10. Long-term impact of COVID-19 pandemic: Moral tensions, distress, and injuries of healthcare workers.

Abstract

Given the longevity of the COVID-19 pandemic, it is important to address the perceptions and experiences associated with the progression of the pandemic. This narrative can inform future strategies aimed at mitigating moral distress, injury, and chronic stress that restores resilience and well-being of HCWs. In this context, a longitudinal survey design was undertaken to explore how health care workers are experiencing the COVID-19 pandemic over time. A qualitative design was employed to analyze the open ended survey responses using a thematic analysis approach. All physicians and staff at an academic health science centre in Toronto, Ontario, Canada were invited to participate in the survey. The majority of survey respondents were nurses and physicians, followed by researchers/scientists, administrative assistants, laboratory technicians, managers, social workers, occupational therapists, administrators, clerks and medical imaging technologists. The inductive analysis revealed three themes that contributed to moral tensions and injury: 1) experiencing stress and distress with staffing shortages, increased patient care needs, and visitor restrictions; 2) feeling devalued and invisible due to lack of support and inequities; and 3) polarizing anti- and pro-public health measures and incivility. Study findings highlight the spectrum, magnitude, and severity of the emotional, psychological, and physical stress leading to moral injury experienced by the healthcare workforce. Our findings also point to continued, renewed, and new efforts in enhancing both individual and collective moral resilience to mitigate current and prevent future moral tensions and injury. 17. Pediatric syncope diagnosis and management: Validation of continuous blood pressure monitoring and alternatives to 24-hour urine sodium sampling

Abstract

Syncope, or fainting, is common and has a devastating impact on quality of life. Diagnosis and management of syncope is challenging. In pediatric populations the current diagnostic gold standard, a tilt test, cannot be safely or properly performed, because the necessary non-invasive beat-to-beat blood pressure monitoring is not validated for children. In addition, low sodium intake is common in syncope patients, and salt supplementation is recommended. However, standard assessments of sodium from urine collections are difficult and unpleasant. This thesis demonstrated that: (i) continuous non-invasive finger blood pressure monitoring provides a novel, comfortable, and accurate approach for use in children compared to an intra-arterial catheter, (ii) Quantab test strips provide a valid alternative to flame photometry for the determination of 24-hour urine sodium levels, and corrected spot sample averages offer an acceptable and convenient alternative to 24-hour urine sampling. These advancements in diagnostic tools for syncope will enhance quality of life for affected individuals.

Abstract

First year orientation activities at post-secondary institutions serve the purpose of facilitating peer-to-peer and peer-to-instructor relationships [1,2]. It has been shown that the creation of these relationships is positively correlated with increased student retention rates [2-4]. Several studies have evaluated activities pertaining to outdoor/physical education, small focus groups, program and faculty information sessions, and facility orientation to try and determine which are most effective in increasing student outcomes [1-4]. However, little research has been conducted to date regarding the evaluation and effectiveness of multi-activity programs. Therefore, the primary purpose of this study is to assess the effectiveness of first year orientation activities (e.g., peer mentoring, outdoor team building) in an Ontario university Kinesiology program for developing relationships between first year students and others within the department. It is hoped that by improving these relationships, there will be an associated long term increase in academic achievement and retention rates. Each winter semester over the next five years, first year undergraduate students will complete an online survey regarding their orientation experiences from the previous fall. The survey will ask students to reflect on their first year orientation experiences and provide feedback on how effective they thought each activity was at orienting them to the campus, developing relationships among their peers, staff and faculty, and other success criteria. Quantitative results will be summarized using descriptive statistics and stratified based on factors such as year and sex. Qualitative responses will be assessed using open and axial coding techniques using QSR Nvivo software. The results of these analyses will be used to modify orientation activities in future years, with the intent of improving relationships between students, staff, and faculty on a continual basis.
