Abstract

Background T1‐weighted dynamic contrast‐enhanced (DCE) perfusion magnetic resonance imaging (MRI) has been broadly utilized in the evaluation of brain tumors. We aimed at assessing the diagnostic accuracy of DCE‐MRI in discriminating between low‐grade gliomas (LGGs) and high‐grade gliomas (HGGs), between tumor recurrence and treatment‐related changes, and between primary central nervous system lymphomas (PCNSLs) and HGGs. Methods We performed this study based on the Preferred Reporting Items for Systematic Reviews and Meta‐Analysis of Diagnostic Test Accuracy Studies criteria. We systematically surveyed studies evaluating the diagnostic accuracy of DCE‐MRI for the aforementioned entities. Meta‐analysis was conducted with the use of a random effects model. Results Twenty‐seven studies were included after screening of 2945 possible entries. We categorized the eligible studies into three groups: those utilizing DCE‐MRI to differentiate between HGGs and LGGs (14 studies, 546 patients), between recurrence and treatment‐related changes (9 studies, 298 patients) and between PCNSLs and HGGs (5 studies, 224 patients). The pooled sensitivity, specificity, and area under the curve for differentiating HGGs from LGGs were 0.93, 0.90, and 0.96, for differentiating tumor relapse from treatment‐related changes were 0.88, 0.86, and 0.89, and for differentiating PCNSLs from HGGs were 0.78, 0.81, and 0.86, respectively. Conclusions Dynamic contrast‐enhanced‐Magnetic resonance imaging is a promising noninvasive imaging method that has moderate or high accuracy in stratifying gliomas. DCE‐MRI shows high diagnostic accuracy in discriminating between HGGs and their low‐grade counterparts, and moderate diagnostic accuracy in discriminating recurrent lesions and treatment‐related changes as well as PCNSLs and HGGs.

21. Extraktion von Arzneidrogen

Abstract

In den Arzneibüchern werden bestimmte Extraktionsmethoden bei bestimmen Arzneipflanzen vorgeschrieben. Der Grund dafür ist nicht immer klar. Anhand der Diplomarbeit sollte die Effektivität und die Reproduzierbarkeit verschiedener Extraktionsverfahren vergleichend untersucht werden. Es wurden sowohl offizinelle Extraktionsmethoden, wie Mazeration und Perkolation, als auch nicht offizinelle Extraktionsmethoden, wie Ultraschall-, Ultra-Turrax- und Turboextraktion, durchgeführt. Als Extraktionsmittel wurde 70 % Ethanol, mit Ausnahme der Teezubereitung mit Wasser, verwendet. Die Quantifizierung der Wirkstoffe erfolgte mit der Hochleistungsflüssigchromatographie. Die untersuchten Arzneipflanzen und deren Wirkstoffgruppen waren Arnikablüten (Sesquiterpenlactone), Baldrianwurzel (Sesquiterpensäuren), Efeublätter (Saponine) und Rotkleeblüten (Isoflavone). Es konnte festgestellt werden, dass Die Perkolation die besten Extraktionsausbeuten lieferte. Die Ultra-Turrax- Extraktion und die Turboextraktion ebenfalls gute Ergebnisse zeigten. Die Ultraschallextraktion und die Mazeration vergleichsweise schlechtere Ausbeuten ergaben. Die Extraktion mit heißem Wasser (Teezubereitung) die geringsten Ausbeuten lieferte., In pharmacopoeias certain extraction methods on specific medicinal plants are used. The reason is sometimes unclear. On the basis of the diploma thesis the effectiveness and reproducibility of different extraction methods was studied comparatively. There are both officinal extraction methods, such as maceration and percolation, and non-officinal extraction methods, such as ultrasound, ultra-Turrax and Turboextraction performed. As solvent 70% ethanol was used, with the exception of tea-preparation with water. Quantification was performed using high performance liquid chromatography. The studied medicinal plants and their active groups were Arnica flowers (sesquiterpenlactones), Valerian root (sesquiterpenacid), Ivy leaves (Saponins) and Red clover flowers (isoflavones). It was found that The percolation provided the best results The Ultra-Turrax-extraction and the Turboextraction also provided good results Ultrasonic-extraction and maceration were comparatively poor. The extraction of hot water provided the least amount of ingredients.

22. Pulmonary vasculitis in Behçet's disease: reference atlas computed tomography pulmonary angiography (CTPA) findings and risk assessment-management proposal.

Abstract

Background and Aim:  Pulmonary artery aneurysms (PAAs) are the most well-defined type of pulmonary vascular complication in Behçet's disease (BD).The aim of this study is to analyze which CT pulmonary angiography (CTPA) signs are associated with serious morbidity and mortality., Methods: The study included 42 BD patients with pulmonary vascular complications. All patients' medical records were reviewed retrospectively in terms of demographics, disease characteristics, laboratory investigations, pulmonary manifestations, arterial and/or venous thrombosis and CTPA vascular and parenchymal findings., Results: Deep venous thrombosis was observed in 31(73.8%) patients, arterial thrombosis in 13(31%), peripheral arterial aneurysms in 12(286%), haemoptysis in 38 (90.5%), and fatal haemoptysis in 8(19 %) patients. CTPA revealed: in situ thrombosis in 14(33.3%) patients, true stable PAAs in 13(31), true unstable PAAs in 11(26.2%), stable pulmonary artery pseudoaneurysms (PAPs) in 7(16.7%), unstable PAPs in 17(40.5%), the latter were associated with perianeurysmal leaking in 26(61.9%) and bronchial indentation in 19(45.2%).In regression analysis, fatal outcomes were associated with age in years (p=0.035), arterial thrombosis (p=0.025), peripheral arterial aneurysms (p=0.010), intracardiac thrombosis (p=0.026) and positively associated with haemoptysis severity (p<0.001)., Conclusion: Peripheral arterial thrombosis and/or aneurysms, intracardiac thrombosis and haemoptysis severity are predictor of fatal outcomes in BD pulmonary vasculitis. PAPs with perianeurysmal alveolar haemorrhage and/or bronchial indentation are serious CTPA signs that require prompt identification and aggressive treatment. PAPs are a more serious aneurysmal pattern than true PAAs because they are a contained rupture of a PA branch in the context of pulmonary vasculitis.
