Abstract

Recent reconnaissance geochemical investigations have unveiled Cryogenian magmatism linked to the compressional accretionary phase, contributing to the growth of the Afif Terrane in the eastern Arabian Shield. The Cryogenian Suwaj intrusive suite, within the Afif Terrane, displays a compositional range from gabbro-diorite to tonalite-granodiorite. The uniform compositional variation is primarily due to magmatic differentiation within parental magma across multiple pulses. The I-type calc-alkaline Suwaj granitoids represent arc-related juvenile materials resulting from the partial melting of a metabasaltic source. The geochemical characteristics of the Suwaj granitoids suggest typical adakite formation resulting from the melting of the basaltic portion of the young, hot oceanic slab. The age correlation between the Halaban ophiolite and the Suwaj Suite aligns with adakite formation in convergent margins. Tectonically, the Suwaj suite's origin is attributed to the subduction of the Paleo-Ad-Dawadmi oceanic slab during microplate amalgamation, reflecting an integral part of the Arabian-Nubian Shield's evolution, underscoring crustal thickening after arc collision. Abstract

Introduction About 80% of patients receiving chemotherapeutics suffer from side effects related to the gastrointestinal tract. Irinotecan (CPT-11) is a chemotherapeutic agent usually used in treating solid tumors. Quercetin (QRT), a bioflavonoid, is an antioxidant and scavenger reactive oxygen species scavenger. Objective The current study explored the possible protective effects of QRT against mucosal tongue injury caused by CPT-11. Methods The study included four equal groups: group 1/control, group 2/QRT, group 3/CPT-11, and group 4/CPT-11 + QRT. Results CPT-11-induced tongue injury in the form of non-healed ulcers, absent lingual papillae, mononuclear cells infiltration, marked deposition of collagen fibers, and overexpression of CD86 and tumor necrosis factor- α (TNF-α). The increased malondialdehyde levels, decreased superoxide dismutase and total antioxidant capacity revealed that there was an oxidative stress. Also, there was a decreased countenance of Ki-67 and Bcl-2 and an increased countenance of NF-κB. The QRT-treated group showed complete ulcer healing, with histological features almost like the control group, along with minimal collagen fiber deposition, decreased reactivity to CD86 and TNF-α and improvement of oxidative stress status and the molecular study results as well. 15. Hf Isotopes and Detrital Zircon Geochronology of the Silasia Formation, Midyan Terrane, Northwestern Arabian Shield: An Investigation of the Provenance History.

Abstract

U–Pb ages and Hf isotopic data from detrital zircon of the Silasia Formation in the Midyan Terrane record evidence for the provenance and tectonic evolution of the northern Arabian Shield. Given that the youngest acknowledged age of these detritus sediments is 735 ± 13 Ma, it is likely that the Silasia Formation was deposited during the closure of the Mozambique Ocean. The U–Pb ages define a major Mesoproterozoic peak, with two minor peaks of Neoproterozoic and Archean age. Combined with zircon Hf isotopic compositions, the sedimentary detritus of the Silasia Formation was mainly derived from source rocks formed during the Grenville Orogeny during the assembly of Rodinia, with a minor contribution from Archean and Paleoproterozoic crustal material, in addition to a limited arc‐basement supply related to the early Mozambique Ocean. The youngest Concordia age of 735 ± 13 Ma with highly variable εHf(t) values (11 to −35) indicates a complex mixture of sources from juvenile to extremely ancient. The Concordia ages at 1113 ± 11 and 1046 ± 10 Ma have positive hafnium isotope signatures (up to +10.45) that are consistent with juvenile source rocks formed during the Grenville Orogeny. Several detrital zircons with ages of 2622 ± 22 Ma and 2690 ± 7 Ma are similar to those reported in Yemen, whereas 1818 ± 19 Ma, 2071 ± 8 Ma and 2001 ± 19 Ma Paleoproterozoic ages are similar to dated outcrops in the Khida terrane in the eastern Arabian Shield. 16. Dexmedetomidine versus ketamine in improving tolerance to noninvasive ventilation after blunt chest trauma: A randomized, double-blinded, placebo-controlled trial.

Abstract

Background and Aims: Even though patient tolerance is critical to the success of noninvasive ventilation (NIV), research on using sedation to improve tolerance to NIV after traumatic chest injuries is limited. We hypothesized that dexmedetomidine would be superior to ketamine in terms of patient tolerance and lengthening the NIV sessions after blunt chest trauma. Material and Methods: This randomized, double-blinded, placebo-controlled trial included 45 patients of both genders aged 18–60 who needed NIV after blunt chest trauma. The patients were randomly assigned to one of three groups (n = 15) for receiving dexmedetomidine, ketamine, or placebo (0.9% sodium chloride solution) infusion to maintain a Richmond Agitation Sedation Scale (RASS) score between 0 and − 3 during two successive NIV sessions. Patients were evaluated for the duration of the NIV sessions, RASS, Visual Analog Scale (VAS), and the total amount of rescue analgesia consumed. Results: The mean duration of the NIV sessions was significantly longer in patients who received dexmedetomidine (P < 0.001) or ketamine (P < 0.001) compared to placebo. However, the NIV durations did not differ significantly between the dexmedetomidine and ketamine groups (P > 0.05). The dexmedetomidine group had a significantly lower RASS score compared to the ketamine (P < 0.001) and placebo (P < 0.001) groups, whereas the ketamine group had a significantly lower VAS compared to the dexmedetomidine (P = 0.005) and placebo (P = 0.022) groups and required significantly less total morphine (P = 0.001) compared to the other groups. Conclusion: The duration of the NIV sessions for patients with blunt chest trauma did not differ significantly between the dexmedetomidine and ketamine groups. [ABSTRACT FROM AUTHOR]

Abstract

Owing to their excellent structural characteristics, aesthetic appearance, low maintenance cost, and efficient use of structural materials, cable-stayed bridges have gained much popularity in recent decades. Stay cables of a cable stayed bridge are post-tensioned to counteract the effect of the bridge dead load. The solution for an optimum distribution of post-tensioning cable forces is considered one of the most important and difficult tasks in the design of cable-stayed bridges. A novel approach that utilizes the finite element method, B-spline curves, and real coded genetic algorithm to determine the global optimum post-tensioning cable forces is developed. The effect of geometric nonlinearity on the determination of the post-tensioning cable forces is assessed. The study is further extended to develop the first surrogate polynomial functions that can be used to evaluate the post-tensioning cable forces in semi-fan cable stayed bridges. The developed post tensioning functions are then used to investigate the optimal geometric configurations, which lead to the most uniform distribution of the post-tensioning cable forces. Details of an optimization code developed in-house specifically to optimize the design of composite cable-stayed bridges with semi-fan cable arrangement are then reported. The optimization design code integrates a finite element model, the real coded genetic algorithm, the post tensioning polynomial functions, and the design provisions provided by the Canadian Highway Bridge Design Code. An extensive parametric study is then conducted using this optimization code to develop a database for the optimum design of semi-fan cable stayed bridges. The database covers bridge lengths ranging from 250 m to 700 m. It describes the variations of the optimum design parameters, such as the main span length,height of the pylon, number of stay cables, and cross-sectional dimensions with the total length of the bridge.

Abstract

The optimum design solution of cable-stayed bridges is a complicated task. The large number of design parameters, strict design constraints imposed by design codes, highly geometrically nonlinear behaviour and robust effect of post-tensioning cable forces make traditional design methods not capable of achieving this task. In this study, an optimisation technique that integrates a finite element model, a genetic algorithm and polynomial functions for evaluating post-tensioning cable forces is adopted to develop a database for the optimum design of cable-stayed bridges. The database describes the variations of the optimum design parameters, including the main span length, height of the pylon, number of stay cables and cross-sectional dimensions of all elements with the total length of the bridge. The study concerns the optimisation of three-span composite cable-stayed bridges with semi-fan cable arrangements. The study covers bridge lengths ranging from 250 to 700 m. Abstract

We aimed to determine the impact of pre-operative isolation on postoperative pulmonary complications after elective surgery during the global SARS-CoV-2 pandemic. We performed an international prospective cohort study including patients undergoing elective surgery in October 2020. Isolation was defined as the period before surgery during which patients did not leave their house or receive visitors from outside their household. The primary outcome was postoperative pulmonary complications, adjusted in multivariable models for measured confounders. Pre-defined sub-group analyses were performed for the primary outcome. A total of 96,454 patients from 114 countries were included and overall, 26,948 (27.9%) patients isolated before surgery. Postoperative pulmonary complications were recorded in 1947 (2.0%) patients of which 227 (11.7%) were associated with SARS-CoV-2 infection. Patients who isolated pre-operatively were older, had more respiratory comorbidities and were more commonly from areas of high SARS-CoV-2 incidence and high-income countries. Although the overall rates of postoperative pulmonary complications were similar in those that isolated and those that did not (2.1% vs 2.0%, respectively), isolation was associated with higher rates of postoperative pulmonary complications after adjustment (adjusted OR 1.20, 95%CI 1.05–1.36, p=0.005). Sensitivity analyses revealed no further differences when patients were categorised by: pre-operative testing; use of COVID-19-free pathways; or community SARS-CoV-2 prevalence. The rate of postoperative pulmonary complications increased with periods of isolation longer than 3days, with an OR (95%CI) at 4–7days or ≥8days of 1.25 (1.04–1.48), p=0.015 and 1.31 (1.11–1.55), p=0.001, respectively. Isolation before elective surgery might be associated with a small but clinically important increased risk of postoperative pulmonary complications. Longer periods of isolation showed no reduction in the risk of postoperative pulmonary complications. These findings have significant implications for global provision of elective surgical care.
