Abstract

The design and optimization of laser-Compton x-ray systems based on compact distributed charge accelerator structures can enable micron-scale imaging of disease and the concomitant production of beams of Very High Energy Electrons (VHEEs) capable of producing FLASH-relevant dose rates. The physics of laser-Compton x-ray scattering ensures that the scattered x-rays follow exactly the trajectory of the incident electrons, thus providing a route to image-guided, VHEE FLASH radiotherapy. The keys to a compact architecture capable of producing both laser-Compton x-rays and VHEEs are the use of X-band RF accelerator structures which have been demonstrated to operate with over 100 MeV/m acceleration gradients. The operation of these structures in a distributed charge mode in which each radiofrequency (RF) cycle of the drive RF pulse is filled with a low-charge, high-brightness electron bunch is enabled by the illumination of a high-brightness photogun with a train of UV laser pulses synchronized to the frequency of the underlying accelerator system. The UV pulse trains are created by a patented pulse synthesis approach which utilizes the RF clock of the accelerator to phase and amplitude modulate a narrow band continuous wave (CW) seed laser. In this way it is possible to produce up to 10 $\mu$A of average beam current from the accelerator. Such high current from a compact accelerator enables production of sufficient x-rays via laser-Compton scattering for clinical imaging and does so from a machine of "clinical" footprint. At the same time, the production of 1000 or greater individual micro-bunches per RF pulse enables > 10 nC of charge to be produced in a macrobunch of < 100 ns. Abstract

The development of compact quasimonoenergetic x-ray radiation sources based on laser Compton scattering (LCS) offers opportunities for novel approaches to medical imaging. However, careful experimental design is required to fully utilize the angle-correlated x-ray spectra produced by LCS sources. Direct simulations of LCS x-ray spectra are computationally expensive and difficult to employ in experimental optimization. In this manuscript, we present a computational method that fully characterizes angle-correlated LCS x-ray spectra at any end point energy within a range defined by three direct simulations. With this approach, subsequent LCS x-ray spectra can be generated with up to 200 times less computational overhead.

10. TLC-NOSF dressings as a first-line local treatment of chronic wounds: a systematic review of clinical evidence

Abstract

Objective:Several national and international guidelines recommend lipidocolloid technology with a nano-oligosaccharide factor (TLC-NOSF) dressings (UrgoStart dressing range, Laboratoires Urgo, France) for treating patients with chronic wounds. However, these dressings are still often reported as second-line options, potentially leading to loss of opportunity for patients and additional costs for payers. This review aimed to explore the reported wound healing and patient outcomes as well as the related costs when the dressings were used as first-line treatment in patients with different types of chronic wounds.Method:A systematic review of the literature was conducted. Databases (MEDLINE, Embase, Emcare, and Google Scholar) were searched up to 1 February 2024, without any language or time period limitations. Studies were eligible if the evaluated dressings had been used as a first-line treatment for chronic wounds, that is, as an integral part of the standard of care (SoC) at the patient's first presentation and/or in recent wounds. The main evaluation criteria included: wound healing rate; time to reach wound closure; change in patients' quality of life (QoL); and associated costs. The quality of evidence of the included studies was appraised using well-recognised risk-of-bias tools suitable for different study designs. A narrative synthesis describes the findings in three sections depending on the type of comparison. This report followed the principles of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines.Results:A total of 17 studies published between 2017 and 2024 met the eligibility criteria. A comparative analysis between TLC-NOSF dressings and standard dressings, both of which were used as first-line treatment, was reported in nine studies. A comparative analysis between the use of TLC-NOSF dressings as first-line and second-line treatments was reported in eight studies, and five studies reported a systematic use of the TLC-NOSF dressing as first-line treatment without a control group. Overall, the included studies had a relatively low risk of bias for the respective types of evidence. Data of 10,191 patients of both sexes and different age groups with a total of 10,203 wounds (diabetic foot ulcers, leg ulcers, pressure injuries, and other types of chronic wounds) were included in the analysis: 7775 treated with the evaluated dressing and 2428 treated with a comparator dressing. The data suggested that using TLC-NOSF as a first-line treatment for chronic wounds consistently resulted in significantly higher healing rates, shorter healing times, and cost savings compared with standard dressings used under similar conditions. Real-life evidence confirmed the results obtained in clinical trials and economic models, within similar ranges, regardless of the settings involved or of the characteristics of the patients and wounds treated. The wound healing rates ranged around 70–80% by week 20/24 and time-to-heal was reported on average around seven weeks, with slightly longer times reported in wounds with a more severe prognosis. Furthermore, the dressings were shown to improve patient QoL, and were well tolerated and accepted, supporting a wider adoption approach.Conclusion:The results of this review are aligned with the current guidelines recommending the use of TLC-NOSF dressings in the treatment of patients with chronic wounds. They support its wider implementation as a first-line treatment and as an integral part of SoC for these wounds in the daily practice of all centres involved in their management.

Abstract

El pie diabético es un importante problema de salud pública. La tasa de amputaciones de miembros inferiores sigue siendo muy elevada, incluso en los países de alto nivel socioeconómico. Las complicaciones podológicas de la diabetes están dominadas por la neuropatía diabética, la arteriopatía diabética y la infección de ulceraciones del pie. El tratamiento del pie diabético necesita un equipo multidisciplinario, agrupado en la unidad de tratamiento del pie diabético en los servicios de diabetología. Se ocupa de los diversos problemas planteados por las heridas: diagnóstico precoz de la gravedad con hospitalización si es necesario, evaluación etiológica, optimización de la descarga, posible revascularización, tratamiento médico o quirúrgico adaptado de la infección de tejidos blandos o huesos, optimización del equilibrio de la diabetes, cuidados locales, prevención de las recidivas, seguimiento a largo plazo. La hipótesis patogénica actual del pie de Charcot es la respuesta inflamatoria a un microtraumatismo local indoloro del pie que se ha vuelto osteopénico. La destrucción osteoarticular mayor clásica del pie de Charcot se puede evitar mediante la descarga precoz en la fase aguda previa a la fractura. La prevención de las heridas multirrecidivantes del pie diabético de riesgo, difícil pero crucial, está dominada por la educación terapéutica de los pacientes, la pedicura, la ortoplastia, el tratamiento con ortesis plantares y los zapatos ortopédicos. Las unidades de referencia multidisciplinarias del pie diabético pueden mejorar la tasa de cicatrización y reducir las recidivas de ulceraciones y la tasa de amputaciones.

International audience; FFP2 respiratory protective devices in the general population to fight against the spread of the SARS-CoV-2 virus in relation to the emergence of the Omicron variantIn this statement, the French High Council for Public Health or HCSP examines the strategies for indicating the use of FFP2-type respirator protective devices (RPE) in the general population or in populations at risk, as part of the fight against the spread of the SARS-CoV-2 virus in the context of the emergence of the Omicron variant.It specifies, in a supplement sent to the Directorate-General for Health (DGS) on 7 January 2022, (updated on 15 January 2022), that the correct wearing of FFP2-type respirators is essential.The HCSP takes into account that :The effectiveness and protective performance (filtration) of masks, whatever they may be, are closely dependent on the constancy of wear, the fit to the face and the quality of wear covering the nose, mouth and chin imperatively;The mask alone cannot completely stop the risk of transmission; it is one of the protective measures to be respected (vaccination, hand hygiene, ventilation of premises, social distancing, etc.).The HCSP stresses the high level of uncertainty and lack of scientific data on several important aspects of the analysis of the benefit/risk ratio in favour of extending the use of FFP2-type respirators in the general population.The HCSP defines 4 possible scenarios relating to the indications for wearing FFP2-type respirators in the general population.In a supplement sent to the DGS on 7 January 2022, (updated on 15 January 2022), it specifies that the correct wearing of FFP2 type respirators in the general population may be indicated for people at risk of severe forms of Covid-19 and who have failed immunosuppression vaccination, and who are able to tolerate it, on medical advice. Abstract

In patients with diabetes, peripheral arterial disease, particularly below the knee, is associated with medial arterial calcification. This is a frequent and potentially serious complication, affecting all types of diabetes. In recent years, our understanding of the pathophysiology and clinical significance of medial arterial calcification has improved considerably. Here, we offer a short narrative review of the epidemiology, clinical consequences, and pathophysiology of this complication. Abstract

The current pandemic caused by COVID-19 has underlined the importance of a joint effort and approach to ensure patient and health care worker safety in medical care throughout Europe. In addition, the recent flood disasters in Germany and other countries called for immediate joint action, in this case with regard to the prevention of water-borne infections. Environmental disasters will increase with consequences for hospitals and nursing homes. Cooperative efforts are needed for preventing and controlling associated infection outbreaks, new pathogens will appear and a geographic shift of infectious diseases previously not detected in certain areas has already been observed.This approach to infection prevention and control must entail structural as well as regulatory aspects. The principle of equal protection against infections in all European countries must be implemented. Prevention and control of infections, including nosocomial infections, infections caused by antibiotic-resistant bacteria as well as pandemics, need to be based on equal standards in all of Europe.Protection against infections and other public health risks in all European countries is the best guarantor for building trust and identification of citizens in our common Europe. Experts in the fields of hygiene, microbiology, infectiology and epidemiology have to pool the expertise on the prevention and control of infections from different European countries and define key targets for achieving a high standard of hygiene measures throughout Europe. The participants of the Rudolf Schülke Foundation International Symposium call for immediate action and priority to be given to the realization of the proposed 16-point plan. Die aktuelle COVID-19-Pandemie hat deutlich gemacht, wie wichtig gemeinsame Anstrengungen und ein gemeinsames Vorgehen sind, um die Sicherheit von Patienten und Mitarbeitenden in der medizinischen Versorgung in ganz Europa zu gewährleisten. Auch die jüngsten Hochwasserkatastrophen in Deutschland und anderen Ländern haben gezeigt, dass sofortige überregionale Maßnahmen, in diesem Fall zur Prävention von durch Wasser übertragenen Infektionen, dringend erforderlich sind. Umweltkatastrophen werden zunehmen, und sie haben auch für Krankenhäuser und Pflegeheime Folgen.Ausbrüche müssen verhindert und unter Kontrolle gebracht werden. Zugleich ist mit dem Auftreten neuer Krankheitserreger sowie mit einer bereits jetzt zu beobachtenden geographischen Verschiebung von Infektionskrankheiten zu rechnen.Der Ansatz zur Prävention und Bekämpfung von Infektionskrankheiten muss sowohl strukturelle als auch politische Aspekte beinhalten. Es gilt, den Grundsatz des gleichen Schutzes vor Infektionen in allen europäischen Ländern umzusetzen. Prävention und Bekämpfung von Infektionen, einschließlich nosokomialer Infektionen, Infektionen durch antibiotikaresistente Bakterien sowie Pandemien, müssen in ganz Europa auf den gleichen Standards beruhen.Der Schutz vor Infektionen und anderen Risiken für die öffentliche Gesundheit in allen Ländern Europas ist der beste Garant für die Schaffung von Vertrauen und Identifikation der Bürger in unserem gemeinsamen Europa. Expertinnen und Experten auf den Gebieten der Hygiene, Mikrobiologie, Infektiologie und Epidemiologie müssen ihre Expertise zur Prävention und Bekämpfung von Infektionen aus verschiedenen europäischen Ländern bündeln und die wichtigsten Ziele zur Erreichung eines hohen Standards von Hygienemaßnahmen in Europa definieren. Over the past years, it has become clear that the microbial ecosystem in the gut has a profound capacity to interact with the host through the production of a wide range of bioactive metabolites. The microbially produced metabolite imidazole propionate (ImP) is clinically and mechanistically linked with insulin resistance and type 2 diabetes, but it is unclear how ImP is associated with heart failure.

Abstract

L’utilisation d’appareils adhérant à la peau chez les patients vivants avec un diabète ne cesse d’augmenter. Cette étude française, multicentrique transversale, a évalué la prévalence et les conséquences des réactions cutanées chez 851 adultes diabétiques ayant utilisé des capteurs de glucose ou des pompes à insuline au cours des 10 dernières années. Les résultats ont montré que 28 % des utilisateurs de capteurs et 29 % des utilisateurs de pompes ont présenté des réactions cutanées, plus fréquemment chez les femmes et les patients ayant des antécédents d’eczéma. Les symptômes suggèrent une irritation cutanée plutôt qu’une allergie. Les réactions cutanées sont survenues dans les premières 24heures dans 22 à 24 % des cas, et après plus de 6 mois dans 38 et 47 % des cas pour les capteurs et les pompes, respectivement. Enfin, 12 % des patients présentant une réaction au capteur et 7 % des patients présentant une réaction à une pompe ont arrêté d’utiliser le dispositif.

Abstract

Diabetic foot ulcers are one of the complications of diabetes. Malnutrition is one of the risk factors for wounds but, on the other hand, diabetic foot ulceration may promote malnutrition. In this single-centre retrospective study we evaluated the frequency of malnutrition at first admission and the severity of foot ulceration. We demonstrated that malnutrition at admission correlated with duration of hospitalisation and with death rate rather than with the risk of amputation. Our data challenged the concept that protein-energy deficiency may worsen the prognosis of diabetic foot ulcers. Nevertheless, it is still important to screen nutritional status at baseline and during the follow-up in order to start specific nutritional support therapy as soon as possible in order to reduce morbidity/mortality related to malnutrition.

En médecine générale elle s’est développé dès les années 80 de façon structurée et approfondie pendant que la formation à l’hôpital et à la faculté de médecine ne s’en préoccupait pas et ont intégré ces enseignements de façon tardives, diverses, encore limités à quelques facultés de médecine. Leur évaluation reste difficile. Ces enseignements doivent se développer dès la formation initiale des étudiant(e)s puis tout au long de l’exercice.

Abstract

Chemical disinfection is an indispensable means of preventing infection. This holds true for healthcare settings, but also for all other settings where transmission of pathogens poses a potential health risk to humans and/or animals. Research on how to ensure effectiveness of disinfectants and the process of disinfection, as well as on when, how and where to implement disinfection precautions is an ongoing challenge requiring an interdisciplinary team effort. The valuable resources of active substances used for disinfection must be used wisely and their interaction with the target organisms and the environment should be evaluated and monitored closely, if we are to reliable reap the benefits of disinfection in future generations. In view of the global threat of communicable diseases and emerging and re-emerging pathogens and multidrug-resistant pathogens, the relevance of chemical disinfection is continually increasing. Although this consensus paper pinpoints crucial aspects for strategies of chemical disinfection in terms of the properties of disinfectant agents and disinfection practices in a particularly vulnerable group and setting, i.e., patients in healthcare settings, it takes a comprehensive, holistic approach to do justice to the complexity of the topic of disinfection., Die chemische Desinfektion ist ein unverzichtbares Mittel zur Infektionsprävention. Dies gilt für das Gesundheitswesen, aber auch für alle anderen Bereiche, in denen die Übertragung von Krankheitserregern ein potenzielles Gesundheitsrisiko für Mensch und/oder Tier darstellt. Zu erforschen, wie die Wirksamkeit von Desinfektionsmitteln und des Desinfektionsprozesses sichergestellt werden kann und wann, wie und wo Desinfektionsmaßnahmen durchzuführen sind, ist eine ständige Herausforderung, die eine interdisziplinäre Teamarbeit erfordert. Die wertvollen Ressourcen der für die Desinfektion verwendeten Wirkstoffe müssen klug genutzt werden und ihre Wechselwirkung mit den Zielorganismen und der Umwelt sollte genau bewertet und überwacht werden, wenn wir die Vorteile der Desinfektion in zukünftigen Generationen zuverlässig nutzen wollen. Angesichts der globalen Bedrohung durch übertragbare Krankheiten sowie durch neu- und wiederauftretende Krankheitserreger und multiresistente Erreger nimmt die Bedeutung der chemischen Desinfektion ständig zu. Dieses Konsensuspapier befasst sich vor allem mit den entscheidenden Aspekten für Strategien der chemischen Desinfektion im Hinblick auf die Eigenschaften von Desinfektionsmitteln und Desinfektionspraktiken in einer besonders gefährdeten Gruppe und Umgebung, nämlich bei Patienten in Einrichtungen des Gesundheitswesens, jedoch verfolgt es dabei auch einen umfassenden, ganzheitlichen Ansatz, um der Komplexität des Themas Desinfektion gerecht zu werden.

Abstract

The formation of the insulating barrier in YBaCuO-insulator-normal metal tunnel junctions was studied. A CHF3 treatment incorporated in a reactive ion etching system was employed to induce the formation of the barrier during the deposition of several YBaCuO films onto SrTiO3 substrates. The structure and chemical composition of the barrier layer were then analyzed by x-ray photoelectrons spectroscopy and Auger characterizations. Results reveal that the chemical composition and thickness of this layer barrier are different from those obtained by wet chemical treatments.
