Abstract

Objective To elucidate the role of advanced maternal age (AMA) in determining the outcome of pregnancies complicated by SARS-CoV-2 infection. Methods Multinational cohort study included women with laboratory-confirmed SARS-CoV-2 infection from 76 centers in 27 different countries in Europe, United States, South America, Asia and Australia from 04 April 2020 till 28 October 2020. The primary outcome was a composite measure of maternal mortality and morbidity including admission to intensive care unit (ICU), use of mechanical ventilation (defined as intubation, need for continuous positive airway pressure, extra-corporeal membrane oxygenation), severe respiratory symptoms (including dyspnea and shortness of breath) or death. Results Eight hundred and eighty seven pregnant women were included in the study who were positive SARS-CoV-2 results by RT-PCR (reverse transcriptase-polymerase chain reaction) on their nasal and pharyngeal swab specimens (352 with and 652 with no AMA). The risk of composite adverse maternal outcome was higher in AMA group compared to that of under 35 years of age group, with an OR of 1.99 (95% CI 1.4–2.9; p=0.002). Abstract

It has still to be ascertained whether severe acute respiratory syndrome coronavirus 2 infection in pregnancy is associated with worse maternal and fetal outcomes compared to low risk gestations.This study aimed to evaluate maternal and perinatal outcomes in high- and low-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection.This was a multinational retrospective cohort study involving women with laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection from 76 centers from 25 countries in Europe, the United States, South America, Asia, and Australia from April 4, 2020, to October 28, 2020. The primary outcome was a composite measure of maternal mortality and morbidity, including admission to the intensive care unit, use of mechanical ventilation, or death. The secondary outcome was a composite measure of adverse perinatal outcome, including miscarriage, fetal loss, neonatal and perinatal death, and admission to the neonatal intensive care unit. All outcomes were assessed in high- and low-risk pregnancies. Pregnancies were considered high risk in case of either preexisting chronic medical conditions in pregnancy or obstetrical disorders occurring in pregnancy. The Fisher exact test and logistic regression analysis were used to analyze the data.A total of 887 singleton pregnancies who tested positive for severe acute respiratory syndrome coronavirus 2 infection using reverse transcription-polymerase chain reaction of nasal and pharyngeal swab specimens were included in the study. The risk of composite adverse maternal outcomes was higher in high-risk pregnancies than in low-risk pregnancies (odds ratio, 1.52; 95% confidence interval, 1.03-2.24; P=.035). In addition, women carrying high-risk pregnancies were at higher risk of hospital admission (odds ratio, 1.48; 95% confidence interval, 1.07-2.04; P=.002), presence of severe respiratory symptoms (odds ratio, 2.13; 95% confidence interval, 0.41-3.21; P=.001), admission to the intensive care unit (odds ratio, 2.63; 95% confidence interval, 1.42-4.88), and invasive mechanical ventilation (odds ratio, 2.65; 95% confidence interval, 1.19-5.94; P=.002). When exploring perinatal outcomes, high-risk pregnancies were at high risk of adverse perinatal outcomes (odds ratio, 1.78; 95% confidence interval, 0.15-2.72; P=.009). However, such association was mainly because of the higher incidence of miscarriage in high-risk pregnancies compared with that in low-risk pregnancies (5.3% vs 1.6%, P=.008); furthermore, there was no difference in other explored outcomes between the 2 study groups. At logistic regression analysis, maternal age (odds ratio, 1.12; 95% confidence interval, 1.02-1.22; P=.023) and high-risk pregnancy (odds ratio, 4.21; 95% confidence interval, 3.90-5.11; P.001) were independently associated with adverse maternal outcomes.High-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection were at higher risk of adverse maternal outcomes than low-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection.

Abstract

Objective The performance of the first-trimester preeclampsia (PE) screening among women with traditional high-risk factors remains controversial. Administration of prophylactic aspirin regardless of the result of the screening is still a common practice considered the basal risk of placental disease. Our aim was to evaluate the performance of the FMF first trimester screening to detect preterm PE in women with at least one high-risk factor. Methods Retrospective cohort study performed at a tertiary-level teaching hospital including singleton pregnancies with first-trimester PE screening and complete pregnancy outcomes with delivery at the same hospital between January 2023 and September 2023. The study was approved by the local Institutional Review Board. Gestational age was calculated according to crown-rump length at the first-trimester ultrasound. Screening was performed at 12 (11-14) weeks with PlGF determination at 11 (9-13) weeks. High-risk factors were considered as the presence of chronic hypertension, pregestational diabetes mellitus, systemic lupus erythematosus, antiphospholipid syndrome, or prior preeclampsia. Women with a risk >1:100 at screening were prescribed 150mg aspirin until 36 weeks. Those with a low-risk screening but with a high-risk factor were offered the possibility to either be considered as low-risk and not receive aspirin or still take aspirin. The main maternal basal characteristics as well as perinatal outcomes were collected from the clinical electronic history. PE was defined as the presence of hypertension (>140 / 90 mmHg) and proteinuria (>300 mg/24 hours or a urine protein-to-creatinine ratio >0.3 mg/mg). Preterm PE was considered as the one that required delivery before 37 weeks of gestation. For the purpose of this study, patients with a high-risk factor or positive screening were contacted by telephone and asked about their adherence to aspirin. Descriptive analysis was performed using mean (standard deviation) and median (interquartile range) or n (%) where appropriate. The performance of the screening to detect PE was tested using logistic regression analysis, sensitivity, specificity, positive and negative predictive value, and the area under the ROC curve. Statistical analysis was computed using STATA 14.2 and significance was considered with p values <0.05 in a two-tailed distribution. Results A total of 1831 screenings were performed, of which 1330 had complete pregnancy outcomes and composed the final study population. The mean (SD) maternal age was 31.8 (6.1) years old and 48% were nulliparous. There were 52/1330 (3.9%) who had at least one major risk factor of PE: prior PE (3.1%), chronic hypertension (2.6%), antiphospholipid syndrome (0.8%), diabetes mellitus (0.5%), and systemic lupus erythematosus (0.2%). In 126/1330 (9.5%) women a positive screening was obtained, of which 115/126 (91.3%) were treated with aspirin and 111/115 (96.5%) reported >90% adherence. Among those with at least one high-risk factor, the rate of positive screening was 27/52 (51.9%). All with a positive screening and 7/16 with a negative result were treated with prophylactic aspirin. The PE incidence was of 4.96% and 1.73% of preterm PE. Among women with at least one high risk factor, the incidence of preterm PE was 3.85% (7.41% vs 0% in those with that screened positive vs negative, respectively). The performance of the screening for preterm PE on the whole population vs those with a high-risk factor was as follows: sensitivity of 52.2% (95%CI 30.6% - 73.2%) vs 100% (95%CI 15% - 100%), specificity of 91.3% (95%CI 89.6% - 92.8%) vs 50% (95%CI 35.5% - 64.5%), positive predictive value of 9.5% (95%CI 5% - 16%) vs 7.4% (95%CI 0.9%-24.3%), negative predictive value of 99.1% (95%CI 98.4% - 99.5%) vs 100% (95%CI 86.3% -100%), AUC of 0.71 (95%CI 0.61 – 0.82) vs 0.75 (95%CI 0.68 – 0.82). Conclusion In our setting, half of women with at least one high-risk factor have a positive first-trimester screening for preterm PE. The overall performance is similar to the general population with excellent negative predictive values in both. The rates of preeclampsia were similar in women who screened negative regardless of the presence of major risk factors. We categorized the risk into four groups, a basal one G0 (>1:200), G1 between 1:200 and 1:101, G2 between 1:100 and 1:51, and G3 ≥1:50. PE was defined as the presence of hypertension (≥140 / 90 mmHg) and proteinuria (≥300 mg/24 hours or a urine protein-to-creatinine ratio of ≥ 0.3). FGR was considered as a newborn weight <3rd customized centile according to GROW software. The main maternal basal characteristics as well as perinatal outcomes were collected from the clinical electronic history. Descriptive analysis was performed using mean (standard deviation-SD) and median (interquartile range) or n (%) where appropriate. The performance of the screening to detect FGR was tested using logistic regression analysis, sensitivity, specificity, positive and negative predictive value, and the area under the ROC curve. Classification of the risk of preterm PE showed that 1127/1330 (84.7%) were part of of G0, 77/1630 (5.8%) of G1; 70/1330 (5.3%) of G2, and 56/1330 (4.2%) of G3. The mean (SD) gestational age at delivery was 39.5 (2.1) weeks, with a mean weight of 3194 (493) g. The overall PE incidence was 5.0% and 1.7% of preterm PE. There were 45/1330 (3.4%) of newborns born <3rd centile of which 8/45 (17.8%) had concomitant PE. When compared with the G0 group, the RR for FGR was 2.4 (95%CI 0.9 – 6.3) for G1; 1.6 (95% CI 0.5 – 5.2) for G2, and 4.7 (95% CI 2.1 – 10.7) for G3. Considering those at highest risk (G3), the performance of the screening to detect FGR had a sensitivity of 25% (95% CI 15.3% - 37%), specificity of 95.8% (95% CI 94.6% - 96.8%), positive predictive value of 23.3% (95% CI 14.2% - 34.6%), negative predictive value of 96.2% (95% CI 95% - 97.1%), and AUC of 0.60 (95% CI 0.55 - 0.66). Conclusion: A very high risk (>1:50) of preterm PE in the first-trimester screening is strongly associated with both PE and FGR but is a poor predictor of fetal growth. Abstract

Objective: To investigate if cerebroplacental ratio (CPR) adds to the predictive value of umbilical artery pulsatility index (UA PI) alone–standard of practice–for adverse perinatal outcome in singleton pregnancies. Design and setting: Meta-analysis based on individual participant data (IPD). Population or sample: Ten centres provided 17 data sets for 21661 participants, 18731 of which could be included. Sample sizes per data set ranged from 207 to 9215 individuals. Patient populations varied from uncomplicated to complicated pregnancies. Methods: In a collaborative, pooled analysis, we compared the prognostic value of combining CPR with UA PI, versus UA PI only and CPR only, with a one-stage IPD approach. After multiple imputation of missing values, we used multilevel multivariable logistic regression to develop prediction models. We evaluated the classification performance of all models with receiver operating characteristics analysis. We performed subgroup analyses according to gestational age, birthweight centile and estimated fetal weight centile. Main outcome measures: Composite adverse perinatal outcome, defined as perinatal death, caesarean section for fetal distress or neonatal unit admission. Results: Adverse outcomes occurred in 3423 (18%) participants. The model with UA PI alone resulted in an area under the curve (AUC) of 0.775 (95% CI 0.709–0.828) and with CPR alone in an AUC of 0.778 (95% CI 0.715–0.831). Addition of CPR to the UA PI model resulted in an increase in the AUC of 0.003 points (0.778, 95% CI 0.714–0.831). These results were consistent across all subgroups. Conclusions: Cerebroplacental ratio added no predictive value for adverse perinatal outcome beyond UA PI, when assessing singleton pregnancies, irrespective of gestational age or fetal size. Tweetable abstract: Doppler measurement of cerebroplacental ratio in clinical practice has limited added predictive value to umbilical artery alone.

Abstract

BACKGROUND: Pre-eclampsia is a leading cause of maternal and perinatal mortality and morbidity. Early identification of women at risk during pregnancy is required to plan management. Although there are many published prediction models for pre-eclampsia, few have been validated in external data. Our objective was to externally validate published prediction models for pre-eclampsia using individual participant data (IPD) from UK studies, to evaluate whether any of the models can accurately predict the condition when used within the UK healthcare setting. METHODS: IPD from 11 UK cohort studies (217,415 pregnant women) within the International Prediction of Pregnancy Complications (IPPIC) pre-eclampsia network contributed to external validation of published prediction models, identified by systematic review. Cohorts that measured all predictor variables in at least one of the identified models and reported pre-eclampsia as an outcome were included for validation. We reported the model predictive performance as discrimination (C-statistic), calibration (calibration plots, calibration slope, calibration-in-the-large), and net benefit. Performance measures were estimated separately in each available study and then, where possible, combined across studies in a random-effects meta-analysis. RESULTS: Of 131 published models, 67 provided the full model equation and 24 could be validated in 11 UK cohorts. Most of the models showed modest discrimination with summary C-statistics between 0.6 and 0.7. The calibration of the predicted compared to observed risk was generally poor for most models with observed calibration slopes less than 1, indicating that predictions were generally too extreme, although confidence intervals were wide. There was large between-study heterogeneity in each model's calibration-in-the-large, suggesting poor calibration of the predicted overall risk across populations. In a subset of models, the net benefit of using the models to inform clinical decisions appeared small and limited to probability thresholds between 5 and 7%. CONCLUSIONS: The evaluated models had modest predictive performance, with key limitations such as poor calibration (likely due to overfitting in the original development datasets), substantial heterogeneity, and small net benefit across settings. The evidence to support the use of these prediction models for pre-eclampsia in clinical decision-making is limited. Any models that we could not validate should be examined in terms of their predictive performance, net benefit, and heterogeneity across multiple UK settings before consideration for use in practice. Abstract

Background There is little knowledge about the significance of extremely high values (>655) for the ratio of sF lt‐1 (soluble fms‐like tyrosine kinase 1) to Pl GF (placental growth factor). We aim to describe the time‐to‐delivery interval and maternal and perinatal outcomes when such values are demonstrated while assessing suspected or confirmed placental dysfunction based on clinical or sonographic criteria. Methods and Results A multicenter retrospective cohort study was performed on 237 singleton gestations between 20+0 and 37+0 weeks included at the time of first demonstrating a sF lt‐1/Pl GF ratio >655. Clinicians were aware of this result, but standard protocols were followed for delivery indication. Main outcomes were compared for women with and without preeclampsia at inclusion. In those with preeclampsia (n=185, of whom 77.3% had fetal growth restriction), severe preeclampsia features and fetal growth restriction in stages III or IV were present in 49.2% and 13.5% cases, respectively, at inclusion and in 77.3% and 28.6% cases, respectively, at delivery. In the group without preeclampsia (n=52, 82.7% had fetal growth restriction), these figures were 0% and 30.8%, respectively, at inclusion and 21.2% and 50%, respectively, at delivery. Interestingly, 28% of women without initial preeclampsia developed it later. The median time to delivery was 4 days (interquartile range: 1–6 days) and 7 days (interquartile range: 3–12 days), respectively ( P Conclusions An sF lt‐1/Pl GF ratio >655 is almost invariably associated with preeclampsia or fetal growth restriction that progresses rapidly. In our tertiary care settings, we observed that maternal adverse outcomes were high throughout gestation, whereas perinatal adverse outcomes diminished as pregnancy advanced.

Abstract

Objective: The aim of this study was to report the rate of additional anomalies detected exclusively at prenatal magnetic resonance imaging (MRI) in fetuses with isolated severe ventriculomegaly undergoing neurosonography. Method: Multicenter, retrospective, cohort study involving 20 referral fetal medicine centers in Italy, United Kingdom, Spain and Denmark. Inclusion criteria were fetuses affected by isolated severe ventriculomegaly (≥15 mm), defined as ventriculomegaly with normal karyotype and no other additional central nervous system (CNS) and extra-CNS anomalies on ultrasound. In all cases, a multiplanar assessment of fetal brain as suggested by ISUOG guidelines on fetal neurosonography had been performed. Abstract

Background: There is little evidence about how novel coronavirus (SARS-CoV-2) affects pregnant women and their newborns. Comparisons with other members of the coronavirus family responsible for severe acute respiratory syndrome (SARS) have been done to predict maternal and neonatal outcomes; however, more information is required to establish clinical patterns, disease evolution and pregnancy prognosis in this group of patients.Methods: This paper is reporting a series of 91 women diagnosed with SARS-CoV-2 infection during pregnancy and puerperium. The analysis showed that 40 patients developed pneumonia, bilateral in most cases, with a 46.2% rate of hospitalization and 4 patients requiring intensive care unit (ICU) admission. In confront with previous publications, we have found a higher rate of coronavirus disease (COVID-19) severe forms, even when compared to non-pregnant women with the same baseline characteristics. We have analyzed the demographic characteristics, pregnancy-related conditions and presenting symptoms to identify features that could determine which patients will need hospitalization because of COVID-19 (Group 1-G1) and those who not (Group 2-G2). We have found that obesity and Latin-American origin behave as risk factors: OR: 4.3; 95% CI: 1.4-13.2, and OR: 2.6; 95% CI: 1.1 - 6.2, respectively. Among the 23 patients that delivered with active SARS-CoV-2, the overall rate of cesarean section (CS) and preterm birth were 52.2% and 34.8%, respectively, but we observed that the rate of CS was even higher in G1 compared to G2: 81.8% versus 25%, p = .012. However, prematurity was equally distributed in both groups and only one preterm delivery was determined by poor maternal condition. There were no deaths among the patients neither their newborns.Conclusion: In conclusion, the results of our cohort reveal that SARSC-CoV-2 infection may not behave as mild as suggested during pregnancy, especially when factors as obesity or Latin-American origin are present. No evidence of late vertical transmission was noticed but prematurity and high CS rate were common findings, although it is difficult to establish any causality between these conditions and COVID-19. Further evidence is required to establish if pregnancy itself can lead to severe forms of COVID-19 disease and whether risk factors for the general population are applicable to obstetric patients.
