Abstract

PurposeThe Accreditation Council for Graduate Medical Education (ACGME) states that "residents should participate in scholarly activity." However, there is little guidance for effectively integrating scholarly activity into residency. This study was conducted to understand how pediatric residency programs meet ACGME requirements and to identify characteristics of successful programs.MethodThe authors conducted an online cross-sectional survey of all pediatric residency program directors in October 2012, assessing program characteristics, resident participation in scholarly activity, program infrastructure, barriers, and outcomes. Multivariate logistic regression was used to identify characteristics of programs in the top quartile for resident scholarly activity participation.ResultsThe response rate was 52.8% (105/199 programs). Seventy-seven (78.6%) programs required scholarly activity, although definitions were variable. When including only original research, systematic reviews or meta-analyses, and case reports or series with references, resident participation averaged 56% (range 0%-100%). Characteristics associated with high-participation programs included a scholarly activity requirement (odds ratio [OR] = 5.5, 95% confidence interval [CI] = 1.03-30.0); program director belief that all residents should present work regionally or nationally (OR = 4.7, 95% CI = 1.5-15.1); and mentorship by >25% of faculty (OR = 3.6, CI = 1.2-11.4). Only 47.1% (41) of program directors were satisfied with resident participation, and only 30.7% (27) were satisfied with the quality of research training provided.ConclusionsThe findings suggest that resident scholarly activity experience is highly variable and suboptimal. Identifying characteristics of successful programs can improve the resident research training experience.

13. Empatins risker : En kvalitativ intervjustudie om chefers uppfattning och förståelse av empatitrötthet inom socialt arbete.

Abstract

This study aims to examine the understanding of compassion fatigue, the burdens of social work, and its relevance within different social work organisations. Interviews were conducted with eight social work managers from various workplaces across Sweden. Participants volunteered to take part in the study. The interviewees had between 14 - 36 years of experience in the field of social work. This study used a qualitative study method and semi-structured interviews were conducted. A thematic analysis was used and three main themes were created: a varying understanding of compassion fatigue, the vulnerability of working with vulnerable people and workplace wellness. The results found that when asked to define compassion fatigue participants identified it as synonymous with secondary traumatic stress disorder and as a form of empathic numbness. They acknowledged that working with people in need can be difficult and it is important to have a healthy workplace to stay in the field of social work. The main findings are that the respondents' understanding of the phenomenon was not complete and that a lack of distance from their clients and an unawareness of oneself are risk factors for suffering from compassion fatigue. In summary, we concluded that more knowledge is needed about the phenomenon and increased opportunities for students to prepare for what impact the social work profession can have on their health and wellbeing., Denna studie syftar till att undersöka chefers upplevelse och förståelse av empatitrötthet samt arbetets påverkan på medarbetarna inom olika verksamheter för socialt arbete. Intervjuer genomfördes med åtta chefer för verksamheter inom socialt arbete på olika arbetsplatser över hela Sverige. Respondenternas medverkan i studien var frivillig och de hade mellan 14 - 36 års erfarenhet inom socialt arbete. Studien har utgått från en kvalitativ ansats genom användning av semistrukturerade intervjuer. Utifrån en tematisk analys skapades tre teman; varierande förståelse av empatitrötthet, utsattheten i att jobba med utsatta samt att må väl på arbetsplatsen. Studiens resultat visar att när respondenterna ombads definiera empatitrötthet beskrevs fenomenet nära sammanvävt med sekundär traumatisering och som en form av empatisk avdomnad. Respondenterna uppmärksammade den emotionella påverkan som klientnära arbete kan innebära och vikten av att ha en hälsosam arbetsplats för att medarbetaren ska klara av arbetet. De huvudsakliga fynd som studien redovisar är att respondenternas förståelse av fenomenet var ofullständig samt att avsaknad av distans till ens klienter och en omedvetenhet om sig själv är riskfaktorer för att drabbas av empatitrötthet. Sammanfattningsvis landade vi i slutsatserna att det behövs mer kunskap om fenomenet och ökad möjlighet för studenter att förbereda sig på hur yrket kan påverka en.

Abstract

Basal/squamous (Ba/Sq) subtype represents an intrinsic and robust group in the consensus molecular classification of muscle‐invasive bladder cancer (MIBC), with poor outcome and controversial chemosensitivity. We aimed to investigate the spectrum of intratumor heterogeneity (ITH) in the Ba/Sq subtype. First, we validated a 29‐gene NanoString CodeSet to predict the Ba/Sq subtype for FFPE samples. We identified heterogeneous Ba/Sq tumors in a series of 331 MIBC FFPE samples using dual GATA3/KRT5/6 immunohistochemistry (IHC). Heterogeneous regions with distinct immunostaining patterns were studied separately for gene expression using the 29‐gene CodeSet, for mutations by targeted next‐generation sequencing, and for copy number alteration (CNA) by microarray hybridization. Among 83 Ba/Sq tumors identified by GATA3/KRT5/6 dual staining, 19 tumors showed heterogeneity at the IHC level. In one third of the 19 cases, regions from the same tumor were classified in different distinct molecular subtypes. The mutational and CNA profiles confirmed the same clonal origin for IHC heterogeneous regions with possible subclonal evolution. Overall, two patterns of intratumoral heterogeneity (ITH) were observed in Ba/Sq tumors: low ITH (regions with distinct immunostaining, but common molecular subtype and shared CNA) or high ITH (regions with distinct immunostaining, molecular subtype, and CNA). These results showed multilayer heterogeneity in Ba/Sq MIBC. In view of personalized medicine, this heterogeneity adds complexity and should be taken into account for sampling procedures used for diagnosis and treatment choice. © 2021 The Pathological Society of Great Britain and Ireland. Abstract

International audience; This study examined regret following HIV serostatusdisclosure and associated factors in under-investigatedcontexts (Mali, Morocco, Democratic Republic ofthe Congo, Ecuador and Romania). A community-basedcross-sectional study was implemented by a mixed consortium[researchers/community-based organizations (CBO)].Trained CBO members interviewed 1,500 PLHIV in contactwith CBOs using a 125-item questionnaire. A weightedmultivariate logistic regression was performed. Among the1,212 participants included in the analysis, 290 (23.9 %)declared that disclosure was a mistake. Female gender,percentage of PLHIV’s network knowing about one’sseropositivity from a third party, having suffered rejectionafter disclosure, having suffered HIV-based discriminationat work, perceived seriousness of infection score, dailyloneliness, property index and self-esteem score wereindependently associated with regret. Discrimination, aswell as individual characteristics and skills may affect thedisclosure experience. Interventions aiming at improvingPLHIV skills and reducing their social isolation mayfacilitate the disclosure process and avoid negativeconsequences.

Characteristics associated with high-participation programs included a scholarly activity requirement (odds ratio [OR] = 5.5, 95% confidence interval [CI] = 1.03-30.0); program director belief that all residents should present work regionally or nationally (OR = 4.7, 95% CI = 1.5-15.1); and mentorship by >25% of faculty (OR = 3.6, CI = 1.2-11.4). Only 47.1% (41) of program directors were satisfied with resident participation, and only 30.7% (27) were satisfied with the quality of research training provided. CONCLUSIONS: The findings suggest that resident scholarly activity experience is highly variable and suboptimal. Identifying characteristics of successful programs can improve the resident research training experience.

Abstract

Despite the widespread dissemination of HIV information through public awareness campaigns in Mali, disclosing seropositivity to one's steady sexual partner (SSP) remains difficult for people living with HIV (PLHIV). Disclosure is a public health concern with serious implications and is also strongly linked to the quality of life of PLHIV. This study aimed to analyse factors associated with voluntary HIV disclosure to one's SSP, using a community-based cross-sectional study on 300 adult PLHIV in contact with a Malian community-based organization working in the field of AIDS response. A 125-item questionnaire was administered by trained personnel to study participants between May and October 2011. Analysis was restricted to the 219 participants who both reported having a SSP and answered to the question on disclosure to their SSP. A weighted multivariate logistic regression was used to determine variables independently associated with disclosure. In total, 161 participants (73%) reported HIV disclosure to their SSP. Having children (odds ratio [95% confidence interval]: 4.52 [1.84–11.12]), being accompanied to the survey site (3.66 [1.00–13.33]), knowing others who had publicly declared their seropositivity (3.12 [1.59–6.12]), having higher self-esteem (1.55 [1.09–2.19]) and using means other than anti-retroviral treatment to treat HIV (0.33 [0.11–1.00]) were independently associated with disclosure. This study identified several factors that should be considered for the design of interventions aimed at facilitating disclosure if/when desired in this cultural context. [ABSTRACT FROM PUBLISHER]
