We investigated fluid shifts and regulatory responses to variations of posture, exercise, Gz level and radius of rotation in subjects riding NASA Ames 20G centrifuge. Results are from 4 protocols that address radius and exercise effects only. Protocol A: After 10 min supine control, 12 healthy men (35 plus or minus 9 yr, 82.8 plus or minus 7.9 kg) were exposed to rotational 1 Gz (2.5 m radius) for 2 min followed by 20 min alternating between 1 and 1.25 Gz. Blood samples were taken pre and post spin. Protocol B: Same as A, but lower limb exercise (70% V02max) preceded ramps to 1.25 Gz. Protocol C: Same as A but radius of rotation 8.3 m. Protocol D: Same as B but at 8.3 m. The 8 subjects who completed all protocols, increased heart rate (HR) from control, on average, by: A: 5, B: 39, C: 11, D: 44 bpm. For thoracic fluid volume, (bioimpedance), the 8 subjects changed from control, on average: A: -394, B: -548, C: -537, D: -708 mL. For thigh fluid volume, changes from control, on average, were: A: -137, B: 129, C: -75, D: 159 mL. Hematocrit changes from control were: A: 2.3, B: 3.5, C: 2.3, D: 4.3 %. Radius effects were mild and included greater loss of fluid from the thorax, less fluid loss from the thigh and increased heart rate at the longer radius. Pre-acceleration exercise effects were more dramatic and included additional loss of fluid from the chest, increased fluid volume of the thigh, increased hematocrit and greater heart rate increases. We propose that short bouts of intense exercise can be used to magnify the cardiovascular stress delivered by artificial gravity (AG) training and the combination of AG with exercise training can be fine-tuned to preserve orthostatic tolerance of astronauts during spaceflight.

For thoracic fluid volume, (bioimpedance), the 8 subjects changed from control: A: -394, B: -548, C: -537, D: -708 mL; and the 4: A: -516, B: -652, C: -583, D: -1263 mL. The 4 subjects lost more thoracic fluid volume than the 8, especially in protocol D. A slightly greater increase in HR for the 4 compared to the 8 was not adequate to maintain cardiac output during D. Our data support the concept that thoracic impedance can detect inability to return adequate fluid to the heart, thereby predicting presyncope.

Dissociation between beverage sodium [Na(+)] and osmotic [Osm] concentrations for increasing plasma volume (PV, hypervolemia) appears to refute the high theoretical correlation between plasma [pNa(+)] and [pOsm].

To investigate the possible effect of hypobaria on ventilation (VE) at high altitude, we exposed nine men to three conditions for 10 h in a chamber on separate occasions at least 1 week apart. These three conditions were: altitude (PB = 432, FIO2 = 0.207), normobaric hypoxia (PB = 614, FIO2 = 0.142) and normoxic hypobaria (PB = 434, FIO2 = 0.296). In addition, post-test measurements were made 2 h after returning to ambient conditions at normobaric normoxia (PB = 636, FIO2 = 0.204). In the first hour of exposure VE was increased similarly by altitude and normobaric hypoxia. The was 38% above post-test values and end-tidal CO2 (PET(CO2) was lower by 4 mmHg. After 3, 6 and 9 h, the average VE in normobaric hypoxia was 26% higher than at altitude (p < 0.01), resulting primarily from a decline in VE at altitude. The difference between altitude and normobaric hypoxia was greatest at 3 h (+ 39%). In spite of the higher VE during normobaric hypoxia, the PET(CO2) was higher than at altitude. Changes in VE and PET(CO2) in normoxic hypobaria were minimal relative to normobaric normoxia post-test measurements. One possible explanation for the lower VE at altitude is that CO2 elimination is relatively less at altitude because of a reduction in inspired gas density compared to normobaric hypoxia; this may reduce the work of breathing or alveolar deadspace. The greater VE during the first hour at altitude, relative to subsequent measurements, may be related to the appearance of microbubbles in the pulmonary circulation acting to transiently worsen matching. Results indicate that hypobaria per se effects ventilation under altitude conditions.

The mechanism for reduction of total body water and plasma volume (PV) during initial exposure to acute attitude (ALT) is not clear. Ten man (25+/-SD 3yr. 1.99+/-0.l9sq m) were confined (sitting) for 12 hr in a chamber a 2800m ALT or at 305m(ground). They ate a controlled breakfast (450 kcal + 3ml/kg H2O) on the ground, and lunch and dinner at ALT for a total daily intake of 2,850 kcal (14%PRO, 67%CHO, 16%FAT, 26gNaCl). At hr 10 they consumed fluid-electrolyte drinks or water (12ml/kg, 948ml/day) with sessions at weekly intervals: (a) 185mEq/L Na, 283mOsm/kg; (b) 21.6Na, 365mOsm: and (c) water at ALT: and (d) water on the ground. After 10 hr at ALT%(Delta)PV(Hb-Hct) decreased (P<0.05) by (a) 9.0+/-SE1.5%, (b) 6.2+/-1.7%, (c) 7.4+/-2.2%. and (d) 9.0+/-2.4%. respectively. After drinking at 1200 hr PV increased by (a) 8.3% (P<0.05), (b) 2.8% (NS), (c) -0.9% (NS). and (d) by 0.8% (NS). respectively. The ground-induced hypovolemia suggests a confinement rather than an ALT effect. The increase in PV after the NaCl-NaCitrate drink (a) indicates that drink composition is mom important than as Osm for restoring PV in these Conditions.

A report presents data on the effectiveness of each of six rehydration fluids in restoring total body water and plasma volume in human subjects during rest and exercise. One of the six fluids was water sweetened with aspartame: the others were water containing various amounts of sodium chloride and/or sodium citrate, plus various amounts of aspartame and/or other carbohydrates. In one experiment, five men who had previously dehydrated themselves for 24 hours drank one of the rehydration fluids, then sat for 70 minutes. Pretest plasma volumes were measured and changes in plasma volumes were calculated. This procedure was repeated at weekly intervals until all six rehydration fluids had been tested. Another similar experiment involved four men who exercised on a cycle ergometer for 70 minutes in the supine position after drinking the fluids.

We measured blood density (BD). plasma density (PD) and hematocrit (Ht) during repeated 70 deg head-up tilt (HUT) consisting of a pre-drink period with two supine (P and P3) and two HUT (P2 and P4) phases of 45 min each. At the end of P4 test subjects (N=8) drank 10 ml/kg body weight of isotonic (290 mOsm/kg) sodium chloride (ISO) or hypotonic (<10 mOsm/kg) unsweetened us (HYP), or nothing (control: CON). The following periods continued the supine (P5, P7) / upright (P6) sequence. Mass density of plasma and whole blood using a "mechanical oscillation" technique were measured from ear lobe blood. Tilting altered (p<0.001) BD, PD, and Ht in all consecutive Phases. BD and PD were different (p<0.05) between CON. HYP. and ISO Phases 6 and 7. The computed mass density of fluid (FD). which moved between intra. and extravascular compartments. averaged 1008 g/l and was the same in any conditions. In the non-drink treatment (P3, P5, P7), supine PV steadly decreased when compared to P1. Tilt-induced PV shifts ranged from 9.7 to 16.7% when compared to PV du. ring the respective previous Phases. After drinking, PV increased above CON values at the end of Phase 7 by 12.9% with ISO, and by 6.6% with HYP. Progressive hemoconcentration. occurred during non-drink supine; isotonic saline ingestion increased supine PV a control level but did not stop or reverse the decrease of upright hemoconcentration. Upright density values were not slowed with repeated upright tilting; decreased fluid loss occurred in consecutive upright periods. Upright capillary pressure balance was achieved with diminished plasma volume loss during repeated tilting.

The purpose of this study was to formulate and to evaluate rehydration drinks, which would restore total body water and plasma volume (PV), for astronauts to consume before and during extravehicular activity, a few hours before reentry, and immediately after landing. In the first experiment (rest, sitting), five healthy men (23-41 yr), previously dehydrated for 24 hr., drank six (1a, 2, 4, 5, 6, 7) fluid formulations (one each at weekly intervals) and then sat for 70 min. Pre-test PV were measured with Evans blue dye and changes in PV were calculated with the hematocrit-hemoglobin transformation equation. This rest experiment simulated hypohydrated astronauts preparing for reentry. The second experiment (exercise, supine) followed the same protocol except four healthy men (30-46 yr) worked for 70 min. in the supine position on a cycle ergometer at a mean load of 71+/-1 percent of their peak aerobic work capacity. This exercise experiment simulated conditions for astronauts with reduced total body water engaging in extravehicular activity.

The effect of the lower-body positive pressure (LBPP) on the orthostatic fluid and protein shifts were investigated in five men during combined tilt-table/antigravity suit inflation and deflation experiments. Changes in the mass densities of venous blood and plasma were measured and the values were used to calculate the densities of erythrocytes, whole-body blood, and shifted fluid. It was found that the application of 60 mm Hg LBPP during 60-deg head-up tilt prevented about half of the postural hemoconcentration occurring during passive head-up tilt.

Use of on-line high-precision mass densitometry for the continuous monitoring of blood volume changes in humans was demonstrated by recording short-term blood volume alterations produced by changes in body position. The mass density of antecubital venous blood was measured continuously for 80 min per session with 0.1 g/l precision at a flow rate of 1.5 ml/min. Additional discrete plasma density and hematocrit measurements gave linear relations between all possible combinations of blood density, plasma density, and hematocrit. Transient filtration phenomena were revealed that are not amenable to discontinuous measurements.

The thermal expansion of human blood, plasma, ultrafiltrate, and erythrocycte concentration at temperatures in the range of 4-48 C is studied. The mechanical oscillator technique which has an accuracy of 1 x 10 to the -5 th g/ml is utilized to measure fluid density. The relationship between thermal expansion, density, and temperature is analyzed. The study reveals that: (1) thermal expansion increases with increasing temperature; (2) the magnitude of the increase declines with increasing temperature; (3) thermal expansion increases with density at temperatures below 40 C; and (4) the thermal expansion of intracellular fluid is greater than that of extracellular fluid in the temperature range of 4-10 C, but it is equal at temperatures greater than or equal to 40 C.

The effects of head-up, one-step, slow-graded, and rapid-graded tilt positions on blood density (BD), plasma density (PD), and erythrocyte density (ED) in male and female subjects are evaluated using a mass density detection method. The preparation of the samples and procedures for density and hematocrit measurements are described. The formula for calculating the density and volume of fluid shifted between intravascular and extravascular compartments is derived. The data reveal that ED does not change with varying tilt positions; however, PD and BD increase linearly with increasing angles of tilt, thereby resembling the time course of hemodilution and hemoconcentration.

Rapid fluid shifts in four pigs and two dogs subjected to graded hemorrhage are investigated. Arterial blood density (BD), mean arterial pressure (MAP), central venous pressure (CVP), arterial plasma density (PD), hematocrit (Hct) and erythrocyte density were measured. The apparatus and mechancial oscillator technique for measuring density are described. Fluid shifts between red blood cells and blood plasma and alterations in the whole-body-to-large vessel Hct, F(cell) are studied using two models. The bases of the model calculations are discussed. A decrease in MAP, CVP, and BP is detected at the beginning of hemorrhaging; continued bleeding results in further BD decrease correlating with volume displacement. The data reveal that at 15 ml/kg blood loss the mean PD and BD dropped by 0.99 + or - 0.15 and 2.42 + or 0.26 g/liter, respectively, and the Hct dropped by 2.40 + or 0.47 units. The data reveal that inward-shifted fluid has a higher density than normal ultrafiltrate and/or there is a rise in the F(cell) ratio. It is noted that rapid fluid replacement ranged from 5.8 + or - 0.8 to 10.6 + or - 2.0 percent of the initial plasma volume.

The reliability of the mechanical oscillator technique for blood and plasma density measurements on samples of volumes less than 0.1 ml is examined, and a precision of 0.001 g/l is found if plasma-isodensic heparin solution and siliconized densitometers are employed. Sources of measurement errors in the density determinations include storage of plasma samples, inhomogeneity of blood samples, and density reading before adequate temperature equilibration. In tests of plasma sample storage, the best reproducibility was obtained with samples kept at 4 C. Linear correlations were found between plasma density and plasma protein concentration, blood density and blood hemoglobin concentration, and erythrocyte density and MCHC.

Methods for measuring absolute levels and changes in plasma volume are presented along with derivations of pertinent equations. Reduction in variability of the Evans blue dye dilution technique using chromatographic column purification suggests that the day-to-day variability in the plasma volume in humans is less than + or - 20 m1. Mass density determination using the mechanical-oscillator technique provides a method for measuring vascular fluid shifts continuously for assessing the density of the filtrate, and for quantifying movements of protein across microvascular walls. Equations for the calculation of volume and density of shifted fluid are presented.
