Abstract

Abstract Diffuse leptomeningeal glioneuronal tumours (DLGNT) represent rare enigmatic CNS tumours of childhood. Most patients with this disease share common radiological and histopathological features but the clinical course of this disease is variable. A radiological hallmark of this disease is widespread leptomeningeal enhancement that may involve the entire neuroaxis with predilection for the posterior fossa and spine. The classic pathologic features include low- to moderate-density cellular lesions with OLIG2 expression and evidence of ‘oligodendroglioma-like’ appearance. The MAPK/ERK signaling pathway has recently been reported as a potential driver of tumourigenesis in up to 80% of DLGNT with KIAA1549:BRAF fusions being the most common event seen. Until now, limited analysis of the biological drivers of tumourigenesis has been undertaken via targeted profiling, chromosomal analysis and immunohistochemistry. Our study represents the first examples of comprehensive genomic sequencing in DLGNT and shows that it is not only feasible but crucial to our understanding of this rare disease. Moreover, we demonstrate that DLGNT may be more genomically complex than single-event MAPK/ERK signaling pathway tumours.

Abstract

Introduction: Autosomal recessive renal tubular dysgenesis (ARRTD) is a rare disorder of renal tubular development. ARRTD is a severe condition with high risk of fetal demise and early neonatal death, with only limited case reports of survival over 2 years [Clin Kidney J. 2012 Feb 1;5(1):56–8]. Prenatal diagnosis of ARRTD is challenging, and diagnosis has only previously been confirmed after postnatal or post-mortem investigation. Case: To the best of our knowledge, we describe the first reported case of utilizing targeted genetic testing on the chorionic villous sample (CVS) to identify a homozygous variant in the angiotensinogen (AGT) gene. Discussion: By substantiating the diagnosis of ARRTD prenatally, we allow timely and appropriate counseling during pregnancy.

Abstract

Adeno-associated virus (AAV) vectors are proving to be clinically transformative tools in the treatment of monogenic genetic disease. Rapid ongoing development of this technology promises to not only increase the number of monogenic disorders amenable to this approach but also to bring diseases with complex multigenic and nongenetic etiologies within therapeutic reach. In this study, we explore the broader paradigm of converting the liver into a biofactory for systemic output of therapeutic molecules using AAV-mediated delivery of the endonuclease DNaseI as an exemplar. DNaseI can clear neutrophil extracellular traps (NETs), which are nuclear-protein structures possessing antimicrobial action, also involved in the pathophysiology of clinically troubling immune-mediated diseases. Abstract

Public health practitioners are called to effectively communicate with the public on climate change. The climate crisis requires swift action that starts with public awareness of climate-related health impacts and leads to public support for individual, community and systemic actions to mitigate and adapt to climate change.This paper discusses learnings about public opinion research and communication strategies and how a health-focused climate communication campaign-#MakeItBetter-could help to increase awareness and engage new audiences, including public health partners, in conversations about climate change in order to reduce climate-related health impacts for current and future generations. The #MakeItBetter campaign was grounded in evidence-informed messaging, being sensitive to health inequities. Emerging research and pre-campaign testing suggest that framing climate change as a health issue is a promising practice.The #MakeItBetter campaign appeals to parents/caregivers to learn more about climate-related health impacts, take protective action for children and support multi-level climate action. The campaign launch secured 89 news stories, including multicultural media coverage. Longer-term evaluation is required to determine the campaign's effectiveness in building public support for climate action.An innovative approach to climate communication that draws on the intersections between behavioural and climate sciences and engages in multi-sectoral collaboration can spur both climate action and health protection, aiding public health practitioners and partners in effectively communicating the urgency for climate action. More work is needed to support communication on climate change as an inequity multiplier and promote climate action and community resilience for health equity co-benefits.RéSUMé: LIEU: Les praticiens de la santé publique sont appelés à transmettre à la population des messages efficaces sur le changement climatique. La crise climatique nécessite une action rapide qui commence par la sensibilisation du public aux incidences du climat sur la santé et mène à l’aide publique aux actions individuelles, collectives et systémiques pour atténuer le changement climatique et s’y adapter. INTERVENTION: Notre article porte sur les enseignements de la recherche sur l’opinion publique et des stratégies de communication et sur le moyen pour une campagne d’information axée sur les effets du climat sur la santé, #MakeItBetter, de favoriser une prise de conscience et de toucher de nouveaux publics, notamment les partenaires de la santé publique, en nouant un dialogue sur le changement climatique afin de réduire les incidences du climat sur la santé des générations actuelles et futures. Fondée sur des messages factuels, la campagne #MakeItBetter est sensible aux inégalités de santé. Selon la recherche émergente et les essais menés avant la campagne, la pratique qui consiste à présenter le changement climatique comme une question de santé est prometteuse. RéSULTATS: La campagne #MakeItBetter demande aux parents/proches aidants de s’informer des incidences du climat sur la santé, de prendre des mesures pour protéger leurs enfants et d’appuyer une action climatique à plusieurs niveaux. Le lancement de la campagne a été mentionné dans 89 articles, y compris dans la presse multiculturelle. Une évaluation à plus long terme est nécessaire pour déterminer dans quelle mesure la campagne a réussi à convaincre le public d’appuyer l’action climatique. CONSéQUENCES: Une démarche de communication novatrice qui mise sur les liens entre les sciences du comportement et du climat et sur la collaboration multisectorielle peut stimuler à la fois l’action climatique et la protection de la santé, aidant ainsi les praticiens de la santé publique et leurs partenaires à communiquer efficacement l’urgence de l’action climatique. Abstract

The complement system is a key component of innate immunity, but impaired regulation influences disease susceptibility, including age-related macular degeneration and some kidney diseases. While complete complement inhibition has been used successfully to treat acute kidney disease, key unresolved challenges include strategies to modulate rather than completely inhibit the system and to deliver therapy potentially over decades. Elevating concentrations of complement factor I (CFI) restricts complement activation in vitro and this approach was extended in the current study to modulate complement activation in vivo. Sustained increases in CFI levels were achieved using an adeno-associated virus (AAV) vector to target the liver, inducing a 4- to 5-fold increase in circulating CFI levels. This led to decreased activity of the alternative pathway as demonstrated by a reduction in the rate of inactive C3b (iC3b) deposition and more rapid formation of C3 degradation products. In addition, vector application in a mouse model of systemic lupus erythematosus (NZBWF1), where tissue injury is, in part, complement dependent, resulted in reduced complement C3 and IgG renal deposition. Collectively, these data demonstrate that sustained elevation of CFI reduces complement activation in vivo providing proof-of-principle support for the therapeutic application of AAV gene delivery to modulate complement activation.

Abstract

We present eight families with arthrogryposis multiplex congenita and myopathy bearing a TTN intron 213 extended splice-site variant (NM_001267550.1:c.39974-11T>G), inherited in trans with a second pathogenic TTN variant. Muscle-derived RNA studies of three individuals confirmed mis-splicing induced by the c.39974-11T>G variant; in-frame exon 214 skipping or use of a cryptic 3' splice-site effecting a frameshift. Confounding interpretation of pathogenicity is the absence of exons 213-217 within the described skeletal muscle TTN N2A isoform. However, RNA-sequencing from 365 adult human gastrocnemius samples revealed that 56% specimens predominantly include exons 213-217 in TTN transcripts (inclusion rate ≥66%). Further, RNA-sequencing of five fetal muscle samples confirmed that 4/5 specimens predominantly include exons 213-217 (fifth sample inclusion rate 57%). Contractures improved significantly with age for four individuals, which may be linked to decreased expression of pathogenic fetal transcripts. Abstract

Three distinct clusters were identified from a survey study of a sample of 127 unit coordinators from a regional Australian University. The clusters emerged after a survey that explored perceptions of pedagogical practices that incorporated the use of Information Communication and Technology (ICT). The key components of the survey were based on seven constructs derived from the Technological Pedagogical and Content Knowledge (TPACK). For future investigations of TPACK application in university contexts, a three-cluster configuration of teacher-practitioners is proposed that requires empirical confirmation. Alongside the theorised clusters of university lecturers according to their perceived engagement with ICT, several layers of technology policy disconnect have also been discovered. The relevance of the findings of the inquiry and their implications on universities that conduct ICT intensive courses are also discussed, especially in relation to improving teaching practices.

Abstract

Background Heat wave early warning systems help alert decision-makers and the public to prepare for hot weather and implement preventive actions to protect health. Prior to harmonization, public health units across Ontario either used independent systems with varying methodologies for triggering and issuing public heat warnings or did not use any system. The federal government also issued heat warnings based on different criteria. During heat events, adjacent public health units in Ontario and the federal government would routinely call heat warnings at different times with separate public messages, leading to confusion. This article describes the collaborative process and key steps in developing a harmonized Heat Warning and Information System (HWIS) for Ontario. Setting Public health units across Ontario, Canada, collaborated with the federal and provincial government to develop the harmonized HWIS for Ontario. Intervention In 2011, stakeholders identified the need to develop a harmonized system across Ontario to improve heat warning services, warning criteria, and health messaging. Through a 5-year process facilitated by a non-governmental organization, the three levels of government collaborated to establish the Ontario HWIS. Outcomes The province-wide HWIS was implemented in 2016 with the Ontario Ministry of Health and Long-Term Care’s release of the harmonized HWIS Standard Operating Practice, which outlined the notification and warning process. Implications The lessons learned could help spur action in other provinces and jurisdictions internationally in the development of similar health evidence-based warning systems, including in particular those for protecting public health during extreme heat events.

Abstract

Over the past few years the cataloguing community has seen radical changes in cataloguing standards, changes which appear to have been largely ignored by legal information professionals. This is a mistake according to Helen Doyle; the new cataloguing model can have enormous implications for the legal community, particularly in the spheres of information and knowledge management, and the profession is missing a huge opportunity by ignoring it. A new cataloguing standard (RDA) has been developed based on an alternative theoretical approach, known as “Functional Requirements for Bibliographic Records” (FRBR). FRBR seeks to change the way we approach the bibliographic universe: from stand-alone, individual repositories of information to networks of linked data built on a structured hierarchy. Commercial law firms are constantly trying to make connections between their traditional resources, online repositories, internal know-how, etc, but struggle to achieve complete synchronicity. FRBR provides a solution to this knowledge management problem: all resources (including people, events and subjects) become searchable, and because everything is linked, users can access information by navigating to it, establishing their own pathway through the data. Moreover, the major legal databases are already utilising linked data in this way – it is time for law firms to catch up.

The constructs emerged after employing Exploratory Factor Analysis (EFA) on respondents' perceptions of pedagogical practices incorporating the use of Information Communication and Technology (ICT). The key components of the survey were derived from a Technological Pedagogical and Content Knowledge (TPACK) survey developed for a national project. For future investigations of TPACK application in university contexts, a four-construct configuration of pre-service teacher TPACK perceptions is proposed requiring empirical confirmation. This inquiry depicts a portrait of emerging domains of TPACK. The relevance of the findings and their implications for universities that rely heavily on ICT in the delivery of are discussed, especially in relation to improving teaching practices.

Abstract

Background: Treatment guidelines recommend a stepwise approach to primary biliary cholangitis: all patients begin treatment with ursodeoxycholic acid (UDCA) monotherapy and those with an inadequate biochemical response after 12 months are subsequently considered for second-line therapies. However, as a result, patients at the highest risk can wait the longest for effective treatment. We determined whether UDCA response can be accurately predicted using pretreatment clinical parameters. Methods: We did logistic regression analysis of pretreatment variables in a discovery cohort of patients in the UK with primary biliary cholangitis to derive the best-fitting model of UDCA response, defined as alkaline phosphatase less than 1·67 times the upper limit of normal (ULN), measured after 12 months of treatment with UDCA. We validated the model in an external cohort of patients with primary biliary cholangitis and treated with UDCA in Italy. Additionally, we assessed correlations between model predictions and key histological features, such as biliary injury and fibrosis, on liver biopsy samples. Findings: 2703 participants diagnosed with primary biliary cholangitis between Jan 1, 1998, and May 31, 2015, were included in the UK-PBC cohort for derivation of the model. Abstract

There is increasing evidence to show that the circumstances of people with visual impairment in the UK have worsened in recent years and that statutory services are increasingly under pressure. As a voluntary sector provider, changes in the visual impairment sector and in the provision of health and social care meant that an assessment of a new model of service delivery needed to be investigated. This article reviews the changing landscape of service provision for people with sight loss, outlines the findings of current research into the needs of our client group and discusses the implications of these findings, which ultimately led to a new model of service delivery—Henshaws Pathway to Independence.

Abstract

Primary biliary cirrhosis (PBC) is a classical autoimmune liver disease for which effective immunomodulatory therapy is lacking. Here we perform meta-analyses of discovery data sets from genome-wide association studies of European subjects (n=2,764 cases and 10,475 controls) followed by validation genotyping in an independent cohort (n=3,716 cases and 4,261 controls). We discover and validate six previously unknown risk loci for PBC (Pcombined, Primary biliary cirrhosis is an autoimmune liver disease with poor therapeutic options. Here Cordell et al. a perform meta-analysis of European genome-wide association studies identifying six novel risk loci and a number of potential therapeutic pathways.

Abstract

Chronic low back pain (CLBP) is endemic in Western societies, and while a good deal of attention has been paid to the lay experience of such pain, much less sociological attention has been paid to the way in which medical ideas about it have been formulated. This paper takes the latter course, tracing the development of clinical notions about the relationship between pathological signs and expressed symptoms from the 1820's to the 1930's, and then placing these developments in the context of postwar notions of 'somatization'. We point to the extent to which the disparity between expressed symptoms, pathological signs and perceived disability in CLBP has led to the moral character of the sufferer forming a constant subtext to medical discourse about the condition. We also note the extent to which medical ideas themselves have been constructed in intimate linkage with socio-legal questions of compensation and worker's insurance.

Research has become increasingly important within social science since the introduction of the Research Assessment Exercise (RAE). The RAE has served to simultaneously focus the attention of academic institutions upon research output and emphasise the obvious penalties of poor performance in this area in terms of potentially lower academic status, lower income and lower student numbers. Academic institutions have, therefore, begun to pay an increasing amount of attention to postgraduate research and the needs of postgraduates. This article questions the extent to which institutions are actually meeting these needs and reviews the extent to which institutions reap the potential benefits that employment of postgraduates can bring.

Academic authors typically assign full copyrights to their articles to the publishers of the journals in which the works appear. In recent years, however, several technological and legal innovations have led a growing number of scientists to begin to question the sagacity of this arrangement. The advent of electronic publishing and the Internet itself have made technically possible a slew of novel uses of primary research papers. Simultaneously, the traditional “all rights reserved” copyright license has been supplemented by a variety of alternative licenses—of equal legal validity and available at no charge to anyone who wants them—that allow copyright holders to prevent some uses of a work without permission, but to authorize others. Different licenses created by the nonprofit organization Creative Commons (www.creativecommons.org), for example, allow copyright holders to mark their work with freedoms—to permit a work's reproduction for any noncommercial purpose (the Noncommercial License) or for any purpose at all provided that the original authorship is properly attributed (the Attribution License). The upshot of these developments is that copyright holders can now permit a spectrum of uses of a paper by prospective researchers, anthologizers, archivists, teachers, patients, policy makers, journalists, and other interested parties. Precisely which uses are permitted and which are not is far from a trivial matter. The particular copyright license under which an article is published largely determines how the document can be stored, searched, and built upon by other scientists.

Abstract

Scientific societies serve their members, their broader scholarly communities, and the different components of their missions in many important ways. Making peer-reviewed literature immediately accessible, searchable, and reusable to anyone in the world with an Internet connection is a uniquely direct means of achieving a number of goals that are common to most scholarly associations and of advancing the diverse interests of their constituencies. Setting aside for the moment the question of how feasible it is for societies to alter their journals' access policies, there is by now a broad consensus that widespread open access to scientific publications is good for scientists and good for science. Society members want to maximize the impact of their work—and articles that are freely available online are cited more frequently than those that are not (Lawrence 2001). Most societies are committed to catalyzing innovations within and across scientific disciplines—and open-access archives of full-text literature provide a valuable tool for sharing information globally in order to accelerate the rate of scientific progress. Many societies articulate in their mission statements the goal of communicating the benefits of their members' discoveries with the public—and open-access publishing is a direct means to accomplish this goal.

There has been much concern about the possibility of discrimination in the allocation of distinction awards to consultants. The aim of this study was to assess whether there is any disparity between white and non-white consultants in the receipt of distinction awards. We used the list of all consultants in England and Wales who are currently in receipt of a distinction award published by the Advisory Committee on Distinction Awards1 to determine the ethnicity of award holders using the surname of the award holder as a proxy for ethnicity. We classified consultants with Asian, Chinese, and African names as non-white and consultants with Anglo-Saxon, East European, and South European names as white. All consultants are eligible for distinction awards, so for denominator data we obtained the number of consultants by region and specialty from the 1996 census of the NHS workforce carried out by the Department of Health.2 This …

Abstract

In the wake of declarations supporting open access to research literature from international bodies including the Organization for Economic Cooperation and Development (OECD) and the United Nations' World Summit on the Information Society (WSIS), advocates and critics of the movement appear to have agreed that the issue warrants a robust, ongoing dialogue—a development undoubtedly in the interest of the scientific community, regardless of its ultimate outcome. To the extent that listserv messages, editorials, and conference presentations are representative of more widespread reactions to the debate, there appear to be a number of common misconceptions about what open access is and what problems it can or cannot solve. Over the next few months in PLoS Biology, we plan to explore the more pervasive of these misunderstandings, in an effort to expose the real challenges that need to be overcome and to identify some possible solutions. Here we address the first of these—the perception that the publication-charge model puts an unfair burden on authors. Subsequently, we will address concerns about the long-term economic viability of the open-access model, the integrity and quality of work published in open-access journals, and the effect that open access will have on scholarly societies.

Abstract

Open-access publication is inarguably valuable to science and to the public, yet skepticism about the long-term sustainability of open-access publications persists (see Nature, October 9, and Science, October 24). While some fear the effects of the open-access model—which shifts revenue streams from subscription fees to publication fees—on scientific societies and others, support for open access among funders is already increasing. Funding agencies that have announced policies supporting open access include the Howard Hughes Medical Institute and the Wellcome Trust (the largest private biomedical research funders in the United States and the United Kingdom, respectively) and, more recently, the Max Planck Institute, the Centre National de la Recherche Scientifique (CNRS), and other European agencies.
