Abstract

Although RNI improves breast cancer survival, it increases risk of upper extremity lymphedema. We hypothesized that hypofractionated RNI may reduce lymphedema risk. Patients with a recommendation for RNI for cT0-T3, N0-N2a, N3a invasive breast cancer were randomized between standard RNI (STD-RNI: 50 Gy to breast/chest wall and 45 Gy to RN) or shorter RNI (SH-RNI: 40.05 Gy to breast/chest wall and 37.5 Gy to RN). Patients were stratified by receipt of chemotherapy, body mass index (BMI), type of axillary surgery, and difference in arm volume prior to RNI. RN targets included the internal mammary, infraclavicular, and supraclavicular nodal basins; the level I and II axilla was treated if axillary lymph node dissection was not performed. Boost to the tumor bed or chest wall was permitted. Lymphedema was assessed via standard toxicity grading by the treating physician and by serial perometry measurement prior to surgery, post-operatively and then 6, 12, 18, and 24 months after radiation. The primary outcome was defined as a ≥ 10% relative difference in arm volume on at least one post-radiation perometry assessment, normalized by the pre-operative perometry measurement. Secondary outcomes included comparison of physician-reported toxicities using the NCI CTCAE version 4.0 scale graded weekly during RT, at 6 months, and then annually. Fisher's exact tests compared groups. Local-regional recurrence (LRR) was calculated using the Kaplan-Meier method and compared using the log-rank test. There were three hundred twenty-four patients across 5 treatment centers were enrolled and randomized from 2017 to 2022 with median follow up of 4.75 years. Clinical-pathologic covariates were well-balanced by treatment arm. Median age was 54 years, 64% were non-Hispanic White, and 39% had body mass index (BMI) > 30. 57% underwent mastectomy with or without reconstruction and 42% underwent segmental mastectomy. Sixty-eight percent underwent axillary lymph node dissection and 90% received chemotherapy. Perometry-assessed lymphedema, the primary outcome, was less common after SH-RNI (29%) than STD-RNI (36%), but the difference was not statistically significant (P = 0.24). In contrast, physician-assessed lymphedema was significantly less common with SH-RNI than STD-RNI (15% vs. 27%, P = 0.009). Patients randomized to SH-RNI were less likely to experience any grade ≥ 2 toxicity (52% vs. 78%, P < 0.001). Pneumonitis was uncommon and similar between groups (3% vs 2%, P = 0.46). There were no brachial plexopathy events. Five-year LRR risk was 3% with SH-RNI and 2% with STD-RNI (P = 0.48). In this primary outcome analysis of a multisite phase III randomized clinical trial, SH-RNI did not lower risk of perometry-assessed lymphedema. However, SH-RNI conferred a low risk of LRR and reduced the risk of physician-reported lymphedema and grade 2 or higher toxicity when compared to STD-RNI. [ABSTRACT FROM AUTHOR]

Abstract

The randomized FORMULA-509 trial demonstrated that for patients with a PSA > 0.5 after prostatectomy receiving salvage radiation and 6 months of GnRH Agonist, the addition of abiraterone acetate plus prednisone (AAP) and apalutamide (Apa) improved metastasis-free survival compared to bicalutamide. Here we present the patient-reported health-related quality of life (HRQoL) results through three years. Validated questionnaires were administered at baseline, end of treatment, and 1 and 3 years after completion of treatment. Patients completed Expanded Prostate Cancer Index Composite (EPIC)-26, PROMIS Fatigue, and Saint Louis University Mental Status Exam (SLUMS). EPIC-26 is scored from 0 to 100 with 100 indicating higher function. PROMIS Fatigue is scored using a standardized T score with higher score indicating greater fatigue. SLUMS is scored from 0 to 30 with 27 to 30 interpreted as normal, 21 to 26 as mild neurocognitive disorder and 20 or less as dementia. Scores between treatment arms were compared using the t-test. Results were interpreted using established thresholds for clinically meaningful differences (4-6 for EPIC-26 hormonal domain, 5-10 for PROMIS Fatigue). Three hundred forty-five patients were randomized (172 bicalutamide and 173 AAP/Apa). Completion rates at baseline, end of treatment, 1 year, and 3 years, were 96%, 80%, 70%, and 57% for EPIC-26; 95%, 79%, 67%, and 57% for PROMIS Fatigue; and 96%, 80%, 70%, and 34% for SLUMS. From baseline to end of treatment, both arms demonstrated clinically meaningful declines in EPIC-26 hormonal domain (median change -15 bicalutamide; -15 AAP/Apa) and increase in PROMIS Fatigue (median change 6 bicalutamide; 7.4 AAP/Apa). From end of treatment to 1 year after treatment patient-reported HRQoL improved to near baseline for both EPIC-26 hormonal function (bicalutamide median score baseline = 95, end = 75, 1 year = 90; AAP/Apa baseline = 95, end = 75, 1 year = 90) and fatigue (bicalutamide median score baseline = 43.1, end = 48.6, 1 year = 46.0; AAP/Apa baseline = 43.1, end = 51.0, 1 year = 46.0) with further improvement in EPIC-26 hormonal function at 3 years (bicalutamide = 95.0; AAP/Apa = 95.0) and stable fatigue at 3 years (bicalutamide = 46.0; AAP/Apa = 46.0). The cMedian SLUMS score was within normal range at baseline (27 bicalutamide; 27 AAP/Apa), end of treatment (28, 28) and 1 (27, 27) and 3 (27, 27) years after treatment. There was no difference in patient-reported hormonal function, fatigue, or mental status between treatment arms, at end of treatment (P = 0.40; P = 0.78; and P = 0.41, respectively), 1 year after treatment (P = 0.78; P = 0.89; and P = 0.76), and 3 years after treatment (P = 0.45; P = 0.95; and P = 0.86). The addition of AAP/Apa improved oncologic outcomes without causing a detectable difference in patient-reported hormonal function, fatigue, or mental status compared to bicalutamide through three years after treatment. Abstract

Purpose Urologists have been criticized for overtreating men with low risk prostate cancer and for passively observing older men with higher risk disease. Proponents of active surveillance for low risk disease and critics of watchful waiting for higher risk disease have advocated for more judicious use of observation. Thus, we compared 2 population based cohorts to determine how expectant management has evolved during the last 2 decades. Materials and Methods A total of 5,871 men with localized prostate cancer were enrolled in the PCOS (Prostate Cancer Outcomes Study) or the CEASAR (Comparative Effectiveness Analysis of Surgery and Radiation) study. We compared the use of definitive treatment vs expectant management (watchful waiting or active surveillance) across cohorts, focusing on the influence of disease risk, age and comorbidities. Results Use of watchful waiting or active surveillance was similar in PCOS and CEASAR (14% in each). Compared to the PCOS, more men in the CEASAR study with low risk disease selected watchful waiting or active surveillance (25% vs 15%, respectively), whereas fewer men with intermediate (7% vs 14%) and high risk (3% vs 10%) disease chose watchful waiting or active surveillance (p <0.001 for each). The association of disease risk with watchful waiting or active surveillance was significantly larger in CEASAR than in PCOS (OR 7.3, 95% CI 3.4 to 15.7). Older age was associated with watchful waiting or active surveillance in both cohorts but there was no association between comorbidity and watchful waiting or active surveillance in the CEASAR study. Conclusions Use of watchful waiting or active surveillance was more aligned with disease risk in CEASAR compared to PCOS, suggesting there has been a pivot from watchful waiting to active surveillance.
