Background: The extent to which Point-of-care of ultrasound (POCUS) is used in different European helicopter EMS (HEMS) is unknown. We aimed to study the availability, perception, and future aspects of POCUS in the European HEMS. Methods: A survey about the use of POCUS in HEMS was conducted by a multinational steering expert committee and was carried out from November 30, 2020 to December 30, 2020 via an online web portal. Invitations for participation were sent via email to the medical directors of the European HEMS organizations including two reminders. Results: During the study period, 69 participants from 25 countries and 41 different HEMS providers took part in the survey. 96% (n=66) completed the survey. POCUS was available in 75% of the responding HEMS organizations when needed (56% always, and 19% occasionally). 17% were planning to establish POCUS in the near future. Responders who provided POCUS used it in approximately 15% of the patients. Participants thought that POCUS is important in both trauma and non-trauma-patients (73%, n=46). The extended focused assessment sonography for trauma (eFAST) protocol (77%) was the most common used protocol. A POCUS credentialing process including documented examinations was requested in less than one third of the HEMS organizations.Conclusions: The majority of the HEMS organizations in Europe are able to provide different POCUS protocols in their services. The most used POCUS protocols were eFAST, FATE and RUSH. Despite the enthusiasm for POCUS, comprehensive training and clear credentialing processes are lacking in about two thirds of the European HEMS organizations.

Er liebte Intensivmedizin, aber die IMC hatte fur ihn wenig mit Intensivmedizin zu tun, sondern entsprach eher einem Aufwachraum der Anasthesie. „Wahrscheinlich wieder ein akutgeriatrischer Notfall“, dachte er, denn der typische IMC-Patient war 90 Jahre alt, hatte eine Schenkelhalsfraktur und war im Delir. In diesem Kapitel geht es um IMC, Postaggressionsstoffwechsel, Tokolyse, nichtinvasive Beatmung, ARDS, Ultraschall, dilatative Kardiomyopathie, postoperative Ernahrung, Lungenodem, die Starling-Gleichung sowie das Bauchgefuhl.

Akute Notfalle, die sich wahrend des stationaren Aufenthalts eines Patienten ereignen, sind meist sehr komplex und herausfordernd. Teammitglieder eines MET mussen in der Lage sein, die in dieser Situation notwendigen intensivmedizinischen Masnahmen suffizient anzuwenden. Bei der Erkennung und Behandlung kritischer Notfalle sind noch weitere Akteure in uberaus wichtiger Funktion. Eine Zustandsverschlechterung muss erkannt werden und oft sind bereits Masnahmen notwendig, bevor ein intensivmedizinisch geschultes MET eingetroffen ist. Diese Elemente greifen in einer „Chain of Survival“ ineinander. Innerklinische Notfalle zu verhindern, ist der beste Ansatz, das Ergebnis eines unvorhergesehenen Ereignisses zu verbessern. Deshalb sollte die „Chain of Survival“ um eine „Chain of Prevention“ erganzt werden. Das wichtigste Glied dieser Kette ist das Element „Education“.

Die diagnostische Anwendung von Ultraschall ist interdisziplinar ein Standardverfahren, zu dessen traditioneller Anwendung in Funktionseinheiten in den letzten Jahren ein neues Einsatzprofil hinzugekommen ist: das am kritisch kranken Patienten. Henryk Pich hat sich damit beschaftigt, wie man Medizinstudenten den problemfokussierten, bettseitigen Ultraschall bei kritisch Kranken und Notfallpatienten nachhaltig vermittelt kann. Das entstandene, E-Learning-basierte Ausbildungscurriculum ist eines von 2 Projekten, die auf dem Deutschen Anasthesiecongress (DAC) vergangenen Mai in Dusseldorf mit dem diesjahrigen „Thieme Teaching Award“ ausgezeichnet wurden.
