Abstract

Background Evaluating COVID-19 vaccine effectiveness (VE) domestically is crucial for assessing and determining national vaccination policy. This study aimed to evaluate VE of mRNA COVID-19 vaccines in Japan. Methods We conducted a multicenter test-negative case-control study. The study comprised individuals aged ≥16 visiting medical facilities with COVID-19-related signs or symptoms from 1 January to 26 June 2022, when Omicron BA.1 and BA.2 were dominant nationwide. We evaluated VE of primary and booster vaccination against symptomatic SARS-CoV-2 infections and relative VE of booster compared with primary. Results We enrolled 7,931 episodes, including 3,055 test positive. The median age was 39, 48.0% were male, and 20.5% had underlying medical conditions. In individuals aged 16 to 64, VE of primary vaccination within 90 days was 35.6% (95% CI, 19.0–48.8%). After booster, VE increased to 68.7% (60.6–75.1%). In individuals aged ≥65, VE of primary and booster was 31.2% (−44.0–67.1%) and 76.5% (46.7–89.7%), respectively. Relative VE of booster compared with primary vaccination was 52.9% (41.0–62.5%) in individuals aged 16 to 64 and 65.9% (35.7–81.9%) in individuals aged ≥65. Conclusions During BA.1 and BA.2 epidemic in Japan, mRNA COVID-19 primary vaccination provided modest protection. Booster vaccination was necessary to protect against symptomatic infections.

Publisher Summary The presence of the four distinct seasons with appropriate temperature and humidity in Japan surrounded by sea fostered the Japanese food eating habit and created a distinctive Japanese food culture. The term “health foods” (HF) is commonly used in Japan to describe what may be called “functional foods” (FF) or “dietary or food supplements” in the USA and other countries. This chapter serves to enhance understanding of HF in relation to various regulatory systems and safety concerns in consumers. The “Foods with Specified Health Uses” (FOSHU) concept is described as it evolved from FF which was first developed and published for the health benefits of foods in our country in 1984. In 1991, the Ministry of Health and Welfare (MHW), now known as the Ministry of Health, Labor and Welfare (MHLW), officially presented the FOSHU system as a basis for making the functional claims of foods (FD). It was in reference to the increase in the occurrence of lifestyle-related diseases (LSRD) that include diabetes mellitus, high blood pressure, cerebro- and cardiovascular diseases and cancer. In 2001, a new regulatory system, “Foods with Health Claims” (FHC) was established and consisted of a new concept of “Foods with Nutrient Function Claims” (FNFC) and innovated FOSHU. In 2005, the MHLW additionally changed the existing FOSHU system. Such change includes the new subsystems of FOSHU: standardized FOSHU, qualified FOSHU and disease risk reduction claims. Lastly, issues directed to the safety and quality of raw materials and HF products are also discussed with the focus on two guidelines that require good manufacturing practice (GMP) and self-investigating systems for safety of raw materials.

Abstract

A new function claim system within the framework of health foods, Foods With Function Claims (FFC), was added in Apr. 2015 into preexisting health claim systems such as Foods With Specified Health Uses (FOSHU) and Foods With Nutrient Function Claims, while so-called health foods continued to be a large part of health foods. A striking difference between FFC and FOSHU in the categorization is that FFC adopts a registration or notification system. Consequently, the safety and efficacy (function claims) of any FFC product should have scientifically evidence for which applicants (sellers) take responsibilities in documenting, whereas FOSHU is performed within a government approval system. The Consumer Affair Agency's major aims of implementing FFC are to provide consumers with an opportunity to select foods with function/health claims and vitalize the health foods market, allowing small to medium-size enterprises to capitalize on FFC.

Abstract

Tokyo Olympic and Paralympic Games are coming up in 2020, and the sports supplement/nutrition market is steadily increasing. It is common for top-level athletes to use various types of sport supplements. However, the variety of products is sometimes limited by the Japanese unique regulation, and furthermore, it is not easy to know the functions of a product appropriately because of lack of information or complex food classification system. With this situation, privately importing sport supplement via internet is increasing, but it should be noted that the risk of inadvertent doping is increasing by those products. Academic societies have trained human resources such as sports dietitians or nutrition representative & supplement advisors who are expected to help athletes and consumers. Antidoping is another important challenge in Japan, although positive cases are small. A global risk management system for antidoping certification to a sport supplement was introduced in Japan recently, and its benefit and possibility are explained in this chapter.

Abstract

Holoparasitism is a special life cycle of flowering plants. All carbon resources are provided by photosynthetic host plants. A recent study revealed the presence of endophytic fungi in holoparasitic plants, but their ecological and evolutionary roles are still unknown. In this study, we examined endophytic fungi isolated from the holoparasitic plant Balanophora japonica (Balanophoraceae), collected from Kochi, Shikoku in western Japan. We isolated 23 fungal strains on inflorescences and tubers from three B. japonica plants at two locations and on one sample of the host plant (Symplocos lancifolia, Symplocaceae). Predominant isolates were Trichoderma-Hypocrea, Penicillium and Phialemonium. The first group was also predominant in the host plant. Fungal composition revealed in this study differed from the composition on B. harlandii or other root holoparasites with endophytic fungal (Rafflesia cantleyi) data. Those differences might be caused by various factors, including growth habits, location, phylogenetic position or host-parasite relationship.

Abstract

Inflammatory mediators are involved in the pathophysiology of neutrophilic bronchial disorders presenting with chronic productive cough. Accumulating evidence indicates that prostanoids are key elements in the pathophysiology of these disorders. However, little is known about the role of prostacyclin in neutrophilic bronchial inflammation.The effect of beraprost, a chemically and biologically stable analog of prostacyclin, on cough response to inhaled capsaicin was examined in 14 patients with chronic bronchitis, a neutrophilic bronchial disorder, in a randomized, placebo-controlled crossover study. Capsaicin cough threshold, defined as the lowest concentration of capsaicin eliciting five or more coughs, was measured as an index of the airway cough reflex sensitivity.After a 2-week treatment with beraprost (80 μg twice a day orally), the cough threshold was significantly (P.05) decreased as compared with placebo [12.2 (geometric standard error of the mean [GSEM] 1.5) μM vs. 24.4 (GSEM 1.3)].These findings indicate that prostacyclin is involved in the pathophysiology of cough reflex sensitivity in patients with chronic bronchitis, a frequently encountered neutrophilic bronchial disorder presenting with chronic productive cough.

Abstract

Since its inception in Japan, functional food has continued to deliver a true added value to a wide spectrum of customers, especially in aging subpopulations. Japanese companies have strong "R&D" capabilities and strong know-how in the field of functional foods. They have the opportunity to grow overseas by promoting and marketing their products. The main challenge is to understand the foreign markets and their regulations to be able to promote Japanese products overseas. To achieve this goal, the study reports a scientific review of the relevant literature and official legislative reports published by the authorized entities in several countries to create a comparison between the rules and regulations in different countries such as China, the European Union, South Korea, Singapore, Taiwan, and the United States. The study results provide suggestions for entry strategies to recommended foreign markets based on regulatory situations. The study also provides a comparison for the different functional food regulations in Japan (FOSHU, FNFC, FFC), along with an introduction for the new local government certification system.

Abstract

Increasing number of patients with advanced non-small cell lung cancer are receiving outpatient maintenance chemotherapy. It is very important to maintain these patients' quality of life(QOL). Pemetrexed has been reported to be an effective maintenance chemotherapy. However, its effects on the QOL of patients with non-small cell lung cancer who are undergoing outpatient maintenance chemotherapy are unknown; therefore, we conducted this study. To investigate factors that influence the QOL of these patients, we provided a QOL questionnaire,"The QOL Questionnaire for Cancer Patients Treated with Anticancer Drugs(QOL-ACD)"to 7 patients with non-small cell lung cancer. The medical factors related to the overall QOL scores, as well as other categories indicating"activity","physical condition","psychological condition","social relationship", and"face scale", were analyzed. No significant reductions in any of the factors were observed in this study.

Abstract

The terms 'nutraceuticals' and 'dietary or food supplements' are not very popular in Japan as compared to most of other countries. However, the concept of 'functional foods', which benefits the structure and function of the human body, is known as a result of research studies initiated on the health benefits of foods in 1984. The Ministry of Education organized a national research and development project to evaluate the functionalities of various foods. Researchers from diverse scientific fields succeeded to define new functions of food, successfully incorporating the previously recognized functions of nutrition, sensory/satisfaction and physiological effects of ingredients in foods. Some of the food manufacturers and distributors unfortunately capitalized on such food functionalities to promote 'health foods' by claiming drug-like effects and violating laws. In 1991, the Ministry of Health and Welfare (MHW) now as the Ministry of Health, Labor and Welfare (MHLW) introduced a 'foods for specified health uses' (FOSHU) system, for the control of such exaggerated and misleading claims. The other reason for such enforcement is due to an increase in the population of elderly people and lifestyle-related diseases that include obesity, diabetes mellitus, high blood pressure, cerebro- and cardiovascular diseases and cancer. In 2001, a new regulatory system, 'foods with health claims' (FHC) with a 'foods with nutrient function claims' (FNFC) system and newly established FOSHU was introduced. In addition, MHLW has changed the existing FOSHU, FNFC and other systems in 2005. Such changes include the new subsystems of FOSHU such as (1) standardized FOSHU, (2) qualified FOSHU and (3) disease risk reduction claims for FOSHU. In the present chapter, two guidelines that require good manufacturing practice (GMP) and self-investigative systems for ensuring the safety of raw materials used for products in the dosage forms such as capsules, tablets, etc. have been discussed. Furthermore, issues related to positioning and definition of supplements are also discussed in the light of the enhancement of understanding the beneficial roles that supplements may play for human health in Japan.

A 48-year-old woman was admitted to our hospital for 7-th line chemotherapy. Repeated treatment with carboplatin (AUC 6, day 1), nab-paclitaxel (100 mg/m2, day 1, 8, 15) were effective in this patient. Chmotherapy with nab-paclitaxel may be effective for patients with multi-recurrent adenocarcinoma.

Abstract

e18564 Background: In non-small cell lung cancer, epidermal growth factor receptor (EGFR) and KRAS are known as driver mutations. However the association between these gene alteration and small cell lung cancer (SCLC) has been unclear. The aim of this study is to investigate the gene status of EGFR and Kras in SCLC. Methods: This retrospective study included 66 SCLC patients with available tumor tissue from primary lung tumor or metastatic and clinical data including survival. We evaluated for the presence of EGFR and KRAS mutation those patients using Scorpion-ARMS method. This study was approved by IRB. Results: Sixty-six patients (52 male,14 female median age 68 range 49-89, LD/ED 34/32, PS0-1/2-3 57/9, smoker/never smoker 64/2) were evaluated. Their median overall survival (OS) were 484 days, progression-free survival (PFS) were 217 days. Sixty-six samples were evaluated EGFR mutation test and 61 were evaluated KRAS mutation test by Scorpion-ARMs methods. SCLC patients with EGFR mutation were the only one patients (1.5%,1/66). KRAS mutation were also the only one patients (1.6%,1/61). A 54 years-old nevere-smoker female was already treated with EGFR tyrosine kinase inhibitor for non-small cell lung cancer, the patients with EGFR mutation had a transformation into SCLC. The KRAS mutation patient was combined SCLC with adenocarcinoma. Conclusions: This study suggested that EGFR and KRAS patients associated with SCLC is poor, these driver mutation is unnecessary for common SCLC.

Abstract

SRPX2 (Sushi repeat-containing protein, X-linked 2) has recently emerged as a multifunctional protein that is involved in seizure disorders, angiogenesis and cellular adhesion. Here, we analyzed this protein biochemically. SRPX2 protein was secreted with a highly post-translational modification. Chondroitinase ABC treatment completely decreased the molecular mass of purified SRPX2 protein to its predicted size, whereas heparitinase, keratanase and hyaluroinidase did not. Secreted SRPX2 protein was also detected using an anti-chondroitin sulfate antibody. These results indicate that SRPX2 is a novel chondroitin sulfate proteoglycan (CSPG). Furthermore, a binding assay revealed that hepatocyte growth factor dose-dependently binds to SRPX2 protein, and a ligand–glycosaminoglycans interaction was speculated to be likely in proteoglycans. Regarding its molecular architecture, SRPX2 has sushi repeat modules similar to four other CSPGs/lecticans; however, the molecular architecture of SRPX2 seems to be quite different from that of the lecticans. Taken together, we found that SRPX2 is a novel CSPG that is overexpressed in gastrointestinal cancer cells. Our findings provide key glycobiological insight into SRPX2 in cancer cells and demonstrate that SRPX2 is a new member of the cancer-related proteoglycan family.

Abstract

Background The hepatocyte growth factor/MET pathway has been shown to cause tumor progression in several types of carcinomas. We reported that c-Met up-regulated topoisomease I (Topo I) in non-small-cell lung cancer cell lines (submitted for publication). The aims of this study were (i) to examine the prognostic influence of c-MET/phospho-MET (p-Met) or Topo I expression and (ii) to elucidate correlation between c-Met/p-Met expression and Topo I expression in small-cell lung cancer (SCLC). Methods This retrospective study included 72 SCLC patients (pts) with available tumor tissue from primary lung tumor or metastatic lesions and clinical data including survival. We carried out immunohistochemistry to detect c-MET/p-MET and Topo I expression. Results Tumor tissues were obtained from 72 SCLC pts. Sixty-six pts, (51 male, 15 female, median age 67.5 range 43–91, LD/ED 38/28, PS0,1/2,3,4 57/9) were evaluated. c-Met overexpression was seen in 40.9%, p-Met in 74.2%, and Topo I expression in 59.1%, respectively. High expression of Topo I associated with lower response rate (RR) (96.7% in low group versus 78.4% in high group, P = 0.029) and shorter progression-free survival (PFS) (65W versus 39W, P = 0.040) but did not correlate with overall survival (OS). Phosphorylation of Met protein did not correlate with RR, PFS or OS. Interestingly, intensity of p-Met significantly correlated with Topo I expression (P = 0.048). High expression of c-Met protein did not associated with RR or PFS but significantly associated with shorter OS (high 53W, low 95W, P = 0.018). Multivariate analysis which included c-Met, stage, PS, and age resulted that c-Met was an independent prognostic factor for OS of pts with SCLC (HR; 2.144, 95% confidence interval 1.162–3.956, P = 0.015). Conclusions This study suggested c-Met high expression was an independent prognostic factor, and Topo I was up-regulated by Met in SCLC.
