In this study, we aimed to evaluate our pneumothorax rates and risk factors associated with pneumothorax in patients who underwent percutaneous transthoracic lung biopsy (PTAB). Radiological images, reports, and pathology results of 754 PTAB procedures performed in our clinic were reviewed retrospectively. The incidence of pneumothorax and pulmonary hemorrhage (PH) and PH types were determined. Biopsy-associated pneumothorax was detected in 143 patients (42.56%). Additional treatment was not required in 130 (90.91%) of these patients, while catheter drainage was performed in 13 (9.09%) patients. PH occurred in 153 (45.54%) patients, 124 (36.91%) had type 1 (tract hemorrhage) and 29 (8.63%) had type 2 (perilesional hemorrhage) hemorrhage. The presence of emphysema, increased pleural lesion distance, decreased pleural skin distance, passing through fissure with a needle during the procedure, and perifissurel localization of the lesion were found to increase the risk of pneumothorax (p, Bu çalışmada, perkütan transtorasik akciğer biyopsi (PTAB) işlemi yapılan hastalarda pnömotoraks oranlarımızı ve pnömotoraks ile ilişkili risk faktörlerinin değerlendirilmesini amaçladık. Kliniğimizde yapılan 754 PTAB işlemine ait radyolojik görüntüler, raporları ve patoloji sonuçları retrospektif olarak incelendi. Objective: It was aimed to investigate whether the fractional anisotropy (FA) differs for the benign and metastatic axillary lymph nodes (LNs).Material and Methods: 58 women with benign (n=33) and metastatic (n=25) axillary LNs who underwent diffusion weighted and tensor imaging with a 3T scanner were enrolled. Apparent diffusion coefficient (ADC) and FA, cortex thickness, long and short axes were measured retrospectively and compared statistically. Observer reliabilities were also assessed in terms of intra and inter-reviewer variability.Results: Metastatic LNs showed significantly lower ADC and FA values and greater cortex thickness, long and short axes. ROC test showed the area under the curve values of 0.876 for ADC, 0.661 for FA and 0.960 for cortex thickness. Cortex thickness had excellent sensitivity, specificity, and accuracy. A cutoff value of 3.5 mm for cortex thickness had 92% sensitivity, 94% specificity, 92% positive predictive value (PV), 93% negative PV, and 93% accuracy. A cutoff value of 0.774x10-3 mm2/s for ADC had 84% sensitivity, 82% specificity, 79% positive PV, 90% negative PV, and 84% accuracy. A cutoff value of 0.423 x10-3 mm2/s for FA had 64% sensitivity, 76% specificity, 67% positive PV, 71% negative PV, and 71% accuracy. High intra- and inter-observer reliabilities were seen.Conclusion: Among the parameters assessed by our study, cortex thickness had superior accuracy. ADC and FA showed a respectable diagnostic performance, especially the first one having a high negative PV and the second one relatively high specificity.

Objective: The aim was to evaluate the outcomesof preoperative endovascular embolization for glomus tumors including glomuscaroticum, glomus jugulare, and glomus vagale.Material and Methods: Between 2012 and 2017, eighteenpatients with glomus tumors were diagnosed by angiography and some of them wereembolized. Data were extracted for demographics, tumor subtype and size,embolization, and the rate of complication. The efficacy of tumor embolizationwas determined by comparing angiography before and after the intervention.Patients were followed-up for at least 24-48 hours for vital signs and possiblecomplications.Results: A total of 18 patients underwentdiagnostic angiography and of these, tumor was embolized in 10 patients. Sixpatients had glomus jugulare, 1 had glomus vagale, 10 had glomus caroticum and1 had concurrent glomus caroticum and vagale. The most common embolized arterywas ascending pharyngeal artery with a rate of 50%. Post-embolizationangiography revealed that blood flow to tumor had decreased at least by 70%. Nocomplications related to the procedure were experienced in any of the patients.Conclusion: Endovascular selective artery embolizationprior to surgery is a safe and effective method in treatment of glomus tumors., Amaç: Glomuskarotikum, vagale ve jugulare tümörlerinde preoperatif yapılan endovaskülerembolizasyon işlem sonuçlarının değerlendirilmesi amaçlandı.Gereçve Yöntemler: 2012-2017 yılları arasında glomus tümörü nedeniyle 18hastaya tanısal anjiyografi ve bazılarına da embolizasyon yapıldı. Hastalarındemografik bilgileri, tümör tipleri, embolizasyon ve komplikasyon oranlarıanaliz edildi. Tümör embolizasyonunun etkinliğine, girişim öncesi ve sonrasıalınan anjiyografiler karşılaştırarak karar verildi. Hastalar girişim sonrasıvital bulgular ve olası komplikasyonlar açısından en az 24-48 saat takipedildi.Bulgular: Tanısalanjiyografi yapılan 18 hastadan 10’una eş zamanlı embolizasyon da uygulandı.Altı hastada glomus jugulare, 1 hastada glomus vagale, 10 hastada glomuskarotikum ve 1 hastada eş zamanlı glomus jugulare ve vagale mevcuttu. En sıkembolize edilen arter %50 oranı ile asendan faringeal arterdi. Embolizasyonyapılan hastalarda tümör kan akımında en az %70 oranında bir azalma meydanageldiği saptandı. Yapılan işlemler ile ilgili herhangi bir komplikasyonoluşmadı.Sonuç: Endovasküler selektif arter embolizasyonu glomustümörlerinin preoperatif tedavisinde güvenilir ve etkin bir yöntemdir.

Objective: The aimof this study is to share our experience of ultrasound-guided percutaneusthrombin injection in the treatment in iatrogenic femoral arterypseudoaneurysms and discuss about the safety and efficacy of this minimallyinvasive treatment.Material and Methods: Fourty-two patients (26 female, 16 male) withiatrogenic femoral artery pseudoaneurysms treated with percutaneousultrasound-guided thrombin injection between June 2012 and August 2017 wereretrospectively evaluated. The diameter and the number of the pseudoaneurysmsac, artery of origin, pseudoaneurysm shape and structure were identified byDoppler ultrasound and thrombin was injected via a 21 G needle. Control withDoppler ultrasound was performed on the first day and one weekafter theprocedure.Results: The mean age of the patients included in the study was66 years (range: 46-88 years). Thirty-three percent of pseudoaneurysm sacs hadoriginated from the superficial femoral artery and 67%from the common femoralartery. Mean pseudoaneurysm diameter was 36.7 mm (range: 19-55 mm), mean necklenght was 21 mm and mean neck diameter was 3.2 mm. Pseudoaneurysm in 50%of thepatients was partially thrombosed, while 8 patients had two or more synchronouspseudoaneurysms and 8 patients had multilobulated pseudoaneurysm sacs. Pseudoaneurysmswere completely thrombosed with a single injection in 83%of patients. In twopatients, pseudoaneurysms were thrombosed with mild compression in addition tothrombin injection during the first procedure. Four patients’ pseudoaneurysmswere thrombosed after a second injection within the first week because ofrecurrence. In one patient, the pseudoaneurysm sac enlarged after procedure andrequired stent-graft owing to superficial femoral artery compression andocclusion. Technical, primary and secondary success rates were 100%, 93% and 98%respectively. No complications occured during the procedure.Conclusion: Ultrasound-guided percutaneus thrombininjection is a reliable and effective method that can be easily applied even innumerous and multilobulated pseudoaneurysms., Amaç: Bu çalışmada, iyatrojenik femoral arter psödoanevrizmalarındaultrason eşliğinde perkütan trombin enjeksiyonu tedavisi hakkında tecrübemiziaktarmak ve bu minimal invaziv tedavinin etkinliği ile güvenilirliğininsunulması amaçlandı.Gereç ve Yöntemler: Bu çalışmada Haziran 2012 ileAğustos 2017 arasında ultrason eşliğindeperkütan trombin enjeksiyonu ile tedavi edilmiş iyatrojenik femoral arterpsödoanevrizmalı 42 hasta (26 kadın, 16 erkek) retrospektif olarakdeğerlendirildi. Doppler ultrason ile psödoanevrizma kesesi sayısı, çapı,psödoanevrizma şekli ve yapısı, orijin aldığı arter tanımlandı ve sonrasında 21G iğne ile trombin enjeksiyonu gerçekleştirildi. İşlem sonrası kontrol, ilk günve birinci hafta sonrası Doppler ultrason ile yapıldı.Bulgular: Çalışmaya dahil edilen hastların ortalama yaşı 66 idi(46-88 yaş). Psödoanevrizmalar hastaların %33’ünde yüzeyel femoral arterden,%67’sinde ana femoral arterden köken almaktaydı. Ortalama psödoanevrizma çapı36.7 mm (19-55 mm), ortalama psödoanevrizma uzunluğu 21 mm, ortalama boyun çapı3.2 mm olarak ölçüldü. Psödoanevrizmalar hastaların %50’sinde parsiyeltromboze, 8 hastada aynı zamanda iki veya daha fazla sayıda ve 8 hastadamultilobuleydi. Hastaların %83’ünde psödoanevrizma kesesi tek enjeksiyondatotal tromboze oldu. İki hastanın psödoanevrizma kesesi ilk prosedür esnasındatrombin enjeksiyonuna ek olarak hafif kompresyon ile tromboze edildi. Dörthastada ilk hafta içinde gelişen rekürrens nedeniyle ikinci bir enjeksiyon iletotal tromboz sağlandı. Bir hastada psödoanevrizma boyutunun artması sonucuyüzeyel femoral artere bası yapması ve akımı engellemesi nedeniylepsödoanevrizma stent-greft ile tedavi edildi. Teknik, primer ve sekonder başarıoranları ise sırasıyla %100, %93 ve %98 olarak hesaplandı. İşlemler esnasındakomplikasyon gelişmedi.Sonuç: İyatrojenik femoral arter psödoanevrizmalarınıntedavisinde uygulanan ultrason eşliğinde perkütan trombin enjeksiyonu;güvenilir, etkin ve çok sayıda, multilobule psödoanevrizmalarda bile kolayuygulanabilir bir yöntemdir.

Üst ve alt gastrointestinal (GI)fistüllerin saptanmasında, yerlerinin belirlenmesinde ve uzanımlarınıngörülmesinde konvansiyonel fluoroskopik kontrastlı çalışmaların yararlılığınıaraştırmayı amaçladık. Çalışmamıza, klinik olarak çeşitli tiplerde GI fistülleriolduğu düşünülen, yaşları 1 ay ile 75 yaş arasında değişen 24 hasta dahil edildi. Fistülleri göstermekiçin suda çözünür iyotlu kontrast ajanlar her olgu için en uygun şekilde oral,perkütan veya rektal yoldan verildi.Olgular etiyolojik olarak sınıflandırıldı. Fistüllerin, orokutanöz/tiroglossal(n=1), trakeoözofageal (n=4), özofagoplevral (n = 4), gastrokolik (n=1),duodenorenal (n=2), duodenokütanöz (n=1) , enterokutanöz (n = 2), enterovezikal(n=1), kolokütanöz (n = 1), kolovezikal (n=1), rektovezikal (n=1), rektovajinal(n=1), anorektokütanöz (n= 4) olduğu ve uzanımları etkin olarak gösterilebildi.İatrojenik etyolojiler (cerrahi, radyoterapi vb.), 24 hastanın 11'inde(%45.8) GI fistüllerin başlıca nedeniolarak tespit edildi. Kontrast madde geçişini gerçek zamanlı olarakgörebilmemiz, seçtimiz bölgeye yönelik çalışabilmemiz, farklı projeksiyonlardayüksek uzaysal çözünürlükte görüntüler elde edebilmemiz nedeniyle, değişiktürlerde GI fistüllerin ilk görüntülemesinde konvansiyonel fluoroskopikkontrastlı çalışmaların tercih edilen radyolojik yöntem olmaya devam ettiğisonucuna vardık., Our aim was to investigate theutility of conventional fluoroscopic contrast studies in detecting, localizingthe upper and lower gastrointestinal (GI) fistulas and seeing their extensions.Our study included 24 patients between 1 month to 75 years of ages who wereclinically suspected to have various types of GI fistulas. We administeredwater soluble iodinated contrast agents orally, percutaneously and rectally in an appropriate way in each case todemonstrate the fistulas. The cases were etiologically classified. We were ableto demonstrate orocutaneous/thyroglossal (n=1), tracheoesophageal (n=4),esophagopleural (n=4), gastrocolic (n=1), duodenorenal (n=2), duodenocutaneous(n=1), enterocutaneous (n=2), enterovesical (n=1), colocutaneous (n=1),colovesical (n=1), rectovesical (n=1), rectovaginal (n=1), anorectocutaneous(n=4) fistulas and their extensions effectively. Iatrogenic etiologies(surgery, radiotherapy etc.) were found to be the leading cause of GI fistulas by 11 patients out of 24 (45.8%). Since we are able to see contrast material flow real-time, makeselective studies and get highest spatial resolution images in differentprojections, we conclude that conventional fluoroscopic contrast studies remainto be the radiologic method of choice particularly for the initial imaging ofvarious types of GI fistulas.
