Abstract

Various assumptions on a right-censoring mechanism to ensure consistency of the Kaplan--Meier and Aalen--Johansen estimators in a competing risks setting are studied. Specifically, eight different assumptions are seen to fall in two categories: a weaker identifiability assumption, which is the weakest possible assumption in a precise sense, and a stronger representativity assumption which ensures the existence of an independent censoring time. When a given censoring time is considered, similar assumptions can be made on the censoring time. This allows for a characterization of so-called pointwise independence as well as full independence of censoring time and event time and type. Examples illustrate how the various assumptions differ.

Abstract

OBJECTIVE: Juvenile idiopathic arthritis (JIA) is a chronic rheumatic disease causing joint inflammation and pain. Previous studies have indicated affected mental health and increased risk of psychiatric conditions among patients with JIA. We aimed to explore differences in psychiatric morbidity between children with JIA and their peers. We further studied if parental socioeconomic status (SES) influences the association between JIA and the risk of psychiatric morbidity.METHODS: We used a matched cohort design to estimate the association between JIA and psychiatric disease. Children with JIA, born between 1995 and 2014, were identified in Danish national registers. Based on birth registers we randomly selected 100 age- and sex-matched children per index child. Index date was date of fifth JIA diagnosis code or date of matching for reference children. End of follow-up was date of psychiatric diagnosis, death, emigration, or December 31st, 2018, whatever came first. Data were analysed using a Cox proportional hazard model.RESULTS: We identified 2,086 children with JIA with a mean age at diagnosis of 8.1 years. Children with JIA had 17% higher instantaneous risk of a psychiatric diagnosis when compared with the reference group, adjusted HR=1.17 (95% CI 1.02-1.34). Relevant associations were only found for depression and adjustment disorders. Stratifying our analysis for SES, showed no modifying effect of SES.CONCLUSION: Children with JIA had a higher risk of psychiatric diagnoses compared to their peers, especially for diagnosis of depression and adjustment disorders. The association between JIA and psychiatric disease did not depend on parental SES.

38. Stoppage in Autism Spectrum Disorders

Abstract

Stoppage refers to changes in reproductive behavior following the birth of a child with a severe disease. The presence of stoppage can bias estimates of sibling recurrence risk if not properly addressed. If stoppage occurs non-randomly (differential stoppage), it is possibly an additional source of bias in sibling recurrence risk estimation. This study investigated whether stoppage occurs in Danish families with a firstborn child diagnosed with autism spectrum disorders, and if stoppage was differential. We found that stoppage occurs moderately in Danish families affected by autism spectrum disorders, and that stoppage is differential. However, differential stoppage is a minor source of estimation bias in Danish sibling recurrence risk studies of autism spectrum disorders.
