Objectives: Compulsory mental health care includes compulsory hospitalisation and outpatient commitment with medication treatment without consent. Uncertain evidence of the effects of compulsory care contributes to large geographical variations and a controversy on its use. Some argue that compulsion can rarely be justified and should be reduced to an absolute minimum, while others claim compulsion can more frequently be justified. The limited evidence base has contributed to variations in care that raise issues about the quality/appropriateness of care as well as ethical concerns. To address the question whether compulsory mental health care results in superior, worse or equivalent outcomes for patients, this project will utilise registry‐ based longitudinal data to examine the effect of compulsory inpatient and outpatient care on multiple outcomes, including suicide and overall mortality; emergency care/injuries; crime and victimisation; and participation in the labour force and welfare dependency. Methods: By using the natural variation in health providers' preference for compulsory care as a source of quasi‐randomisation we will estimate causal effects of compulsory care on short‐ and long‐term trajectories. Conclusions: This project will provide valuable insights for service providers and policy makers in facilitating high quality clinical care pathways for a high risk population group.

Helsevesenet har blitt en sentral samfunnsaktør og medisinen dets førende fag. Hvorfor har det blitt slik? En grunn er at medisinen fører sammen sfærer som ellers har vært adskilt: det sanne, det gode og det skjønne. Medisinen som fag og helsevesenet som institusjon, har blitt et fascinerende skjæringspunkt nettopp mellom evidens, etikk og estetikk. Her kobles kunnskap til det som er vondt og skjønnhet til det som er friskt. Kunnskapsproduksjonen dirigeres ut fra ønsket om å gjøre det gode ved å unngå det vonde. Samtidig styres helsevesenets normer også av estetikkens normer. Koblingene mellom evidens, estetikk og etikk gir noen grunnleggende utfordringer, som det er viktig å være oppmerksom på. Riktig håndtert gjør de helsevesenet generelt og medisinen spesielt til et fascinerende og fruktbart skjæringspunkt mellom det sanne, det gode og det skjønne.

Background Overuse, underuse, and significant variation in the utilisation of radiological services are well documented in the literature. Several radiological examinations are identified as low-value examinations as they do not lead to a change in diagnosis or course of treatment. Even so, such examinations are frequently performed. Many measures for reducing low-value imaging have been carried out with variable outcomes. While there is little evidence as to why some measures work and others do not, adjusting to the context seems important for success. The objective of this study was to investigate which measures stakeholders consider appropriate for reducing the use of low-value imaging and what it takes to make them work. Methods Semi-structured interviews were conducted among radiographers, radiologists, radiological department managers, hospital clinicians, general practitioners, and health government/authorities’ representatives. The interview guide covered two broad areas: Experience with low-value services, and possible future measures deemed appropriate for reducing low-value services. Data were analysed in line with a qualitative framework analysis. Results The analysis included information from 27 participants. All participants acknowledged that low-value imaging was a problem, but few had very specific suggestions on reducing this in practice. Suggested measures were to stop referrals from being sent, provide support in assessing referrals, or change the healthcare system. Identified facilitators were categorised as management and resources, evidence, and experienced value. In general, appropriate measures should be practical, well-founded, and valuable. Conclusions This study provides insight into various stakeholders’ perceptions of suitable interventions to reduce low-value imaging. While many measures for reducing low-value imaging are available, contextual sensitivity is crucial to make them work.

La pandemia de COVID-19 ha generado en todo el mundo una amenaza sin precedentes que no solo impone la urgente necesidad de reflexionar críticamente y de producir nuevo conocimiento, sino que también exige la acción inmediata, a pesar de las lagunas de información que aún prevalecen y la incertidumbre existente a múltiples niveles. En cuanto al papel de la ética de la investigación en estos tiempos de pandemia, algunos defienden el excepcionalismo mientras que otros, incluidos los autores de este artículo, hacen énfasis en la urgente necesidad de mantener el compromiso con los principios éticos fundamentales y con las obligaciones de derechos humanos más fundamentales, que se reflejan en las pautas y directrices para la investigación que han sido cuidadosamente elaboradas a lo largo del tiempo. En este artículo analizamos algunos de los argumentos que se plantean en el debate actual sobre los estudios de desafío humano o estudios de infección humana controlada (EIHC) y el papel concomitante de la ética de la investigación en temas de salud en épocas de pandemia. Sugerimos que resultaría útil pensar la cuestión a partir de la diferenciación entre riesgo, incertidumbre en sentido estricto y la ignorancia. Presentamos algunos ejemplos de casos en los que se causó daño en nombre de la investigación científica y discutimos la relevancia de este legado en la situación actual.

Filosofi og etikk har fått en stadig større plass i det offentlige rom i Norge. 2017 ble et år der filosofer sørget for overskrifter i en rekke norske medier. En av sakene som fikk størst oppmerksomhet, var debatten om sorteringssamfunnet og Aksel Braanen Sterris påstand om at personer med Downs syndrom ikke kan leve fullverdige liv. Utsagnet skapte en voldsom debatt og kraftige reaksjoner. Temaet for debatten er interessant i seg selv, men den reiser også spørsmål om hvordan slike debatter endrer filosofiens anseelse og rolle i det offentlige ordskiftet i Norge. I denne artikkelen stiller vi derfor spørsmålet: På hvilken måte har debatten om sorteringssamfunnet i 2017 påvirket forholdet mellom filosofi og samfunn? Som perspektiv for analysen anvender vi tradisjonelle kvalitetskriterier innen filosofi, slik som konsistens, klare premisser og evnen til å klargjøre begreper, fremstille motargumenter og begrunne grenser. Vi finner at debatten om sorteringssamfunnet utvilsomt har gitt filosofien mer oppmerksomhet i det offentlige ordskiftet, og at filosofisk argumentasjon kan bidra til å løfte frem skjulte problemstillinger og sette ord på uuttalte intuisjoner, samt å stimulere til bedre argumentasjon. Dette bør hilses velkommen. Samtidig finner vi at filosofiens tilpasning til mediediskursen fører til at akademiske forbehold tradisjonelle kvalitetskrav og nyansering forsvinner. Dersom skjulte premisser, manglende konsistens, begrepslige og vurderingsmessige uklarheter, samt ignorering av empiriske premisser, motargumenter og viktige implikasjoner blir utbredt, vil resultatet kunne bli en fattigere offentlig debatt, et dårligere samfunn og et svekket omdømme for filosofien. Løsningen må være at vi som fagpersoner er villige til å gjøre klart og grundig rede for våre påstander, perspektiver, premisser, argumenter og konklusjoner, og at vi bør revidere eller trekke dem tilbake dersom vi ikke makter å gjøre dette. Ellers står vi i fare for å gjøre filosofien til en form for «villedningskunst» – en ny form for sofisme – og et lett bytte for platonsk fordømmelse. Problemstilling: I hvilken grad er informasjonen som blir gitt kvinner i forbindelse med mammografiscreening i Norge balansert og nøytral, slik at den fremmer kvinners egne valg? Formål: Formålet med denne oppgaven er å analysere informasjonsmateriellet gitt til norske kvinner angående mammografiscreeningprogrammet, med tanke på om informasjonen er balansert og nøytral, og fremmer kvinners selvbestemmelse. Metode: Det er benyttet en kvalitativ litteraturstudie, med tidligere forskning og analyse av informasjonsmateriell gitt til kvinner i forbindelse med mammografiscreening. Resultat: Resultatene viser utviklingen av informasjonen gitt i Norge fra oppstarten frem til i dag. I tillegg sammenlignes Norges nyeste informasjonsmateriell med Sverige og Danmark. Konklusjon: Norge har hatt en stor utvikling de siste årene. Det har blitt mer fokus på både positive og negative sider, slik at de inviterte kan ta et bedre informert valg. Slik type informasjon har likevel alltid forbedringspotensiale, da ny forskning stadig bidrar til nye synspunkter. Research question: To what extent is information given to women regarding mammography screening in Norway unbiased and neutral, and how does it promote women`s own choices? Purpose: The purpose of this study is to examine information given to Norwegian women regarding the Mammography Screening Program, and to evaluate whether or not the given information is neutral and unbiased. Method: We have conducted a literature review with former research and analysis of the information material given to women. Results: The results show the development in Norway from the beginning of the Screening Program until today. It also includes a comparison between the latest information presented in Norway, compared to material given in Sweden and Denmark. Conclusion: Norway has shown major development in the recent years. Both positive and negative effects have been highlighted. This can increase the quality of choices made by those who are invited. This type of information, however, always has potential for improvement, as new research will contribute to new views.
