This feasibility study aims to evaluate the usefulness of the parametric g-formula within a target trial for application to an extensive healthcare database in order to address various sources of time-related biases and time-dependent confounding.Patients and Methods: Based on the German Pharmacoepidemiological Research Database (GePaRD), we estimated the pancreatic cancer incidence comparing two hypothetical treatment strategies for type 2 diabetes mellitus (T2DM), i.e., (A) sustained metformin monotherapy vs (B) combination therapy with DPP-4 inhibitors after one year metformin monotherapy. We included 77,330 persons with T2DM who started metformin therapy at baseline between 2005 and 2011. Key aspects for avoiding time-related biases and time-dependent confounding were the emulation of a target trial over a 7-year follow-up period and application of the parametric g-formula.Results: Over the 7-year follow-up period, 652 out of the 77,330 study subjects had a diagnosis of pancreatic cancer. Assuming no unobserved confounding, we found evidence that the metformin/DPP-4i combination therapy increased the risk of pancreatic cancer compared to a sustained metformin monotherapy (risk ratio: 1.47; 95% bootstrap CI: 1.07– 1.94). The risk ratio decreased in sensitivity analyses addressing protopathic bias.Conclusion: While protopathic bias could not fully be ruled out, and computational challenges necessitated compromises in the analysis, the g-formula and target trial emulation proved useful: Self-inflicted biases were avoided, observed time-varying confounding was adjusted for, and the estimated risks have a clear causal interpretation.Keywords: target trial emulation, electronic health data, parametric g-formula, time-related bias, time-dependent confounding, type-2 diabetes mellitus

Abstract

BACKGROUND: In May 2020, the ACCESS (The vACCine covid-19 monitoring readinESS) project was launched to prepare real-world monitoring of COVID-19 vaccines. Within this project, this study aimed to generate background incidence rates of 41 adverse events of special interest (AESI) to contextualize potential safety signals detected following administration of COVID-19 vaccines.METHODS: A dynamic cohort study was conducted using a distributed data network of 10 healthcare databases from 7 European countries (Italy, Spain, Denmark, The Netherlands, Germany, France and United Kingdom) over the period 2017 to 2020. A common protocol (EUPAS37273), common data model, and common analytics programs were applied for syntactic, semantic and analytical harmonization. Incidence rates (IR) for each AESI and each database were calculated by age and sex by dividing the number of incident cases by the total person-time at risk. Age-standardized rates were pooled using random effect models according to the provenance of the events.FINDINGS: A total number of 63,456,074 individuals were included in the study, contributing to 211.7 million person-years. A clear age pattern was observed for most AESIs, rates also varied by provenance of disease diagnosis (primary care, specialist care). Thrombosis with thrombocytopenia rates were extremely low ranging from 0.06 to 4.53/100,000 person-years for cerebral venous sinus thrombosis (CVST) with thrombocytopenia (TP) and mixed venous and arterial thrombosis with TP, respectively.INTERPRETATION: Given the nature of the AESIs and the setting (general practitioners or hospital-based databases or both), background rates from databases that show the highest level of completeness (primary care and specialist care) should be preferred, others can be used for sensitivity. Cases were patients first hospitalized for hip–pelvis or other non-vertebral fractures. Up to 100 controls per case were selected using incidence density sampling. AD use was ascertained at index date (ID) based on the supply of last dispensing. Adjusted odds ratios (aORs) and 95% confidence intervals (CIs) were estimated using conditional logistic regression with current users of mirtazapine as reference (active comparator).Results: A total of 39,853 cases of hip–pelvis fracture (80% women, median age 81 years) and 31,577 cases of other fractures (84% women, median age 79 years) were matched to > 3 million controls. For hip–pelvis fracture, aORs in current users were about 1.3 with little variation between individual ADs, ranging from 1.33 for citalopram (95% CI 1.27– 1.39) to 1.28 for amitriptyline (1.21– 1.35). For other fractures, the aORs were highest in current users of citalopram (1.50; 1.42– 1.58) and duloxetine (1.54; 1.39– 1.71) and lowest for amitriptyline (1.18; 1.11– 1.26) and trimipramine (1.16; 1.03– 1.29). For all examined ADs, the aORs were higher for other fractures than for hip–pelvis fracture.Conclusion: The risk of fractures varies between ADs, but for most agents is higher than the risk for mirtazapine. Abstract

Background: The socioeconomic status is often an important confounder in the context of epidemiological studies. However, this information is typically not included in studies based on claims data, possibly due to a lack of knowledge regarding the completeness and validity of the available information.Objectives: Our aim was to assess and evaluate the socioeconomic status in the German Pharmacoepidemiological Research Database (GePaRD).Methodology: First, an algorithm was developed to assign an educational status to individuals insured in 2017 in order to estimate the socioeconomic status at the individual level. For comparison, the socioeconomic status was estimated based on the place of residence. For plausibility, we examined whether the known association between the presence of obesity and the socioeconomic status could be reproduced.Results: Based on different age groups it was possible to dichotomously estimate the socioeconomic status for 86-93% of those under 60 years of age and for 67% of the 60- to 69-year-olds. For individuals 70 years and older, the proportion of missing values was very high. In all subgroups, the prevalence of obesity was higher among persons of lower socioeconomic status than among persons of higher socioeconomic status. The estimation based on the place of residence produced plausible results, but with smaller differences by socioeconomic status.Discussion and conclusion: Overall, the study results suggest that in terms of plausibility and completeness the information available in GePaRD is well suited for estimating the individual socioeconomic status for age groups up to 69 years., Hintergrund: Der sozioökonomische Status stellt bei vielen epidemiologischen Fragestellungen eine wichtige Einfluss- bzw. Störgröße dar. Dennoch wird diese Information in Studien basierend auf Versichertendaten oft nicht berücksichtigt, vermutlich auch da unklar ist, wie vollständig bzw. aussagekräftig die vorliegenden Informationen sind.Ziel der Arbeit: Ziel war, den sozioökonomischen Status für die pharmakoepidemiologische Forschungsdatenbank GePaRD zu erfassen und zu bewerten.Methodik: Zunächst wurde ein Algorithmus entwickelt, der den Versicherten im Jahr 2017 einen Schulbildungsstatus zuordnet und so den sozioökonomischen Status auf individueller Ebene schätzt. Zum Vergleich wurde der sozioökonomische Status basierend auf dem Wohnort geschätzt. Zur Plausibilisierung wurde untersucht, ob sich der bekannte Zusammenhang zwischen dem Vorliegen einer Adipositas und dem sozioökonomischen Status reproduzieren lässt.Ergebnisse: Es konnte je nach Altersgruppe für 86-93% der unter 60-Jährigen und für 67% der 60- bis 69-Jährigen der sozioökonomische Status dichotom geschätzt werden. Für Personen ab 70 Jahren war der Anteil fehlender Werte sehr hoch. In allen Subgruppen war die Prävalenz der Adipositas bei Personen mit niedrigem sozioökonomischen Status höher als bei Personen mit höherem sozioökonomischen Status. Die wohnortbasierte Schätzung zeigte zwar auch plausible Ergebnisse, doch mit schwächeren Unterschieden nach sozioökonomischem Status. Diskussion und Fazit: Insgesamt legen die Studienergebnisse nahe, dass sich die in GePaRD verfügbaren Informationen für eine Abschätzung des individuellen sozioökonomischen Status bei Altersgruppen bis 69 Jahren, was Plausibilität und Vollständigkeit betrifft, gut eignen.

19. Does the socioeconomic status differ between users of potentially NDMA-contaminated generic valsartan and users of brand-name valsartan in Germany?

Abstract

A recently published study from Germany suggested an increased risk of liver cancer among NDMA-contaminated generic valsartan users. However, studies from the US showed that generic versus brand-name drug users differ regarding socioeconomic characteristics. If also true for Germany, this might have biased the results on NDMA-contaminated valsartan given that the socioeconomic status was not considered in the analysis. We, therefore, investigated for the first time whether users of brand-name versus generic preparations in Germany differ by socioeconomic status. Based on a large claims database (GePaRD), we conducted a cross-sectional study including individuals with >=1 valsartan dispensation in 2017. Among 495,832 individuals, we observed no meaningful differences between generic versus brand-name valsartan users regarding educational level or socioeconomic deprivation. Our results suggest brand-name versus generic drug users in Germany to be comparable regarding the socioeconomic status, thus, supporting the validity of findings on harmful effects of NDMA-contaminated valsartan exposure., Eine kürzlich publizierte Studie aus Deutschland zeigte Hinweise auf ein erhöhtes Leberkrebsrisiko bei Nutzern von NDMA-kontaminierten Valsartan-Generika. Studien aus den USA hingegen zeigten, dass sich Generika-Nutzer von Nutzern der Originalpräparate hinsichtlich sozioökonomischer Charakteristika unterscheiden. Sollte dies auch für Deutschland zutreffen, könnte dies zu einer Verzerrung der Ergebnisse zu NDMA-kontaminiertem Valsartan geführt haben, da der sozioökonomische Status in den Analysen nicht berücksichtigt wurde. Daher untersuchten wir erstmals, ob sich Generika- und Originalpräparat-Nutzer in Deutschland hinsichtlich des sozioökonomischen Status unterscheiden. Basierend auf einer großen Datenbank mit Abrechnungsdaten (GePaRD) führten wir eine Querschnittsanalyse durch, die Personen mit mindestens einer Valsartan-Abgabe in 2017 umfasste. Bei 495.832 Personen beobachteten wir keine bedeutsamen Unterschiede zwischen Generika-Nutzern und Nutzern von Originalpräparaten hinsichtlich des Bildungsniveaus oder der sozioökonomischen Deprivation. Unsere Ergebnisse weisen darauf hin, dass Nutzer von Originalpräparaten und Generika-Nutzer in Deutschland hinsichtlich des sozioökonomischen Status vergleichbar sind, was die Gültigkeit der Ergebnisse zu den schädlichen Auswirkungen einer NDMA-kontaminierten Valsartan-Exposition unterstützt.

Abstract

Rationale and background: The global rapid spread of COVID-19 caused by the SARS-CoV-2 triggered the need for developing vaccines to control for this pandemic. This study aimed to generate background incidence rates of adverse events of special interest (AESI) that may be used to monitor benefit-risk profile of COVID-19 vaccines. Data sources: This study included 10 data sources from 7 European countries (Denmark, Germany, France, Italy, Netherlands, Spain, United Kingdom). Data sources contain health insurance data (GePaRD, SNDS), hospitalisation record linkage data (PHARMO, Danish registries (DCE-AU), SIDIAP, ARS) or data from general practitioners (CPRD, PEDIANET, BIFAP, FISABIO). For this final report data from 9 data sources were included. Study size: The study population for the total study comprised approximately 141.6 million individuals. In this final report, a total number of 45 million individuals were included. An update including French data is expected later this year Results This report comprises background rate data on AESI from 6 countries (UK, ES, IT, DK, NL, DE) and 9 data sources(BIFAP, Pedianet (children only), CPRD, ARS, Danish registries, FISABIO, SIDIAP, PHARMO, GeParD). Data from France (SNDS) could not be generated in a timely manner due to administrative constraints in data release. Data sources included different subpopulations based on the availability of numerator data of the observed persontime (Hosp= hospital based, PC= primary care, HOSP-PC= overlap between hospitalization and primary care). This entry also includes the results in excel format and also the links to the codes and event definitions, This protocol has been accepted by EMA as a deliverable of the framework contract No EMA/2018/28/PE. Patients and methods: We conducted a case–control study nested in a cohort of new users of ADs aged ≥65 years, identified in the German Pharmacoepidemiological Research Database during 2005–2014. Cases were patients first hospitalized for TBI. Up to 100 controls per case were selected using incidence density sampling. AD use was ascertained at the index date based on the supply of last dispensing (adding 150% of the defined daily doses [DDDs]; in sensitivity analysis, no additional DDDs were considered). We estimated adjusted ORs (aORs) and 95% CIs using conditional logistic regression. Results: Among 701,309 cohort members, 16,750 cases were identified and matched to 1,673,320 controls (in both groups: 70.4% women; median age 80 years). Compared with remote users of the same AD, current users had an aOR (95% CI) of 1.87 (1.56–2.24) for duloxetine, 1.74 (1.41–2.15) for escitalopram, 1.70 (1.58–1.83) for citalopram, 1.66 (1.40–1.97) for sertraline, 1.64 (1.24–2.15) for fluoxetine and 1.57 (1.20–2.06) for paroxetine. The aOR was lower for amitriptyline (1.45; 1.32–1.58), trimipramine (1.17; 0.99–1.38) and opipramol (1.11; 0.99–1.25). Mirtazapine had an aOR of 1.03 (0.94–1.12). Sensitivity analysis confirmed the findings. Abstract

Purpose: The prevalence of colorectal cancer is higher among patients with type 2 diabetes mellitus (T2D) than among patients without diabetes. Furthermore, men are at higher risk for developing colorectal cancer than women in the general population and also subsite-specific risks differ per sex. The aim was to evaluate the impact of T2D on these associations. Methods: A population-based matched cohort study was performed using data from the PHARMO Database Network. Patients with T2D were selected and matched (1:4) to diabetes free controls. Cox proportional hazards models were used to estimate hazard ratios (HRs) for CRC and its subsites. HRs were determined per sex and adjusted for age and socioeconomic status. The ratio of distal versus proximal colon cancer was calculated for people with T2D and controls per sex and stratified by age. Results: Over 55,000 people with T2D were matched to > 215,000 diabetes free controls. Men and women with T2D were 1.3 times more likely to develop colorectal cancer compared to controls. Men with T2D were at higher risk to develop distal colon cancer (hazard ratio (95% confidence interval), 1.42 (1.08–1.88)), and women with T2D were at higher risk for developing proximal colon cancer (hazard ratio (95% confidence interval), 1.58 (1.13–2.19)). For rectal cancer, no statistically significant risk was observed for both men and women. Conclusions: Sex-specific screening strategies and prevention protocols should be considered for people with T2D. More tailored screening strategies may optimize the effectiveness of colorectal cancer screening in terms of reducing incidence and mortality.

Abstract

Objectives: To compare diclofenac use before and after implementation of European risk minimization measures in 2013, focusing on diclofenac initiators and prevalence of congestive heart failure (NYHA class II-IV), ischaemic heart disease, peripheral arterial disease and cerebrovascular disease (new contraindications) in these patients in Germany.Methods: We included adults with health insurance coverage on 1 January 2011 (cohort 2011) or 1 January 2014 (cohort 2014) and during a 1-year pre-observation period. We defined diclofenac initiators as persons filling a prescription of systemic diclofenac in 2011 (cohort 2011) or 2014 (cohort 2014) and without such a prescription during the respective pre-observation period.Results: Each cohort comprised >10 million persons. Between 2011 and 2014, the age-standardized proportion of persons initiating diclofenac decreased by 29% (from 8.2% to 5.8%) amongst female patients and by 26% (from 8.5% to 6.3%) amongst male patients; in the subgroup of persons with new contraindications, this proportion decreased by 33% (from 9.8% to 6.6%) amongst female patients and by 31% (from 10.0% to 6.7%) amongst male patients. Amongst diclofenac initiators, the proportion of those with new contraindications did not change between 2011 (12.0%) and 2014 (11.8%).Conclusion: The overall decline of about 30% in diclofenac initiation between 2011 and 2014 was largely independent of the presence or absence of new contraindications. The proportion of diclofenac initiators with a new contraindication remained at a high level (more than one in ten patients), demonstrating the need for research at the prescriber level (e.g. interventional studies) and further measures to improve patient safety. Abstract

Researchers are actively pursuing the development of a new non-invasive test (NIT) for colorectal cancer (CRC) screening as an alternative to fecal occult blood tests (FOBTs). The majority of pilot studies focus on the detection of invasive CRC rather than precursor lesions (i.e., adenomas). We aimed to explore the relevance of adenoma detection for the viability of an NIT for CRC screening by considering a hypothetical test that does not detect adenomas beyond chance. We used the Simulation Model of Colorectal Cancer (SimCRC) to estimate the effectiveness of CRC screening and the lifetime costs (payers' perspective) for a cohort of US 50-years-old persons to whom CRC screening is offered from age 50-75. We compared annual screening with guaiac and immunochemical FOBTs (with sensitivities up to 70 and 24% for CRC and adenomas, respectively) to annual screening with a hypothetical NIT (sensitivity of 90% for CRC, no detection of adenomas beyond chance, specificity and cost similar to FOBTs). Screening with the NIT was not more effective, but was 29-44% more costly than screening with FOBTs. The findings were robust to varying the screening interval, the NIT's sensitivity for CRC, adherence rates favoring the NIT, and the NIT's unit cost. A comparative modelling approach using a model that assumes a shorter adenoma dwell time (MISCAN-COLON) confirmed the superiority of the immunochemical FOBT over an NIT with no ability to detect adenomas. Information on adenoma detection is crucial to determine whether a new NIT is a viable alternative to FOBTs for CRC screening. Current evidence thus lacks an important piece of information to identify marker candidates that hold real promise and deserve further (large-scale) evaluation. What's new? The development of new non-invasive tests (NITs) for colorectal cancer (CRC) screening has centered primarily on improving the detection of invasive disease. But according to this study, whether new NITs are viable alternatives to immunochemical fecal occult blood testsalready established screening tests for CRC-depends also on their ability to detect adenomas, the precursor lesions of CRC. Without that ability, immunochemical fecal occult blood tests retain their superiority over newer NITs, many of which attempt to detect biomarkers in stool or blood. The identification of adenoma-specific markers could be critical to the advance of NITs for CRC screening.
