In this work, we studied the pregnancy-associated differences in peripheral blood gene expression profiles between healthy women and women diagnosed with BC in a prospective design. Methods: Using an integrated system epidemiology approach, we modeled BC incidence as a function of parity in the Norwegian Women and Cancer (NOWAC) cohort (165,000 women) and then tested the resulting mathematical model using gene expression profiles in blood in a nested case–control study (460 invasive case–control pairs) of women from the NOWAC postgenome cohort. Lastly, we undertook a gene set enrichment analysis for immunological gene sets. Results: A linear trend fitted the dataset precisely showing an 8% decrease in risk of BC for each FTP, independent of stratification on other risk factors and lasting for decades after a woman’s last FTP. Women with six children demonstrated 48% reduction in the incidence of BC compared to nulliparous. When we looked at gene expression, we found that 756 genes showed linear trends in cancer-free controls (false discovery rate [FDR] 5%), but this was not the case for any of the genes in BC cases. Gene set enrichment analysis of immunologic gene sets (C7 collection in Molecular Signatures Database) revealed 215 significantly enriched human gene sets (FDR 5%). Conclusion: We found marked differences in gene expression and enrichment profiles of immunologic gene sets between BC cases and healthy controls, suggesting an important protective effect of the immune system on BC risk. Abstract

We present MEGARA (Multi-Espectr\'ografo en GTC de Alta Resoluci\'on para Astronom\'ia) Integral Field Unit (IFU) observations of 5 local type-2 quasars (QSO2s, z $\sim 0.1$) from the Quasar Feedback (QSOFEED) sample. These active galactic nuclei (AGN) have bolometric luminosities of 10$^{45.5-46}$ erg/s and stellar masses of $\sim$10$^{11}$ M$_{\odot}$. We explore the kinematics of the ionized gas through the [O~III]$\lambda$5007 $\r{A}$ emission line. The nuclear spectra of the 5 QSO2s, extracted in a circular aperture of $\sim$ 1.2" ($\sim$ 2.2 kpc) in diameter, show signatures of high velocity winds in the form of broad (full width at half maximum; 1300$\leq$FWHM$\leq$2240 km/s and blueshifted components. We find that 4 out of the 5 QSO2s present outflows that we can resolve with our seeing-limited data, and they have radii ranging from 3.1 to 12.6 kpc. In the case of the two QSO2s with extended radio emission, we find that it is well-aligned with the outflows, suggesting that low-power jets might be compressing and accelerating the ionized gas in these radio-quiet QSO2s. In the four QSO2s with spatially resolved outflows, we measure ionized mass outflow rates of 3.3-6.5 Msun/yr when we use [S~II]-based densities, and of 0.7-1.6 Msun/yr when trans-auroral line-based densities are considered instead. We compare them with the corresponding molecular mass outflow rates (8 - 16 Msun/yr), derived from CO(2-1) ALMA observations at 0.2" resolution. Both phases show lower outflow mass rates than those expected from observational scaling relations where uniform assumptions on the outflow properties were adopted. This might be indicating that the AGN luminosity is not the only driver of massive outflows and/or that these relations need to be re-scaled using accurate outflow properties. We do not find a significant impact of the outflows on the global star formation rates., Comment: 24 pages, 13 figures and 4 tables. However, some areas of health practice, particularly many of the allied health areas, are generally considered to have a low evidence base. In order to successfully build research capacity in allied health, a clearer understanding is required of what assists and encourages research as well as the barriers and challenges.Participants and methods: This study used written surveys to collect data relating to motivators, enablers, and barriers to research capacity building. Respondents were asked to answer questions relating to them as individuals and other questions relating to their team. Allied health professionals were recruited from multidisciplinary primary health care teams in Queensland Health. Eighty-five participants from ten healthcare teams completed a written version of the research capacity and culture survey.Results: The results of this study indicate that individual allied health professionals are more likely to report being motivated to do research by intrinsic factors such as a strong interest in research. Barriers they identified to research are more likely to be extrinsic factors such as workload and lack of time. Allied health professionals identified some additional factors that impact on their research capacity than those reported in the literature, such as a desire to keep at the “cutting edge” and a lack of exposure to research. Some of the factors influencing individuals to do research were different to those influencing teams. These results are discussed with reference to organizational behavior and theories of motivation.Conclusion: Supporting already motivated allied health professional individuals and teams to conduct research by increased skills training, infrastructure, and quarantined time is likely to produce better outcomes for research capacity building investment.Keywords: research capacity building, allied health professionals, motivation theory

Abstract

We present a framework for quantifying the uncertainty that results from the choice of exchange-correlation (XC) functional in predictions of phonon properties and thermal conductivity that use density functional theory (DFT) to calculate the atomic force constants. The energy ensemble capabilities of the BEEF-vdW XC functional are first applied to determine an ensemble of interatomic force constants, which are then used as inputs to lattice dynamics calculations and a solution of the Boltzmann transport equation. The framework is applied to isotopically-pure silicon. We find that the uncertainty estimates bound property predictions (e.g., phonon dispersions, specific heat, thermal conductivity) from other XC functionals and experiments. We distinguish between properties that are correlated with the predicted thermal conductivity [e.g., the transverse acoustic branch sound speed ($R^2=0.89$) and average Gr\"uneisen parameter ($R^2=0.85$)] and those that are not [e.g., longitudinal acoustic branch sound speed ($R^2=0.23$) and specific heat ($R^2=0.00$)]. We find that differences in ensemble predictions of thermal conductivity are correlated with the behavior of phonons with mean free paths between $100$ and $300$ nm. Abstract

Considerable uncertainties remain about the nature of warm, AGN-driven outflows and their impact on the evolution of galaxies. This is because the outflows are often unresolved in ground-based observations. As part of a project to study the AGN outflows in some of the most rapidly evolving galaxies in the local Universe, here we present HST/STIS observations of F14394+5332E that resolve the sub-kpc warm outflow for the first time in a ULIRG. The observations reveal a compact, high-ionization outflow region (r_max~0.9 kpc) set in a more extensive (r_max~1.4 kpc) halo that is kinematically quiescent and has a lower ionization state. A large line width (600 < FWHM < 1500 km/s) is measured throughout the outflow region, and the outflowing gas shows a steep velocity gradient with radius, with the magnitude of the blueshifted velocities increasing from ~500 to 1800 km/s from the inner to the outer part of the outflow. We interpret the observations in terms of the local acceleration, and hydrodynamic destruction, of dense clouds as they are swept up in a hot, low density wind driven by the AGN. We discuss the implications for measuring the mass outflow rates and kinetic powers for the AGN-driven outflows in such objects., Comment: 10 pages, 5 figures

Abstract

Accurate prediction of molecular vibrational frequencies is important to identify spectroscopic signatures and reaction thermodynamics. In this work, we develop a method to quantify uncertainty associated with density functional theory predicted harmonic vibration frequencies utilizing the built-in error estimation capabilities of the BEEF-vdW exchange-correlation functional. The method is computationally efficiency by estimating the uncertainty at nearly the same computational cost as a single vibrational frequency calculation. We demonstrate the utility and robustness of the method by showing that the uncertainty estimates bounds the self-consistent calculations of six exchange correlation functionals for small molecules, rare gas dimers, and molecular complexes from the S22 dataset. Ten rare-gas dimers and the S22 dataset of molecular complexes provide a rigorous test as they are systems with complicated vibrational motion and non-covalent interactions. Using coefficient of variation as a uncertainty metric, we find that modes involving bending or torsional motion and those dominated by non-covalent interactions are found to have higher uncertainty in their predicted frequencies than covalent stretching modes. Abstract

The goal of PrecisionTox is to overcome conceptual barriers to replacing traditional mammalian chemical safety testing by accelerating the discovery of evolutionarily conserved toxicity pathways that are shared by descent among humans and more distantly related animals. An international consortium is systematically testing the toxicological effects of a diverse set of chemicals on a suite of five model species comprising fruit flies, nematodes, water fleas, and embryos of clawed frogs and zebrafish along with human cell lines. Multiple forms of omics and comparative toxicology data are integrated to map the evolutionary origins of biomolecular interactions that are predictive of adverse health effects, to major branches of the animal phylogeny. These conserved elements of adverse outcome pathways (AOPs) and their biomarkers are expected to provide mechanistic insight useful for regulating groups of chemicals based on their shared modes of action. PrecisionTox also aims to quantify risk variation within populations by recognizing susceptibility as a heritable trait that varies with genetic diversity. This initiative incorporates legal experts and collaborates with risk managers to address specific needs within European chemicals legislation, including the uptake of new approach methodologies (NAMs) for setting precise regulatory limits on toxic chemicals.

13. An Anglo Swedish comparison of employee participation in the banking sector

Abstract

The purpose of this research was to compare employee participation practices in a Swedish and a British bank. There has been considerable interest in human resource management over the past decade, of which employee participation forms an important part, but there have been very few studies which attempt a qualitative comparison of international aspects of this subject. By using a wider study, the Price Waterhouse Cranfield Project on . International Human Resource Management, a European context is provided for the case study material, which examines in depth the forms and outcomes of employee participation in a Swedish and British setting. A triangulation methodology was employed using two questionnaires given to employees of each organisation, a series of in-depth interviews, a reading of company documentation and personal visits. This enabled the use of a multiple of approaches with the questionnaires providing a framework for the in-depth interviews. Four hypotheses were posed which offered tentative explanations for the similarities and differences in employee participation practices in Sweden and Britain. The findings were then analysed using Poole's Framework of Participation which proposes a number of contingent factors which influence the outcomes of employee participation. The thesis showed that Swedes allow greater participation in the workplace than the British, explanations of which are rooted in the cultural and ideological differences of the two societies. Secondly, it was shown that the drive for profit or financial stability will override participation mechanisms if it is felt necessary for survival. Thirdly, HRM techniques of employee participation are used mainly at a micro (workplace) level in the organisation as they can safely be distanced from any strategic decision making. Thus the strength of employee participation is very much anchored to the latent power of employees which is influenced by convergent forces such as economic, technological and political factors, and divergent forces such as cultural and ideological factors.

Abstract

Introduction: Spinal stenosis is a chronic, debilitating condition that is expected to affect an increasing number of people as the population ages. Symptomatic spinal stenosis, like other spine pathologies, including disc herniation and degenerative disc disease, traditionally required an open decompressive surgical approach if more conservative approaches failed. An emerging alternative has been developed to address the needs of this population of patients in the form of endoscopic spine surgery (ESS). Advantages of ESS include minimal tissue trauma, decreased risk of damage to the neurovascular structures, minimal epidural fibrosis/scarring, reduced hospital stay, early functional recovery, and improved cosmetic outcomes. The purpose of this study was to review the outcomes of patients undergoing transforaminal endoscopic spinal decompression at an academic pain program. Methods: We conducted a retrospective review of electronic medical records with approval from the University of Florida Institutional Review Board (IRB #202001529). Twenty patients underwent successful transforaminal endoscopic lumbar spinal decompression surgery at UF Health Pain Medicine from July 1, 2019, to June 1, 2020. The majority of cases were performed at L4-5 (n = 14), followed by an equal number (n = 3) of cases at L3-4 and L5-S1. Preoperative and postoperative visual analog scale (VAS) pain scores from patients' pain clinic appointments were obtained from the electronic health records system to assess the intervention as a pain relief strategy. Results: Patients had an average pain reduction of 82% (SD = 31%), resulting in an average postoperative pain score of 1.8 (SD = 2.8) on a 10-point VAS. Conclusion: This study highlights the benefits of endoscopic spine surgery for patients, including pain reduction and reduced scarring.

Abstract

Lumbar synovial cysts (LSC) can impinge neural structures, causing radicular back pain. Conservative treatment options; however, are often ineffective, and traditional surgical techniques can cause joint instability. We describe two cases in which interventional pain physicians used transforaminal endoscopic spine surgery to treat LSC. The patients reported complete resolution of their lower back and radicular pain and the procedures preserved their motor function and sensation in their bilateral lower extremities. This technique is a viable option for remediation of LSC and can be performed by well-trained pain physicians.

Abstract

Coronavirus disease 2019 (COVID-19) causes significant mortality and morbidity, with an unknown impact in the medium to long term. Evidence from previous coronavirus epidemics indicates that there is likely to be a substantial burden of disease, potentially even in those with a mild acute illness. The clinical and occupational effects of COVID-19 are likely to impact on the operational effectiveness of the Armed Forces. Collaboration between Defence Primary Healthcare, Defence Secondary Healthcare, Defence Rehabilitation and Defence Occupational Medicine resulted in the Defence Medical Rehabilitation Centre COVID-19 Recovery Service (DCRS). This integrated clinical and occupational pathway uses cardiopulmonary assessment as a cornerstone to identify, diagnose and manage post-COVID-19 pathology.

Abstract

OBJECTIVE The aim of the study was to assess the relationship between leisure time exercise and whether workers ever had modified duty or lost time (MD/LT) due to low back pain (LBP) in an occupational cohort. METHODS Workers (N = 827) completed a structured interview assessing characteristics of their LBP, whether or not the pain caused modified or lost work time, and their participation in leisure time exercise. Odds ratio of modified/lost time and minutes of exercise participation were assessed. RESULTS Workers who participated in over 316 min/wk of leisure time exercise incurred significantly less modified/lost time, adjusted odds ratio = 0.46 (95% confidence interval, 0.23 to 0.98). There also lies a significant trend between increases in leisure time exercise and reductions in modified/lost time (P = 0.0016). CONCLUSION These results suggest exercise reduces risk of MD/LT from LBP.
