1. Inflow control can be safely used in laparoscopic subsegmentectomy of the liver: a single-center 10-year experience

Abstract

Abstract Background Several techniques have been developed to reduce blood loss in liver resection. The half-Pringle and Pringle maneuvers are commonly used for inflow control. This study compared the outcomes of different inflow control techniques in laparoscopic subsegmentectomy. Methods From October 2010 to December 2020, a total of 362 laparoscopic liver resections were performed by a single surgeon (C.C. Yong) in our institute. We retrospectively enrolled 133 patients who underwent laparoscopic subsegmentectomy during the same period. Perioperative and long-term outcomes were analyzed. Results The 133 patients were divided into 3 groups: no inflow control (n = 49), half-Pringle maneuver (n = 46), and Pringle maneuver (n = 38). A lower proportion of patients with cirrhosis were included in the half-Pringle maneuver group (P = .02). Fewer patients in the half-Pringle maneuver group had undergone previous abdominal (P = .01) or liver (P = .02) surgery. 2. Low Lung Function Is Associated with Low Baseline Calcaneus Ultrasound T-Score but a Slow Decline in T-Score in a Taiwanese Follow-Up Population with No History of Smoking, Bronchitis, Emphysema, or Asthma

Abstract

Osteoporosis is a common disease, and the prevalence is increasing in patients with chronic respiratory diseases, with important implications with regard to fractures, hospitalization, and death. Due to inconsistent data and a lack of large cohort follow-up studies on the association between lung function and osteoporosis, the aim of this study was to investigate this issue. We enrolled and followed for a median of 4 years a total of 9059 participants with no history of smoking, bronchitis, emphysema, or asthma from the Taiwan Biobank. Spirometry data, including forced expiratory volume in 1 second (FEV1) and forced vital capacity (FVC), were used to assess lung function. Changes in the calcaneus ultrasound T-score (ΔT-score) were calculated as follow-up T-score—baseline T-score. A ΔT-score ≤ −3 (median value of ΔT-score) meant a fast decline in T-score. Multivariable analysis showed that lower values of FEV1 (β, 0.127, p < 0.001), FVC (β, 0.203, p < 0.001), and FEV1/FVC (β, 0.002, p = 0.013) were significantly associated with a low baseline T-score. In addition, after follow-up, higher values of FEV1 (odds ratio (OR), 1.146, p = 0.001), FVC (OR, 1.110, p = 0.042), and FEV1/FVC (OR, 1.004, p = 0.002) were significantly associated with ΔT-score ≤ −3. FEV1/FVC < 70% (OR, 0.838, p < 0.001) was significantly associated with ΔT-score ≤ −3. In conclusion, lower FEV1, FVC, and FEV1/FVC were associated with a low baseline T-score, and higher FEV1, FVC, and FEV1/FVC were associated with a rapid decline in T-score in follow-up. This suggests that lung disease may be associated with bone mineral density in the Taiwanese population with no history of smoking, bronchitis, emphysema, or asthma. Further research is needed to establish causality.

Abstract

In this research, a set of TFT-LCD rib mark depth detection system was proposed. The system is mainly divided into three parts: hardware, system control and software. For the hardware part, a line scan camera coupled with a telecentric coaxial lense were adopted for shooting. As for the light source, an internal coaxial white light source combined with a white line light source were employed to strengthen characteristic information. For the system control part, Nivdia Xavier AGX was applied. The model weighted data format was changed to INT8 to accelerate the model image prediction speed and shorten the time to 0.18 seconds. For the software part, in order to detect rib mark features, the Unet network was mainly used to carry out feature segmentation, with splitting accuracy reaching 100%.

Abstract

The placenta plays a crucial role in successful mammalian reproduction. Ruminant animals possess a semi-invasive placenta characterized by a highly vascularized structure formed by maternal endometrial caruncles and fetal placental cotyledons, essential for full-term fetal development. The cow placenta harbors at least two trophoblast cell populations: uninucleate (UNC) and binucleate (BNC) cells. However, the limited capacity to elucidate the transcriptomic dynamics of the placental natural environment has resulted in a poor understanding of both the molecular and cellular interactions between trophoblast cells and niches, and the molecular mechanisms governing trophoblast differentiation and functionalization. To fill this knowledge gap, we employed Stereo-seq to map spatial gene expression patterns at near single-cell resolution in the cow placenta at 90 and 130 days of gestation, attaining high-resolution, spatially resolved gene expression profiles. Based on clustering and cell marker gene expression analyses, key transcription factors, including YBX1 and NPAS2, were shown to regulate the heterogeneity of trophoblast cell subpopulations. Cell communication and trajectory analysis provided a framework for understanding cell-cell interactions and the differentiation of trophoblasts into BNCs in the placental microenvironment. Differential analysis of cell trajectories identified a set of genes involved in regulation of trophoblast differentiation. Additionally, spatial modules and co-variant genes that help shape specific tissue structures were identified. Abstract

Introduction: Currently, the impact of body mass index (BMI) on the outcomes of laparoscopic liver resections (LLR) is poorly defined. This study attempts to evaluate the impact of BMI on the peri-operative outcomes following laparoscopic left lateral sectionectomy (L-LLS). Methods: A retrospective analysis of 2183 patients who underwent pure L-LLS at 59 international centers between 2004 and 2021 was performed. Associations between BMI and selected peri-operative outcomes were analyzed using restricted cubic splines. Results: A BMI of >27kg/m2 was associated with increased in blood loss (Mean difference (MD) 21 mls, 95% CI 5–36), open conversions (Relative risk (RR) 1.13, 95% CI 1.03–1.25), operative time (MD 11 min, 95% CI 6–16), use of Pringles maneuver (RR 1.15, 95% CI 1.06–1.26) and reductions in length of stay (MD -0.2 days, 95% CI -0.3 to −0.1). The magnitude of these differences increased with each unit increase in BMI. However, there was a “U” shaped association between BMI and morbidity with the highest complication rates observed in underweight and obese patients. Conclusion: Increasing BMI resulted in increasing difficulty of L-LLS. Consideration should be given to its incorporation in future difficulty scoring systems in laparoscopic liver resections.

8. Machine Vision-based Cutting Process for LCD Glass Defect Detection System

Abstract

In this research, the automatic optical detection system is developed for detecting the sectional profile and the surface of the TFT-LCD panel after being treated through the cutting process. When tested with the sectional profile photodetector, uniformed lighting effect is achieved by combining the concentrated light source of the inner coaxial lens with the line light source in order to test the surface coarseness related characteristics of the glass sectional profile so as to indicate the defect while intensifying the contrast effect of the sectional profile background. When detecting the sectional profile features, it is conducted by separating the rib mark features through the U-Net network model developed for the deep learning; in result, 100% accuracy can be achieved. When detecting the sectional profile defect, Auto Encoder network model of the deep learning is used to learn the background picture retrieved from the original picture through the linear regression process. As a next step, the model is used again to predict the result by subtracting the background picture from the original picture and then the defect position is highlighted; in result, 96% accuracy is achieved. When detecting the model acceleration, it is conducted by revising the model weighting data format. In terms of U-Net Model, the reading time has been shortened for 4.28 seconds; in individual picture, the prediction time has been shortened for 0.29 second; in Auto Encoder, the model reading time has been shortened for 19.23 seconds; and the individual picture prediction time has been shortened for 0.94 second. As for the surface detection, the circular polariscope is developed. During the detection, the photoelastic reflection theory is employed by projecting the circularly polarized light vertically onto the panel surface in order to produce the interfering halo at the deformed area surrounding the denting defect and the resulting features are referenced to identify the denting defect. By screening the mean luminance value of the sliding window and the discrete value, 90% detection accuracy can be achieved. In the meantime, it can also be used to pinpoint the denting defect that is over 3˚ in average angle change as seen on the normal line surrounding the defect.

Abstract

Background: Despite the rapid advances that minimally invasive liver resection has gained in recent decades, open conversion is still inevitable in some circumstances. In this study, we aimed to determine the risk factors for open conversion after minimally invasive left lateral sectionectomy, and its impact on perioperative outcomes. Methods: This is a post hoc analysis of 2,445 of 2,678 patients who underwent minimally invasive left lateral sectionectomy at 45 international centers between 2004 and 2020. Factors related to open conversion were analyzed via univariate and multivariate analyses. One-to-one propensity score matching was used to analyze outcomes after open conversion versus non-converted cases. Results: The open conversion rate was 69/2,445 (2.8%). On multivariate analyses, male gender (3.6% vs 1.8%, P = .011), presence of clinically significant portal hypertension (6.1% vs 2.6%, P = .009), and larger tumor size (50 mm vs 32 mm, P < .001) were identified as independent factors associated with open conversion. The most common reason for conversion was bleeding in 27/69 (39.1%) of cases. After propensity score matching (65 open conversion vs 65 completed via minimally invasive liver resection), the open conversion group was associated with increased operation time, blood transfusion rate, blood loss, and postoperative stay compared with cases completed via the minimally invasive approach. Conclusion: Male sex, portal hypertension, and larger tumor size were predictive factors of open conversion after minimally invasive left lateral sectionectomy. Open conversion was associated with inferior perioperative outcomes compared with non-converted cases.

Abstract

Awareness coverage control for multiple targets in a large-scale mission domain is considered in this work. Most previous results require prior knowledge of the loss of awareness to design coverage controllers to guarantee the desired state of awareness can be achieved. Other studies neglect the loss of awareness, so that the dynamics of the state of awareness can be solved and the solution can be used as feedback for coverage controllers. Uncertain loss of awareness can lead to unexpected variation in the state of awareness even when the target is covered by agents, and the control strategies to tackle the uncertain dynamics is still unclear. To eliminate the need of the knowledge and to extend to more general applications, systems with unknown loss of the awareness are considered in this paper. An overall controller composed of a nominal controller and a perturbation controller is developed. The nominal controller does not require the knowledge of the loss of awareness for implementation, and it ensures each agent is controlled to a local minimum. The perturbation controller drives the agent outside the local minimum to achieve large-scale coverage control. Stability analysis is conducted to guarantee all targets are covered with a satisfactory state of awareness, and infinite switching between the two controllers (i.e., Zeno behavior) is proven to be avoided. Finally, a numerical simulation is provided to demonstrate the performance of the developed controller.

Abstract

To investigate NF-kappaB and IkappaBalpha activities in HL-60 induced by TNF-alpha in order to understand the molecular mechanism of GbE in asthma treatment.The amount of IkappaBalpha in HL-60 cells stimulated by TNF-alpha and GbE was measured by western blotting. Plasmid pNF-kappaB-LuC was transfected and NF-kappaB activity was analyzed by measuring the expression level of luciferase.It showed in the luciferase assay that the activity of NF-kappaB could significantly be suppressed in HL-60 cells after the pretreatment with CGbE. However, the phosphorylation and subsequent degradation of IKBalpha induced by TNF-alpha can not be inhibited in HL-60 cells even we prolonged the treatment time or increased the concentration of GhE.GhE can suppress the NF-kappaB gene expression actively on independent of NIK/ IKK/ IkappaBalpha pathway in HL-60 cells.
