Background. Total knee arthroplasty is one of the most effective methods for the treatment of end-stage knee osteoarthritis, but 10% of patients still show insufficient function, strength, and mobility. Continuous nursing service plays an important role in the rehabilitation of patients undergoing total knee arthroplasty. For discharged and convalescent patients, the traditional follow-up model cannot solve the nursing problems of discharged patients. How to meet the health needs of discharged patients under the limited nursing resources has become an existing problem. Objective. To explore the effect of proprioception and balance training combined with continuous nursing on Berg balance scale (BBS) score and Hospital for Special Surgery (HSS) score of patients undergoing total knee arthroplasty (TKA) is the objective of this study. Methods. Sixty patients undergoing TKA in our hospital from December 2019 to April 2021 were enrolled. The patients were randomly assigned into the control group and the study group. The control group received continuous nursing, and the study group received proprioception and balance training combined with continuous nursing. Results. The nursing satisfaction of the study group was higher than that of the control group ( P < 0.05 ). The HSS scores at discharge, 1 month, 3 months, and 6 months after discharge in the study group were higher than those in the control group ( P < 0.05 ). It was higher in the study group than in the control group at 1 month, 3 months, and 6 months after discharge ( P < 0.05 ). The pain catastrophizing score of the study group at discharge was lower than that of the control group at 1 month, 3 months, and 6 months after discharge ( P < 0.05 ). The BBS scores at discharge, 1 month, 3 months, and 6 months after discharge in the study group were higher than those in the control group ( P < 0.05 ). The Lindmark balance scores at discharge, 1 month, 3 months, and 6 months after discharge in the study group were lower than those in the control group ( P < 0.05 ). Conclusion. Proprioception and balance training combined with continuous nursing can effectively promote the recovery of knee joint function of patients after TKA, enhance patients’ pain catastrophizing grade, enhance patients’ quality of life, effectively promote patients’ knee joint function and BBS score, and promote the improvement of disease.

The transfer of large-area and clean graphene to arbitrary substrates, especially to those with raised nanostructures, represents a great challenge. Polymer-based supporting layers generally lead to organic residues, while graphene transfer using alternative supporting materials like paraffin suffers from breaking and thus limits the transfer area. We demonstrated an improved poly(methyl methacrylate) (PMMA)/paraffin double layer, enabling the large-area transfer of graphene with high cleanliness and high coverage (81%) onto gold nanopyramid (AuNP)-structured substrates. The impact of supporting layers including single PMMA or paraffin and mixed PMMA/paraffin was clarified. The properties of graphene on AuNPs were theoretically and experimentally examined in detail. Raman spectra show a polarization-dependent D peak due to the folding of large-curvature graphene. The graphene on AuNPs shows a slightly tensile strain and provides extra surface-enhanced Raman scattering (SERS) with an enhancement factor of ∼20 times. These findings open a pathway to extend the applications of transferred graphene on raised nanostructures in many fields, such as SERS detection, catalysis, biosensors, light-emitting diodes, solar cells, and advanced transparent conductors.

Consequently, developing a CD73 inhibitor as monotherapy and a potential beneficial combination partner with immune-checkpoint inhibitors needs investigation.Methods: CD73 inhibitors were evaluated in vitro with soluble and membrane-bound CD73 enzymes, as well as its PD biomarker responses in human peripheral blood mononuclear cells (PBMC) by flow cytometry and ELISA. The binding modes of the molecules were analyzed via molecular modeling. The anti-tumor activity and synergistic effect of SHR170008 in combination with anti-PD-1 mAb were evaluated in a syngeneic mouse breast cancer model.Results: SHR170008 was discovered during the initial structural modifications on the link between the ribose and the α-phosphate of AMPCP, which significantly improved the stability of the compound confirmed by the metabolite identification study. Further modifications on the adenine base of AMPCP improved the potency due to forming stronger interactions with CD73 protein. It exhibited potent inhibitory activities on soluble and endogenous membrane-bound CD73 enzymes, and induced IFNγ production, reversed AMP-suppressed CD25+ and CD8+/CD25+ expression, and enhanced granzyme B production on CD8+ T cells in human PBMC. SHR170008 showed dose-dependent anti-tumor efficacy with suppression of adenosine in the tumors in EMT6 mouse breast tumor model. The increase of adenosine in tumor tissue by anti-PD-1 mAb alone was suppressed by SHR170008 in the combination groups. Simultaneous inhibition of CD73 and PD-1 neutralization synergistically enhanced antitumor immunity and biomarkers in response, and exposures of SHR170008 were correlated with the efficacy readouts.Conclusion: Our findings suggest that CD73 may serve as an immune checkpoint by generating adenosine, which suppresses the antitumor activity of anti-PD-1 mAb, and inhibition of CD73 may be a potential beneficial combination partner with immune-checkpoint inhibitors to improve their therapeutic outcomes in general.Keywords: small-molecule inhibitor of CD73, adenosine, anti-PD-1 mAb, checkpoint blockade, immunotherapy, combination therapy

To review cases of newly developed AHP and secondary esotropia (ET) after surgical correction for IXT.Medical records of cases with newly developed AHP and secondary ET after surgical correction for IXT were retrospectively reviewed.Fifteen cases were included and the mean pre-operative exodeviation was 31.7 ± 10.6 prism diopters (PD) (range: 16-50) at distance. They received bilateral lateral rectus (LR) recessions (13/15), unilateral LR recession and medial rectus (MR) resection (1/15) or bilateral LR recession plus unilateral MR resection (1/15). Fourteen patients developed chin-down AHP with secondary V-pattern ET and 1 presented chin-up AHP with secondary A-pattern ET. The mean esodeviation in primary gaze was 22.7 ± 10.0 PD (range: 3-40) at distance. Twelve cases underwent a second surgery for correction of AHP and secondary ET, and approximately half (5/11) of the recessed LR muscles explored showed a 3-5 mm downward displacement of the new attachment. Surgical management mainly included advancement of LR to the original insertion with a 3-5 mm upward transposition. The AHP dissipated and ocular alignment returned to normal in all of these cases. Three cases did not require further surgery, and their AHP and secondary ET were no longer present at 2-5 months after the first surgery.A downward shift of the recessed LR in IXT may lead to secondary V-pattern ET and development of a chin-down AHP. Advancement of LR with an upward transposition serves as an effective procedure for the correction of this complication.

Piston compressors are necessary dynamic equipment in chemical engineering and petrochemical industry. Establishment of a quality evaluation system for Chinese manufacturing enterprises of large piston compressors is important for the proper evaluation of production quality. The evaluation system can serve both government decision-making and user selection-making. Furthermore, the quality evaluation system is beneficial for identifying production bottleneck and improving production quality. A hierarchical analytic process is adopted in this paper to establish a comprehensive quality evaluation system. The system characteristics can be summarized into three levels, two main lines, and one correction. The first level consists of two main routes, external effects and intrinsic effects. The second level includes 6 external factors and 7 intrinsic factors. The third level is composed of 81 elements. In addition, the product accident history is introduced at the first level as a correction item. The evaluation system was implemented using B/S architecture and the standard SSM framework with MVC design pattern.

In a former paper by the authors (Guangfan Gao et al., 2017), an evaluation system for Chinese manufacturing enterprises of large piston compressors was established for the proper evaluation of production quality. In addition to the first three levels, the fourth level indices are needed, and therefore introduced in this paper. There are 35 and 47 items for the two branches of “Man” and “Soft power” respectively. In addition to the four levels, potential faults and accidents are also an integral part of compressor quality. Piston rod, for example, has its complexity of fault factors illustrated. We put forward two ways for malfunction factors to participate in evaluation. One is weight allocation of each element in all levels based on the multi-factor malfunction correlation matrix. The other is a general first-level correction of malfunctions based on product accident statistics.

To evaluate the feasibility and safety of a revised technique of botulinum toxin type A (BTA) injections for the treatment of infantile esotropia.Forty-seven patients with infantile esotropia were randomly divided into two groups. In group A, 23 cases were treated with a bilateral injection of 2.5-3.75 U BTA combined with sodium hyaluronate (SH) to the medial rectus muscle. In group B, 24 cases were treated with a bilateral injection of 2.5-3.75 U BTA solution alone to the medial rectus muscle. Electromyography was not used in the study. All patients received one injection and were evaluated 2 weeks, 3 months, and 6 months following injection.The measured changes between groups A and B included the frequencies of good alignment 6 months after injections (30.4% vs 37.5%), complicated ptosis (2.2% vs 20.8%), and vertical deviation (2.2% vs 2.1%).BTA injections combined with or without SH in the absence of electromyography demonstrated effectiveness and feasibility in the treatment of infantile esotropia. A relative decrease in the frequency of complicated ptosis resulted from injections of BTA+SH.

Aim To investigate the prevalence of hyperuricemia and the association between the serum uric aci (SUA) levels and incidence of metabolic syndrome (MetS) in the Mongolian area of China. Methods This cross-sectional survey was based on a population of 1,426 subjects (809 men and 617 women) 20-80 years of age who were recruited from Inner Mongolia, China. Metabolic and anthropometric indicators were measured according to standard methods. Hyperuricemia was defined as an SUA level of ≥7.0 mg/dL for men and ≥6.0 mg/dL for women. MetS was diagnosed based on the consensus criteria released in 2009 from a joint collaboration organization. Results The prevalence of hyperuricemia was 17.7% in men and 5.2% in women. The prevalence of MetS in men was higher than that observed in women(36.7% vs. 17.8%). Waist circumference, BMI and the level of triglycerides were most strongly correlated with the SUA level in both sexes. Men with hyperuricemia had an increased risk of MetS [OR(95%CI)=2.95(2.00-4.35)], while men with a "normal" SUA level(＞5.0 mg/dL and ＜6.3 mg/dL) had a higher risk of MetS, central obesity and hypertriglyceridemia than men in the lower level group (≤5 mg/dL). Women with a higher SUA level(≥4.3 mg/dL) had an increased risk of MetS, central obesity, hypertriglyceridemia and hypertension compared with women in the lowest tertile SUA group (≤3.5 mg/dL). Conclusions The SUA level is significantly associated with various metabolic indicators. In this study, waist circumference and the level of triglycerides were most strongly correlated with the SUA level in both sexes. Individuals with a normal level of SUA had an increased risk of MetS and other metabolic disorders. Further research on appropriate cut-off values for pre-hyperuricemia is expected, and the early detection of hyperuricemia is essential for the prevention of MetS.

Ophthalmic complications associated with endoscopic sinus surgery (ESS) are quite rare. There is a paucity of reliable data and limited experience on the clinical findings and treatments of these injuries. Our study here is to characterize the types of orbital injury following ESS, in particular extraocular muscle injury, and to evaluate the long-term therapeutic outcomes as compiled from a relatively large sample of Chinese patients. A series of 27 patients (21 males and 6 females; mean age = 42.6 years, ranges: 10–60 years) were retrospectively reviewed. The mean duration of orbital complication was 6.6 months (ranges: 1 day to 24 months). The right eye was affected in 19 patients and the left in 8 patients. All patients had various extraocular muscle dysfunction, including contusion, oculomotor nerve damage, muscle entrapment, muscle transection, and muscle destruction. All patients subjected to strabismus surgery showed an obvious reduction in deviation. Three patients achieved orthophoria without any surgery during the period of observation. All patients displayed mild to complicated orbital hemorrhage that often disappeared within 2 weeks. Optic nerve injury occurred in 29.6% of patients and vision damage in these patients was often irreversible. All patients with ophthalmic complications after ESS had strabismus and extraocular muscle dysfunction. Timing and type of strabismus surgery performed depended on the severity and number of muscles involved as well as the type of injury. This surgery is less effective in cases of restriction factor adhesion and/or entrapment as compared to that of patients with other types of strabismus. Orbital hemorrhages were usually resolved spontaneously, but optic nerve injury was mostly irreversible.

To determine the distribution of the toxin among individual muscles after intramuscular injection of botulinum toxin gel.One eye of 7 New Zealand white rabbits was randomized into group A, and the contralateral eye was into group B. Eyes in group A received medial rectus intramuscular injection of 2.5 IU of 125I-BTX-A gel in 0.1 ml, and those in group B received equivalent amount of 125I-BTX-A solution by medial rectus intramuscular injection. Four rectus muscles and the levator palpebrae superioris were harvested and the radioactivity of muscles was measured 16 hours after the injection.In group A, the radioactivity of per gram medial rectus was significantly higher than that of other muscles (P0.01), and there was no statistically significant difference in the radioactivity of per gram muscles among other muscles (P0.05). In group B, the radioactivity of per gram medial rectus and levator palpebrae superioris was significantly higher than that of other muscles respectively(P0.05), and the difference in the radioactivity of per gram muscles between medial rectus and levator palpebrae superioris was not statistically significant (P0.05). The radioactivity of per gram medial rectus in group A was higher than that in group B (t=3.731, P-0.01), and there was no significant difference in the radioactivity of per gram muscles among other homonymous muscles (P0.05).The toxin dispersed principally in the injected muscle and the local concentration of the toxin was much high following intramuscular injection of botulinum toxin gel.

To investigate the ocular torsion of V patterns.Twenty-two in hospital V patterns patients underwent bilateral inferior oblique muscles myectomy for inferior oblique overacting and hortizon recti underwent recess-resection procedure to correct hortizon devation. Objective cycloposition was assessed by fundus photograph before and 7 days after surgery in all cases. The photographs was stored in computer, and the disc-fovea angle was measured by means of drawing picture software. As well as the deviation, oblique muscle function was recorded before and after surgery.Seventeen cases were bilateral ocular excyclotropia, 4 cases were monocular excyclotropia and one case was annual. The disc-fovea angle of right eyes and left eyes before operation is 15.67 degrees +/- 5.72 degrees and 16.59 degrees +/- 6.69 degrees. The comparisons among V patterns and normal average showed significant differences (P0.001). The disc-fovea angle of right eyes and left eyes after operation is 5.24 degrees +/- 5.87 degrees and 7.17 degrees +/- 5.40 degrees. There is no statistic difference between normal average (P0.5). The deviation between up- and down 25 degrees is 23.1 +/- 6.8 before surgery, as well as the one is 2.0 +/- 2.25 one week postoperatively. All cases have bilateral inferior oblique overaction, and diminished but one case still has unilateral moderate overaction 7 days after surgery.There is objective cyclotropia in case of V patterns, and the objective torsion is normal after weakening overacted inferior oblique muscles.

To observe and compare the effect of 0.005% latanoprost and 0.5% timolol solutions in the treatment of post-iridectomy residual angle-closure glaucoma.Laser peripheral iridectomy was performed and 68 cases (68 eyes) of post-iridectomy residual angle-closure glaucoma were selected and treated with 0.005% latanoprost once at night as the study group and 0.5% timolol twice a day as the control group Intraocular pressure (IOP) was measured before treatment, on day 3, week 1, week 2, week 4, month 2, month 3 and month 6 in the follow-up, and visual acuity, visual field and C/D ratio were monitored.Of 68 patients, 56 (82.4%) complete the 6 month follow-up. The basic IOP for all subjects in two groups before treatment was not statistically different (t = 0.236, P = 0.814). After the treatment for three days, the efficiency in IOP reduction was statistically different between 2 groups with repeated measure ANOVA (F = 287.441, P0.001). In each group, the IOP after treatment was statistically different from that at baseline, with t = 14.103, P0.001 and t = 10.308, P0.001 in the latanoprost and timolol groups respectively. For those patients who finished 6 months' follow-up, the IOP difference between the 2 groups was statistically significant with repeated measure ANOVA (F = 74.259, P0.001). The IOP was statistically reduced in both the latanoprost and the timolol groups with F = 148.787, P0.001 and F = 54.875, P0.001 respectively. At each follow-up time, the IOP in latanoprost group was statistically different from that of the timolol group with t values to be -6.127, -5.930, -6.201, -5.931, -7.412, -6.640 and -6.756 (P0.001). The IOP was statistically different from that before treatment in each group with a mean difference to be -7.034 approximately -6.897 in the latanoprost group and -4.704 approximately -5.000 in the timolol group (P0.001). The IOP was not statistically different within each group in the follow-up with mean difference of 0.034 approximately 0.138 (P = 0.904 approximately 0.629) in the latanoprost group and a mean difference of 0.037 approximately 0.296 (P = 0.910 approximately 0.366) in the timolol group. The best-corrected visual acuity, visual field and C/D ratio were not significantly changed during follow-up.0.005% latanoprost and 0.5% timolol can be used in the drug therapy of post-iridectomy angle-closure glaucoma and the efficiency of 0.005% latanoprost is better than of 0.5% timolol. Drug therapy of post-iridectomy angle-closure glaucoma is a practical strategy.

To review the major progress in primary angle closure glaucoma (PACG).Contents of this article were selected from the original papers or reviews related to primary angle closure glaucoma published in Chinese and foreign journals. A total of 76 articles were selected from several hundred original articles or reviews. The content of selected articles is in accordance with our purpose and the authors are authorized scientists in the study of glaucoma.Primary angle closure glaucoma is the most common type of glaucoma in the Sino-Mongoloid population. PACG in Chinese can be classified into three types depending on the mechanism of angle closure: 1. Multimechanism: 54.8% of Chinese PACG is caused by co-existing factors. The pattern of angle closure appears to mainly be creeping closure. After iridectomy, almost 40% of the cases still manifest a positive response to the darkroom provocative test and progressive synechial closure or recurrent angle closure may occur. Several mechanisms are involved in this form of PACG such as pupillary blocking component, iris crowding component and anterior positioned ciliary body. These factors can coexist in the follow patterns: pupillary blocking and iris crowding coexist; pupillary blocking and anterior positioned ciliary body coexist or three of them co-exist. 2. Pupillary block: (38.1% of Chinese PACG) is caused by iris bombe due to pupillary block with acute or subacute attack. It responds well to iridectomy or laser iridotomy. 3. Non-pupillary blocking: (7.8% of Chinese PACG). They usually have a deeper anterior chamber, and tend to be younger (below 40 years of age). Angle closure in this form of PACG is caused by: iris crowding mechanism or/and anteriorly positioned ciliary body against iris root to angle. It is critical to distinguish multi-mechanism PACG from other types. The initial treatment for this type of PACG is also iridectomy, but after the pupillary block component is eliminated by iridectomy, the residual non-pupillary blocking components should be highlighted by a diagnostic treatment procedure or by a ultrasound biomicroscopy (UBM) provocative test. Finally, the role of UBM in the observation and evaluation of the mechanism of angle closure is discussed and future research directions on PACG in Asians are proposed.Chinese eyes have been recognized to be prone to the development of creeping angle closure. There is some direct evidence that creeping angle closure is caused by multiple mechanisms. Further study on this topic is needed.

To investigate in vivo survival of retinal ganglion cells (RGCs) after partial blockage of optic nerve (ON) axoplasmic flow by sub-retinal space or vitreous cavity injection of brain-derived neural factor (BDNF) produced by genetically modified neural progenitor cells (NPCs).Adult Sprague-Dawley (SD) rat RGCs were labeled with granular blue (GB) applied to their main targets in the brain. Seven days later, the left ON was intra-obitally crushed with a 40 g power forceps to partially block ON axoplasmic flow. Animals were randomized to three groups. The left eye of each rat received a sham injection, NPCs injection or an injection of genetically modified neural progenitors producing BDNF (BDNF-NPCs). Seven, 15 and 30 days after ON crush, retinas were examined under a fluorescence microscope. By calculating and comparing the average RGCs densities and RGC apoptosis density, RGC survival was estimated and the neuro-protective effect of transplanted cells was evaluated.Seven, 15 and 30 days after crush, in the intra-vitreous injection group, mean RGC densities had decreased to 1885 +/- 68, 1562 +/- 20, 1380 +/- 7 and 1837 +/- 46, 1561 +/- 58, 1370 +/- 16, respectively with sham injection or neural progenitors injection. However, RGCs density in the groups treated with intra-vitreous injection of BDNF-NPC was 2101 +/- 15, 1809 +/- 19 and 1625 +/- 34. Similar results were found in groups after sub-retinal injection. Higher densities were observed in groups treated with BDNF-NPCs. There were statistically significant differences among groups through nonparametric tests followed by the Mann-Whitely test. RGC apoptosis density in BDNF-NPC at each follow-up time was less than in other groups.A continuous supply of neurotrophic factors by the injection of genetically modified neural progenitors presents a highly effective approach to counteract optic neuropathy and RGC degeneration after partial ON axoplasmic flow blockage.

To evaluate the safety, effectiveness and complications of non-penetrating trabecular surgery (NPTS). METHODS A total of 168 patients ( 258 eyes) of primary open angle glaucoma were divided into 2 groups randomly, and they were matched according to the age, sex and severity of disease. NPTS with reticulated hyaluronic acid implant was taken in one group, and traditional trabeculectomy was taken in the other. Pre-a2operative mean intraocular pressure (IOP) in the NPTS group and trabeculectomy group was (31.85 +/- 4.83) and (32.59 +/- 4.62) mm Hg (1a(c)mm Hg = 0.133 kPa), respectively. The post-operative IOP and complications in these 2 groups were compared.(1) IOP: The mean IOP on post-operative 7th, 14th day, 1st, 3rd and 6th month was (6.67 +/- 2.43), (11.42 +/- 2.89), (12.59 +/- 2.24), (15.45 +/- 1.82), and (17.99 +/- 1.80) mm Hg respectively in the NPTS group, while that in the trabeculectomy group for the same time a(c)series was (4.87 +/- 1.65), (10.48 +/- 2.38), (12.01 +/- 2.83), (15.01 +/- 2.66), an.d (17.48 +/- 2.97) mm Hg respectively. Statistically significant differences between these 2 groups were found on the post-operative 7th day and 14th day (t = 7.03, 2.89; P0.000 1, P = 0.004). However, significant difference was not noted for pre-operative and other post-operative IOP in the follow-up (t = 1.28, 1.78, 1.55, 1.60; P = 0.202, 0.077, 0.124, 0.112). (2) Visual acuity: There were no statistical differences of visual acuity between the 2 groups (kappa(2) = 0.135, 0.151, 0.024, 0.076; P = 0.935, 0.927, 0.988, 0.963). (3) COMPLICATIONS: I and II degree shallow anterior chamber was encountered in 57 eyes in the NPTS group, while that in the trabeculectomy group was 70. Twelve eyes with III degree shallow anterior chamber were found exclusively in the trabeculectomy group. Ciliary body or choroidal detachment was found in 66 and 70 eyes respectively in the NPTS and trabeculectomy group. Hypotony was encountered in 5 eyes exclusively in the trabeculectomy group. COMPLICATIONS were found to be statistically different in 2 groups (kappa(2) = 9.95, 15.27, 4.60, 6.19; P0.05). Hyphema was found in 2 and 6 eyes respectively in the NPTS and trabeculectomy group. Light anterior uveitis was found in 5 and 9 eyes respectively in the NPTS and trabeculectomy group. Vitreous extrusion was experienced in 1 eye of trabeculectomy group. Some complications (16.2%) related to operative skill were also experienced in the NPTS group.Over-filtration and surgical injury related complications are significantly reduced in the NPTS group compared to those in the trabeculectomy group. NPTS is safer than traditional trabeculectomy. Mastering surgical skills is required for reducing complications of NPTS.

To investigate the damage of the cells of the lateral geniculate bodies (LGB) of Sprague-Dawley (SD) rats after partial optic nerve injury.The optic nerve was pressed by a forceps (40 g) for 30 seconds to set up the animal model with optic nerve injury. LGB was chopped in serial coronal slices (10 microm thickness) by a cryomicrotome. The apoptosis of LGB cells was detected with terminal deoxynucleotidyl transferase (TDT)-mediated dUTP nick end-labeling (TUNEL) technique after one week. After retrograde labeling with Granular Blue (GB) through injecting GB into optic center, the average density of the LGB cells was counted under the fluorescent microscope. Using monoclonal rabbit anti-rat neurofilament (NF) antibody, the neurons of LGB were detected by immunochemistry stains, and the standard avidin-biotin complex immunoperoxidase technique was used for immunostaining.The results of TUNEL analysis showed that the mechanism of the LGB cells pathogenesis was apoptosis. The LGB cells appeared clear striation in the control group, and the cells in experimental group decreased obviously after immunohistrochemical staining. The average density of LGB cells was (5 172 +/- 248) cells/mm(2) in control group, and (4 144 +/- 61) cells/mm(2) in experimental group.After partial optic nerve injury, the LGB cells are obviously damaged, which may be related to apoptosis of LGB cells.
