoperatively diagnosed as HCC, a surgery was performed on these lesions. The pathological examinations of resected specimens showed of the two nodules were HCC and cholangiocellular carcinoma (ICC), respectively. There are only a few reports for double cancer complicated HCC and ICC at the same time. Then, it should be noted that many of them are preoperatively assumed as multiple HCC. Our case indicates that we need to take into consideration the likelihood of ICC even when multiple HCC are suggested from imaging diagnosis, because the treatment strategies for HCC and ICC are quite different.

Abstract

Aim The aim of the present study was to evaluate the relationship between baseline white matter lesions ( WML) and changes in regional cerebral blood flow during longitudinal follow up of patients with Alzheimer's disease ( AD). Methods A total of 38 patients with AD were included in the study (16 men, 22 women; mean age 77.8 years). All patients were evaluated using the Mini- Mental State Examination and brain perfusion single-photon emission computed tomography at baseline with an approximately 2-year follow up. The patients were divided into two subgroups according to the presence of WML on magnetic resonance imaging. Single-photon emission computed tomography data were analyzed using a voxel-by-voxel group analysis with Statistical Parametric Mapping 8 and region of interest analysis using FineSRT. Changes in Mini- Mental State Examination scores and regional cerebral blood flow were analyzed using the Wilcoxon signed-rank test. Results Mean Mini- Mental State Examination scores in AD patients with WML significantly decreased from 19.4 ± 4.8 to 15.5 ± 6.5 ( P = 0.003). Statistical Parametric Mapping 8 and FineSRT analysis showed more severe and widespread regional cerebral blood flow reduction, mainly in the frontal and mesial temporal regions in AD patients with WML compared with those without WML. 36. Objectively measured prolonged sleep is associated with plasma cytokines in older adults with mild cognitive impairment.

Abstract

Summary This study aimed to determine whether objective sleep time is associated with the concentrations of various plasma cytokines in older adults with mild cognitive impairment (MCI). In total, 118 adults with MCI (66 women; mean age: 75.7 years) participated in this prospective cohort study. All participants were required to wear a wristband sensor for 7.8 days, on average, every 3 months for 1 year and undergo measurement of 27 plasma cytokines using multiplex immunoassays. After adjusting for potential confounders, the associations of total sleep time with cytokine concentrations were assessed by multiple linear regression analysis. The total sleep time was significantly correlated with plasma interleukin (IL)‐9 and macrophage inflammatory protein (MIP)‐1β levels (r = 0.239, p = 0.009, and r = 0.242, p = 0.008, respectively). Moreover, these associations remained significant after adjusting for covariates, including demographic characteristics, lifestyle‐related diseases, and apolipoprotein E status (β = 0.272, 95% confidence interval: 0.095–0.448, p = 0.003, and β = 0.27, 95% confidence interval: 0.092–0.449, p = 0.003, respectively). Thus, this study is the first to demonstrate the association between objective prolonged sleep and higher plasma IL‐9 and MIP‐1β levels in older adults with MCI. [ABSTRACT FROM AUTHOR]

Abstract

We report herein two cases with pathologically confirmed tumor-like demyelinating lesion and reviewed the clinical features in the literature. In comparison with the conventional primary demyelinating disease, atypical radiological features are a monofocal and large lesion with mild brain edema and the open ring-like or focal enhancement on the magnetic resonance imaging. Clinical manifestations include the focal neurological signs owing to the lesions, monophasic episode without relapse for long follow-up period and the efficacy of corticosteroid therapy. Our cases were not consistent with MS criteria, and similar to the published cases of monofocal acute inflammatory demyelination (MAID). The all reported cases of MAID, as well as our cases revealed the identical clinical, radiological, and pathological features. We speculate that an immunologic mechanism, differencing from that of MS, ADEM and MDS may contributes to the pathogenesis of MAID based on the clinical and pathological manifestation. Abstract

We investigated retrospectively 157 neurological patients who underwent percutaneous endoscopic gastrostomy (PEG) since 2003 May to 2006 January, focusing on their neurological diagnosis, complication, relation to ventilatory support and survival prognosis. Among 157 patients, 42 patients had amyotrophic lateral sclerosis, 39 Parkinson's disease, and 27 multiple system atrophy. The percentage for the neurodegenerative diseases was 68.0%. The most frequent complication during PEG procedure was transient hypoxia (8 patients), three of whom needed oxygen therapy, but no patients received emergent artificial ventilation. After PEG. six patients experienced accidental self-removal of the catheter, but there was no subsequent peritonitis. Seven patients were on tracheostomy positive-pressure ventilation during PEG. and two on non-invasive intermittent positive-pressure ventilation around the PEG period, in all of whom the PEG was carried out without any trouble. Investigation on survival after PEG showed that nine ALS patients died within six months because of respiratory failure. We conclude that PEG for neurological patients was generally performed safely. For ALS patients, however, respiratory function should be carefully monitored, and PEG should be performed in ALS patients before their respiratory function becomes worse.
