Abstract

The elimination half life of gentamicin after intravenous administration of 80 mg of Refobacin was determined experimentally in 24 patients with normal and variably reduced renal function in the agar diffusion test. Experimentally found elimination half lives were compared to values determined from the creatinine clearance. Although there was good correlation between both parameters, satisfactory agreement between experimentally determined and calculated half life was found only in patients with normal or slightly reduced renal function. With decreasing creatinine clearance the calculated value was constantly bigger than the measured value, and the range of both values increased. Gentamicin treatment in patients with grossly decreased renal function should thus be performed under control of serum levels.

Abstract

The efficacy and safety of Iosartan and valsartan were evaluated in a multicenter, double-blind, randomized trial in patients with mild to moderate essential hypertension. Blood pressure responses to once-daily treatment with either losartan 50 mg (n = 93) or valsartan 80 mg (n = 94) for 6 weeks were assessed through measurements taken in the clinic and by 24-hour ambulatory blood pressure monitoring (ABPM). Both drugs significantly reduced clinic sitting systolic (SiSBP) and diastolic blood pressure (SiDBP) at 2, 4, and 6 weeks. Maximum reductions from baseline in SiSBP and SiDBP on 24hour ABPM were also significant with the two treatments. The reduction in blood pressure was more consistent across patients in the losartan group, as indicated by a numerically smaller variability in change from baseline on all ABPM measures, which achieved significance at peak (P = .017) and during the day (P = .002). In addition, the numerically larger smoothness index with losartan suggested a more homogeneous antihypertensive effect throughout the 24-hour dosing interval. The anti hypertensive response rate was 54% with losartan and 46% with valsartan. Three days after discontinuation of therapy, SiDBP remained below baseline in 73% of losartan and 63% of valsartan patients. Both agents were generally well tolerated. Losartan, but not valsartan, significantly decreased serum uric acid an average 0.4 mg/dL at week 6. In conclusion, once-daily losartan 50 mg and valsartan 80 mg had similar anti hypertensive effects in patients with mild to moderate essential hypertension. Losartan produced a more consistent blood pressure-lowering response and significantly lowered uric acid, suggesting potentially meaningful differences between these two A II receptor antagonists.

40. Effects of age and smoking on the pharmacokinetics of pindolol and propranolol.

Abstract

1 Pharmacokinetic investigations were carried out in a group of 32 ambulant normal male volunteers in order to determine the effect of age and smoking on steady-state plasma levels of pindolol and propranolol. There were four groups of 8: young non-smokers (YNS), young smokers (YS), old non-smokers (ONS) and old smokers (OS). Each subject received, in a randomized cross-over sequence, 5 mg pindolol and 80 mg propranolol three times daily for 2 days with an interval of at least 14 days between the two treatment periods. 2 Neither age nor smoking was shown to have any influence on the time to reach a peak plasma level after pindolol or propranolol. Age, on the other hand, significantly increased the peak plasma levels and the areas under the plasma concentration time curves, and decreased the elimination rate constants, the differences between the age groups being more pronounced for propranolol than for pindolol. No effect of smoking on these parameters was observed. 3 Differences were found between pindolol and propranolol in respect of time to reach a peak plasma level, peak plasma levels and area under the plasma concentration time curves in the groups ONS and YS when related to the YNS control group. Changes observed (with the exception of the time to reach a peak level) tended to be less for pindolol and were considered to be of little clinical relevance. [ABSTRACT FROM AUTHOR]

Abstract

1. The haemodynamic mechanism of action of guanfacine 4 mg intravenously was investigated in resting conditions and during exercise for up to 20 h after administration of the drug. Cardiac output and pulmonary arterial pressure were determined by the Swan-Ganz thermodilution method. Blood pressure was measured directly. 2. During and immediately after intravenous administration of guanfacine, blood pressure peripheral resistance and pulmonary arterial pressure increased (in keeping with an alpha-sympathomimetic effect of the compound), whereas heart rate and cardiac output decreased. 3. Subsequently blood pressure fell as a result of a decrease in cardiac output. From the third hour peripheral resistance decreased, whereas cardiac output increased again, sometimes exceeding the control value. 4. During exercise blood pressure was reduced from the third hour after administration, as in resting conditions, as a result of the reduction in peripheral resistance. 5. In resting conditions guanfacine reduced heart rate at the beginning and during the whole course after administration of the drug. 6. Side-effects noted included fatigue, drowsiness and bradycardia. Abstract

The effect of submaximal exercise upon haemodynamic and biochemical variables was investigated in healthy male subjects, aged 17–27 years, before and at the end of 2 weeks treatment with propranolol (40 mg p.o., q.i.d.). Propranolol reduced the resting blood pressure in normal subjects significantly. This effect was due to reduction of cardiac output and of systemic vascular resistance. No effect of propranolol on BP was seen during maximal exercise, since a reduced cardiac output was accompanied by an increased peripheral resistance. The reduction of cardiac output during exercise can be compensated in part by an increase in stroke volume. The sympathetic activity induced by physical exercise in normotensives increased plasma renin concentration (PRC) and plasma aldosterone (PA), and suppressed urinary excretion of c-AMP. PRC returned to basal levels after 45 min. No increase of PRC was observed after exercise in subjects treated with propranolol. Yet the increase of PA was not completely suppressed. No direct relation was demonstrated between PRC and the haemodynamic variables before or during the administration of propranolol.

Abstract

The metabolism of the polyvalent saluretic agent (2-cyclopentyl-6,7-dichloro-2-methyl-1-oxo-5-indanyloxy)acetic acid was studied in chimpanzee and man. The drug was well absorbed and extensively metabolized by man. Peak levels of drug (5--8 microgram/ml) occurred within 1.5--4.5 hr of drug administration. The plasma half-life was estimated to be 2 hr; a similar half-life was observed in the chimpanzee. Little unchanged drug (less than 10%) was excreted in the urine of either species. Similar metabolic profiles were obtained for man and chimpanzee. The major urinary metabolites resulted from hydroxylation of the cyclopentyl moiety, giving rise to a number of diastereomers. The alcohol metabolites were subsequently oxidized to the ketone. The excretion of the metabolites coincided with maximal excretion of sodium and chloride ions. The hydroxylated metabolites have intrinsic pharmacological activity.

50. Biotransformation of tetroxoprim in man

Abstract

After oral administration in man of 14C-labelled 200 mg tetroxoprim, intact parent substance was eliminated through the kidneys (55·8% of the dose) and in the faeces (34·2%). A metabolite (UH-1) representing 5·1% of the dose can be detected up to 48 h after administration. The 14C-activity in serum is derived trom tetroxoprim alone, at each time point. The elimination half-life was 6·1 h; the maximum serum level was 2·9 mg/l at 1·4 h. Whilst the total serum clearance of tetroxoprim is about 120 mllmin, the renal clearance of the drug was calculated as 67 ml/min. Therefore, tetroxoprim undergoes about 46% tubular reabsorption.
