Abstract

Successful subscapularis repair in stemless shoulder arthroplasty is crucial to reduce complications and improve postoperative function. As stemless shoulder arthroplasty continues to grow in popularity, several subscapularis tendon repair techniques are being developed, with a current trend toward knotless devices and double-row anchor-based constructs. In this article, we present our technique for repair of a subscapularis tendon peel using a suture-capture construct that aids in compression of the tendon onto its footprint and then gradually releases the tension as the capture resorbs and tendon healing occurs. The suture-capture tissue repair technology uses a disk or button that pushes down a larger area of tissue onto the bone, eliminating gap formation between the bone and tendon; as the button’s material degrades, compressive, shearing, and tensile forces are transferred to the tendon tissue to achieve complete remodeling of the repair.

6. Indications for Surgery, Activities After Surgery, and Pain Are the Most Commonly Asked Questions in Anterior Cruciate Ligament Injury and Reconstruction

Abstract

Purpose: To leverage Google’s search algorithms to summarize the most commonly asked questions regarding anterior cruciate ligament (ACL) injuries and surgery. Methods: Six terms related to ACL tear and/or surgery were searched on a clean-installed Google Chrome browser. The list of questions and their associated websites on the Google search page were extracted after multiple search iterations performed in January of 2022. Questions and websites were categorized according to Rothwell’s criteria. The Journal of the American Medical Association (JAMA) Benchmark criteria were used to grade website quality and transparency. Descriptive statistics were provided. χ2 and Student t-tests identified for categorical differences and differences in JAMA score, respectively (significance set at P < .05). Results: A total of 273 unique questions associated with 204 websites were identified. The most frequently asked questions involved Indications/Management (20.2%), Specific Activities (15.8%), and Pain (10.3%). The most common websites were Medical Practice (27.9%), Academic (23.5%), and Commercial (19.5%). In Academic websites, questions regarding Specific Activities were seldom included (4.7%) whereas questions regarding Pain were frequently addressed (39.3%, P = .027). Although average JAMA score was relatively high for Academic websites, the average combined score for medical and governmental websites was lower (P < .001) than nonmedical websites. Conclusions: The most searched questions on Google regarding ACL tears or surgery related to indications for surgery, pain, and activities postoperatively. Health information resources stemmed from Medical Practice (27.9%) followed by Academic (23.5%) and Commercial (19.5%) websites. Medical websites had lower JAMA quality scores compared with nonmedical websites. Clinical Relevance: These findings presented may assist physicians in addressing the most frequently searched questions while also guiding their patients to greater-quality resources regarding ACL injuries and surgery.

Abstract

Background Skin cancer is one of the most frequent neoplasms in México, squamous cell carcinoma has an aggressive behavior, invading deep tissue and more risk of metastasis. The resection of this type of lesion leads to a defect that needs to be covered and is when a reconstructive surgeon needs to perform some techniques like skin flaps. In this case, we present an 82-years-old female with an ulcerated lesion on the nasal dorsum, left nasal wing, and inner third of the cheek, having a positive biopsy for highly differenced squamous carcinoma. It was handled with a cervicocutaneous flap with favorable clinical evolution. Keywords: Skin cancer, facial reconstruction, basal cell carcinoma.

20. Acute Variceal Bleeding: Does Octreotide Improve Outcomes in Patients with Different Functional Hepatic Reserve?

Abstract

Background. Current guidelines do not differentiate in the utilization of vasoactive drugs in patients with cirrhosis and acute variceal bleeding (AVB) depending on liver disease severity.Material and methods. In this retrospective study, clinical outcomes in 100 patients receiving octreotide plus endoscopic therapy (ET) and 216 patients with ET alone were compared in terms of failure to control bleeding, in-hospital mortality, and transfusion requirements stratifying the results according to liver disease severity by Child-Pugh (CP) score and MELD.Results. In patients with CP-A or those with MELD < 10 octreotide was not associated with a better outcome compared to ET alone in terms of hospital mortality (CP-A: 0.0 vs. 0.0%; MELD < 10: 0.0 vs. 2.9%, p = 1.00), failure to control bleeding (CP-A: 8.7 vs. 3.7%, p = 0.58; MELD < 10: 5.3 vs. 4.3%, p = 1.00) and need for transfusion (CP-A: 39.1 vs. 61.1%, p = 0.09; MELD < 10: 63.2 vs. 62.9%, p = 1.00). Those with severe liver dysfunction in the octreotide group showed better outcomes compared to the non-octreotide group in terms of hospital mortality (CP-B/C: 3.9 vs. 13.0%, p = 0.04; MELD > 10: 3.9 vs. 13.3%, p = 0.03) and need for transfusion (CP-B/C: 58.4 vs. 71.6%, p = 0.05; MELD > 10: 50.6 vs. 72.7%, p < 0.01). In multivariate analysis, octreotide was independently associated with in-hospital mortality (p = 0.028) and need for transfusion (p = 0.008) only in patients with severe liver dysfunction (CP-B/C or MELD > 10).Conclusion. Patients with cirrhosis and AVB categorized as CP-A or MELD < 10 had similar clinical outcomes during hospitalization whether or not they received octreotide.

Abstract

After a mastectomy, the breast may be managed in a number of ways, including with implants or autologous tissue flaps taken from a variety of donor locations. About 7.5% of women who have had a mastectomy choose for breast reconstruction. However, that worth is growing as surgical oncologists are increasingly teaming together with reconstructive surgeons to improve outcomes in both areas (patient satisfaction and psychological effects). The transverse rectus abdominis musculocutaneous (TRAM) flap and the deep inferior epigastric perforator (DIEP) flap are the two most common autogenous reconstructive procedures used today. Despite being a significant improvement in breast reconstruction, the deep inferior epigastric perforator (DIEP) flap is often avoided by surgeons because of fears of a greater flap loss rate compared to free or muscle-sparing transverse rectus abdominis myocutaneous (TRAM) flaps. Smoking, chemotherapy, prereconstruction radiation, postreconstruction radiotherapy, hypertension, diabetes mellitus, abdominal scarring, obesity, age, flap size, number of venous anastomoses, and number of perforators were all factors associated with greater flap morbidity. Fat necrosis, partial flap loss, complete flap loss, arterial thrombosis, venous congestion, postoperative hernia, infection, seroma, and hematoma are some of the issues associated with breast reconstruction. The tricarbocyanine dye indocyanine green (ICG) fluoresces upon near-infrared stimulation and strongly binds to plasma proteins that have not left the intravascular space. It has been utilized for some time now in the examination of vascularization; it tells us about the perfusion of the flap's cutaneous island.

Abstract

Four Pressure-equipped Inverted Echo Sounders (PIES) were deployed at about 10°W, between 19 and 35°S, the South Atlantic Gateway (SAGA), in order to determine the zonal flows in the South Atlantic. Those PIES will allow to observe the circulation of two water masses, the South Atlantic Central Water (SACW) and the North Atlantic Deep Water (NADW), that flow in opposite directions across the South Atlantic, between Cape town and Brazil, through the SAGA. The measurements from the PIES, together with historical hydrographic data, permit to estimate the profiles of temperature and salinity of the water column, and therefore the density. Besides, using the thermal-wind equation, it is possible to retrieve the geostrophic velocity from an array of PIES. In order to get those estimations of temperature and salinity, it is necessary to determine the Gravest Empirical Mode (GEM), a relationship between the acoustic travel time observed by the PIES and the historical observations of salinity and temperature in the study area. In this work, we will show the GEM estimated for the SAGA, calculated using historical hydrographic data from CTDs and Argo Float, as well as, the estimations of the error in the geostrophic transport.

Abstract

NMR-based metabolomics investigations of human biofluids offer great potential to uncover new biomarkers. In contrast to protocols for sample collection and biobanking, procedures for sample preparation prior to NMR measurements are still heterogeneous, thus compromising the comparability of the resulting data. Herein, we present results of an investigation of the handling of cerebrospinal fluid (CSF) samples for NMR metabolomics research. Origins of commonly observed problems when conducting NMR experiments on this type of sample are addressed, and suitable experimental conditions in terms of sample preparation and pH control are discussed. Sample stability was assessed by monitoring the degradation of CSF samples by NMR, hereby identifying metabolite candidates, which are potentially affected by sample storage. A protocol was devised yielding consistent spectroscopic data as well as achieving overall sample stability for robust analysis. We present easy to adopt standard operating procedures with the aim to establish a shared sample handling strategy that facilitates and promotes inter-laboratory comparison, and the analysis of sample degradation provides new insights into sample stability.
