Purpose Pathologic fractures of the extremities due to carcinoma metastases require individual and patient prognosis-related stabilization procedures. Quick remobilization of the patient to restore the quality of life is of high importance, especially in the case of subtrochanteric and diaphyseal femoral fractures. In our retrospective cohort study, we evaluated intraoperative blood loss, length of operation, complication rate, and regain of lower extremity function in plate compound osteosynthesis (PCO) versus intramedullary nailing (IM) for subtrochanteric and diaphyseal pathologic fractures of the femur. Methods Between January 2010 and July 2021, we retrospectively reviewed 49 patients who were treated at our institution for pathologic fractures of the subtrochanteric and diaphyseal femurs for group differences in terms of blood loss, length of operation, implant survival, and Musculoskeletal Tumor Society (MSTS) score. Results We included 49 stabilization procedures of the lower extremity due to pathologic fractures of the proximal or diaphyseal femur, with a mean follow-up of 17.7 months. IM (n = 29) had a significantly shorter operation time than PCO (n = 20) (112.4 ± 9.4 and 163.3 ± 15.96 min, respectively). We did not detect any significant differences in terms of blood loss, complication rate, implant survival, or MSTS score. Conclusion Based on our data, pathologic subtrochanteric and diaphyseal fractures of the femur can be stabilized with IM, which has a shorter operation time than PCO, but the complication rate, implant survival, and blood loss remain unaffected.

Purpose In view of our aging society, co-management with a geriatrician is becoming increasingly important. While such collaborations have been working successfully in trauma surgery for years, it is still unclear whether they are also helpful for non-trauma patients in orthopedics. The aim of this study was to investigate the effect of such a cooperation in orthopedic non-trauma patients with native and periprosthetic joint infections on the basis of five key areas. Results Analysis was carried out with 59 patients “with” and 63 “without” geriatric co-management. In the co-management group, delirium was detected significantly more often (p Conclusion Orthogeriatric co-management in orthopedic patients with native and periprosthetic joint infections and nontraumatic surgery appears to have positive impacts on recognition and treatment of delirium, pain management, transfer performance, and attention to renal function. Further studies should follow in order to conclusively assess the value of such co-management in orthopedic nontraumatic surgery patients.

Introduction Acetabular defect recognition and classification remains a challenging field of practice for orthopedic surgeons. Recently, the Acetabular Defect Classification (ADC) has been introduced to provide a reliable, reproducible and intuitive classification system. In order to improve ease of use and efficiency of the ADC, a browser-based application has been created. We hypothesized that the ADC application can improve rating performance of non-specialists (medical students) to achieve good inter- and intra-rater agreement and will compare favorable to the results of specialists (experienced surgeons) without the help of the application. Materials and methods The ADC is based on the integrity of the acetabular rim and the supporting structures. It consists of four main types of defects ascending in severity. These defects are further subdivided in A–C, narrowing down defect location. 80 randomized radiographs were graded according to ADC by three non-specialists (medical students) with help of the ADC application and by three specialists (orthopedic surgeons) without help of the application to evaluate the difference in inter-rater agreement between groups. To account for intra-rater agreement, the rating process was repeated after a reasonable wash-out period. Results Inter-rater and intra-rater agreement within the non-specialist group rated lower when compared to the specialist group while still falling into the good agreement range. The student group presented with k values of 0.61 for inter-rater agreement and 0.68 for intra-rater agreement, while the surgeon group displayed k values of 0.72 for inter-rater agreement and 0.83 for intra-rater agreement. Conclusion The app-guided assessment of acetabular defects offers a promising innovative approach to simplify complex situations. It makes the challenging field of acetabular revision arthroplasty more approachable especially for less experienced surgeons and offers insight and guidance in the planning stage as well as intra-operative setting.

Wiederherstellung des Streckapparates bei fortgeschrittenem Sehnendefekt im Rahmen der Revisionsendoprothetik des Kniegelenkes. Rekonstruktion und Augmentation mittels vaskularisiertem Muskel- und Sehnengewebe als gefasgestielte Augmentationsplastik. Fortgeschrittene Degeneration des Streckapparates (Patellasehne; Quadrizepssehne) mit und ohne Kontinuitatsunterbrechung. Persistierende Infektion oder anstehende Revision einer einliegenden Knietotalendoprothese (TEP). Schadigung von Gastrocnemius, Soleus oder Achillessehne. Erweiterung des Zuganges uber den Unterschenkel medial, Praparation des M. gastrocnemius medialis, Auftrennen der Raphe und Praparation der Sehne vom Soleusanteil. Weit distale Abtrennung der Sehne, Transposition auf den Defekt. Einschlagen der Sehne und Augmentation bzw. Rekonstruktion des Defektes, der Muskelbauch dient dabei der suffizienten Bedeckung der Sehne wie auch des ventralen Kniegelenkes. Meshdeckung des Muskels. Immobilisation von Knie und Sprunggelenk bis zur Einheilung der Meshgraft fur 10 Tage. Schrittweise Steigerung der Bewegung mit 30°/60°/90° alle 2 Wochen. Belastung mit gestrecktem Knie, keine Belastung bei gebeugtem Knie fur 6 Wochen postoperativ. Nachuntersucht wurden insgesamt 9 Patienten nach orthopadisch notwendiger Wechselendoprothetik des Kniegelenkes (3 aseptisch, 6 septisch), von denen 3 eine vollstandige Ruptur der Patellasehne, 5 eine fortgeschrittene Destruktion des Streckapparates und 1 Patient eine vollstandiger Ruptur der Quadrizepssehne vorwiesen. Der mittlere Nachuntersuchungszeitraum betrug 11,2 Monate. Bereits nach 6 Monaten zeigte das nachuntersuchte Kollektiv ein gutes funktionelles Ergebnis mit aktiver Streckung des Kniegelenkes sowie eine Stand- und Gangstabilitat.

Due to the current COVID-19 pandemic, the German Health Ministry has issued restrictions applying to the field of orthopaedics and trauma surgery. Besides postponement of elective surgeries, outpatient consultations have been drastically reduced. Parallel to these developments, an increase in telemedical consultations has reflected efforts to provide sufficient patient care. This study aims to evaluate the feasibility of a clinical examination of the hip joint and pelvis by way of a telemedical consultation.Twenty-nine patients of a German university clinic were recruited and assessed in both telemedical and conventional examinations. Agreement between the two examinations was then assessed, and connections between the observed agreement and patient-specific factors such as age, BMI and ASA classification were investigated.The inspections agreed closely with a mean Cohen's kappa of 0.76 ± 0.37. Palpation showed adequate agreement with a mean Cohen's kappa of 0.38 ± 0.19. Function showed good agreement with a mean Cohen's kappa of 0.61 ± 0.26 and range of motion showed adequate agreement with a mean Cohen's kappa of 0.36 ± 0.19. A significant positive correlation was observed between the number of deviations in the different examinations and age (p = 0.05), and a significant positive correlation was shown between the number of non-feasible examinations and age (p 0.01), BMI (p 0.01) and ASA classification score (p 0.01).Inspection and function can be reliably evaluated, whereas the significance of palpation, provocation and measurement of range of motion is limited. The small sample size puts limitations on the significance of a statistically relevant correlation between patient-specific factors such as age, BMI and ASA classification score and valid and successful implementation of a telemedical examination. The authors recommend targeted patient selection. If, however, patients are being evaluated who are very old ( 75 years), obese (BMI 30) or with multiple comorbidities (ASA 3 and above), caution is advised. Large, prospective studies are needed in the future to fully validate telemedical consultations in the fields of orthopaedics and trauma surgery.A telemedical examination of the hip joint and pelvis can be performed with certain limitations. Patient-specific factors such as age, BMI, and extent of comorbidities appear to have a relevant impact on validity and execution of the examination. Patients with multiple comorbidities (ASA 3 and above), advanced age ( 75 years) or obesity (BMI 30) should, whenever possible, be examined in a conventional outpatient setting.Aufgrund der aktuellen COVID-19-Pandemie erfolgten Einschränkungen im Fach der Orthopädie und Unfallchirurgie durch das Gesundheitsamt. Neben der Verlegung von elektiven operativen Eingriffen erfolgte auch die drastische Reduktion der Sprechstunden. Hierdurch kam es zu einem deutlichen Anstieg von telemedizinisch durchgeführten Sprechstunden zur Gewährleistung der Patientenversorgung. Die Zielsetzung dieser Arbeit ist, die Umsetzbarkeit einer klinisch-orthopädischen Untersuchung der Hüfte und des Beckens durch telemedizinische Verfahren im Rahmen einer Videosprechstunde zu untersuchen.Im Rahmen der vorliegenden Auswertung wurden 29 stationäre Patienten eines deutschen Universitätsklinikums rekrutiert und im Rahmen einer Videosprechstunde und konventionell untersucht. Anschließend wurde die Übereinstimmung der erhobenen Befunde ausgewertet und Zusammenhänge mit patientenspezifischen Daten wie Alter, BMI und ASA-Klassifikation untersucht.Es zeigte sich eine gute Übereinstimmung der Inspektion mit einem durchschnittlichen Cohens Kappa von 0,76 ± 0,37, eine angemessene Übereinstimmung der Palpation mit einem durchschnittlichen Cohens Kappa von 0,38 ± 0,19, eine gute Übereinstimmung der Funktion mit einem durchschnittlichen Cohens Kappa von 0,61 ± 0,26 und eine angemessene Übereinstimmung der Bewegungsausmaße mit einem Cohens Kappa von 0,36 ± 0,19. Es zeigte sich ein signifikanter positiver Zusammenhang zwischen Anzahl der Abweichungen der verschiedenen Untersuchungen und Alter (p = 0,05) und ein positiver signifikanter Zusammenhang zwischen Anzahl der nicht durchführbaren Untersuchungen und Alter (p 0,01), BMI (p 0,01) und ASA-Klassifikationswert (p 0,01).Vor allem die Inspektion und Funktion sind gut untersuchbare Parameter während Palpation, Provokation und die Erfassung der Bewegungsausmaße eine limitierte Aussagekraft zeigen. Bei begrenzter Anzahl der Probanden ist die Aussagekraft eines statistisch relevanten Zusammenhangs zwischen patientenspezifischen Faktoren wie Alter, BMI und ASA-Klassifikationswert und der validen und erfolgreichen Durchführung einer telemedizinischen Untersuchung limitiert. Die Autoren sprechen sich für eine gezielte Patientenselektion aus. Sollten dennoch sehr alte ( 75 Jahren), adipöse (BMI 30) oder stark vorerkrankte Patienten (ab ASA 3) untersucht werden, ist Vorsicht geboten. Es bedarf in der Zukunft weiterer großangelegter, prospektiver Studien, um eine ausreichende Validierung einer telemedizinischen Untersuchung in der Orthopädie und Unfallchirurgie zu erreichen.Die telemedizinische Untersuchung von Hüftgelenk und Becken ist mit Einschränkungen möglich. Patientenspezifische Faktoren wie Alter, BMI und Grad der Vorerkrankungen scheinen einen relevanten Einfluss auf die Validität und die Durchführung zu haben. Verwendung von modularen achsgefuhrten Revisionsimplantaten zur Wiederherstellung einer stabilen Kniegelenkkinematik in der Revisionsendoprothetik. Revisionsimplantat bei Knochendefekten Grad I) oder multidirektionaler Instabilitat. Persistierender Gelenkinfekt, genereller Infekt (Pneumonie, Harnwegsinfekt [HWI]), fehlende/nicht ausreichende Knochenverankerungsmoglichkeit femoral und tibial (AORI >IIb), destruierter, nichtrekonstruierbarer Streckapparat. Standardzugang uber bestehende Hautnarbe, Arthrotomie, Entfernung Zementspacer, wenn notwendig, und Entnahme multipler Gewebeproben; Praparation Femur first. Beginn mit Reibahlen aufsteigender Durchmesser bis zum kortikalen Kontakt. Anschliesend wird ein Referenzschaft mit einer Gelenklinienplatte so verankert, dass die Gelenklinie an der gewunschten Position zu liegen kommt. Referenzierung der Rotation und Gelenklinienhohe mit dementsprechender Entscheidung zur Verwendung von Augmenten. Femorale Osteotomie nach Ausrichtung des Schnittblockes 5‑in‑1. Nach Entfernung der osteotomierten femoralen Flachen erfolgen die Boxpraparation und anschliesend die Probeimplantation. Tibiale Schaftpraparation analog der femoralen Praparation. Anschliesend erfolgt die Verankerung des Referenzschaftes mit der tibialen Gelenklinienplatte entsprechend der gewunschten Position. Montage der tibialen Sagefuhrung und Kontrolle der Resektate; ggf. mit der Notwendigkeit zur Verwendung von Augmentaten. Durchfuhrung der Osteotomie und Entfernen der Resektate. Finale Praparation der tibialen Flache und Einschlagen der Probeprothese. Durchbewegen des Gelenkes; bei klinisch und rontgenradiologisch regelhaftem Befund und Stellung Entfernung der Probeimplantate und Implantation der definitiven Prothese. Vollbelastung; regelmasige Wundkontrolle und steriler Wundverband; Einschrankung des Flexionsgrades nur bei geschwachtem Streckapparat. Im Zeitraum von 03/2011 bis 05/2018 wurden an den Kliniken der Autoren insgesamt 48 Patienten mit einem modularen Knierevisionssystem (Typ BPK‑S, Fa. Brehm, Weisendorf, Deutschland) versorgt. In 30 Fallen erfolgte die Implantation eine „Rotating-hinge“, in 18 Fallen eine „semiconstrained“ Variante. Das mittlere Follow-up lag bei 24 Monaten. Indikationen: konsolidierte Infektion nach Prothesenausbau im zweizeitigen Intervall (n = 22), aseptische Lockerung (n = 11), Instabilitat (n = 11), periprothetische Fraktur (n = 3), Zementallergie (n = 1); 80 % der Patienten zeigten gute klinische und radiologische Ergebnisse. Insgesamt wurden 11 Re-Revisionen durchgefuhrt (5 aseptische Ursachen [jeweils 1 Luxation, periprothetische Fraktur, symptomatische Retropatellararthrose, Schmerzen und Zementallergie], 6 periprothetische Infekte [davon 4 Re-Infekte]). Alle Patienten wurden mit einem zweizeitigen Komplettwechsel behandelt, wobei in 2 Fallen schlieslich eine Arthrodese durchgefuhrt werden musste. Die Uberlebensrate im 2‑Jahres-Follow-up betrug 77 %.

Verwendung von distalen Femurersatzimplantaten bei fortgeschrittenen Knochendefekten nach mehrfachen Revisionseingriffen am Kniegelenk. Hierdurch soll trotz zahlreicher, den Knochen schadigender Voroperationen durch Verwendung eines modularen distalen endoprothetischen Femurersatzes eine die Kniegelenkfunktion erhaltende Rekonstruktion ermoglicht werden. Fortgeschrittene femorale Knochendefekte AORI-IIb- und -III-Defekte bei der Revisionsendoprothetik des Kniegelenks. Akuter oder chronischer periprothetischer Gelenkinfekt, genereller Infekt, nicht rekonstruierbarer Streckapparat, unzureichende Muskeldeckung. Standardzugang unter Einbezug bestehende Hautnarbe, Arthrotomie, Entfernung Zementspacer, wenn notwendig, und Entnahme multipler Gewebeproben; Praparation Tibia zur Festlegung der Gelenklinie; anschliesend Praparation Femur. Festlegung der Resektionshohe des verbliebenen Femurs nach praoperativer Planung und Praparation mit Femurraspeln. In Abhangigkeit der zu verwendenden Schaftlange kann ein Standard MUTARS-Schaft (90, 120 und 160 mm) oder ein Revisionsschaft (RS) (150, 200 und 250 mm) benutzt werden. Die jeweilige benotigte femorale Rekonstruktionslange kann mittels zusatzlicher Verlangerungshulsen in verschiedenen Langen modular aufgebaut werden. Probekopplung und Reposition. Kontrolle der Implantatlage als auch des Patellatrackings und der Rotation der femoralen als auch tibialen Komponente. Vollbelastung bei zementierter Versorgung, Teilbelastung von maximal 10 kg fur 6 Wochen bei zementfreier Versorgung; regelmasige Wundkontrolle; Limitation des Flexionsgrades nur bei geschwachtem Streckapparat. Im Fall einer Reimplantation bei saniertem Gelenkinfekt nach zweizeitigem Vorgehen postoperative adaptierte antibiotische Therapie. Zwischen Februar 2015 und August 2018 wurden 34 distale Femora implantiert. Bei 19 Patienten erfolgte die Implantation nach Lockerung einer einliegender Knieprothesen. Alle Patienten wiesen intraoperativ einen AORI-III-Defekt des Femurs auf. Von den 19 Patienten, bei denen eine Implantation eines distalen Femurs durchgefuhrt wurde, erfolgte bei 7 Patienten eine erneute Revision aufgrund eines neuen Infektes; 4 dieser 7 Patienten wurden im Verlauf mehrfach revidiert und erhielten eine spatere Konvertierung in eine Arthrodese. Bei 1 Patienten ist aufgrund einer sekundaren aseptischen Lockerung des zementierten Schaftes eine Revision mit Schaftwechsel erfolgt. Der mittlere Nachuntersuchungszeitraum betrug 11,2 Monate. Metaphysare Defekte entsprechend AORI(Anderson Orthopaedic Research Institute)-II- (a und b) und -III-Defekten in der Revisionsendoprothetik des Kniegelenks. Persistierender oder aktueller Gelenkinfekt, genereller Infekt (Pneumonie, Harnwegsinfekt [HWI]), fehlende Metaphyse, insuffizienter Streckapparat. Standardzugang uber bestehende Hautnarbe, Arthrotomie, Entfernung Zementspacer, wenn notwendig, und Entnahme multipler Gewebeproben; Praparation Femur first; anschliesend Praparation Tibia. Referenzierung der Rotation und Gelenklinienhohe mit entsprechender Entscheidung zur Verwendung von Augmenten. Tibiale Osteotomie intramedullar referenziert. Bestimmung der metaphysaren Defektgrose und dementsprechend Wahl der Komponentengrose. Positionierung der Raspelfuhrung fur die metaphysare Komponente (Sleeve); stufenweise Praparation des Bettes fur die metaphysare Komponente. Belassen der Komponente in der gewunschten Tiefe (bei Verwendung von Augmenten +5 oder +10 mm dementsprechend 5 bzw. 10 mm oberhalb des Tibiaplateaus). Montage des Tibiaplateaus auf dem liegenden Sleeve und Reposition. Probekopplung, Kontrolle der Implantatlage und der Gelenklinienhohe rontgenradiologisch. Verwendung der definitiven Implantate. Vollbelastung; regelmasige Wundkontrolle; Limitation des Flexionsgrades nur bei Augmentation oder geschwachtem Streckapparat. Zwischen Mai 2018 und August 2019 wurden 14 metaphysare Komponenten bei 14 Patienten tibial implantiert. Der mittlere Nachuntersuchungszeitraum betrug 10,4 Monate. Die Nachuntersuchung umfasste die klinische Untersuchung, den KSS (Knee Society Score) sowie eine Rontgen- und Versagensanalyse. Eine signifikante Verbesserung der ROM („range of movement“) von 75 ± 16° auf 100 ± 14° (p 200 m) nach 6 Monaten; tibiale Schaftschmerzen wurden von allen Patienten verneint. Das nachuntersuchte Kollektiv zeigt im Beobachtungsintervall eine Implantatuberlebensrate von 100 %.

Numerous studies have shown that geriatric cooperation models in traumatology lead to significantly reduced morbidity and mortality rates. Even though national and international orthopedic and trauma societies recommend expanding these concepts to include nontrauma-related musculoskeletal diseases, unfortunately, to date only a few of these projects have been implemented. The present article deals with the necessary personnel and infrastructural implementation of a so-called orthogeriatric concept and puts the special features of this new type of treatment in direct comparison with known traumatological cotreatment concepts.Zahlreiche Studien konnten belegen, dass geriatrische Kooperationsmodelle in der Traumatologie zu einer signifikant reduzierten Morbiditäts- und Mortalitätsrate führen. Auch wenn nationale und internationale Fachgesellschaften seit Jahren eine Ausweitung dieser Konzepte auch auf andere, nichttraumabedingte muskuloskelettale Erkrankungen fordern, sind bisher wenige derartige Projekte umgesetzt worden. Der vorliegende Artikel beschäftigt sich mit der personellen, inhaltlichen und infrastrukturellen Umsetzung eines sog. orthogeriatrischen Konzeptes und stellt die Besonderheiten dieser neuartigen Therapieform in direktem Vergleich zu den bekannten traumatologischen Co-Behandlungskonzepten. Essenziell ist dabei die Möglichkeit, orthogeriatrische Patienten oft lange im Vorfeld einer Operation präoperativ vorbereiten zu können und so das postoperative Outcome zu verbessern. Die stationären als auch poststationären Konzepte halten sich eng an die Vorgaben der zertifizierten Traumazentren Alterstrauma-Zentren (ATZ) der Deutschen Gesellschaft für Unfallchirurgie (DGU) und können überwiegend in orthogeriatrische Behandlungskonzepte übernommen werden.

The aim of this study was to determine if a negative pressure wound therapy (NPWT) system can facilitate wound healing and withstand mechanical stress owing to the high range of motion of the knee joint in total knee arthroplasty. The benefits of NPWT include aiding wound granulation and a reduction in wound edema and secretion, our goal is to investigate its benefits in primary wound closure. Within an eight-day duration of treatment, we compared standard wound dressing (gauze and compression) with the disposable NPWT system (PICO)TM. A total of 79 patients were recruited in a controlled, randomized, prospective, single-institution clinical trial. A total of 40 patients were allocated to the study group and 39 to the control group. In terms of blood loss and infection parameters, there was no significant difference between both groups. In the NPWT group, we determined a significant reduction in wound edema but none in wound secretion. While the NPWT dressings survived the mechanical effects of movement during postoperative physiotherapy, conventional compressive dressings were often renewed. Even though the risk factors that could lead to delayed wound healing were reduced in the test group, we could not identify any significant difference pertaining to delayed wound healing that required surgical treatment.

The goal of this study is to evaluate the primary stability of a cementless augment-and-modular-cage system with and without the addition of cranial straps in a standardized in vitro setting. As the surrogate parameter for the evaluation of primary stability, the measurement of relative motion between the implant components themselves and the bone will be used. Acetabular revision components with a trabecular titanium augment in combination with a large fourth-generation composite left hemipelvis were assembled. These constructs were divided into two groups with (S) and without cranial straps (nS). A total of 1000 cycles was applied at each of three load levels. Relative movements (RM) between the components were measured. Load levels display a significant effect on the amount of RM at all interfaces except between shell/augment. The group assignment appears to have an effect on RM due to significantly differing means at all interfaces. Between bone/shell RM increased as load increased. NS displayed significantly more RM than S. Between shell/augment RM remained constant as load increased. Between shell/cup S showed more RM than nS while both groups’ RM increased with load. We conclude a significant increase of primary stability between the shell and the bone through the addition of cranial straps. Relative motion between components (shell/cup) increases through the addition of cranial straps. A clinical impact of this finding is uncertain and requires further investigation. Finally, the cementless fixation of the augment against the rim-portion of the shell appears stable and compares favorably to prior investigation of different fixation techniques.

This article offers a German perspective on orthopaedic management of geriatric populations in higher-income countries. Specific orthopaedic procedures necessitate a more intensive level of care in older adult patients, especially with co-morbidities, than in younger patients. During the pre-, peri- and post-operative periods, several aspects such as patient blood management principles improve the outcomes of geriatric patients. New interdisciplinary geriatric models of care including geriatric and orthopaedic specialties have to be established to take care of the special needs of geriatric patients. Introduction The complex field of femoral defects in revision hip arthroplasty displays a lack of standardized, intuitive pre- and intraoperative assessment. To address this issue, the femoral defect classification (FDC) is introduced to offer a reliable, reproducible and an intuitive classification system with a clear therapeutic guideline. Materials and methods The FDC is based on the integrity of the main femoral segments which determine function and structural support. It focuses on the femoral neck, the metaphysis consisting of the greater and lesser trochanter, and the femoral diaphysis. The four main categories determine the location of the defect while subcategories a, b and c are being used to classify the extent of damage in each location. In total, 218 preoperative radiographs were retrospectively graded according to FDC and compared to intraoperatively encountered bone defects. To account for inter-rater and intra-rater agreement, 5 different observers evaluated 80 randomized cases at different points in time. Results A Cohens kappa of 0.832 ± 0.028 could be evaluated, accounting for excellent agreement between preoperative radiographs and intraoperative findings. To account for inter-rater reliability, 80 patients have been evaluated by 5 different observers. Testing for inter-rater reliability, a Fleiss Kappa of 0.688 could be evaluated falling into the good agreement range. When testing for intra-rater reliability, Cohens Kappa of each of the 5 raters has been analyzed and the mean was evaluated at 0.856 accounting for excellent agreement. Conclusion The FDC is a reliable and reproducible classification system. It combines intuitive use and structured design and allows for consistent preoperative planning and intraoperative guidance. A therapeutic algorithm has been created according to current literature and expert opinion. Due to the combination of the FDC with the recently introduced Acetabular Defect Classification (ADC) a structured approach to the entire field of hip revision arthroplasty is now available.

The role and diagnostic value of the synovial complement system in patients with low-grade periprosthetic joint infection (PJI) are unclear. We sought to evaluate, for the first time, the usefulness of synovial complement factors in these patients by measuring the individual synovial fluid levels of complement factors (C1q, C3b/iC3b, C4b, C5, C5a, C9, factor B, factor D, factor H, factor I, properdin, and mannose-binding lectin [MBL]). The patients (n = 74) were classified into septic (n = 28) and aseptic (n = 46). Receiver-operator characteristic curves and a multiple regression model to determine the feasibility of a combination of the tested cytokines to determine the infection status were calculated. The synovial fluid levels of C1q, C3b/C3i, C4b, C5, C5a, MBL, and properdin were significantly elevated in the PJI group. The best sensitivity and specificity was found for C1q. The multiple regression models revealed that the combination of C1q, C3b/C3i, C4b, C5, C5a, and MBL was associated with the best sensitivity (83.3%) and specificity (79.2%) for a cutoff value of 0.62 (likelihood ratio: 4.0, area under the curve: 0.853). Nevertheless, only a combined model showed acceptable results. The expression patterns of the complement factors suggested that PJI activates all three pathways of the complement system.

Eine standardisierte und strukturierte Herangehensweise ist im Rahmen der körperlichen Untersuchung unerlässlich und stellt für den ärztlichen Behandler das wichtigste Diagnostikum für eine zielgerichtete Diagnosestellung und die Grundlage für einen richtigen Behandlungsansatz dar. Periprosthetic joint infection (PJI) is a feared complication after arthroplasty. Our hypothesis was that PJI caused by difficult-to-treat (DTT) pathogens has a worse outcome compared with non-DTT PJI. Routine clinical data on 77 consecutive patients with confirmed PJI treated with 2-stage exchange arthroplasty were placed in DTT and non-DTT PJI groups and analyzed. The main outcome variable was that the patient was definitively free of infection after 2 years. We found definitive infection resolution in 31 patients in the DTT group (68.9%) and 28 patients (87.5%) in the non-DTT group (P < 0.05). The necessity for revision surgery until assumed resolution of infection was significantly more frequent in the DTT group with 4.72 ± 3.03 operations versus 2.41 ± 3.02 operations in the non-DTT group (P < 0.05). PJI caused by DTT bacteria is associated with significantly higher numbers of revision operations and significantly inferior definitive infection resolution.

Extracellular vesicles (EVs) comprise an as yet insufficiently investigated intercellular communication pathway in the field of revision total joint arthroplasty (RTJA). This study examined whether periprosthetic joint synovial fluid contains EVs, developed a protocol for their isolation and characterized them with respect to quantity, size, surface markers as well as documented their differences between aseptic implant failure (AIF) and periprosthetic joint infection (PJI). EV isolation was accomplished using ultracentrifugation, electron microscopy (EM) and nanoparticle tracking analysis evaluated EV presence as well as particle size and quantity. EV surface markers were studied by a bead-based multiplex analysis. Using our protocol, EM confirmed the presence of EVs in periprosthetic joint synovial fluid. Higher EV particle concentrations and decreased particle sizes were apparent for PJI. Multiplex analysis confirmed EV-typical surface epitopes and revealed upregulated CD44 and HLA-DR/DP/DQ for AIF, as well as increased CD40 and CD105. Our protocol achieved isolation of EVs from periprosthetic joint synovial fluid, confirmed by EM and multiplex analysis. Characterization was documented with respect to size, concentration and epitope surface signature. Our results indicate various differences between PJI and AIF EVs. This pilot study enables new research approaches and rising diagnostic opportunities in the field of RTJA.

AIM To assess serum levels of RANK-ligand (RANKL) and osteoprotegerin (OPG) as biomarkers for periprosthetic joint infection (PJI) and compare their accuracy with standard tests. METHODS One hundred and twenty patients presenting with a painful total knee or hip arthroplasty with indication for surgical revision were included in this prospective clinical trial. Based on standard diagnostics (joint aspirate, microbiological, and histological samples) and Musculoskeletal Infection Society consensus classification, patients were categorized into PJI, aseptic loosening, and control groups. Implant loosening was assessed radiographically and intraoperatively. Preoperative serum samples were collected and analyzed for RANKL, OPG, calcium, phosphate, alkaline phosphatase (AP), and the bone-specific subform of AP (bAP). Statistical analysis was carried out, testing for significant differences between the three groups and between stable and loose implants. RESULTS All three groups were identical in regards to age, gender, and joint distribution. No statistically significant differences in the serum concentration of RANKL (P = 0.16) and OPG (P = 0.45) were found between aseptic loosening and PJI, with a trend towards lower RANKL concentrations and higher OPG concentrations in the PJI group. The RANKL/OPG ratio was significant for the comparison between PJI and non-PJI (P = 0.005). A ratio > 60 ruled out PJI in all cases (specificity: 100%, 95%CI: 89, 11% to 100.0%) but only 30% of non-PJI patients crossed this threshold. The positive predictive value remained poor at any cut-off. In the differentiation between stable and loose implants, none of the parameters measured (calcium, phosphate, AP, and bAP) showed a significant difference, and only AP and bAP measurements showed a tendency towards higher values in the loosened group (with P = 0.09 for AP and P = 0.19 for bAP). CONCLUSION Lower RANKL and higher OPG concentrations could be detected in PJI, without statistical significance.

The number of hip and knee arthroplasty replacement surgeries is increasing steadily. In combination with demographic aging and the number of periprosthetic complications, this development has lead to the phenomenon where the need for revision surgery is rising. The problem is, that, while implantation procedures of hip arthroplasties are more or less standardized, explantation is a non-standardized task for experienced specialists, due to the unpredictability of the adequate loosening method. The surgeon often only gets to decide on which tools and methods may be applied to detach the prosthesis, only after getting access to the operation site. The time taken to detach the prosthesis is hardly predictable and mainly depends on the surgeons’ skills. To gain objective data on the mechanics of explantation, new measurement methods are required. One technical base for studies on revision procedures are standard force plates, available in biomechanics laboratories.

Approximately one in three patients has untreated preoperative anaemia, which in turn is associated with an increased need for transfusion of allogenic red blood cell concentrates (RBC) and complications in the context of a surgical intervention. Here, the prevalence of preoperative and postoperative anaemia as well as their effects on transfusion rate, hospital length of stay and hospital mortality in primary hip and knee arthroplasty has been analysed.From January 2012 to September 2018, 378,069 adult inpatients from 13 German hospitals were analysed on the basis of an anonymized registry. Of these, n = 10,017 patients had a hip and knee joint primary arthroplasty. The primary endpoint was the incidence of preoperative anaemia, which was analysed by the first available preoperative haemoglobin value according to the WHO definition. Secondary endpoints included in-hospital length of stay, number of patients with red blood cell concentrate transfusion, incidence of hospital-acquired anaemia, number of deceased patients, and postoperative complications.The preoperative anaemia rate was 14.8% for elective knee joint arthroplasty, 22.9% for elective hip joint arthroplasty and 45.0% for duo-prosthesis implantation. Preoperative anaemia led to a significantly higher transfusion rate (knee: 8.3 vs. 1.8%; hip: 34.5 vs. 8.1%; duo-prosthesis: 42.3 vs. 17.4%), an increased red blood cell concentrate consumption (knee: 256 ± 107 vs. 29 ± 5 RBC/1000 patients; hip: 929 ± 60 vs. 190 ± 16 RBC/1000 patients; duo-prosthesis: 1411 ± 98 vs. 453 ± 42 RBC/1000 patients). Pre-operative anaemia was associated with prolonged hospital stay (12.0 [10.0; 17.0] d vs. 11.0 [9.0; 13.0] d; p 0.001) and increased mortality (5.5% [4.6 - 6.5%] vs. 0.9% [0.7 - 1.2%]; Fisher p 0.001) compared to non-anaemic patients. Vor diesem Hintergrund könnte sich in der Zukunft vor allem in der elektiven orthopädischen Chirurgie ein relevantes Potenzial ergeben, im Sinne von Patient Blood Management die elektiven Patienten besser vorzubereiten, um unnötige Transfusionen zu vermeiden und so die wertvolle Ressource Blut zu schonen.

Diagnosing a (low-grade) periprosthetic joint infection (PJI) after hip or knee arthroplasty remains a diagnostic challenge. The aim of this study was to evaluate the utility of using a novel multiplex protein microarray system for synovial biomarkers in determining PJI in patients undergoing revision knee or hip arthroplasty. The individual synovial fluid levels of 12 cytokines (IL-1b, IL-2, IL-4, IL-5, IL-6, IL-8, IL-10, IL-12, IL-17, GM-CSF, TNF-α, and INF-γ) were analysed with a novel multiplex protein microarray system in 32 patients undergoing revision hip (n = 22) or knee (n = 10) arthroplasty. Cases were classified into septic and aseptic groups on basis of pre- and interoperative findings: [PJI (n = 14) vs. non-PJI (n = 18)]. Receiver operator characteristic (ROC) curves were calculated to assess the discriminatory strength of the individual parameters. A multiple regression model was used to determine the utility of using a combination of the tested cytokines to determine the infection status. The levels of all of the evaluated cytokines were significantly elevated in the PJI-group. Best sensitivity and specificity were found for IL-6, followed by IL-1b, IL-10, and IL-17. The multiple regression models revealed a combination of IL-2, IL-4, IL-5, IL6, lL-12, and GM-CSF to be associated with the best sensitivity (100%) and specificity (88.9%) for a cut-off value of 0.41, with a likelihood ratio of 9.0. Analysis of individual synovial fluid cytokine levels showed both high sensitivity and high specificity in diagnosing PJI. A combined model using several cytokines showed even higher sensitivity and specificity in diagnosing PJI and could thus be a useful predictive tool to determine the probability of PJI in patients with a painful prosthesis. Periprosthetic joint infection is among the most common and severe complications in total joint arthroplasty. Today, a combination of different methods is used for diagnosis because no single method with sufficient sensitivity and specificity is available. In this study, we explored the usability of single-molecule microscopy to characterize synovial fluid samples from periprosthetic joint infections. Patients (n = 27) that needed revision arthroplasty underwent the routine diagnostic procedures for periprosthetic joint infection of the University Hospital in Bonn. Additionally, the diffusion rate of two probes, dextran and hyaluronan, was measured in small volumes of periprosthetic synovial fluid samples using single-molecule microscopy. To evaluate the suitability of single-molecule microscopy to detect PJI the AUC for both markers was calculated. The diffusion rate of hyaluronan in periprosthetic synovial fluid from patients with septic loosening was faster than in samples from patients with aseptic loosening. Single-molecule microscopy showed excellent diagnostic performance, with an area under the receiver operating characteristic curve of 0.93, and allowed the detection of periprosthetic joint infection in patients that would be challenging to diagnose with current methods. For the first time, single-molecule microscopy was used to detect periprosthetic joint infection. Our results are encouraging to study the value of single-molecule microscopy in a larger patient cohort. The speed and accuracy of single-molecule microscopy can be used to further characterize synovial fluid, potentially allowing intraoperative diagnosis of periprosthetic joint infections in the future.

Accurate diagnosis of periprosthetic joint infections (PJI) is one of the most widely researched areas in modern orthopedic endoprosthesis. However, our understanding of the immunological basis of this severe complication is still limited. In this study, we developed a flow cytometric approach to precisely characterize the immune cell composition in periprosthetic joints. Using high-dimensional multi-parametric data, we defined, for the first time, the local immune cell populations of artificial joints. We identified significant differences in the cellular distribution between infected and non-infected samples, and revealed that myeloid-derived suppressor cells (MDSCs) act as potential regulators of infiltrating immune cells in PJI. Further, we developed an algorithm to predict septic and aseptic samples with high sensitivity and specificity, that may serve as an indispensable addition to the current criteria of the Musculoskeletal Infection Society. This study describes a novel approach to flow cytometrically analyze the immune cell infiltrate of joint fluid that not only improves our understanding of the pathophysiology of PJI, but also enables the development of a novel screening tool to predict infection status. Indikationen: Hüftdysplasie im Erwachsenenalter. Fortgeschrittene symptomatische Coxarthrose. Hohe Hüftluxation gemäß CroweIII/IV. Symptomatische Beinlängendifferenz. Kontraindikationen: Zerebrospinale Dysfunktionen. Muskeldystrophien. Manifeste Störung des Knochenmetabolismus. Infektion. Immunsupprimierte Patienten. Operationstechnik: Seitenlage (variabel). Gerade laterale Inzision über dem Trochanter major und Eingehen zwischen dem Vorderrand des M.gluteus maximus und dem Hinterrand des M.gluteus medius, dem sog. Gibson-Intervall. Fixation der Pfanne je nach Halt und Knochenqualität mit 3-5Schrauben. Resektion des proximalen Femurs je nach Spannung der Weichteile bis auf Höhe oder sogar unter das Niveau des Trochanter minor. Anschließend Aufraspeln des Femurs und unter steter Spannungskontrolle des N.ischiadicus Reposition des Femurs in die Hüftpfanne; je nach Spannung des N.ischiadicus und der umgebenden Weichteile erfolgt eine Nachresektion des proximalen Femurs. Anfrischen des proximalen Femurs mit dem Meißel zur besseren Konsolidation des Trochanter-major-Fragments. Refixation des Trochanters mittels Cerclage, wobei die Muskulatur des M.gluteus medius an seinem Ursprung am Os ilium oder am muskulotendinösen Übergang am Trochanterfragment etwas mobilisiert werden muss, um den Trochanter major wieder distal am proximalen Femur refixieren zu können. Weiterbehandlung: Während der Hospitalisierung regelmäßige Behandlung auf der passiven Bewegungsschiene mit maximal 70° Flexion. Keine aktive Abduktion, keine passive Adduktion über die Mittellinie, kein Heben des gestreckten Beins, 10-15kg Teilbelastung an zwei Unterarmgehstöcken während 8Wochen. Je nach Compliance des Patienten und intraoperativer Stabilität der Prothese trägt der Patient eine Antiluxationsbandage (in Bern Hohmann-Bandage) für die ersten 6-8Wochen postoperativ. Anschließend erste klinische und radiologische Nachkontrolle und je nach Konsolidation des Trochanter major schrittweiser Übergang zur Vollbelastung. Thromboseprophylaxe bis zur Vollbelastung. Ergebnisse: Im beobachteten Zeitraum von 1998-2012 erfolgten insgesamt 28Hüftprothesenimplantationen bei hoher Hüftluxation im Erwachsenenalter im Stadium CroweIV, die mit einer Hakendachschale und einer femoralen Verkürzungsosteotomie behandelt wurden. Aktuell liegen von 14Patienten nach 8 ± 1,2Jahren (3,5-12Jahren) klinische Nachuntersuchungen vor. In recent years, the incidence of overweight and obesity has increased in the German population. Thus the number of obese patients treated with primary total joint arthroplasty has also increased. It is therefore predicted that the number of obese patients undergoing revision total joint arthroplasty will also increase. Nevertheless almost every manufacturer of commercially available revision arthroplasty implants states in his product safety guarantee that obesity is a relative or absolute contraindication. Data on revision total joint arthroplasty in obese patients are sparse. The aim of this systematic review is to assess the current literature on re-revision rate, infection rate, postoperative clinical outcome, and implant survival rate in obese patients undergoing revision total knee arthroplasty. Moreover, potential legal consequences and aspects are discussed which are essential for the surgeon.We conducted a systematic review of the online databases PubMed and identified clinical studies on obesity and overweight in revision total knee arthroplasty. Study quality was assessed using levels of evidence and the modified Jadad score. We also included descriptive data on case numbers, age, gender, height, weight and follow-up time. Current legal aspects were also analysed.Five studies met the inclusion criteria and were included in the systematic review. The average Jadad score was 1, the average level of evidence 3. In two studies, infection and revision occured more often in obese patients. Patients with morbid obesity had a higher risk of wound revision and periprosthetic joint infections. Three studies showed that obese patients had significantly lower scores for clinical outcome measures in function and pain. The legal aspect was not discussed in any of the five studies. Overall, published data are sparse and very heterogeneous. Therefore comparing these five studies is difficult. However, the results of our review suggest that obesity in revision total knee arthroplasty may have a negative influence on reoperation rate, infection rate and postoperative functional outcome. From a legal point of view, potential weight limitations of the implants intented for use have to be part of the oral preoperative discussion. Als Folge der hohen Anzahl von primärendoprothetischen Gelenkersätzen steigt aktuell auch die Zahl der revisionsendoprothetischen Eingriffe, sodass auch hier mit einer Zunahme der Anzahl übergewichtiger Patienten zu rechnen ist. Trotz dieser Tatsachen beschreiben viele Prothesenhersteller Adipositas als eine relative, teilweise sogar absolute Kontraindikation, und auch die Datenlage in der Revisionsendoprothetik bez. Adipositas und deren Auswirkungen ist gering. Ziel dieses systematischen Reviews ist es daher, Fragen bez. der Re-Revisionsoperation, der Infektionsrate, des postoperativen Outcomes, der Implantatüberlebensrate und der rechtlichen Konsequenzen bei adipösen Patienten in der Revisionsendoprothetik des Kniegelenks zu erörtern.Basierend auf einem systematischen Review der Datenbank PubMed identifizierten wir Studien, die sich mit der Thematik Adipositas und Übergewicht in der Wechselendoprothetik des Kniegelenks befassten. Die Bewertung der Studienqualität erfolgte anhand des Evidenzgrades sowie des modifizierten Jadad-Scores. Zusätzlich wurden die Studien nach Fallzahl, Alter, Größe, Gewicht, Geschlecht und Follow-up-Zeit analysiert. Zusätzlich erfolgte eine Analyse des aktuellen rechtlichen Aspekts.Insgesamt wurden aus n = 118 Suchergebnissen 5 Studien eingeschlossen. Der durchschnittliche Jadad-Score lag bei 1, das durchschnittliche Level of Evidence bei 3. Bezüglich der Infektionsrate und Revisionsoperationsrate zeigten 2 Studien deutlich schlechtere Werte für adipöse Patienten in der Wechselendoprothetik. Patienten mit einer morbiden Adipositas hatten in der Literatur ein deutlich erhöhtes Risiko für Wundheilungsstörungen und periprothetische Gelenkinfekte. Drei Studien gaben schlechtere Werte für adipöse Patienten an bez. des postoperativen Outcomes bei Funktion und Schmerz. Der rechtliche Aspekt wurde in keiner der 5 Studien genauer erörtert. Insgesamt ist die Datenlage bez. der Fragestellung gering und sehr heterogen, sodass die Gruppenbildung der 5 Studien nur bedingt vergleichbar ist. Insgesamt weisen jedoch die Ergebnisse des Reviews darauf hin, dass Adipositas auch in der Revisionsendoprothetik des Kniegelenks eine prognostisch ungünstige Einflussgröße ist. Rechtlich gesehen muss der Faktor Adipositas/Übergewicht in der Aufklärung der Wechselendoprothetik zwingend berücksichtigt werden, da sonst die Möglichkeit besteht, dass die Aufklärung nicht dem zu fordernden Fachstandard entsprechend erfolgt.

Background Traumatic knee dislocation represents a rare but devastating injury. Several controversies persist regarding type of treatment, surgical timing, graft selection, repair versus reconstruction of the medial and lateral structures, surgical techniques and postoperative rehabilitation. A new technique for primary ACL stabilization, dynamic intaligamentary stabilization (DIS) was developed at the authors’ institution. The purpose of this study was to analyze the clinical and radiological outcomes of surgically treated traumatic knee dislocations by means of the DIS technique for the ACL, primary suturing for PCL, MCL and LCL. Methods Between 2009 and 2012, 35 patients treated surgically for traumatic knee dislocation with primary anterior cruciate ligament (ACL) reconstruction with DIS, suturing of the posterior cruciate ligament (PCL) and primary complete repair of collaterals, were evaluated clinically (IKDC score, SF12 health survey, Lysholm score, Tegner score) and radiologically with a mean follow up of 2.2 years (range 1.00–3.50 years) years. Instrumented anterior–posterior translation was measured (KT-2000). Results Anterior/posterior translation (KT-2000) for the healthy and injured limb was 4.8 mm (range 3–8 mm) and 7.3 mm (range 5–10) (89 N) respectively. Valgus and varus stress testing in 30° flexion was normal in 26 (75%) and 29 (83%) patients, respectively. The IKDC score was B in 29 (83%) and C in 6 (17%) patients, while the mean Tegner score was 6 (range 4–8). The mean Lysholm score was 90.83 (range 81–95) and mean SF-12 physical and mental scores were 54.1 (range 45–60) and 51.0 (range 39–62) respectively. In 2 patients, a secondary operation was performed. Conclusions Early, one stage reconstruction with DIS can achieve good functional results and patient satisfaction with overall restoration of sports and working capacity without graft requirements.
