- Abstract
Background: A global epidemic of allergic contact dermatitis to (meth)acrylates has been described in relation to the widespread use of manicure products. Objectives: To evaluate the frequency of sensitization to 2‐hydroxyethyl methacrylate (2‐HEMA) among consecutively patch tested patients with eczema in Spain; the percentage of current relevance; the MOAHLFA index; and, the potential sources of exposure to (meth)acrylates. Methods: From January 2019 to December 2022, 2‐HEMA 2% pet. was prospectively patch tested in 24 REIDAC (Spanish Allergic Contact Dermatitis Registry) centres. Results: Six thousand one hundred thirty‐four patients were consecutively patch tested with 2‐HEMA 2% pet. 265/6134 (4.3%) were positive. Positive reactions of current relevance were identified to involve 184/265 (69%). The efficiency (number of patch tests needed to detect relevant positive patch test reactions) was 34 (6134/184). The variable 'occupational' was found to be significantly associated with a higher risk for relevant positive reactions to 2‐HEMA (OR: 10.9; 95% CI: 8.1–14.9). Conclusion: (Meth)acrylate sensitization is a prevalent health issue in Spain. 2‐HEMA 2% pet. has been identified to be a highly effective (meth)acrylate allergy marker in the GEIDAC baseline series. The responsible authorities should implement policies guaranteeing accurate labelling of industrial, medical, and consumer materials while ensuring the enforcement of said labelling through appropriate legal means. - Abstract
[EN]Background: Standard patch test series must be updated using objective data on allergen sensitization. The Spanish standard series was last updated in 2016 and the European series in 2019, and the inclusion of several emerging allergens needs to be evaluated. Material and methods: We conducted a prospective, observational, multicenter study of consecutive patients from the registry of the Spanish Contact Dermatitis and Skin Allergy Research Group (GEIDAC) who were patch tested in 2019 and 2020 with linalool hydroperoxide, limonene hydroperoxide, 2-hydroxyethyl-methacrylate, benzisothiazotinone, octylisothiazolinone, textile dye mix (TDM), sodium metabisulfite, propolis, bronopol, Compositae mix II, diazolidinyl urea, imidazolidinyl urea, decyl glucoside, and lauryl glucoside. Results: We analyzed data for 4654 patients tested with diazolidinyl urea, imidazolidinyl urea, and bronopol, and 1890 tested with the other allergens. The values for the MOAHLFA index components were 30% for mate, 18% for occupational dermatitis, 15% for atopic dermatitis, 29% for hand, 6.5% for leg, 23% for face, and 68% for age > 40 years. Sensitization rates above 1% were observed for 7 allergens: linalool hydroperoxide, 2-hydroxyethyl-methacrylate, benzisothiazolinone, limonene hydroperoxide, TDM, sodium metabisulfite, and propolis. Three allergens had a current relevance rate of over 1%: linalool hydroperoxide, 2-hydroxyethyl-methacrylat, and limonene hydroperoxide. Benzisothiazotinone and TDM had a relevance rate of between 0.9% and 1%. Conclusions: Our results indicate that 7 new allergens should be considered when extending the Spanish standard patch test series. The data from our series could be helpful for guiding the next extension of the European baseline series

Una correcta derivación a un especialista y la valoración de una baja laboral en el momento adecuado resultan cruciales para un buen manejo de estos pacientes. En esta guía sobre el eccema crónico de manos analizamos el proceso diagnóstico, las medidas preventivas y los tratamientos, con especial énfasis en el papel del médico de atención primaria y de medicina del trabajo en los estados iniciales de su manejo. Hand eczema is a common condition associated with significantly impaired quality of life and high social and occupational costs. Managing hand eczema is particularly challenging for primary care and occupational health physicians as the condition has varying causes and both disease progression and response to treatment are difficult to predict. Early diagnosis and appropriate protective measures are essential to prevent progression to chronic eczema, which is much more difficult to treat. Appropriate referral to a specialist and opportune evaluation of the need for sick leave are crucial to the good management of these patients. These guidelines cover the diagnosis, prevention, and treatment of chronic hand eczema and highlight the role that primary care and occupational health physicians can play in the early management of this disease.

Background: Methyldibromo glutaronitrile (MDBGN) was one of the most frequent and relevant allergens found in patch testing at the beginning of this century. In 2008, this preservative was banned from cosmetics in Europe and ever since the prevalence of contact allergy to MDBGN has progressively decreased. Despite that gradual decline, MDBGN is still patch‐tested in most baseline series. This study assessed the frequency of MDBGN sensitization, epidemiological characteristics of allergic patients, and the relevance of positive patch tests in a nationwide Spanish registry (REIDAC). Patients and methods: We evaluated consecutively patch‐tested patients in all participating centres. Using these data, we calculated the proportion of patients with positive patch tests to MDBGN from June 2018 to June 2020 and evaluated the relevance of the positive patch tests. Results: One hundred and fourteen out of 5072 (2.24 %) tested patients were sensitized to MDBGN. Clinical current relevance was confirmed in only one case. Conclusion: Although the frequency of contact allergy to MDBGN remains high, no clinical significance was found in most of these patients (5072 tests needed to obtain one relevant positive result). El Kathon CG constituye el nombre comercial de una mezcla de isotiazolinas: Metilcloroisotiazolinona y Metilisotiazolinona. Es un conservante muy utilizado, sobre todo en la industria cosmética, aunque en los últimos años también se ha impuesto su presencia en productos de limpieza de uso doméstico y actualmente a concentraciones más altas en preparados de uso industrial como aceites de corte, emulsiones de látex, pinturas al temple, aceites para motores Diesel, etc. Por su alto poder sensibilizante y amplia utilización, constituye actualmente una de las causas más frecuentes de alergia de contacto por preservativos, tanto en nuestra vida privada, como en el ámbito profesional. Objetivos: Estudiar la capacidad sensibilizante del Kathon CG y su relación con el desarrollo de eczema de contacto alérgico de origen profesional. Métodos: Estudio observacional descriptivo, a través de la revisión de las historias clínicas de los pacientes vistos en el Servicio de Dermatología Laboral del Instituto Nacional de Medicina y Seguridad en el Trabajo durante los años 2008-2012; 1520 pacientes fueron evaluados y estudiados mediante pruebas epicutáneas para descartar una posible dermatosis profesional. Se registraron los casos de sensibilización al Kathon CG y se analizaron las siguientes variables: genero, edad, grupo profesional, localización de las lesiones cutáneas, relevancia y relación profesional. El análisis estadístico se realizó con el programa SPSS 15.0. Resultados: Se observó sensibilización al Kathon CG en 88 pacientes (5,8%), correspondiendo un 42% a sensibilizaciones de origen profesional. Conclusiones: La alergia de contacto profesional por Kathon CG constituye actualmente es un problema de alta prevalencia. Ante un paciente con positividad a este alérgeno hay que interrogar siempre por su profesión. Kathon CG is the tradename for a mixture of isothiazolines: methylchloroisothiazolinone and methylisothiazolinone. It is a widely used preservative, especially in the cosmetic industry, although in recent years is presence have also been imposed in cleaning products for household use and, at higher concentrations in industrial preparations such as cutting oils, latex emulsions, paints, oils for diesel engines, etc. Because of its high sensitizing power and widespread use, is currently one of the most common causes of allergic contact dermatitis, both in our private lives, as in the occupational field. Objectives: To study the sensitizing capacity of Kathon CG and its relation to the development of allergic contact dermatitis of occupational origin. Methods: We performed a retrospective study by reviewing the medical records of patients seen at the Department of Occupationsal Dermatology at the Instituto Nacional de Medicina y Seguridad en el Trabajo, during the years 2008-2012. In that period, 1520 patients were evaluated and studied by patch testing, to rule out an occupational skin condition. We recorded cases of sensitization to Kathon CG and analyzed the following data: gender, age, occupational group, location of skin lesions, relevance and occupational origin. Statistical analysis was performed using SPSS 15.0. Results: We observed sensitization to Kathon CG in 88 patients (5.8%), and 42% were of occupational origin. Conclusions: Occupational contact allergy to Kathon CG is currently a problem of high prevalence. In patients with sensitization to this allergen, we should always ask about their occupation.

Objective To develop a self-administered short questionnaire to assess patient satisfaction with medical treatment for hand eczema (dermatitis) with good psychometric properties. Method The content of the questionnaire was determined on the basis of clinical consultation with groups of patients, from studying the existing instruments, and from discussions with a panel of seven experts. A first draft version containing 38 items organised in six dimensions was tested on a pilot sample of patients to assess its legibility. The extended version was then tested on a sample of 217 patients of both genders enrolled at 18 hospitals representative of the national distribution. The questionnaire was supplied together with the Morisky-Green compliance questionnaire, the health-related quality of life (HRQL) SF-12 questionnaire, and a visual analogue scale (VAS) of perceived health status to assess concurrent validity. The dimensionality was reduced by means of exploratory factor analysis, and reliability was evaluated on the basis of internal consistency and two halves reliability estimates. Item discriminant capability and questionnaire discriminant validity with respect to known groups of patients (by gender, principal diagnosis, age, disease severity and treatment) were also assessed. Results The reduction and validation sample was composed of 54% women and 46% men, of various educational levels with an average age of 43 years (SD = 13.7). Of those who responded, 26% were diagnosed with hyperkeratotic dermatitis of the palms and 27% of the fingertips, and 47% with recurring palmar dyshidrotic eczema. The questionnaire was shortened to a version containing 17 items grouped in six dimensions: effectiveness, convenience, impact on HRQL, medical follow-up, side effects, and general opinion. Cronbach's alpha coefficient reached a value of 0.9. The dimensions showed different degrees of correlation, and the scores had a normal distribution with an average of 58.4 points (SD = 18.01). Treatment satisfaction scores attained correlations between 0.003 and 0.222 with the HRQL measures, and showed higher correlations with the effectiveness (r = 0.41) and tolerability (0.22) measures, but very low correlation with compliance (r = 0.015). Significant differences were observed between some diagnoses and treatments. Conclusions The shortened questionnaire proved to have good psychometric properties, providing excellent reliability, satisfactorily reproducing the proposed structure and supplying evidence of validity.

Retrospectivas [ES] Desde la inauguración del Museo Olavide en 1882 su historia ha sido compleja y en muchos momentos desconocida, no conociéndose de forma exacta el número de figuras que tuvo en sus momentos de apogeo. Además no existen evidencia de que los escultores especialmente E. Zofío fueran o estuvieran contratados por el propio museo. Después de muchos avatares en los años 1966-67 el museo se cierra y comienza un largo peregrinaje de las cajas en donde se encontraban las figuras y documentación del museo de los escultores e incluso del propio fundador DrJose Eugenio de Olavide. Una vez localizadas las figuras se planteaba el problema de encontrar un lugar para surecuperación y posterior exposición. Gracias a la colaboración de la Escuela Nacional de Medicina del Trabajo y especialmente a su Director el Dr. Jerónimo Maqueda, se ha podido habilitar un espacio que reúne las condiciones adecuadas para poder realizar este complejo trabajo.[EN] Museum had a complex history. There are many aspects still unknown, like the exact number of figures. Moreover, there is no evidence that another artists, in addition to E. Zofío, were contracted for the museum. After many events, in the years 1966-67 the museum is closed. The figures and a lot of documentation about the museum and his founder, Dr. José Eugenio de Olavide, were kept in boxes, which remained in several places. Once these figures were localized, the next problem was to have a place to work in the restoration and exposition of them. Thanks to the collaboration of the Escuela Nacional de Medicina del Trabajo (National School of Occupational Medicine), and specially to his director, Dr. Jerónimo Maqueda, we have a place with the adequate conditions to do this work. Artículos originales [ES] Las aplicaciones de las diferentes resinas epoxi basadas en bisfenol A y F son extensas. Las resinas epoxi basadas en bisfenol F son más nuevas y resistentes que las de bisfenol A, y ambas son conocida de dermatitis de contacto alérgica. Se estudian 39 casos de sensibilización a resinas epoxi en los últimos 5 años. Los resultados obtenidos son: prevalencia de sensibilización a resinas epoxi entre los pacientes estudiados del 2%, 27 varones y 8 mujeres, con una edad media de 42.77, un período de medio sensibilización de 23,8 meses. La sensibilización a bisfenol F desde que se incluye para las pruebas del parche en la batería de resinas epoxi es del 100% para los casos sensibilizados a la resina. La localización mayoritaria se dio en las manos, con una relevancia actual del 84%. Se recomendó un cambio de puesto en el trabajo al 46% de estos pacientes.[EN] The applications of epoxy resins based on bisphenol A and F are extensive. Epoxy resins based on bisphenol F are new and more resistant than epoxy resins based on bisphenol A. Both of them cause allergic contact dermatitis. In the last 5 years, we have studied 39 cases of sensitization to epoxy resin. The results of our study were these: the global prevalence of epoxy resin sensitization was 2%, 27 men and 8 women. The mean age was 42.77years and the sensitization period was 23.8 months. The most frequent localization of the lesions were hands (84% of the patients). Since bisphenol F was included in epoxy resin battery for patch testing, a 100% of positives responses in patients sensitized to epoxy resin have been found. Sensitization to epoxy resins was a cause of change in workplace in 46% of cases in our series. There has been an increase in sensitization of bisphenol F that could be explained by a concomitant sensitization with bisphenol A, cross-reactivity between bisphenol A and F or because an incorrect chemical compound information of the resin. Artículos especiales [ES] La Bartonella bacilliformis es un parásito bacteriano intracelular facultativo de los eritrocitos humanos y de lascélulas endoteliales. La enfermedad de Carrión, fiebre de La Oroya y Verruga Peruana son todos términos que describen las consecuencias patológicas de la infección humana por Bartonella bacilliformis. (1) Aunque las infecciones que involucran especies de Bartonella, tales como Bartonella henselae y Bartonella quintana, ocurren en todo el mundo, la enfermedad de Carrión es endémica únicamente en Sudamérica. (14) Las infecciones por B. bacilliformis son un problemade salud en numerosas áreas rurales de Sudamérica y para los viajeros que visitan esas regiones. Se han reportadobrotes de bartonelosis en las regiones montañosas de Perú, Ecuador y Colombia. (1). La Lutzomyia verrucarum es el principal vector para la transmisión de la bacteria al ser humano; la hembra del mosquito transmite el patógeno durante su alimentación nocturna de sangre humana. Presumiblemente el insecto sealimenta de sangre de un individuo infectado y disemina el patógeno por medio de su saliva durante la siguiente ingesta de sangre. Opinión Los agentes de revelado fotográfico son derivados de la parafenilendiamina, que pueden actuar como irritantes y/o sensibilizantes cutáneos en trabajadores de procesos fotográficos, manifestándose las lesiones no sólo en forma de eczema de contacto, sino también como lesiones liquenoides. La incidencia de dermatosis profesionales por reveladores de color ha disminuido en los últimos años debido a la automatización de los procesos de revelado y a la aparición de la fotografía digital. Presentamos un caso de dermatitis de contacto liquenoide secundaria al revelador CD2 en un trabajador de laboratorio fotográfico automatizadoque ocasionalmente reparaba las averías que se presentaban en la maquinaria. Las lesiones eran clínica e histológicamente liquenoides y las pruebas epicutáneas resultaron positivas a CD2. Nuestro caso presenta además la particularidad de la diseminación de las lesiones hacia zonas donde no era evidente el contacto con el líquido revelador. No

Background Limonene and linalool are common fragrance terpenes widely used in cosmetic, household and hygiene products. Their primary oxidation products formed after air exposure, the hydroperoxides, have been recognized as important contact haptens. Objectives To investigate the prevalence of contact allergy to hydroperoxides of limonene (Lim-OOHs) and hydroperoxides of linalool (Lin-OOHs) in Spain, and to define the optimal concentration for screening in consecutive patients. Methods Three different concentrations of Lim-OOHs (0.1%, 0.2% and 0.3% pet.) and Lin-OOHs (0.25%, 0.5% and 1.0% pet.) were simultaneously tested in 3639 consecutive patients at 22 departments of dermatology in Spain. Results Lim-OOHs at 0.1%, 0.2% and 0.3% yielded positive patch test reactions in 1.4%, 3.4% and 5.1% of the tested patients, respectively; and Lin-OOHs at 0.25%, 0.5% and 1.0% yielded positive reactions in 1.3%, 2.9% and 4.9% of the tested patients, respectively. Few irritant (1.5-1.9%) and doubtful reactions (0.4-0.5%) to both terpene hydroperoxides were registered at the highest concentrations tested. Conclusions Lim-OOHs and Lin-OOHs can be considered as common causes of contact allergy, and their inclusion in an extended baseline patch test series therefore seems to be appropriate. The patch test preparations of Lim-OOHs 0.3% pet. and Lin-OOHs 1.0% pet. are useful tools for screening of contact sensitization. [ABSTRACT FROM AUTHOR]

Opinión La exploración física tiene como finalidad la obtención de datos objetivos (signos clínicos) que el médico recoge directamente del paciente. La exploración del enfermo con una probable dermatosis profesional debe tener en cuenta tres aspectos fundamentales, como son la localización de las lesiones, su morfología y si existen estigmas físicos de alguna enfermedad dermatológica en otras áreas de la piel. La principal localización de una dermatosis profesional son las manos. El hecho de que el cuadro sea unilateral orienta hacia una dermatitis de contacto, pudiéndose sospechar la sustancia causante según la zona de afectación de la mano. La afectación bilateral nos orienta, en general, a que la causa principal sea una enfermedad endógena. Son bilaterales, aunque de causa exógena, las dermatitis de contacto aerotransportadas y lumínicas, afectando ambas a regiones no protegidas por la vestimenta. Una localización en las áreas protegidas por las prendas de vestir puededeberse a la retención del antígeno o irritante en la ropa. No

Actualizaciones Resumen: La anamnesis es la principal herramienta diagnóstica en Dermatología Laboral. Puesto que alrededor del 90% de las dermatosis profesionales corresponden a dermatitis de contacto, nuestro interrogatorio se centrará principalmente en conocer las sustancias irritantes o sensibilizantes que se encuentran en el entorno laboral. Esto también incluye las medidas de protección (guantes, mascarilla, botas…) y los jabones o cremas que utilice el paciente en relación con su puesto de trabajo. Es importante además indagar sobre empleos pasados y si presentaba algún problema dermatológico en ellos, ya que podría explicar una sensibilización que no está relacionada con la dermatitis actual. La relación temporal de las lesiones con la actividad profesional, y la mejoría de las mismas cuando no trabaja, es un dato importante para sospechar una causa laboral. Otro dato que apoya un origen laboral es la afectación de varios trabajadores en la misma empresa. La opinión del paciente acerca de cuál es el producto que le produce la dermatitis de contacto nos puede ser de gran ayuda. También debemos conocer cuáles son sus actividades y aficiones extra-profesionales y si presenta rasgos atópicos. Después de una anamnesis bien realizada debemos sospechar el diagnóstico, antes incluso de realizar la exploración física, y utilizar ésta y las pruebas complementarias para confirmarlo. No

The article discusses a study conducted to identify sensitizing capacity of Kathon CG and its association with the development of allergic contact dermatitis of occupational origin. It informs that Kathon CG is a mixture of isothiazolines including methylchloroisothiazolinone and methylisothiazolinone. The study concluded that occupational contact allergy to Kathon CG is a problem of high prevalence.

- Abstract
Background: Standard patch test series must be updated using objective data on allergen sensitization. The Spanish standard series was last updated in 2016 and the European series in 2019, and the inclusion of several emerging allergens needs to be evaluated., Material and Methods: We conducted a prospective, observational, multicenter study of consecutive patients from the registry of the Spanish Contact Dermatitis and Skin Allergy Research Group (GEIDAC) who were patch tested in 2019 and 2020 with linalool hydroperoxide, limonene hydroperoxide, 2-hydroxyethyl-methacrylate, benzisothiazolinone, octylisothiazolinone, textile dye mix (TDM), sodium metabisulfite, propolis, bronopol, Compositae mix II, diazolidinyl urea, imidazolidinyl urea, decyl glucoside, and lauryl glucoside., Results: We analyzed data for 4654 patients tested with diazolidinyl urea, imidazolidinyl urea, and bronopol, and 1890 tested with the other allergens. The values for the MOAHLFA index components were 30% for male, 18% for occupational dermatitis, 15% for atopic dermatitis, 29% for hand, 6.5% for leg, 23% for face, and 68% for age > 40 years. Sensitization rates above 1% were observed for 7 allergens: linalool hydroperoxide, 2-hydroxyethyl-methacrylate, benzisothiazolinone, limonene hydroperoxide, TDM, sodium metabisulfite, and propolis. Three allergens had a current relevance rate of over 1%: linalool hydroperoxide, 2-hydroxyethyl-methacrylat, and limonene hydroperoxide. Benzisothiazolinone and TDM had a relevance rate of between 0.9% and 1%., Conclusions: Our results indicate that 7 new allergens should be considered when extending the Spanish standard patch test series.
