Objectives: To describe and compare cardiopulmonary resuscitation outcomes at a Swiss veterinary teaching hospital before and after publication of the Reassessment Campaign on Veterinary Resuscitation guidelines. Materials and Methods: Between 2018 and 2020, hospital staff underwent various types of yearly Reassessment Campaign on Veterinary Resuscitation‐based cardiopulmonary resuscitation trainings. Canine and feline cardiopulmonary resuscitation events during that period (post‐Reassessment Campaign on Veterinary Resuscitation) and between 2010 and 2012 (pre‐Reassessment Campaign on Veterinary Resuscitation) were identified and animal, arrest and outcome variables recorded retrospectively. Results: Eighty‐one animals were included in the pre‐Reassessment Campaign on Veterinary Resuscitation group and 190 in the post‐Reassessment Campaign on Veterinary Resuscitation group. Twenty‐three percent in the pre‐Reassessment Campaign on Veterinary Resuscitation group and 28% in the post‐Reassessment Campaign on Veterinary Resuscitation group achieved return of spontaneous circulation and 1% and 4% survived to hospital discharge, respectively. Patients undergoing anaesthesia [odds ratio 4.26 (1.76 to 10.27)], elective [odds ratio 5.16 (1.06 to 25.02)] or emergent surgery [odds ratio 3.09 (1.20 to 8.00)], or experiencing cardiopulmonary arrest (CPA) due to arrhythmias [odds ratio 4.31 (1.44 to 12.93)] had higher odds of return of spontaneous circulation, while those with unknown cause of CPA [odds ratio 0.25 (0.08 to 0.78)] had lower odds. Undergoing cardiopulmonary resuscitation in the post‐Reassessment Campaign on Veterinary Resuscitation period was not statistically significantly associated with return of spontaneous circulation [odds ratio 1.38 (0.68 to 2.79)]. Clinical Significance: Unchanged odds of return of spontaneous circulation observed in this study could suggest that once‐yearly cardiopulmonary resuscitation training is insufficient, effects of animal and tertiary referral hospital variables confounded results, guideline benefit is limited, or that compliance during clinical cardiopulmonary resuscitation efforts is too poor for guideline recommendations to have a positive impact. Persistent bacterial infections of the gastrointestinal mucosa are causally linked to gastric carcinoma and mucosa-associated lymphoid tissue (MALT) lymphoma in people and laboratory animals. We examined the relationship of mucosa-associated bacteria to alimentary lymphoma in cats. Intestinal biopsies from 50 cats with alimentary lymphoma (small cell, n = 33; large cell, n = 17) and 38 controls without lymphoma (normal to minimal change on histopathology, n = 18; lymphocytic-plasmacytic enteritis, n = 20) were evaluated. The number and spatial distribution of bacteria (ie, in luminal cellular debris, villus-associated mucus, adherent to epithelium, mucosal invasion, intravascular, or serosal) were determined by fluorescence in situ hybridization with the eubacterial probe EUB-338. Mucosa-invasive bacteria were more frequently observed in cats with large cell lymphoma (82%, P ≤ .001) than in cats with small cell lymphoma (18%), normal to minimal change on histopathology, and lymphocytic-plasmacytic enteritis (3%). Intravascular bacteria were observed solely in large cell lymphoma (29%), and serosal colonization was more common in cats with large cell lymphoma (57%) than with small cell lymphoma (11%, P ≤ .01), normal to minimal change (8%, P ≤ .01), and lymphocytic-plasmacytic enteritis (6%, P ≤ .001). The high frequency of invasive bacteria within blood vessels and serosa of cats with large cell lymphoma may account for the sepsis-related complications associated with large cell lymphoma and inform clinical management. Further studies are required to determine the role of intramucosal bacteria in the etiopathogenesis of feline alimentary lymphoma. [ABSTRACT FROM AUTHOR]

Introduction: The association of screen time and physical activity with body weight in children has been investigated in cross-sectional and prospective studies, as well as randomized controlled trials. The present study extends previous research by examining how longitudinal within-person changes in screen time and physical activity relate to changes in Body Mass Index (BMI) in children, and how changes in screen time and physical activity relate to each other., Methods: The data for the present study came from the PIER Youth Study. Data were collected from 971 children and their parents at two time points approximately 1 year apart. A multilevel modeling approach with measurement occasions nested within children was used to model changes in BMI across age., Results: Within a child, a change in daily hours of TV viewing was associated with a corresponding change in BMI, whereas a change in daily hours of physical activity was associated with an opposite change in BMI. Within-person correlations between changes in screen time and physical activity were small but positive., Conclusion: The present findings have important implications for interventions to reduce and prevent childhood overweight and obesity. The present study examined trajectories of trait positive and trait negative affect as measured by the Positive and Negative Affect Schedule across the lifespan. Increasing levels of measurement invariance of the Positive and Negative Affect Schedule were tested across a wide age range (18-99 years of age; 10 age groups) in a large sample ( N = 3,309; 65.82% women; 74.22% with a university entrance diploma; 92.23% with German as their mother tongue). Strong invariance was established so that the latent factor means of positive and negative affect could be meaningfully compared across age groups. Age had a small effect on positive affect, which showed a reversed U-shaped function with a maximum between the ages of 40 and 50. For negative affect, there was a strong cubic age effect. Specifically, negative affect decreased until about the age of 70 and then increased again. We also found a negative covariance between positive and negative affect, which first increased slightly and then decreased across age groups until becoming positive in the oldest age group. In summary, the present study supports previous findings on age trajectories of positive and negative affect but also highlights the importance of testing for measurement invariance. The emotions attributed to an event can change from occurrence to recall. Autobiographical memories (AMs) exhibit fixed affect (i.e., no change in emotionality), fading affect (i.e., a decrease in emotional intensity), flourishing affect (i.e., an increase in emotional intensity), and flexible affect (i.e., change of valence). Mixed-effects multinomial models were used to predict the likelihood of the different affect change categories. Mixed-effects regression models were used to predict the amount of emotional change within each category. Predictors at the event-level were initial intensity, social rehearsal, and recall frequency. Predictors at the participant-level were components of subjective wellbeing. Analyses were based on 1,748 AMs reported by 117 young participants in response to 16 event cues. Frequency biases, and biases in the amount of change across positive and negative emotionality, were found for all types of emotional change. Specifically, there was more fading of negative (29.98%) than positive affect (11.90%), more flourishing of positive (34.27%) than negative affect (9.61%), and more AMs changing from negative to positive valence (13.33%) than vice versa (3.95%). These biases were also evident in the amount of change within the categories. Moreover, slightly more AMs remained fixed in positive (49.89%) than negative affect (47.08%). Both event and participant level predictors were significantly associated with the likelihood of different affect change categories and the amount of emotional change within the categories. Recalling autobiographical memories (AMs) is closely tied to emotional experience. However, the emotionality associated with an event can change from occurrence to recall. Autobiographical memories show fixed affect (i.e., no change in emotionality), fading affect (i.e., decrease in emotional intensity), flourishing affect (i.e., increase in emotional intensity), and flexible affect (i.e., change of valence). The present study used mixed-effects multinomial models to predict perceived changes in positive and negative valence as well as intensity. Initial intensity, vividness, and social rehearsal were entered into the models as event level predictor variables, whereas rumination and reflection were entered into the models as participant level predictor variables. Analyses were based on 3950 AMs reported by 352 participants (18-92 years old) in response to 12 emotional cue-words. Participants rated the emotionality of each memory from the perspective of event occurrence and event recall. Only the predictors on the event level meaningfully distinguished between memories that stayed fixed in affect and memories that showed fading, flourishing, or flexible affect (R² values ranging from .24 to .65). The present results highlight the importance of considering different aspects of AMs and the ways they change emotionally to fully understand emotional experiencing in autobiographical memory.

Autobiographical memories serve psychosocial functions in daily life and the use of memories is related to their valence. In the present study, we examined whether functions are also related to the intensity of positive and negative memories. Our sample included 110 participants (57-89 years of age). Memories were prompted with 30 emotionally neutral cue words. Participants rated the emotional quality of each memory and indicated how frequently they had recalled it for self-continuity, directing behavior, social-bonding, and mood-enhancement. We used multilevel modeling to test whether individual differences in the use of memories can explain why individuals recall different numbers of positive and negative memories as well as memories high or low in intensity. Each function revealed its specific pattern regarding valence and intensity but also regarding within-person and between-person effects. Positive-pressure ventilation results in ventilator-induced lung injury, and few therapeutic modalities have been successful at limiting the degree of injury to the lungs. Understanding the primary drivers of ventilator-induced lung injury will aid in the development of specific treatments to ameliorate the progression of this syndrome. There are conflicting data for the role of neutrophils in acute respiratory distress syndrome pathogenesis. Here, we specifically examined the importance of neutrophils as a primary driver of ventilator-induced lung injury in a mouse model known to have impaired ability to recruit neutrophils in previous models of inflammation. We exposed Duoxa +/+ and Duoxa -/- mice to low- or high-tidal volume ventilation with or without positive end-expiratory pressure (PEEP) and recruitment maneuvers for 4 hours. Absolute neutrophils in BAL fluid were significantly reduced in Duoxa -/- mice compared with Duoxa +/+ mice (6.7 cells/μl; 16.4 cells/μl; P  = 0.003), consistent with our hypothesis that neutrophil translocation across the capillary endothelium is reduced in the absence of DUOX1 or DUOX2 in response to ventilator-induced lung injury. Reduced lung neutrophilia was not associated with a reduction in overall lung injury in this study, suggesting that neutrophils do not play an important role in early features of acute lung injury. Surprisingly, Duoxa -/- mice exhibited significant hypoxemia, as measured by the arterial oxygen tension/fraction of inspired oxygen ratio and arterial oxygen content, which was out of proportion with that seen in the Duoxa +/+ mice (141, 257, P  = 0.012). These findings suggest a role for dual oxidases to limit physiologic impairment during early ventilator-induced lung injury.

Introduction: Objective: In 2012, the Reassessment Campaign on Veterinary Resuscitation (RECOVER) published the first evidence-based small animal CPR guidelines. Even though a RECOVER-based CPR approach has been shown to improve patient outcomes, guideline awareness and compliance is necessary to see such benefits. Our study aimed to characterize Swiss small animal veterinary CPR practices and assess their compliance with RECOVER guidelines. Methods: A nationwide, internet-based survey was conducted, and invitations distributed via Swiss veterinary society mailing lists. Questions covered respondents' demographics, CPR preparedness, Basic Life Support (BLS) and Advanced Life Support (ALS) techniques, and awareness of RECOVER guidelines. Percentages of group total (95% confidence interval) were calculated. Results: One-hundred and fifty respondents were grouped by level of expertise into board-certified specialists (BCS, n = 19), veterinarians with additional post-graduate training (PGT, n = 27), and general practitioners with (GPE, n = 30), and without emergency duties (GPG, n = 74). Of BCS respondents, 58% (36-77%) were familiar with the RECOVER guidelines, compared to 8% (4-17%) of GPG. Large disparities in preparedness, BLS, and ALS techniques emerged among the levels of expertise. Incompliance with preparedness measures varied from 89% (69-98%) in BCS to 100% (95-100%) in GPG and was predominantly due to failure to attend regular CPR training. BLS compliance ranged from 26% (12-49%) in BCS to 5% (2-13%) in GPG, and incompliance was mostly characterized by targeting lower than recommended chest compression rates. ALS compliance varied from 21% (9-43%) in BCS to 0% (0-5%) in GPG and was compromised by limitations in the resuscitation environment such as lacking access to a defibrillator, monitoring equipment, and rescue drugs. Conclusion: Awareness of RECOVER guidelines in Switzerland is acceptable in specialists, but inadequate among general practitioners and CPR practices are largely not in agreement with RECOVER guidelines. An educational strategy is needed to improve Swiss small animal CPR knowledge and performance.

Introduction: Cardiopulmonary arrest (CPA) is the acute cessation of systemic perfusion and ventilation. It leads to a lack of tissue oxygen delivery and, if not addressed quickly, will inevitably cause death. Cardiopulmonary resuscitation (CPR) is the only available treatment for CPA and several opportunities exist to improve the veterinary team's resuscitation approach and optimize small animal CPR patient outcomes. In 2012, the Reassessment Campaign on Veterinary Resuscitation (RECOVER) initiative generated evidence-based clinical guidelines to form the basis for training and practice of CPR in dogs and cats. When employing an evidence-based, standardized approach to small animal CPR, return of spontaneous circulation can be achieved in up to 58% of patients and up to 7% of dogs and 19% of cats can be discharged from the veterinary hospital alive. Survival for dogs and cats that suffer CPA is best in patients that suffer a peri-anesthetic arrest, so high quality CPR in the anesthesia patient population is of utmost importance and expected to be the most rewarding. To ensure the best possible outcomes for any patient suffering from CPA and undergoing CPR, a comprehensive resuscitation strategy is necessary, that includes preventive and preparedness measures, basic life support (chest compressions and ventilation), advanced life support (optimization of the patient status by targeted drug therapy, cardiac rhythm monitoring, and defibrillation), and post-cardiac arrest critical care. This article summarizes the most important RECOVER CPR guidelines for the small animal practitioner.

Background: Access to basic anesthetic monitoring in the developing world is lacking, which contributes to the 100 times greater anesthesia-related mortality in low- and middle-income countries. We hypothesize that an environmental sensor with a lower sampling rate could provide some clinical utility by providing CO 2 levels, respiratory rate, and support in detection of clinical abnormalities., Materials and Methods: A bench-top lung simulation was created to replicate CO 2 waveforms, and an environmental sensor was compared with industry-available technology. Sensor response time and respiratory rates were compared between devices. Additionally, an in silico model was created to replicate capnography pathology as waveforms would appear using the environmental sensor., Results and Conclusion: Breath simulations using the bench-top lung simulation produced similar results to industry standards with a degree of variability. Respiratory rates did not differ between the environmental sensor and all other devices tested. Finally, pathological waveforms created in silico carried a certain level of detail regarding ventilatory pathology, which could provide some clinical insight to an anesthesiologist. We believe our prototype is the first step toward making low-cost and portable capnography available in the resource-limited setting, and future efforts should focus on bridging the gap to safer anesthesia and surgery globally.

Lung endothelial cells respond to changes in vascular pressure through mechanotransduction pathways that alter barrier function via non-Starling mechanism(s). Components of the endothelial glycocalyx have been shown to participate in mechanotransduction in vitro and in systemic vessels, but the glycocalyx's role in mechanosensing and pulmonary barrier function has not been characterized. Mechanotransduction pathways may represent novel targets for therapeutic intervention during states of elevated pulmonary pressure such as acute heart failure, fluid overload, and mechanical ventilation. Our objective was to assess the effects of increasing vascular pressure on whole lung filtration coefficient (K(fc)) and characterize the role of endothelial heparan sulfates in mediating mechanotransduction and associated increases in K(fc). Isolated perfused rat lung preparation was used to measure K(fc) in response to changes in vascular pressure in combination with superimposed changes in airway pressure. The roles of heparan sulfates, nitric oxide, and reactive oxygen species were investigated. Increases in capillary pressure altered K(fc) in a nonlinear relationship, suggesting non-Starling mechanism(s). nitro-l-arginine methyl ester and heparanase III attenuated the effects of increased capillary pressure on K(fc), demonstrating active mechanotransduction leading to barrier dysfunction. The nitric oxide (NO) donor S-nitrosoglutathione exacerbated pressure-mediated increase in K(fc). Ventilation strategies altered lung NO concentration and the K(fc) response to increases in vascular pressure. This is the first study to demonstrate a role for the glycocalyx in whole lung mechanotransduction and has important implications in understanding the regulation of vascular permeability in the context of vascular pressure, fluid status, and ventilation strategies.
