Abstract Background De-implementation of low-value care can increase health care sustainability. We evaluated the reporting of direct costs of de-implementation and subsequent change (increase or decrease) in health care costs in randomized trials of de-implementation research. Methods We searched MEDLINE and Scopus databases without any language restrictions up to May 2021. We conducted study screening and data extraction independently and in duplicate. We extracted information related to study characteristics, types and characteristics of interventions, de-implementation costs, and impacts on health care costs. We assessed risk of bias using a modified Cochrane risk-of-bias tool. Results We screened 10,733 articles, with 227 studies meeting the inclusion criteria, of which 50 included information on direct cost of de-implementation or impact of de-implementation on health care costs. Studies were mostly conducted in North America (36%) or Europe (32%) and in the primary care context (70%). The most common practice of interest was reduction in the use of antibiotics or other medications (74%). Most studies used education strategies (meetings, materials) (64%). Studies used either a single strategy (52%) or were multifaceted (48%). Of the 227 eligible studies, 18 (8%) reported on direct costs of the used de-implementation strategy; of which, 13 reported total costs, and 12 reported per unit costs (7 reported both). The costs of de-implementation strategies varied considerably. Of the 227 eligible studies, 43 (19%) reported on impact of de-implementation on health care costs. Health care costs decreased in 27 studies (63%), increased in 2 (5%), and were unchanged in 14 (33%). Conclusion De-implementation randomized controlled trials typically did not report direct costs of the de-implementation strategies (92%) or the impacts of de-implementation on health care costs (81%). Lack of cost information may limit the value of de-implementation trials to decision-makers. Trial registration OSF (Open Science Framework): https://osf.io/ueq32 .

Abstract Background Healthcare costs are rising, and a substantial proportion of medical care is of little value. De-implementation of low-value practices is important for improving overall health outcomes and reducing costs. We aimed to identify and synthesize randomized controlled trials (RCTs) on de-implementation interventions and to provide guidance to improve future research. Methods MEDLINE and Scopus up to May 24, 2021, for individual and cluster RCTs comparing de-implementation interventions to usual care, another intervention, or placebo. We applied independent duplicate assessment of eligibility, study characteristics, outcomes, intervention categories, implementation theories, and risk of bias. Results Of the 227 eligible trials, 145 (64%) were cluster randomized trials (median 24 clusters; median follow-up time 305 days), and 82 (36%) were individually randomized trials (median follow-up time 274 days). Of the trials, 118 (52%) were published after 2010, 149 (66%) were conducted in a primary care setting, 163 (72%) aimed to reduce the use of drug treatment, 194 (85%) measured the total volume of care, and 64 (28%) low-value care use as outcomes. Of the trials, 48 (21%) described a theoretical basis for the intervention, and 40 (18%) had the study tailored by context-specific factors. Of the de-implementation interventions, 193 (85%) were targeted at physicians, 115 (51%) tested educational sessions, and 152 (67%) multicomponent interventions. Missing data led to high risk of bias in 137 (60%) trials, followed by baseline imbalances in 99 (44%), and deficiencies in allocation concealment in 56 (25%). Conclusions De-implementation trials were mainly conducted in primary care and typically aimed to reduce low-value drug treatments. Limitations of current de-implementation research may have led to unreliable effect estimates and decreased clinical applicability of studied de-implementation strategies. We identified potential research gaps, including de-implementation in secondary and tertiary care settings, and interventions targeted at other than physicians. Future trials could be improved by favoring simpler intervention designs, better control of potential confounders, larger number of clusters in cluster trials, considering context-specific factors when planning the intervention (tailoring), and using a theoretical basis in intervention design. There is a critical need to establish reference response rates following bladder-sparing therapies administered in the setting of bacillus Calmete-Guerin (BCG)-unresponsive non–muscle-invasive bladder cancer (NMIBC). We sought to determine the efficacy of different interventions in recent trials accruing patients fulfilling the strict BCG-unresponsive definition established by the US Food and Drug Administration. We performed a systematic review and meta-analysis for clinical trials in the BCG-unresponsive disease space to include published and presented results. The primary endpoints were complete response rate for CIS±Ta/T1 tumors, recurrence-free rate for patients with papillary-only disease, and disease-free rate in studies enrolling both papillary CIS tumors (Ta/T1/CIS). I2 was used for assessing heterogeneity. Eleven studies using 9 different therapeutic agents in a total of 909 patients with BCG-unresponsive NMIBC were identified. The resulting outcomes at 3, 6, and 12 months were 44%, 38%, and 25% complete response rate in CIS±Ta/T1 tumors; 73%, 58%, and 48% recurrence-free rate in papillary-only; and 48%, 22%, and 43% disease-free rate in combined Ta/T1/CIS, respectively. Relatively low levels of heterogeneity were observed amongst studies restricted to papillary-only or CIS±Ta/T1 tumors. Future randomized controlled studies are needed and will likely require stratification between papillary-only and CIS±Ta/T1 tumors.

Abstract Background Venous thromboembolism (VTE) and bleeding are serious and potentially fatal complications of surgical procedures. Pharmacological thromboprophylaxis decreases the risk of VTE but increases the risk of major post-operative bleeding. The decision to use pharmacologic prophylaxis therefore represents a trade-off that critically depends on the incidence of VTE and bleeding in the absence of prophylaxis. These baseline risks vary widely between procedures, but their magnitude is uncertain. Systematic reviews addressing baseline risks are scarce, needed, and require innovations in methodology. Indeed, systematic summaries of these baseline risk estimates exist neither in general nor gynecologic surgery. We will fill this knowledge gap by performing a series of systematic reviews and meta-analyses of the procedure-specific and patient risk factor stratified risk estimates in general and gynecologic surgeries. Methods We will perform comprehensive literature searches for observational studies in general and gynecologic surgery reporting symptomatic VTE or bleeding estimates. Pairs of methodologically trained reviewers will independently assess the studies for eligibility, evaluate the risk of bias by using an instrument developed for this review, and extract data. We will perform meta-analyses and modeling studies to adjust the reported risk estimates for the use of thromboprophylaxis and length of follow up. We will derive the estimates of risk from the median estimates of studies rated at the lowest risk of bias. The primary outcomes are the risk estimates of symptomatic VTE and major bleeding at 4 weeks post-operatively for each procedure stratified by patient risk factors. We will apply the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach to rate evidence certainty. Discussion This series of systematic reviews, modeling studies, and meta-analyses will inform clinicians and patients regarding the trade-off between VTE prevention and bleeding in general and gynecologic surgeries. Our work advances the standards in systematic reviews of surgical complications, including assessment of risk of bias, criteria for arriving at the best estimates of risk (including modeling of the timing of events and dealing with suboptimal data reporting), dealing with subgroups at higher and lower risk of bias, and use of the GRADE approach. Determinar la efectividad y seguridad de la cirugía anti-incontinencia retropúbica (TVT) frente a la técnica transobturadora (TOT) en el manejo de la incontinencia urinaria femenina con déficit esfinterico (DE). Se realizó una revisión sistemática, incluyendo ensayos clínicos de intervenciones quirúrgicas de cintilla de uretra media para el manejo de la incontinencia urinaria de esfuerzo asociado a DE. Se realizó la búsqueda en MEDLINE (OVID), EMBASE y Cochrane Central Register of Controlled Trials de la evidencia disponible hasta el 2023. Las opciones quirúrgicas de cintilla de uretra media, TVT y TOT, significativas resultantes fueron comparadas. Un total de 10 estudios fueron incluidos para el análisis, en ellos, se incluyeron 1523 pacientes. El promedio de edad fue 55 años. La cura de la incontinencia urinaria asociada a DE fue mayor con la colocación de cinta TVT (RR 1.22 IC95% [1.08 a 1.36] frente a la técnica de cintilla TOT (I2= 70%). El análisis de sensibilidad ajustado a ensayos clínicos identificó una superioridad en cura con la técnica TVT (RR 1.21 IC 95% % [1.11 a 1.33] con baja heterogeneidad (I2= 0%). No se encontraron diferencias estadísticamente significativas entre las complicaciones postquirúrgicas para ambas técnicas quirúrgicas (RR 1,19 IC 95% [0,90 a 1,56]). La cintilla de uretra media tipo TVT es una técnica quirúrgica superior en cuanto a cura frente a la cintilla tipo TOT para el control de la incontinencia urinaria asociada a DE. Se requieren más ensayos clínicos para confirmar la efectividad de estas técnicas.

Objective: UOR is a transmedia project carried out between the Narratives and Digital Journalism hotbed of the Universidad Autónoma de Occidente and the UROGIV research group of the Universidad del Valle, which was born from the research-creation objective of developing a transmedia narrative. This project would make it possible to contribute to the prevention of urological cancer, using the knowledge and the different investigations of the urology research group at the service of the community. Method: For this, a methodology was carried out that included a search, classification and selection of the papers advanced by UROGIV, the formulation of a model for the URO transmedia narrative and finally its evaluation based on the scope of the proposal in digital media and the analysis in its realization process. Results: A series of contents such as podcasts, video-documentaries and infographics were generated, socialized through the Spotify, Instagram, Facebook and YouTube platforms. In addition, a web repository that would allow uniting all the proposals of the project. Fifty-one communicative products were made. Instagram had follower accounts and interactions that allowed the platform to be evaluated positively. Conclusion: The development of this transmedia narrative showed as conclusions the importance of carrying out initiatives of this nature, and of adapting publications in urology to a more common language, in order to achieve a greater reach in terms of audience.

La cosificación en salud representa una forma de trato peyorativo que el personal de salud da a sus pacientes. Este trato cosificante tiene su origen en la deshumanización en salud, que debido a los avances tecnológicos cada vez es más fuerte. Este es un problema multicausal, que un estudiante y médico pueden abordar desde un enfoque académico promoviendo activades que lleven a un fortalecimiento en la relación médico-paciente y la implementación de una política de humanización en cada institución.

Las prótesis peanas marcaron un cambio importante en la historia del manejo de la disfunción eréctil, principalmente en hombres refractarios a otros métodos existentes. En la actualidad se cuenta con tres opciones de implantes de prótesis peneanas: prótesis de dos piezas, de tres piezas y las semirrígidas; cada una de ellas con una indicación específica. Existen también diferentes tipos de abordajes quirúrgicos, de los cuales los más usados son el abordaje penoescrotal y suprapúbico. En cuanto a las complicaciones quirúrgicas, la infección es una de las más frecuentes, por lo que se han desarrollado implantes impregnados con antibióticos e implantes irrigables con solución antibiótica y/o antifúngicas a elección con el fin de mitigar la infección, puesto que se ha reportado que los dispositivos recubiertos reducen la incidencia de infección del dispositivo en aproximadamente un 50%, al igual que la implementación de otras técnicas como la de no tocar para reducir la infección. La prótesis peneana es una estrategia efectiva para el tratamiento de la disfunción eréctil en hombres refractarios a otras intervenciones.

Objective: The objective of the study was to determine the efficacy and safety of flexible ureteroscopy (F-URS) for the management of intrarenal or proximal ureteral lithiasis in aged patients. Materials and methods: In this retrospective, multicenter observational study, we collected the anonymized clinical data of patients who underwent F-URS at two institutions in Cali, Colombia between June 2015 and May 2018. The patients were divided into two groups based on age: Group A defined as aged patients (> 65 years) and Group B as patients of non-advanced age (< 65 years). Results: A total of 201 patients were included in this study. The average age for Group A was 75 years (± 4.5) and for Group B was 51 years (± 10). The anesthetic risk classification (American Society of Anesthesiology [ASA]) and comorbidities were significantly higher for Group A with an ASA II and III of 74% versus 50% in Group B. No significant differences were shown in the stone-free rates (SFRs) or significant ureteral injury (Grade III and IV). There was no difference in intraoperative or post-operative complications between both groups. Conclusions: Age > 65 years was not associated with a negative impact on the outcomes of F-URS for the management of intrarenal or proximal ureteral lithiasis in this cohort of patients. F-URS appears as a safe and effective procedure and should not be withheld from older patients.

Introducción: El cáncer de vejiga es una patología frecuente del tracto genitourinario, cuyo tratamiento acarrea morbilidad y alteración de la calidad de vida y en particular en el subgrupo de pacientes con tumores vesicales clasificados como invasores de músculo. En los últimos años se han venido buscando alternativas terapéuticas para la cistectomía radical + linfadenectomía pélvica extendida, que es en la actualidad el estándar de manejo para los pacientes con carcinoma de vejiga invasor de músculo. Con el advenimiento de perfiles de manejo oncológico menos ablativos pero sin sacrificar resultados oncológicos y con las nuevas técnicas de radioterapia y quimioterapia, las modalidades terapéuticas preservadoras de órgano como la terapia trimodal (resección transuretral de tumor vesical + quimioterapia + radioterapia) se convierte en una alternativa terapéutica viable y con resultados oncológicos satisfactorios a largo plazo. Objetivo y metodología: Con esta revisión se pretende mostrar la actualidad de la terapia trimodal en el manejo de los tumores vesicales con invasión muscular, definir los mejores pacientes a considerar para recibir esta terapia, exponer los resultados oncológicos comparados con el estándar de manejo y los resultados en calidad de vida. También se propone un algoritmo de manejo y se presentar las recomendaciones al respecto en guías de práctica clínica. Conclusiones: La terapia trimodal es una alternativa al estándar de manejo que conduce a resultados oncológicos aceptables y puede considerarse una opción de tratamiento en pacientes bien seleccionados.

El trauma renal corresponde a un 10% de todos los traumas abdominales; de ellos, un 90% comprende el trauma cerrado.

Purpose: To determine the effectiveness and harm of cytoreductive nephrectomy versus no intervention in patients with metastatic renal carcinoma who undergo targeted therapy to improve overall survival. Materials and Methods: A search strategy was conducted in the MEDLINE, CENTRAL, Embase, HTA, DARE, NHS, and LILACS databases. Searches were also conducted for unpublished literature through references from relevant articles identified through the search, conferences, thesis databases, OpenGrey, Google Scholar, and clinicaltrials.gov, among others. Studies were included without language restrictions. The risk of bias assessment was made by using a modified Cochrane Collaboration tool. A meta-analysis of fixed effects was conducted. The expected outcomes were overall survival, quality of life, adverse effects, mortality, and progression- free survival. The measure of the effect was the hazard ratio (HR) with a 95% confidence interval (CI). The planned comparison was cytoreductive nephrectomy versus no intervention. Results: A total of 22,507 patients were found among seven studies. Seven studies were included in the qualitative analysis (eight publications) and five in the quantitative analysis for overall survival. One study reported progression-free survival and one reported targeted therapy toxicities. A low risk of bias was shown for most of the study items. The HR for overall survival was 0.58 (95% CI, 0.50 to 0.65) favoring cytoreductive nephrectomy compared with no intervention. Conclusions: Cytoreductive nephrectomy is effective for improving overall survival in patients with metastatic renal carcinoma who undergo targeted therapy compared with no intervention.

A lo largo de los años, se ha estudiado epidemiológicamente, los efectos de diferentes micronutrientes en la prevención del cáncer de próstata. Los estudios sugieren que los hábitos en la dieta y los procesos inflamatorios son factores de riesgo para esta enfermedad. Los mecanismos en los que se sustenta la protección de estas sustancias, han sido descritos experimentalmente in vitro y en ratas, demostrando su actuación como antioxidantes, potenciadores de reparación celular y de inhibición de crecimiento celular evitando así, la formación de tumores en la próstata. Existen estudios de observación e intervención en humanos en riesgo de desarrollar cáncer de próstata. Ellos recibieron micronutrientes específicos y evaluaron sus hábitos de ingesta, sin embargo los hallazgos han sido inconsistentes en determinar el impacto en la prevención de la enfermedad. Esto demuestra que se requieren estudios con una metodología más adecuada para establecer una intervención efectiva en las poblaciones en riesgo. En esta revisión, se describen los hallazgos mas importantes descritos en la literatura hasta el moemnto, asi como sus limitaciones.
