Abstract

Background: Schatzker type-4 to 6 tibial plateau fractures most commonly occur when the distal femur is driven through the proximal tibial articular surface. This mechanism of injury can be equivalent to a knee dislocation and carries an increased risk of vascular injury. Our institution screens all Schatzker type-4 to 6 tibial plateau fractures with symmetric pulses for a vascular injury by measuring the ankle-brachial index (ABI). The purpose of this study was to describe our screening protocol and to determine its effectiveness at identifying vascular injuries. Methods: Our screening protocol consisted of measuring the ABI of the injured limb for all Schatzker type-4 to 6 tibial plateau fractures that presented with symmetric pulses. An ABI of ≤0.9 prompted a computed tomographic angiogram (CTA) of the injured extremity. We retrospectively reviewed all Schatzker type-4 to 6 tibial plateau fractures from 2006 to 2023 that presented to a single level-I academic trauma center. We collected demographic, fracture, ABI, and vascular injury data and examined the effectiveness of our screening protocol. Results: We identified 437 Schatzker type-4 to 6 tibial plateau fractures (mean age, 47 years; 59% male). There were 102 (23%) Schatzker type-4 fractures, 4 (1%) type-5 fractures, and 331 (76%) type-6 fractures. Eight fractures (2%) had a concomitant vascular injury; none of the vascular injuries were missed. An ABI of ≤0.9 had a positive predictive value of 0.250, and an ABI of >0.9 had a negative predictive value of 1.000. The sensitivity of the ABI was 1.000, whereas the specificity was 0.056. Thirty-seven fractures were in patients with an ABI of >0.9 at presentation who underwent a CTA of the injured limb for nonorthopaedic indications, with no vascular injuries identified. Conclusions: The ABI has frequently been cited as a valuable screening tool for vascular injury after a knee dislocation, but its application to Schatzker type-4 to 6 tibial plateau fractures had not yet been described. Our results indicate that this screening protocol is both safe and effective, with no missed vascular injuries over a 17-year period. Level of Evidence: Diagnostic Level III. 12. Patient-reported outcomes after tibial plateau fracture: infection confers greatest risk of poor outcome.

Abstract

Introduction: Limited patient-reported outcome measurement (PROM) data exist in peri-articular trauma populations. The purpose of this study was to characterize functional mid-term PROMs and to determine relevant predictors of final PROMs and return to work following tibial plateau fracture. Methods: Patients with a tibial plateau fracture were identified by Current Procedural Terminology (CPT) codes 27535 and 27536. Retrospective chart review was performed to verify diagnosis and collect relevant demographic and perioperative data. PROMIS PF, KOOS Activities of Daily Living (KOOS ADL), PROMIS Preference (PROPr) scores, and return to work were compared between groups. Multivariate regression was used to determine predictors of PROMs and return to work. Results: Two hundred and seven patients had minimum 10-month follow-up (67.9% response rate). There were 114 unicondylar and 93 bicondylar injuries. Twenty patients (9.7%) had a post-operative infection. Body mass index (BMI) (B = −0.22; p = 0.021) and post-operative infection (B = −4.3; p = 0.047) were independent predictors of PROMIS-PF. BMI (B = −0.52, p = 0.008), the presence of an ipsilateral lower extremity injury (B = 10.47, p = 0.038), diabetes (B = −10.60; 0.038), and post-operative infection (B = −10.88; p = 0.014) were independent predictors of KOOS ADL. Final PROMIS-PF, KOOS ADL, and post-operative infection were independent predictors of return to work. In a subgroup analysis, infection was associated with markedly lower rate of return to work (39% vs 81%, p = 0.001), PROMIS-PF score (39.8.4 ± 8.6 vs 46.7 ± 8.6; p = 0.002), KOOS ADL score (67.3 ± 25.3 vs 84.6 ± 17.5; p = 0.003), and PROPr score (0.33 ± 0.30 vs 0.52 ± 0.20; p = 0.003). Conclusion: After operative treatment of tibial plateau fractures, post-operative infection results in large decreases in PROMs and rates of return to work that persist at mid-term follow-up. It is often assumed that the exchange is one-sided and we hope to dispel that assumption. Despite the logistical challenges inherent in these partnerships, our experience has been unanimously reviewed as "greatly beneficial" to visiting residents/faculty and surgeons/trainees at the host location. We hope that this article will (1) encourage faculty at training programs to permit and enable residents to experience international orthopaedics while still in training; (2) encourage faculty to visit international hospitals while contributing expertise in subspecialty surgery, research, and teaching; and (3) encourage international hospitals to create opportunities for clinical and research collaboration with academic orthopaedics departments.

Abstract

Patients with tibial pilon fractures have a higher incidence of post-traumatic osteoarthritis than those with fractures of the tibial plateau. This may indicate that pilon fractures present a greater mechanical insult to the joint than do plateau fractures. We tested the hypothesis that fracture energy and articular fracture edge length, two independent indicators of severity, are higher in pilon than plateau fractures. We also evaluated whether clinical fracture classification systems accurately reflect severity. Seventy-five tibial plateau fractures and 52 tibial pilon fractures from a multi-institutional study were selected to span the spectrum of severity. Fracture severity measures were calculated using objective CT-based image analysis methods. The ranges of fracture energies measured for tibial plateau and pilon fractures were 3.2-33.2 Joules (J) and 3.6-32.2 J, respectively, and articular fracture edge lengths were 68.0-493.0 mm and 56.1-288.6 mm, respectively. Abstract

ObjectivesDetermine the agreement between subjective assessments of fracture severity and an objective computed tomography (CT)-based metric of fracture energy in tibial plateau fractures.MethodsSix fellowship-trained orthopaedic trauma surgeons independently rank-ordered 20 tibial plateau fractures in terms of severity based on anteroposterior and lateral knee radiographs. A CT-based image analysis methodology was used to quantify the fracture energy, and agreement between the surgeons' severity rankings and the fracture energy metric was tested by computing their concordance, a statistical measure that estimates the probability that any 2 cases would be ranked with the same ordering by 2 different raters or methods.ResultsConcordance between the 6 orthopaedic surgeons ranged from 82% to 93%, and concordance between surgeon severity rankings and the computed fracture energy ranged from 73% to 78%.ConclusionsThere is a high level of agreement between experienced surgeons in their assessments of tibial plateau fracture severity, and a slightly lower agreement between the surgeon assessments and an objective CT-based metric of fracture energy. Taken together, these results suggest that experienced surgeons share a similar understanding of what makes a tibial plateau fracture more or less severe, and an objective CT-based metric of fracture energy captures much but not all of that information. Further research is ongoing to characterize the relationship between surgeon assessments of severity, fracture energy, and the eventual clinical outcomes for patients with fractures of the tibial plateau.
