Abstract

Objectives: Since the results of the RECOVERY trial, WHO recommendations about the use of corticosteroids (CTs) in COVID-19 have changed. The aim of the study is to analyse the evolutive use of CTs in Spain during the pandemic to assess the potential influence of new recommendations. Material and methods: A retrospective, descriptive, and observational study was conducted on adults hospitalised due to COVID-19 in Spain who were included in the SEMI-COVID-19 Registry from March to November 2020. Results: CTs were used in 6053 (36.21%) of the included patients. The patients were older (mean (SD)) (69.6 (14.6) vs. 66.0 (16.8) years, p &lt, 0.001), with hypertension (57.0% vs. 47.7%, 0.001), obesity (26.4% vs. 19.3%, 0.0001), and multimorbidity prevalence (20.6% vs. 16.1%, 0.001). These patients had higher values (mean (95% CI)) of C-reactive protein (CRP) (86 (32.7–160) vs. 49.3 (16–109) mg/dL, 0.001), ferritin (791 (393–1534) vs. 470 (236–996) µg/dL, 0.001), D dimer (750 (430–1400) vs. 617 (345–1180) µg/dL, 0.001), and lower Sp02/Fi02 (266 (91.1) vs. 301 (101), 0.001). Since June 2020, there was an increment in the use of CTs (March vs. September, 0.001). Overall, 20% did not receive steroids, and 40% received less than 200 mg accumulated prednisone equivalent dose (APED). Severe patients are treated with higher doses. The mortality benefit was observed in patients with oxygen saturation &lt, /=90%. Conclusions: Patients with greater comorbidity, severity, and inflammatory markers were those treated with CTs. In severe patients, there is a trend towards the use of higher doses. The mortality benefit was observed in patients with oxygen saturation &lt, /=90%.

Abstract

(1) Objectives: To describe the clinical characteristics and clinical course of hospitalized patients with COVID-19 and autoimmune diseases (ADs) compared to the general population. (2) Methods: We used information available in the nationwide Spanish SEMI-COVID-19 Registry, which retrospectively compiles data from the first admission of adult patients with COVID-19. We selected all patients with ADs included in the registry and compared them to the remaining patients. The primary outcome was all-cause mortality during admission, readmission, and subsequent admissions, and secondary outcomes were a composite outcome including the need for intensive care unit (ICU) admission, invasive and non-invasive mechanical ventilation (MV), or death, as well as in-hospital complications. (3) Results: A total of 13,940 patients diagnosed with COVID-19 were included, of which 362 (2.6%) had an AD. Patients with ADs were older, more likely to be female, and had greater comorbidity. On the multivariate logistic regression analysis, which involved the inverse propensity score weighting method, AD as a whole was not associated with an increased risk of any of the outcome variables. Habitual treatment with corticosteroids (CSs), age, Barthel Index score, and comorbidity were associated with poor outcomes. Biological disease-modifying anti-rheumatic drugs (bDMARDs) were associated with a decrease in mortality in patients with AD. (4) Conclusions: The analysis of the SEMI-COVID-19 Registry shows that ADs do not lead to a different prognosis, measured by mortality, complications, or the composite outcome. Considered individually, it seems that some diseases entail a different prognosis than that of the general population. Immunosuppressive/immunoregulatory treatments (IST) prior to admission had variable effects.

Abstract

The production of H 2 via the photocatalytic transformation of aqueous solutions of ethanol, over Pt/anatase was studied in a semi-batch reactor. High H 2 /CO x ratios were obtained indicating that the photoreforming of ethanol only occurred in a small proportion of the ethanol. The main products detected in the liquid phase after the photocatalytic conversion of ethanol solutions were acetaldehyde, acetic acid and 2,3-butanediol, with acetaldehyde being the most abundant. The photoreaction of acetaldehyde (aq) , acetic acid (aq) or 2,3-butanediol (aq) resulted in lower amounts of H 2 compared with that of ethanol (aq) . The main reaction pathway for the photocatalytic transformation of acetaldehyde (aq) is via the formation of acetic acid and H 2 . Then, CO 2 and CH 4 are formed from the decomposition of acetic acid (aq) . The decrease in the H 2 production rate during the photocatalytic reforming of ethanol solutions is related to the presence of the intermediate products formed in the liquid phase that are not removed from the reaction medium. Abstract

Abstract: We examined the effect of the activation process on the structural and morphological characteristics of a cobalt-based catalyst for Fischer–Tropsch synthesis. A 10wt.% Co/SiO2 catalyst prepared by wet impregnation was separately activated under H2, CO or a H2/CO mixture. The structural changes during activation from 298 to 773K were studied by in situ X-ray diffraction. Catalysts were examined by SEM, TEM, XPS and in situ DRIFT-MS. The H2/CO activation produced redispersion of cobalt particles and simultaneous carbon nanostructures formation. The catalyst showed the highest performance in the Fischer–Tropsch synthesis after the H2/CO activation. [Copyright &y& Elsevier]
