The present study aims to demonstrate how biomodels can be used as teaching tools for surgical techniques and training in a medical residency service. A case series was carried out in our orthopedics and traumatology outpatient facility using three-dimensional (3D) printing for surgical planning to contribute to the surgical teaching and training of resident physicians. Two cases were selected as examples in the present article. Biomodels enable a better understanding of the surgery by the surgical team and residents, reducing the surgical time and the risks for the patients. These models can be a good teaching method to plan reconstructions of total hip arthroplasties, evaluate and predict surgical difficulties, and optimize procedures. Resumo O objetivo deste trabalho é demonstrar como os biomodelos podem servir como ferramenta de ensino para o esclarecimento de técnicas cirúrgicas e o treinamento em um serviço de residência médica. Foram realizadas séries de casos em nosso ambulatório de ortopedia e traumatologia, nos quais o uso da prototipagem para o planejamento cirúrgico contribuiu para o ensino cirúrgico e treinamento de médicos residentes. Dois casos foram selecionados como exemplo neste artigo. O uso de biomodelos permite um melhor entendimento da cirurgia pela equipe cirúrgica e pelos residentes, e diminui o tempo e os riscos cirúrgicos aos pacientes. O uso de biomodelos no planejamento de reconstruções da artroplastia total do quadril pode servir como um bom método de ensino, para a avaliação e previsão das dificuldades no momento da cirurgia, e para otimizar os procedimentos.

Vascular complications in the surgical treatment of hip fractures are rare. Depending on the arterial injury, severe intraoperative bleeding or a subacute hematoma formation with arterial pseudoaneurysm development can occur. In the literature, the more frequently described complications are large local hematomas after osteosynthesis with sliding hip screws. This report shows a case of delayed arterial injury after proximal femur osteosynthesis. Resumo Complicações vasculares no tratamento cirúrgico da fratura do quadril são raras. A depender da lesão arterial, pode ocorrer um grave sangramento intraoperatório ou formação de hematoma subagudo comdesenvolvimento de pseudoaneurisma arterial. Na literatura, as complicações mais frequentes relatadas são a formação de grandes hematomas locais após osteossíntese com parafuso deslizante do quadril. O objetivo do presente relato foi demonstrar um caso de lesão arterial tardia após osteossíntese proximal do fêmur.

OBJECTIVE: The Hip Function Recovery Score questionnaire is an instrument that was developed for evaluating the functional recovery of patients over 60 years of age who undergo surgical treatment due to hip fractures. The objective of this study was to make a thorough transcultural translation of this questionnaire, with adaptation to the individual and cultural traits of the Brazilian population. METHOD: This translation method consisted of initial translation, back translation, drafting of a consensual version and pretesting with comments invited. Subsequently, a final version was drawn up after making the necessary adjustments, without altering the semantics of the questions in the original text. RESULTS: The consensual version was applied to thirty patients over the age of 60 years who had undergone surgical treatment due to hip fractures. However, some difficulties in understanding some words and expressions were observed, and these were then replaced with terms that were more easily understood. After the final version had been drawn up, this was applied to the same patients and full understanding was achieved among some of them, without altering the semantics of the questions of the original text. CONCLUSION: The transcultural translation of the Hip Function Recovery Score will have an immediate impact on functional evaluations on patients over 60 years of age who underwent surgery due to hip fracture. It will subsequently be possible for other Brazilian scientific studies to use this questionnaire, which has been standardized and adapted to Brazilian culture, in order to make comparisons between results, thereby enriching Brazilian scientific production. RESUMO OBJETIVO: O questionário Hip Function Recovery Score consiste em um instrumento desenvolvido para avaliação da recuperação funcional de pacientes acima de 60 anos submetidos a tratamento cirúrgico devido a fraturas do quadril. O objetivo deste estudo foi fazer a tradução transcultural de forma criteriosa do questionário e adaptá-lo às características individuais e culturais da população brasileira. MÉTODO: Este método consiste em tradução inicial, retrotradução, elaboração de uma versão de consenso e pré-teste comentado com posterior elaboração de uma versão final após as alterações necessárias sem modificar a semântica das perguntas do texto original. RESULTADOS: A versão de consenso foi aplicada em 30 pacientes, acima de 60 anos, que foram submetidos a tratamento cirúrgico devido a fraturas do quadril. Foram observadas, entretanto, algumas dificuldades no entendimento de algumas palavras e expressões, as quais foram substituídas por termos de mais fácil entendimento. Após a elaboração da versão final, reaplicou-se essa versão aos mesmos pacientes e obteve-se um entendimento total. CONCLUSÃO: A tradução transcultural do questionário terá impacto imediato na avaliação funcional dos pacientes com mais de 60 anos operados devido a fraturas do quadril e, posteriormente, outros trabalhos nacionais podem usar esse questionário padronizado e adaptado à nossa cultura para comparação de resultados e enriquecer a produção científica do Brasil.

Hip arthroscopy is a safe method for treating a variety of pathological conditions that were unknown until a decade ago. Femoroacetabular impingement is the commonest of these pathological conditions and the one with the best results when treated early on. The instruments and surgical technique for hip arthroscopy continue to evolve. New indications for hip arthroscopy has been studied as the ligamentum teres injuries, capsular repair in instabilities, dissection of the sciatic nerve and repair of gluteal muscles tears (injuries to the hip rotator cuff), although still with debatable reproducibility. The complication rate is low, and ever-better results with fewer complications should be expected with the progression of the learning curve.A artroscopia de quadril é um método seguro para o tratamento de diversas patologias desconhecidas até a última década. O impacto femoroacetabular é a patologia mais comum e com melhores resultados quando tratada precocemente. O instrumental e a técnica cirúrgica da artroscopia de quadril continuam em evolução. ResumoObjetivoInvestigar o efeito da artroplastia total do quadril (ATQ) no índice de massa corporal em relação ao pré e ao pós‐operatório.MétodosForam analisados retrospectivamente 100 pacientes submetidos à ATQ. Os pacientes foram estratificados pelo índice de massa corporal (IMC), conforme proposto pela Organização Mundial de Saúde (OMS).ResultadosForam observados 48 pacientes do sexo masculino e 52 do feminino. A média de idade foi de 63,8±13,5 anos. O seguimento médio foi de 24,6±0,6 meses. Os homens apresentaram IMC pré‐operatório médio de 28,4±3,6kg/m2 e as mulheres, de 27,5±5,0kg/m2. O IMC médio pós‐operatório foi 28,9±0,7kg/m2 para os homens e 27,8±0,7kg/m2 para as mulheres. Ocorreu uma média de aumento geral do IMC em 0,4kg/m2. O IMC aumentou em pacientes com peso normal e com sobrepeso, mas diminuiu levemente em pacientes com obesidade. A maioria dos pacientes (73%) permaneceu com o IMC inalterado.ConclusãoA melhoria da mobilidade obtida com a ATQ não promoveu uma redução das medidas antropométricas na maioria dos pacientes.AbstractObjectiveTo investigate the effect of total hip arthroplasty (THA) on body mass index (BMI), from before to after the operation.Methods100 patients who underwent THA were retrospectively analyzed. The BMI increased both in patients with normal weight and in those who were overweight, but it decreased slightly in patients who were obese. BMI remained unchanged in the majority of the patients (73%).ConclusionThe improvement in mobility achieved through THA did not promote any reduction in anthropometric measurements in the majority of the patients.

Hip arthroscopy is an evolving surgical technique that has recently increased in popularity.Although femoroacetabular impingement was an important launch pad for this technique, extra-articular pathology has been described through hip endoscopy.Good clinical results in the medium term will allow improvements in this technique and increase its indications. It has been advocated that total hip arthroplasty (THA) is probably the most successful surgical intervention performed in Medicine. In the 1960s, Sir John Charnley not only introduced, but also modified and improved the technique of cemented arthroplasties. The concepts on biological , fixation established by Pillar and Galante served as the foundation for the development of uncemented implants that are now used worldwide. Currently, THA is a worldwide widespread surgery performed on millions of people. However, keeping abreast of the large number of information available on these procedures, especially on implant fixation, designs, different tribological pairings, and the long-term results can be challenging at times. This article is a brief update on the main aspects of THA. RESUMO A artroplastia total do quadril (ATQ) é uma das cirurgias de maior sucesso na história da medicina. Nos anos 1960,SirJohn Charnley introduziu e aperfeiçoou as artroplastias cimentadas. Pillar e Galante estabeleceram os conceitos da fixação biológica, base para o desenvolvimento das artroplastias não cimentadas. Atualmente, a ATQ é uma cirurgia mundialmente difundida e feita em milhões de pessoas. No entanto, o grande número de informações disponíveis sobre as artroplastias, especialmente quanto à forma de fixação do implante ao osso, aos diferentesdesignsdas hastes e dos acetábulos, aos diferentes pares tribológicos e aos resultados no longo prazo por vezes dificulta a tomada de decisão. Este artigo é uma breve atualização sobre os principais aspectos da ATQ.

Objective: The objective of this paper is to measure the volume occupied by the most widely used internal fixation devices for treating femoral neck fractures, using the first 30, 40 and 50 mm of insertion of each screw as an approximation. The study aimed to observe which of these implants caused least bone aggression. Methods: Five types of cannulated screws and four types of dynamic hip screws (DHS) available on the Brazilian market were evaluated in terms of volume differences through water displacement. Results: Fixation with two cannulated screws presented significantly less volume than shown by DHS, for insertions of 30, 40 and 50 mm (p=0.01, 0.012 and 0.013, respectively), fixation with three screws did not show any statistically significant difference (p= 0.123, 0.08 and 0.381, respectively) and fixation with four cannulated screws presented larger volumes than shown by DHS (p=0.072, 0.161 and 0.033). Conclusions: Fixation of the femoral neck with two cannulated screws occupied less volume than DHS, with a statistically significant difference. The majority of screw combinations did not reach statistical significance, although fixation with four cannulated screws presented larger volumes on average than those occupied by DHS.

Objective: To analyze the clinical and radiological outcomes from total hip arthroplasty using the Biomec prosthesis over a 20-year period (1988 to 2009). Methods: The patients were evaluated clinically in accordance with the Merle d'Aubigne and Postel functional classification, and underwent anteroposterior (AP) radiography of the pelvis and the operated joint in lateral view. The variables evaluated were acetabular and femoral osteolysis, polyethylene wear, nail migration, follow-up and clinical outcomes. Results: The mean follow-up was 10.6 years (range 4 to 21 years). The sample consisted of 84 patients (32 men and 52 women) and the mean preoperative age was 47 years. We found that 88% of the results from the clinical evaluation were good or excellent. In relation to the radiological findings, 92.5% of the results were good. The mean wear on the polyethylene was 1.4 mm. Conclusion: The use of the Biomec prosthesis in this cohort showed good clinical and radiographic results.

OBJECTIVE : The purpose of this study was to evaluate the performance of valgus intertrochanteric osteotomy in femoral neck non-union. METHODS : Forty-two patients with femoral neck fractures with non-union treated using Pauwels' intertrochanteric osteotomy were reviewed. Demographics, time elapsed between fracture and surgery, follow--up, osteosynthesis used, Garden's classification, limb shortening, and x-rays were evaluated. RESULTS : Twenty-two men and 20 women were reviewed. The youngest patient was 18 years old and the oldest 65 years old, with a mean age of 42.4 years (±11.2). The minimum follow-up was 2 years, with a mean of 10.2 years. The average time elapsed between initial fracture and osteotomy was 6.5 months. Twel-ve cases were neglected femoral neck fractures. Nineteen patients were classified as Garden III, and 23 patients as Garden IV. After valgus osteotomy, non-union healing was observed in 38 patients (38/42; 90.4%). Healing of thirty-seven cases of pseudoarthrosis were obtained after the first-attempt osteotomy, and one case required two operations for healing. The osteotomy failed in four cases. Conside-ring the healed osteotomies, good to excellent functional results were achieved in 80.9% (34/42) of the patients. Total hip replacement was subsequently performed in 14.2% (6/42) of the patients for unfavoura-ble outcomes (two for cutting out, two for osteonecrosis, and two for osteoarthritis). CONCLUSIONS : Valgus intertrochanteric osteotomy has a high success rate in archiving healing in femoral neck non-union with good functional results. It is a biological and effective method. Objective: To evaluate the bleeding of the femoral head on hip osteoarthritis in patients who underwent total hip arthroplasty. Methods: One hundred and three hips affected by primary hip osteoarthritis were evaluated. After surgical dislocation, the femoral head was divided into four quadrants, and micro perforations were made in order to observe and assess the presence of bleeding, as early type (EB), late type (LB) or without bleeding (WB). Results: We observed early bleeding (EB) in the upper quadrant in 16 hips (15.5%), late bleeding in 14 hips (13.6%) and no bleeding (WB) in 73 hips (70.9%). The anterior quadrant showed EB in 24 hips (23.3%), LB in 7 hips (6.8%) and WB in 72 hips (69.9%). The lower quadrant presented EB in 40 hips (38.8%), LB 14 hips (13.6%) and WB in 49 hips (47.6%). The posterior quadrant showed EB in 39 hips (37.9%), LB 19 hips (18.4%) and WB in 45 hips (43.7%). Comparing BMI and gender, we found no association between these parameters (p> 0.05). Conclusions: The inferior and posterior quadrant had the highest bleeding levels, following the path of the medial circumflex artery. Level of Evidence III, Therapeutic Study.

Objectives: We evaluated the early results of arthroscopic debridement and lavage procedure in osteoarthritis of the hip.Methods: The hips of 13 patients (7 women, 6 men; mean age 52 years; range 29 to 74 years) were treated by arthroscopic debridement and lavage in lateral position under general anesthesia. Ten patients had primary coxarthrosis; it was due to acetabular dysplasia in two patients, and to avascular necrosis of the femoral head in one patient. The patients were radiographically and arthroscopically evaluated according to the Kellgren-Lawrence grading system and the Tippett’s chondropathy classification, respectively. Preoperative and postoperative assessments of the hips were made according to the Harris hip scoring system. The mean follow-up was 11.2 months (range 5 to 22 months).Results: Harris hip scores increased in eight patients (%62) postoperatively, namely, in only one patient out of six with a radiographically high grade of osteoarthritis (grade 4), and in all seven patients with grade 2 or 3 disease. No complications occurred. Significant negative correlations were found between radiographic and arthroscopic grades and the clinical improvement (p=0.001). However, patients age and preoperative Harris hip score were not correlated with the clinical result.Conclusion: Arthroscopic debridement and lavage provides significant clinical improvement during short-term follow-up of osteoarthritic hips with radiographic grades of 3 or less., Amaç: Kalça osteoartritinde, kalça artroskopisi ile uygulanan debridman ve eklem yıkanması işleminin erken klinik sonuçları değerlendirildi.Çalışma planı: On üç hastanın (7 kadın, 6 erkek; ort. yaş 52; dağılım 29-74) kalçasına genel anestezi altında ve hasta yan yatar pozisyonda artroskopik debridman ve eklem yıkanması uygulandı. On hastada primer osteoartrit vardı. Osteoartrit bir hastada femur başı avasküler nekrozuna, iki hastada ise asetabular displaziye bağlıydı. Hastalar radyografik olarak Kellgren-Lawrence osteoartrit evreleme sistemine, artroskopik olarak Tippett kondropati sınıflandırmasına göre değerlendirildi. Ameliyat öncesi ve sonrasındaki klinik değerlendirilmeler Harris kalça skoru ile yapıldı. Ortalama izlem süresi 11.2 ay (dağılım 5-22 ay) idi.Sonuçlar: Sekiz hastada (%62) Harris kalça skorunda artış saptandı. Kalça skorları, radyografik evresisi 4 olan altı hastanın yalnızca birinde, evre 2 ve 3 olan yedi hastada ise çeşitli oranlarda artış gösterdi. Hiçbir hastada komplikasyon gelişmedi. Hem radyografik hem de artroskopik evreler ile ameliyat sonrası klinik düzelme arasında anlamlı bir ters korelasyon saptandı (p=0.001). Hastanın yaşı ve ameliyat öncesi Harris skorunun klinik iyileşme üzerine etkili olmadığı görüldü.Çıkarımlar: Radyografik olarak evre 3 ve öncesi dönemdeki kalça artrozunda uygulanan artroskopik debridman ve eklem yıkanması, ameliyat sonrası erken dönemde anlamlı derecede semptomatik iyileşme sağlamaktadır.

Objectives: We evaluated total hip arthroplasty in patients with high-riding developmental dysplasia of the hip with regard to surgical method and modifications, problems encountered during surgery, and follow-up results.Methods: A total of 42 hips (22 Crowe type III, 20 type IV) of 31 patients (29 women, 2 men; mean age 45.7 years; range 26-70 years) who underwent total hip arthroplasty were clinically and radiologically evaluated. In all cases, cementless acetabular components were placed in the true acetabulum. Fifteen acetabula required structural autografts. Cementless stems were used on the femoral side. Proximal metaphyseal step-cut osteotomy was performed in patients with high-riding hips of more than 4 cm. The mean follow-up period was 54.7 months (range 11 to 127 months).Results: The mean Harris hip score increased from 37.6 preoperatively to 91.02 at the end of follow-up. No acetabular autograft resorption or non-union were observed. Non-union and delayed union in the femoral osteotomy site were encountered in two patients, respectively. Two components (1 acetabular, 1 femoral) were revised during the follow up period. Two sciatic and two femoral temporary nerve palsies occurred. One patient developed superficial infection and was treated by debridement and antibiotics. Brooker type I heterotopic ossification was observed in seven patients.Conclusion: Total hip arthroplasty proved successful in high riding developmental dysplasia of the hip, Amaç: Yüksekte doğuştan kalça çıkığı nedeniyle total kalça artroplastisi uygulanan olgularda yöntem ve modifikasyonlar, karşılaşılan sorunlar ve klinik sonuçlar değerlendirildi.Çalışma planı: Otuz bir hastanın (29 kadın, 2 erkek; ort yaş 45.7; dağılım 26-70) toplam 42 kalçasına (22 Crowe tip III, 20 tip IV) uygulanan total kalça artroplastileri, klinik ve radyolojik olarak değerlendirildi. Tüm hastalarda gerçek asetabuluma yerleştirilen çimentosuz asetabuler komponent kullanıldı. On beş kalçada asetabulum, otojen yapısal allogreft ile desteklendi. Femoral tarafta da çimentosuz protez uygulandı; femurun 4 cm’den fazla distale indirilmesini gerektiren olgularda, femur proksimal metafiz basamaklı kısaltma osteotomisi uygulandı. Olgular ortalama 54.7 ay (dağılım 11-127 ay) süreyle izlendi.Sonuçlar: Ameliyat öncesinde 37.6 olan Harris kalça skoru, izlem sonunda 91.02’ye yükseldi. Kullanılan otogreftlerin tümünde kaynama sağlandı, greft rezorbsiyonu görülmedi. Femoral kısaltma osteotomisi bölgesinde bir hastada kaynamama, bir hastada geç kaynama gözlendi. Bir hastada asetabuler komponent, bir hastada ise femoral komponent revizyonuna gerek duyuldu. İki hastada geçici siyatik sinir, iki hastada geçici femoral sinir paralizisi gözlendi. Bir hastadaki yüzeyel enfeksiyon, yara debridmanı ve antibiyoterapi ile kontrol altına alındı. Yedi hastada Brooker tip I heterotopik ossifikasyon saptandı.Çıkarımlar: Yüksekte kalça çıkığı zemininde total kalça artroplastisi klinik olarak başarılı bir cerrahi girişimdir.

Objective To describe, in a cadaver study, the anatomical arrangement of the proximal third medial femoral circumflex artery (MFCA). Methods We evaluated the anatomic arrangement of the proximal third of the MFCA through dissection in 12 cadaver hips. After measuring their height in meters (M), several parameters were determined: angle of lateralization of MFCA (Â), Depth MFCA in relation to the superomedial origin of the quadratus femoris muscle (P), and distance of MFCA from the superolateral insertion of the muscle quadratus femoris (D). Results In the comparison of mean parameters between the sexes, it was noted that: Male  = 43.6 degrees, female  = 38.3 degrees. Male D = 6mm, female D = 9.5mm. Male P = 20.8 mm, female P = 18.3mm. The average for parameters A and P were lower in females, but parameter D was higher in females. Conclusion We should use, as the secure surface parameter for the proximal third of the MFCA, the superolateral insertion point of the quadratus femoris. The fact that the parameter D of MFCA is greater in females than in males. Disconnecting the quadratus femoris at its point of origin in the isquio may offer greater security to MFCA than doing it at the point of femoral insertion. Level of Evidence.

OBJETIVO: Avaliar o desfecho clínico e radiográfico da artroplastia total de quadril (ATQ) com prótese Biomec em um período de 20 anos (1988 a 2008). MÉTODOS: Os pacientes foram avaliados clinicamente segundo a classificação funcional de Merle d'Aubigné e Postel e submetidos à radiografia de bacia em anteroposterior (AP) e da articulação operada em perfil. As variáveis avaliadas foram osteólise acetabular e femoral, desgaste do polietileno, migração da haste, seguimento e desfecho clínico. RESULTADOS: O seguimento médio foi de 10,6 anos (4 a 20 anos); a amostra foi de 84 pacientes, sendo 32 do sexo masculino e 52, do feminino; a idade média no pré-operatório foi de 47 anos; obtivemos 88% de resultados bons e muito bons na avaliação clínica. Com relação aos achados radiográficos obtivemos 92,5% de bons resultados. O desgaste médio do polietileno foi de 1,4mm. CONCLUSÃO: A utilização da prótese Biomec nesta coorte apresentou bons resultados clínicos e radiográficos. OBJECTIVE: To analyze the clinical and radiological outcomes of total hip arthroplasty with Biomec prosthesis over a 20-year period (1988 to 2009). METHODS: The patients were evaluated clinically according to the Merle d'Aubigné and Postel functional classification, and submitted to anteroposterior (AP) radiography of the spleen and operated joint in profile. The variables evaluated were acetabular and femoral osteopenia, polyethylene wear, stem migration, follow-up and clinical outcomes. RESULTS: The mean follow-up was 10.6 years (4 to 21years), the sample consisted of 84 patients; 32 male and 52 female; the mean preoperative age was 47 years; we obtained 88% good or excellent results in the clinical evaluation. In relation to the radiological findings, we obtained 92.5% good results. The mean polyethylene wear was 1.4 mm. CONCLUSION: The use of the Biomec prosthesis in this cohort showed good clinical and radiographic results.

ResumoObjetivoO questionário Hip Function Recovery Score consiste em um instrumento desenvolvido para avaliação da recuperação funcional de pacientes acima de 60 anos submetidos a tratamento cirúrgico devido a fraturas do quadril. O objetivo deste estudo foi fazer a tradução transcultural de forma criteriosa do questionário e adaptá‐lo às características individuais e culturais da população brasileira.MétodoEste método consiste em tradução inicial, retrotradução, elaboração de uma versão de consenso e pré‐teste comentado com posterior elaboração de uma versão final após as alterações necessárias sem modificar a semântica das perguntas do texto original.ResultadosA versão de consenso foi aplicada em 30 pacientes, acima de 60 anos, que foram submetidos a tratamento cirúrgico devido a fraturas do quadril. Foram observadas, entretanto, algumas dificuldades no entendimento de algumas palavras e expressões, as quais foram substituídas por termos de mais fácil entendimento. Após a elaboração da versão final, reaplicou‐se essa versão aos mesmos pacientes e obteve‐se um entendimento total.ConclusãoA tradução transcultural do questionário terá impacto imediato na avaliação funcional dos pacientes com mais de 60 anos operados devido a fraturas do quadril e, posteriormente, outros trabalhos nacionais podem usar esse questionário padronizado e adaptado à nossa cultura para comparação de resultados e enriquecer a produção científica do Brasil.AbstractObjectiveThe Hip Function Recovery Score questionnaire is an instrument that was developed for evaluating the functional recovery of patients over 60 years of age who undergo surgical treatment due to hip fractures. The objective of this study was to make a thorough transcultural translation of this questionnaire, with adaptation to the individual and cultural traits of the Brazilian population.MethodThis translation method consisted of initial translation, back translation, drafting of a consensual version and pretesting with comments invited. Subsequently, a final version was drawn up after making the necessary adjustments, without altering the semantics of the questions in the original text.ResultsThe consensual version was applied to thirty patients over the age of 60 years who had undergone surgical treatment due to hip fractures. However, some difficulties in understanding some words and expressions were observed, and these were then replaced with terms that were more easily understood. After the final version had been drawn up, this was applied to the same patients and full understanding was achieved among some of them, without altering the semantics of the questions of the original text.ConclusionThe transcultural translation of the Hip Function Recovery Score will have an immediate impact on functional evaluations on patients over 60 years of age who underwent surgery due to hip fracture. It will subsequently be possible for other Brazilian scientific studies to use this questionnaire, which has been standardized and adapted to Brazilian culture, in order to make comparisons between results, thereby enriching Brazilian scientific production.

Objective The present study aims to evaluate the efficacy of blood cell salvage (CS) as a method of reducing allogeneic blood transfusion in patients submitted to transtrochanteric femoral and hip surgeries due to injury. Methods Prospective cohort of 38 patients froma school hospital submitted to hip or trochanteric surgeries and divided into two groups from August 2015 to February 2017. Patients with any malignancy or infectious condition were excluded from the study. Cell savage group (19 patients) received autologous blood using cell saver, whereas control group (19 patients) received just allogeneic blood, if needed. Red blood cell parameters, blood transfusion requirements, and clinical and surgical characteristics, such as age, gender, ASA scale and type of surgery, were compared both preoperatively and postoperatively. Data was processed in SPSS 20.0. Results There were no differences in the clinical parameters studied (age, gender and ASA scale). Red blood cell parameters on the first day postoperative were higher in the cell savage group (p < 0.05). No significant reduction of intraoperative and postoperative allogeneic blood transfusion requirements was found. Conclusion This study found that CS was not effective in reducing intraoperative and postoperative allogeneic blood transfusion requirements in patients submitted to transtrochanteric femoral and hip surgery. Resumo Objetivo O estudo visa avaliar a eficácia da recuperação intraoperatória de sangue (RIOS) na redução de hemotransfusão alogênica em pacientes submetidos à cirurgia por fratura de fêmur e quadril. Métodos Coorte prospectiva com 38 pacientes submetidos a cirurgia traumatológica para fraturas em quadril e transtrocantéricas de fêmur, divididos em dois grupos em um hospital de ensino de agosto de 2015 a fevereiro de 2017. Pacientes com qualquer enfermidade ou condição infecciosa foram excluídos do presente estudo. O grupo RIOS (19 pacientes) recebeu sangue autólogo com a utilização de Cell Saver, enquanto o grupo controle (19 pacientes) recebeu apenas sangue alogênico, quando necessário.. Grupos comparados em relação ao gênero, idade na cirurgia, escala da Sociedade Americana de Anestesiologistas (ASA) (I, II ou III), uso intraoperatório da RIOS, volume sanguíneo reinfundido pela RIOS, parâmetros hematimétricos pré- e pósoperatórios, volume intra e pós-operatório de sangue alogênico transfundido. Dados processados no software SPSS Statistics for Windows, Versão 20.0 (IBM Corp, Armonk, NY, EUA). Resultados Sem diferenças significativas entre os grupos com as variáveis: idade, gênero e ASA. Percebeu-se que os valores finais de hemoglobina e hematócrito (no 1° dia de pós-operatório) foram mais elevados no grupo que utilizou o dispositivo (p < 0,05). Não houve redução significativa da transfusão alogênica intra e pósoperatória no grupo RIOS em comparação ao controle. Conclusões O presente estudo constatou que a RIOS não foi eficaz em reduzir a transfusão alogênica no intra e pós-operatório de pacientes submetidos à cirurgia de fêmur transtrocantérica e de quadril.
