Abstract

Since most modes of transportation rely on liquid energy carriers like gasoline or diesel, the approaching end of the fossil fuel era poses a special challenge. One should bear in mind that nothing matches the energy density of liquid fuels. It is, therefore, important to consider the efficiency of various modes of transportation and explore alternative options for energy carriers. This includes a feasibility study of the use of solar energy in transportation. In this perspective various modes of transportation are briefly reviewed and the possible alternative energy carriers assessed.

Abstract

An assay that detects carcinoembryonic antigen in tissue samples can identify early metastasis in colorectal cancer patients. Carcinoembryonic antigen is a protein that occurs in cancerous tissue but not normal tissue. Researchers used this assay on 192 lymph nodes from 26 patients with colorectal cancer but no lymph node metastasis. The assay detected microscopic metastasis in half of these patients. The five-year survival rate for these patients was 36%, compared to 75% in patients without microscopic metastasis.

Abstract

Children with autosomal recessive osteopetrosis appear to have very poor chances of survival. Osteopetrosis is an inherited disease characterized by increased bone density, bone remodeling, deficient blood formation and neurological and sensory impairment. Researchers studied 33 osteopetrosis patients until their last follow-up examination or bone marrow transplantation (BMT). Overall, the probability of surviving to age six was about 30%. Eleven of 21 children died during the natural course of disease and seven of 12 children receiving BMT died from various causes. Seventeen children developed visual impairments and 14 developed blood production abnormalities during the first three months of life. The overall probability of developing a visual or blood production impairment during the first year of life was about 75%. Nine children also had hearing impairments and eight had mild, less specific neurological abnormalities., Objective. To determine the variability and the natural course of children suffering from autosomal recessive osteopetrosis to allow optimal counseling and decision making with respect to therapeutic intervention. Design. Retrospective and longitudinal evaluation of clinical symptoms and natural course with emphasis on survival and sensoneurologic and hematologic findings. Setting. Two large referral-based pediatric bone marrow transplantation units in Europe. Patients. Thirty-three patients with autosomal recessive osteopetrosis admitted to units in Paris and Leiden between 1972 and 1988 were analyzed until last follow-up or the time at which bone marrow transplantation was performed. The great number of patients and unprecedented amount of data make this report one of the single largest clinical studies on autosomal recessive osteopetrosis. Main results. Ocular involvement occurring at a median age of 2 months was the symptom at initial examination in half of the patients. A striking variability between patients was found. Retinal degeneration was present in three patients and associated generalized neurodegeneration was present in two. The probability of survival until the age of 6 years was about 30% for the group as a whole. The cumulative risk of developing visual or hematologic impairment in the first year of life was about 75% and leveled off afterward. Patients with early hematologic impairment, ie, before 3 months of age, especially when combined with early visual impairment, had a very poor prognosis regarding life expectancy. Conclusions. Autosomal recessive osteopetrosis seems to be a variable disorder with a poor prognosis, especially in children with early visual and hematologic impairment. Allogeneic bone marrow transplantation remains the only curative approach. Doctors can still harvest cord blood after a stressful delivery unless it would interfere with the delivery. A study of 300 pregnant women showed that stress during delivery increased the level of stem cells in cord blood.

Abstract

Patients with rheumatoid arthritis who experience a flare-up after discontinuing treatment may benefit from a resumption of treatment. This was the conclusion of a study of 51 patients in remission from rheumatoid arthritis who experienced a flare-up after they had stopped treatment. Thirty-five percent responded completely and 65% responded partially. About half responded within three months. Only four patients had to stop treatment due to lack of response. This study shows that many patients can stop treatment if they go into remission.

Abstract

Background. A randomized trial was undertaken comparing the Western R1 resection with limited N1-level lymphadenectomy and the Japanese R2 resection with extended lymphadenectomy, including the N2-level for curative resection of gastric cancer patients. After 389 patients were entered in the trial, protocol deviations were observed that reduced the intended distinction between the two types of lymphadenectomy: noncompliance, i.e., no substantiation of lymphadenectomy by nodal yields at indicated stations, and contamination, i.e., extension of lymphadenectomy outside the allocated level of nodal clearance. Methods. To identify factors underlying these protocol deviations, the authors analyzed the influence of six patient-, tumor, and treatment-related characteristics on the magnitude of deviations per patient, and on the incidence of deviations per lymph node station. Results. Protocol deviations were influenced by the following station-specific factors: (1) the number of nodes per station; (2) the clarity of anatomical station definition; (3) the location of stations; (4) local conventions on the type of gastrectomy; and (5) technical features to allow complete en bloc dissection. Furthermore, nonspecific factors such as inadequate retrieval of nodes, incomplete dissection, and careful selection of clinically overt metastases outside the allocated level of nodal clearance were randomly distributed over stations, and they, too, contributed to the deviations. Conclusions. Based on the findings, the authors took additional steps to preserve the distinction between limited and extended lymphadenectomy and to improve the accuracy of nodal staging. These factors should be considered when standardization of both surgicopathologic trials and clinical protocols for the treatment of gastric cancer is pursued. Cancer 1994; 73:1544-51.
