Introduction: Vertebral body tethering (VBT) has become an alternative option for select patients with idiopathic scoliosis. However, studies have shown a high number of tether breakages, specifically after thoracolumbar (TL) VBT, that can have a negative impact on the outcome, when the breakage occurs within the first year after surgery. In order to overcome this problem, we have started to apply an apical fusion (AF) in combination with TL VBT for select patients. This study aims to analyze the outcome after AF plus VBT. Methods: This is a retrospective single surgeon's data analysis. All patients were included who have had TL VBT after January 2022 and a follow-up of 12 months. Patients were grouped based on whether they only had VBT or VBT + AF. Results: Twenty-five patients were analyzed (15 VBT, 10 VBT + AF). Both groups showed a significant curve correction for thoracic and TL curves. Minor loss of correction was observed in both groups. A significant difference was seen regarding early tether breakages, which were found in 60% of VBT patients and 10% of VBT + AF patients. One key role in industrial control systems (ICSs) is known as Programmable Logic Controller (PLC). However, with the development of the Internet of Things (IoT), PLCs have become exposed to an increasing number of attacks, which may cause malfunctions of the whole ICS. Thus, it is necessary to identify potential attacks on PLCs and propose effective solutions to mitigate them. Unfortunately, to date, there have not been significant efforts made to provide a detailed overview of existing works on PLC security. With such a concern in mind, in this paper, we focus on summarising PLC security from different components running at different layers of a PLC architecture. We first review the framework of PLCs; then, we discuss several models when considering PLC security. After that, we provide an overview of existing attacks on PLCs and general solutions to those issues from different perspectives. Landslide disasters have garnered significant attention due to their extensive devastating impact, leading to a growing emphasis on the prompt and precise identification and detection of landslides as a prominent area of research. Previous research has primarily relied on human–computer interactions and visual interpretation from remote sensing to identify landslides. However, these methods are time-consuming, labor-intensive, subjective, and have a low level of accuracy in extracting data. An essential task in deep learning, semantic segmentation, has been crucial to automated remote sensing image recognition tasks because of its end-to-end pixel-level classification capability. In this study, to mitigate the disadvantages of existing landslide detection methods, we propose a multiscale attention segment network (MsASNet) that acquires different scales of remote sensing image features, designs an encoder–decoder structure to strengthen the landslide boundary, and combines the channel attention mechanism to strengthen the feature extraction capability. The MsASNet model exhibited an average accuracy of 95.13% on the test set from Bijie's landslide dataset, a mean accuracy of 91.45% on the test set from Chongqing's landslide dataset, and a mean accuracy of 90.17% on the test set from Tianshui's landslide dataset, signifying its ability to extract landslide information efficiently and accurately in real time. Background: Although several biomechanical studies have been reported, few clinical studies have compared the efficacy of monoaxial and polyaxial pedicle screws in the surgical treatment of adolescent idiopathic scoliosis (AIS). This study aims to compare the radiological and clinical outcomes of mono- and polyaxial pedicle screws in the surgical treatment of AIS. Methods: A total of 46 AIS patients who underwent surgery to treat scoliosis using pedicle screw instrumentation (PSI) and rod derotation (RD) were divided into two groups according to the use of pedicle screws: the monoaxial group (n = 23) and polyaxial group (n = 23). Results: The correction rate of the main Cobb's angle was higher in the monoaxial group (70.2%) than in the polyaxial group (65.3%) (p = 0.040). No differences in the rotational correction of the apical vertebra were evident between the two groups. SRS-22 scores showed no significant differences according to the type of pedicle screws used. Conclusions: The use of polyaxial pedicle screws resulted in coronal, sagittal, and rotational correction outcomes comparable to those associated with the use of monoaxial pedicle screws for surgical treatment using PSI and RD to treat moderate cases of AIS. The segmentation of crops and weeds from camera-captured images is a demanding research area for advancing agricultural and smart farming systems. Previously, the segmentation of crops and weeds was conducted within a homogeneous data environment where training and testing data were from the same database. However, in the real-world application of advancing agricultural and smart farming systems, it is often the case of a heterogeneous data environment where a system trained with one database should be used for testing with a different database without additional training. This study pioneers the use of heterogeneous data for crop and weed segmentation, addressing the issue of degraded accuracy. Through adjusting the mean and standard deviation, we minimize the variability in pixel value and contrast, enhancing segmentation robustness. Unlike previous methods relying on extensive training data, our approach achieves real-world applicability with just one training sample for deep learning-based semantic segmentation. Moreover, we seamlessly integrated a method for estimating fractal dimensions into our system, incorporating it as an end-to-end task to provide important information on the distributional characteristics of crops and weeds. We evaluated our framework using the BoniRob dataset and the CWFID. When trained with the BoniRob dataset and tested with the CWFID, we obtained a mean intersection of union (mIoU) of 62% and an F1-score of 75.2%. Furthermore, when trained with the CWFID and tested with the BoniRob dataset, we obtained an mIoU of 63.7% and an F1-score of 74.3%. The purpose of this systematic review and meta-analysis is to perform a systematic review and meta-analysis of previous studies on minimally invasive scoliosis surgery (MISS) in adolescents with idiopathic scoliosis (AIS). Some data on MISS in AIS compared with conventional open scoliosis surgery (COSS) are conflicting. A systematic literature search was conducted in Medline, Embase, and Cochrane Library, including studies reporting outcomes for MISS in AIS. The meta-analysis compared the operative, radiological, and clinical outcomes and complications between MISS and COSS in patients with AIS. Of the 208 records identified, 15 nonrandomized studies with 1,369 patients (reviews and case reports are excluded) were included in this systematic review and meta-analysis. The mean scale was 6.1, and eight of the 15 included studies showed satisfactory quality using the Newcastle-Ottawa scale. For operative outcomes, MISS had significant benefits in terms of estimated blood loss (standard mean difference [SMD], −1.87; 95% confidence interval [CI], −2.94 to −0.91) and hospitalization days (SMD, −2.99; 95% CI, −4.45 to −1.53) compared with COSS. However, COSS showed significantly favorable outcomes for operative times (SMD, 1.71; 95% CI, 0.92–2.51). No significant differences were observed in radiological outcomes, including Cobb's angle of the main curve and thoracic kyphosis. For clinical outcomes, MISS showed significant benefits on the visual analog scale score (SMD, −0.91; 95% CI, −1.36 to −0.47). The overall complication rates of MISS were similar to those of COSS (SMD, 0.96; 95% CI, 0.61–1.52). MISS using the posterior approach provides equivalent radiological and clinical outcomes and complication rates compared with COSS. Considering the lower estimated blood loss, shorter hospitalization days, and longer operative times in MISS, COSS is still the mainstay of surgical treatment in AIS; however, MISS using the posterior approach is also one of the surgical options of choice in the case of moderate AIS. (1) Background: The amount of blood loss during oblique lumber interbody fusion (OLIF) surgery is often underestimated and may contribute to adverse postoperative outcomes. This study aims to evaluate hidden blood loss (HBL) in patients who underwent OLIF for degenerative lumbar spine disease and to analyze its risk factors. (2) Methods: The medical records of 179 patients who underwent OLIF surgery from 2015 to 2022 were reviewed. The HBL and total blood loss (TBL) were estimated using the Gross formula. Pearson correlation, Spearman correlation, and multivariate linear regression analyses were used to investigate risk factors for HBL. (3) Results: The mean HBL was 675.2 mL, and the mean hemoglobin loss was 1.7 g/dL during OLIF surgery. In the multivariate linear regression analysis, TBL (p < 0.001), estimated blood loss (p < 0.001), and pedicle screw fixation type (p = 0.039) were identified as independent risk factors of HBL. (4) Conclusions: The OLIF is associated with substantial perioperative HBL, for which we identified risk factors of TBL, EBL, and pedicle screw fixation type. Brain cholesterol metabolic products include neurosteroids and oxysterols, which play important roles in cellular physiology. In neurons, the cholesterol oxidation product, 24S-hydroxycholesterol (24S-HC), is a regulator of signaling and transcription. Here, we examined the behavioral effects of 24S-HC loss, using global and cell-selective genetic deletion of the synthetic enzyme CYP46A1. Mice that are globally deficient in CYP46A1 exhibited hypoactivity at young ages and unexpected increases in conditioned fear memory. Despite strong reductions in hippocampal 24S-HC in mice with selective loss of CYP46A1 in VGLUT1-positive cells, behavioral effects were not recapitulated in these conditional knockout mice. Global knockout produced strong, developmentally dependent transcriptional effects on select cholesterol metabolism genes. These included paradoxical changes in Liver X Receptor targets. Again, conditional knockout was insufficient to recapitulate most changes. Overall, our results highlight the complex effects of 24S-HC in an in vivo setting that are not fully predicted by known mechanisms. The results also demonstrate that the complete inhibition of enzymatic activity may be needed for a detectable, therapeutically relevant impact on gene expression and behavior. The crystal base B (λ) is a colored directed graph that captures the structure of V q (λ) and the action of U q (g) on V q (λ) in a rudimentary manner. Likewise, the crystal base B (∞) holds the bare structure of the negative part U q − (g). In this work, we describe realizations of the crystal B (λ) via two separate approaches for the cases when the base Lie algebra g is of E 6 and E 7 types. Our first description relies on the fact that B (λ) appears as a connected component within the much larger crystal B (∞) ⊗ { r λ } , where { r λ } is a certain single-element crystal. Choosing to represent elements of B (∞) with marginally large tableaux, we identify those elements belonging to the mentioned connected component. Our second description of B (λ) is a translation of the first description into one involving the Kashiwara embedding, which is an embedding of B (∞) into a tensor product of a series of much simpler crystals. Postmenopausal osteoporosis arises from imbalanced osteoclast and osteoblast activity, and mounting evidence suggests a role for the osteoimmune system in bone homeostasis. Bisphosphonate (BP) is an antiresorptive agent, but its treatment failure rate can be as high as 40%. Here, we performed single-cell RNA sequencing on peripheral immune cells from carefully selected postmenopausal women: non-osteoporotic, osteoporosis improved after BP treatment, and BP-failed cases. We found an increase in myeloid cells in patients with osteoporosis (specifically, T cell receptor+ macrophages). Furthermore, lymphoid lineage cells varied significantly, notably elevated natural killer cells (NKs) in the BP-failed group. Moreover, we provide fruitful lists of biomarkers within the immune cells that exhibit condition-dependent differences. The existence of osteoporotic-and BP-failure-specific cellular information flows was revealed by cell-cell interaction analysis. These findings deepen our insight of the osteoporosis pathology enhancing comprehension of the role of immune heterogeneity in postmenopausal osteoporosis and BP treatment failure. Doxorubicin (DOX) is an effective anti-tumor drug accompanied with many side effects, especially heart injury. To explore what effects of sophocarpine (SOP) on DOX-induced heart injury, this study conducted in vivo experiment and in vitro experiment, and the C57BL/6J mice and the H9C2 cells were used. The experimental methods used included echocardiography, enzyme-linked immunosorbent assay (ELISA), dihydroethidium (DHE) staining, terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) staining, western blotting and so on. Echocardiography showed that SOP alleviated DOX-induced cardiac dysfunction, as evidenced by the improvements of left ventricle ejection fraction and left ventricle fractional shortening. DOX caused upregulations of creatine kinase (CK), creatine kinase-MB (CK-MB) and lactate dehydrogenase (LDH), while SOP reduced these indices. The relevant stainings showed that SOP reversed the increases of total superoxide level induced by DOX. DOX also contribute to a higher level of MDA and lower levels of SOD and GSH, but these changes were suppressed by SOP. DOX increased the pro-oxidative protein level of NOX-4 while decreased the anti-oxidative protein level of SOD-2, but SOP reversed these effects. In addition, this study further discovered that SOP inhibited the decreases of Nrf2 and HO-1 levels induced by DOX. The TUNEL staining revealed that SOP reduced the high degree of apoptosis induced by DOX. Besides, pro-apoptosis proteins like Bax, cleaved-caspase-3 and cytochrome-c upregulated while anti-apoptosis protein like Bcl-2 downregulated when challenged by DOX, but them were suppressed by SOP. These findings suggested that SOP could alleviate DOX-induced heart injury by suppressing oxidative stress and apoptosis, with molecular mechanism activating of the Nrf2/HO-1 signaling pathway. Degenerative lumbar spinal disease (DLSD), including spondylolisthesis and spinal stenosis, is increasing due to the aging population. Along with the disease severity, lumbar interbody fusion (LIF) is a mainstay of surgical treatment through decompression, the restoration of intervertebral heights, and the stabilization of motion segments. Currently, pseudoarthrosis after LIF is an important and unsolved issue, which is closely related to osteobiologies. Of the many signaling pathways, the bone morphogenetic protein (BMP) signaling pathway contributes to osteoblast differentiation, which is generally regulated by SMAD proteins as common in the TGF-β superfamily. BMP-2 and -4 are also inter-connected with Wnt/β-catenin, Notch, and FGF signaling pathways. With the potent potential for osteoinduction in BMP-2 and -4, the combination of allogenous bone and recombinant human BMPs (rhBMPs) is currently an ideal fusion material, which has equalized or improved fusion rates compared to traditional materials. However, safety issues in the dosage of BMP remain, so overcoming current limitations will provide significant advancement in spine surgery. This post‐hoc analysis evaluated the efficacy and safety of intranasal esketamine in the Asian subgroup from ASPIRE I. Patients with major depressive disorder and suicidal ideation with intent received intranasal esketamine (n = 26) or placebo (n = 27), plus standard of care for 25 days. The primary endpoint was the change in Montgomery‐Åsberg Depression Rating Scale (MADRS) total score from baseline to Day 2. The MADRS score improved in favor of esketamine (least squares mean difference: −3.8). No unexpected safety concerns were noted. Respiratory syncytial virus (RSV) can lead to serious disease in infants, and no approved RSV vaccine is available for infants. This first in-human clinical trial evaluated a single dose of BLB201, a PIV5-vectored RSV vaccine administrated via intranasal route, for safety and immunogenicity in RSV-seropositive healthy adults (33 to 75 years old). No severe adverse events (SAEs) were reported. Solicited local and systemic AEs were reported by <50% of participants and were mostly mild in intensity. Vaccine virus shedding was detected in 17% of participants. Nasal RSV-specific immunoglobulin A responses were detected in 48%, the highest level observed in adults among all intranasal RSV vaccines evaluated in humans. RSV-neutralizing antibodies titers in serum rose ≥1.5-fold. Peripheral blood RSV F-specific CD4+ and CD8+ T cells increased from ≤0.06% at baseline to ≥0.26 and 0.4% after vaccination, respectively, in >93% participants. This study aims to activate the external urethral sphincter (EUS), which plays a critical role in micturition control, through optogenetics and to determine its potential contribution to the stabilization of sensitized micturition activity. The viral vector (AAV2/8-CMV-hChR2(H134R)-EGFP) is utilized to introduce light-gated ion channels (hChR2/H134R) into the EUS of wild-type C57BL/6 mice. Following the induction of sensitized micturition activity using weak acetic acid (0.1%) in anesthetized mice, optical stimulation of the EUS muscle tissue expressing channel rhodopsin is performed using a 473 nm laser light delivered through optical fibers, and the resulting changes in muscle activation and micturition activity are examined. Through EMG (electromyography) measurements, it is confirmed that optical stimulation electrically activates the EUS muscle in mice. Analysis of micturition activity using cystometry reveals a 70.58% decrease in the micturition period and a 70.27% decrease in the voiding volume due to sensitized voiding. However, with optical stimulation, the micturition period recovers to 101.49%, and the voiding volume recovered to 100.22%. Stimulation of the EUS using optogenetics can alleviate sensitized micturition activity and holds potential for application in conjunction with other micturition control methods. Epidural administration is the leading therapeutic option for the management of pain associated with lumbar spinal stenosis (LSS), which is characterized by compression of the nerve root due to narrowing of the spinal canal. Corticosteroids are effective in alleviating LSS-related pain but can lead to complications with long-term use. Recent studies have focused on identifying promising medications administered epidurally to affected spinal regions. In this study, we aimed to investigate the effectiveness of harpagoside (HAS) as an epidural medication in rats with LSS. HAS at various concentrations was effective for neuroprotection against ferrous sulfate damage and consequent promotion of axonal outgrowth in primary spinal cord neurons. When two concentrations of HAS (100 and 200 μg/kg) were administered to the rat LSS model via the epidural space once a day for 4 weeks, the inflammatory responses around the silicone block used for LSS were substantially reduced. Consistently, pain-related factors were significantly suppressed by the epidural administration of HAS. The motor functions of rats with LSS significantly improved. These findings suggest that targeted delivery of HAS directly to the affected area via epidural injection holds promise as a potential treatment option for the recovery of patients with LSS. Despite the significant number of classification studies conducted using plant images, studies on nonlinear motion blur are limited. In general, motion blur results from movements of the hands of a person holding a camera for capturing plant images, or when the plant moves owing to wind while the camera is stationary. When these two cases occur simultaneously, nonlinear motion blur is highly probable. Therefore, a novel deep learning-based classification method applied on plant images with various nonlinear motion blurs is proposed. In addition, this study proposes a generative adversarial network-based method to reduce nonlinear motion blur; accordingly, the method is explored for improving classification performance. Herein, experiments are conducted using a self-collected visible light images dataset. Evidently, nonlinear motion deblurring results in a structural similarity index measure (SSIM) of 73.1 and a peak signal-to-noise ratio (PSNR) of 21.55, whereas plant classification results in a top-1 accuracy of 90.09% and F1-score of 84.84%. In addition, the experiment conducted using two types of open datasets resulted in PSNRs of 20.84 and 21.02 and SSIMs of 72.96 and 72.86, respectively. The proposed method of plant classification results in top-1 accuracies of 89.79% and 82.21% and F1-scores of 84% and 76.52%, respectively. Zn-Mn aqueous batteries (ZMABs) have the potential for grid-scale energy storage due to their low cost, high safety, and eco-friendliness. Recent advancements in high-capacity cathodes, dendrite-free anodes, and functionalized electrolytes have brought rechargeable ZMABs closer to practical applications. However, challenges remain in stabilizing the interface, particularly in electrolyte regulation. A recent study published in the National Science Review reports on the development of a lean-water quasi-eutectic electrolyte that improves the reversible interfacial deposition and reaction kinetics of Mn-based cathodes in a long cycle process. The study highlights the importance of electrolyte optimization in addressing the energy density and lifespan limitations of ZMABs. pH balance, solvation regulation, and redox mediation are identified as effective strategies for durable ZMABs. The authors propose future research directions, including the design of novel pH buffer electrolytes, regulation of Mn2+ solvation structure, development of stable redox mediators, and the creation of wide-temperature electrolytes for improved performance. To evaluate the learning curve of minimally invasive scoliosis surgery (MISS) in patients with adolescent idiopathic scoliosis (AIS). A total of 76 AIS patients who underwent MISS were included from March 2015 to February 2017. The cases were divided chronologically into early period group (Group A, March 2015–February 2016, n = 31) and late period group (Group B, March 2016–February 2017, n = 45). Radiological parameters, operative parameters, and clinical outcomes were included in this study. The selected criteria for receiver operating characteristic analysis used to assess learning curve were mean operative time (≤389 minutes) and estimated blood loss (EBL) (≤948 mL). The mean operative time was 389 minutes, and the EBL was 948 mL. The mean operative time was significantly reduced in the late period group (360 minutes) compared to the early period group (431 minutes) (P < 0.005). The mean EBL was significantly reduced for the late period group (798 mL) compared to the early period group (1164 mL) (P < 0.001). Forty-six cases were required to achieve the preset criteria (area under the curve 0.858; P < 0.001; sensitivity 0.778; specificity 0.796). A mild learning curve was demonstrated in MISS for AIS with significant improvement in operative times and EBL over time. Pelvic fixation is performed to obtain proper coronal and sagittal alignment when the pelvic obliquity is more than 15º in patients with neuromuscular scoliosis (NMS). Since many NMS patients were wheelchair or bed-ridden status, there has been controversy on the effect of pelvic fixation. Therefore, the purpose of this study is to analyze the effects of pelvic fixation on correction of spinal deformity and quality of life (QoL) in NMS patients. A total of 77 NMS patients who underwent deformity correction were divided into three groups and retrospectively analyzed preoperatively, postoperatively, and at 2-year follow-up: pelvic fixation group (Group A, n = 16), fixed to S1 (Group B, n = 33), and fixed to L5 (Group C, n = 28). The correction rate of scoliosis was 60.0%, 58.0%, and 56.7% in groups A, B, and C, respectively, with no statistical difference (P > 0.05). The correction rate of pelvic obliquity was 61.3%, 42.8%, and 57.5% in respective groups A, B, and C, with no significance (P > 0.05). The correction loss of scoliosis and pelvic obliquity showed no statistical significance between three groups for 2-year follow-up (all Ps > 0.05). There were no significant differences regarding clinical outcomes and postoperative complications among the three groups (all Ps > 0.05). Therefore, pelvic fixation using iliac screws is not substantially influencing radiological and clinical outcomes in the patients with NMS. [ABSTRACT FROM AUTHOR]

(1) Background: Bisphosphonate treatment failure is one of the most difficult clinical problems for patients with osteoporosis. This study aimed to analyze the incidence of bisphosphonate treatment failure, associated radiological factors, and effect of fracture healing in postmenopausal women with osteoporotic vertebral fractures (OVFs). (2) Methods: A total of 300 postmenopausal patients with OVFs who were prescribed bisphosphonate were retrospectively analyzed and divided into two groups according to the treatment response: response (n = 116) and non-response (n = 184) groups. The radiological factors and the morphological patterns of OVFs were included in this study. (3) Results: The initial BMD values of the spine and femur in the non-response group were significantly lower than those in the response group (all Ps < 0.001). The initial BMD value of the spine (odd ratio = 1.962) and the fracture risk assessment tool (FRAX) hip (odd ratio = 1.32) showed statistical significance in logistic regression analysis, respectively (all Ps < 0.001). (4) Conclusions: The bisphosphonate non-responder group showed a greater decrease in BMD over time than the responder group. The initial BMD value of the spine and the FRAX hip could be considered radiological factors influencing bisphosphonate non-response in the postmenopausal patients with OVFs. Extracellular vesicles (EVs) possess great potential in the modulation of cardiovascular diseases. Our current work intended to assay the clinical significance of endothelial cell (EC)‐derived EVs in atherosclerosis (AS). Expression of HIF1A‐AS2, miR‐455‐5p, and ESRRG in plasma from AS patients and mice and EVs from ox‐LDL‐treated ECs was measured. Interactions among HIF1A‐AS2, miR‐455‐5p, ESRRG, and NLRP3 were analyzed. Next, EVs were co‐cultured with ECs, and ectopic expression and depletion experimentations of HIF1A‐AS2, miR‐455‐5p, ESRRG, and/or NLRP3 were carried out to assay their roles in pyroptosis and inflammation of ECs in AS. At last, the effects of HIF1A‐AS2 shuttled by EC‐derived EVs on EC pyroptosis and vascular inflammation in AS were verified in vivo. HIF1A‐AS2 and ESRRG were highly expressed, while miR‐455‐5p was poorly expressed in AS. HIF1A‐AS2 could sponge miR‐455‐5p to elevate the expression of ESRRG and NLRP3. Both in vitro and in vivo experiments revealed that ECs‐derived EVs carrying HIF1A‐AS2 induced the pyroptosis and vascular inflammation of ECs to promote the progression of AS by sponging miR‐455‐5p via ESRRG/NLRP3. Background: Non-curative resection (non-CR) after endoscopic submucosal dissection (ESD) requires additional surgery due to the possibility of lymph node metastasis (LNM). Therefore, it is important to accurately predict the risk of non-CR to avoid unnecessary preoperative procedures. Thus, we aimed to develop and verify a nomogram to predict the risk of non-CR prior to ESD. Methods: Patients who underwent ESD for early gastric cancer (EGC) were divided into CR and non-CR groups based on the present ESD criteria. The pre-procedural factors, such as endoscopic features, radiologic findings, and pathology of the lesion, were compared between the groups to identify the risk factors associated with non-CR. A nomogram was developed using multivariate analysis, and its predictive value was assessed using an external validation group. Results: Among 824 patients, 682 were curative (82.7%) and 142 were non-curative (17.3%). By comparing two groups, endoscopic features including redness, whitish mucosal change, fold convergence, and large lesion size; histologic features such as moderately or poorly differentiated or signet ring cell carcinoma; and abnormal CT findings including non-specific lymph node enlargement and fold thickening were identified as significant predictors of non-CR. The nomogram was developed based on these predictors and showed good predictive performance in the external validation, with an area under the curve of 0.87. Conclusions: We developed a nomogram to predict the risk of non-CR prior to ESD. Human respiratory information is being used as an important source of biometric information that can enable the analysis of health status in the healthcare domain. The analysis of the frequency or duration of a specific respiration pattern and the classification of respiration patterns in the corresponding section for a certain period of time are important for the utilization of respiratory information in various ways. Existing methods require window slide processing to classify sections for each respiration pattern from the breathing data for a certain time period. In this case, when multiple respiration patterns exist within one window, the recognition rate can be lowered. To solve this problem, a 1D Siamese neural network (SNN)-based human respiration pattern detection model and a merge-and-split algorithm for the classification of multiple respiration patterns in each region for all respiration sections are proposed in this study. When calculating the accuracy based on intersection over union (IOU) for the respiration range classification result for each pattern, the accuracy was found to be improved by approximately 19.3% compared with the existing deep neural network (DNN) and 12.4% compared with a 1D convolutional neural network (CNN). Purpose: To compare paraspinal muscle quality between patients with single and multiple osteoporotic vertebral fractures (OVFs) and evaluate the role of the paraspinal muscles in OVFs. Methods: A total of 262 consecutive patients with OVFs were retrospectively analyzed in two groups: those with single OVF (n = 173) and those with multiple OVFs (n = 89). The cross-sectional area (CSA) and fatty degeneration of the paraspinal muscles were calculated from axial T2-weighted magnetic resonance imaging at the level of the L4 upper endplate by manual tracing in ImageJ software. Pearson's correlation analysis was performed to analyze correlations of paraspinal muscle quality to multiple OVFs. Results: FD in all the paraspinal muscles was significantly higher in the multiple OVF group than the single OVF group (all p < 0.005). The functional CSA (fCSA) of the paraspinal muscles was significantly lower in the multiple OVF group than the single OVF group (all Ps < 0.001), except for the erector spine (p = 0.304). The Pearson's correlation analysis showed significant positive inter-correlations for the fCSAs of all the paraspinal muscles and the occurrence of multiple OVFs. Conclusions: The pure muscle volumes of the multifidus, psoas major, and quadratus lumborum were lower in patients with multiple OVFs than in those with a single OVF. Furthermore, the inter-correlation among all the paraspinal muscles indicate that the muscle-bone crosstalk profoundly existed in vertebral fracture cascade. DNA polymerase (pol) η is responsible for error-free translesion DNA synthesis (TLS) opposite ultraviolet light (UV)-induced cis-syn cyclobutane thymine dimers (CTDs) and cisplatin-induced intrastrand guanine crosslinks. POLH deficiency causes one form of the skin cancer-prone disease xeroderma pigmentosum variant (XPV) and cisplatin sensitivity, but the functional impacts of its germline variants remain unclear. We evaluated the functional properties of eight human POLH germline in silico-predicted deleterious missense variants, using biochemical and cell-based assays. In enzymatic assays, utilizing recombinant pol η (residues 1—432) proteins, the C34W, I147N, and R167Q variants showed 4- to 14-fold and 3- to 5-fold decreases in specificity constants (kcat/Km) for dATP insertion opposite the 3'-T and 5′-T of a CTD, respectively, compared to the wild-type, while the other variants displayed 2- to 4-fold increases. A CRISPR/Cas9-mediated POLH knockout increased the sensitivity of human embryonic kidney 293 cells to UV and cisplatin, which was fully reversed by ectopic expression of wild-type pol η, but not by that of an inactive (D115A/E116A) or either of two XPV-pathogenic (R93P and G263V) mutants. Ectopic expression of the C34W, I147N, and R167Q variants, unlike the other variants, did not rescue the UV- and cisplatin-sensitivity in POLH-knockout cells. Our results indicate that the C34W, I147N, and R167Q variants—substantially reduced in TLS activity—failed to rescue the UV- and cisplatin-sensitive phenotype of POLH-deficient cells, which also raises the possibility that such hypoactive germline POLH variants may increase the individual susceptibility to UV irradiation and cisplatin chemotherapy. Background: Subthreshold depression and anxiety are highly prevalent and known to be associated with functional limitations and future onset of major depression or anxiety disorders. However, studies regarding suicidality at subthreshold levels of depression and anxiety are limited.Methods: A total of 17,639 Koreans aged from 18 to 64 responded to the Korean version of the WHO-composite international diagnostic interview (K-CIDI) version 2.1 between 2006 and 2016. Prevalence of subthreshold depression and anxiety, and information on lifetime suicide attempt (SA) were assessed. Multivariable logistic regression was performed to calculate the risk of SA by the level of depression or anxiety.Results: The risk of lifetime SA increased incrementally according to the level of depression and anxiety. In the subthreshold depression group, individuals with subthreshold anxiety increased the odds about threefold (OR = 3.15, 95% CI 3.12-3.18) and the threshold anxiety disorder group increased the odds about fivefold (OR = 5.33, 95% CI 5.27-5.38) for SA, compared to the group without any level of anxiety. In threshold depressive disorder, the subthreshold anxiety group showed about 1.8-fold higher odds (OR = 1.76, 95% CI 1.75-1.77) and the threshold anxiety disorder group showed threefold higher odds (OR = 2.93, 95% CI 2.92-2.95) for SA compared to the group without any level of anxiety.Conclusions: Subthreshold levels of depression and anxiety were prevalent among Koreans and were associated with an increased prevalence of lifetime SA. Background: Minimally invasive scoliosis surgery (MISS) is currently introduced on novel technique for surgical treatment of adolescent idiopathic scoliosis (AIS). This study is aimed to evaluate the efficacy of facet fusion in MISS compared to posterior fusion in conventional open scoliosis surgery (COSS) and compare facet fusion rates based on three bone graft substitutes in MISS for adolescent idiopathic scoliosis (AIS). Methods: Eighty six AIS patients who underwent scoliosis surgery were divided into two groups: the COSS group and the MISS group. COSS was performed through posterior fusion with allograft. MISS was applied via facet fusion with three bone graft substitutes. The MISS group was further divided into three subgroups based on graft substitute: Group A (allograft), Group B (demineralized bone matrix [DBM]), and group C (demineralized cancellous bone chips). Fusion rate was measured using conventional radiographs to visualize loss of correction > 10°, presence of lysis around implants, breaks in fusion mass, and abnormal mobility of the fused segment. Results: The fusion rates showed no significant difference in COSS and MISS groups (p = 0.070). In the MISS group, the fusion rates were 85, 100, and 100% in groups A, B, and C, respectively, with no significant difference (p = 0.221). There were no statistical differences between groups A, B, and C in terms of correction rate, fusion rate, and SRS-22 scores (p > 0.05). Conclusions: The facet fusion in MISS showed comparable to posterior fusion in COSS with regard to radiological and clinical outcomes. Furthermore, the type of graft substitute among allograft, DBM, and demineralized cancellous bone chips did not affect facet fusion rate or clinical outcomes in MISS. Tropospheric ozone is an important atmospheric pollutant as well as an efficient greenhouse gas. Beijing is one of the cities with the most serious ozone pollution. However, long-term date of observed ozone in Beijing are limited. In this paper, we combine the measurements of the In-service Aircraft for a Global Observing System (IAGOS), ozonesonde observations as well as the recently available ozone monitoring network observations to produce a unique data record of surface ozone (at 14:00 Beijing time) in Beijing from 1995 to 2020. Using this merged dataset, we investigate the variability in surface ozone in Beijing on multiple timescales. The long-term change is primarily characterized by a sudden drop in 2011–2012 with an insignificant linear trend during the full period. Based on CAM-chem model simulations, meteorological factors played important roles in the 2011–2012 ozone drop. Before and after this sudden drop, ozone levels in Beijing increased significantly by 0.42 ± 0.27 ppbv year−1 before 2011 and 0.43 ± 0.41 ppbv year−1 after 2013. We also found a substantial increase in the amplitude of the ozone annual cycle in Beijing, which has not been documented in previous studies. This is consistent with ozone increases in summer and ozone decreases in winter. In addition, the results by the Ensemble Empirical Mode Decomposition (EEMD) analysis indicate significant interannual variations in ozone levels in Beijing with different time oscillation periods, which may be associated with natural variabilities and subsequent changes in meteorological conditions. Intervertebral disc degeneration (IDD) is an age-dependent progressive spinal disease that causes chronic back or neck pain. Although aging has long been presented as the main risk factor, the exact cause is not fully known. DNA methylation is associated with chronic pain, suggesting that epigenetic modulation may ameliorate disc degeneration. We examined histological changes in the DNA methylation within the discs and their association with pain-related transient receptor potential vanilloid subtype 1 (TrpV1) expression in rats subjected to IDD. Epigenetic markers (5-hydroxymethylcytosine (5hmC), 5-methylcytosine (5Mc)), DNA methyltransferases (DNMTs), and Ten-eleven translocations (Tets) were analyzed using immunohistochemistry, real-time PCR, and DNA dot-blot following IDD. Results revealed high 5mC levels in the annulus fibrosus (AF) region within the disc after IDD and an association with TrpV1 expression. DNMT1 is mainly involved in 5mC conversion in degenerated discs. However, 5hmC levels did not differ between groups. A degenerated disc can lead to locomotor defects as assessed by ladder and tail suspension tests, no pain signals in the von Frey test, upregulated matrix metalloproteinase-3, and downregulated aggrecan levels within the disc. Thus, we found that the DNA methylation status in the AF region of the disc was mainly changed after IDD and associated with aberrant TrpV1 expression in degenerated discs. [ABSTRACT FROM AUTHOR]

Biometrics is a method of recognizing a person based on one or more unique physical and behavioral characteristics. Since each person has a different structure and shape, it is highly secure and more convenient than the existing security system. Among various biometric authentication methods, finger-vein recognition has advantages in that it is difficult to forge because a finger-vein exists inside one's finger and high user convenience because it uses a non-invasive device. However, motion and optical blur may occur for some reasons such as finger movement and camera defocusing during finger-vein recognition, and such blurring occurrences may increase finger-vein recognition error. However, there has been no research on finger-vein recognition considering both motion and optical blur. Therefore, in this study, we propose a new method for increasing finger-vein recognition accuracy based on a network for the restoration of motion and optical blurring in a finger-vein image (RMOBF-Net). Our proposed network continuously maintains features that can be utilized during motion and optical blur restoration by actively using residual blocks and feature concatenation. Also, the architecture RMOBF-Net is optimized to the finger-vein image domain. Experimental results are based on two open datasets, the Shandong University homologous multi-modal traits finger-vein database and the Hong Kong Polytechnic University finger-image database version 1, from which equal error rates of finger-vein recognition accuracy of 4.290–5.779% and 2.465–6.663% were obtained, respectively. Abstract: In this study, we defined obsessive thoughts (OT) as bothersome, unpleasant thoughts about oneself that keep entering the mind against one's will, and compulsive behavior (CB) as behavior that a person repeats against his or her wishes. The study included 12,532 adults selected randomly through a one-person-per-household method. Each subject selected underwent a face-to-face interview using the Korean version of the Composite International Diagnostic Interview with a questionnaire that examines lifetime suicide attempts (LSAs). Among the participants, 341 (2.74%) had OT and 639 (5.14%) had CB. The highest LSA rate was in subjects with both OT and CB, followed by those with either OT or CB; subjects with neither OT nor CB had the lowest LSA rate. In a multivariate logistic regression analysis of OT and CB, OT, ordering, and rituals of repeating words were significantly associated with LSAs. In subjects with OT and CB, those with MDD had a significantly higher risk of LSAs compared with those without, and MDD with both OT and CB showed odds of approximately 27-fold (adjusted odds ratio, 27.24; 13.29-55.82; p < 0.0001) compared with those without MDD, OT, or CB. Plaque-type psoriasis is a common skin disorder. Tirbanibulin (KX01) is a new Src kinase inhibitor with potent antiproliferative activity against keratinocytes and has been approved for treatment of actinic keratosis. This Phase I study investigates the safety and activity of KX01 ointment in patients with plaque-type psoriasis. We recruited 28 patients from two medical centers in Taiwan. This study was performed in four stages. Double-blind treatments were randomized in stages I (KX01 0.01% + placebo, two rounds of two-week treatment) and II (KX01 0.1% + placebo, four weeks) and open-labelled in stages III (KX01 1%, five days) and IV (KX01 1%, five days weekly for four weeks). The safety, tolerability, KX01 concentration, target area score, physician global assessment, and disease relapse were determined. Most treatment-emergent adverse events were mild-to-moderate application site reactions. Three (50.0%) subjects from the stage IV group showed ≥50% reduction in the target area score (TAS50), while two subjects (33.3%) showed a clinically meaningful improvement in the physician global assessment score. KX01 0.01%, 0.1%, and 1% were safe and well-tolerated. KX01 1% at four weeks showed a promising activity for the treatment of plaque-type psoriasis. [ABSTRACT FROM AUTHOR]

Owing to rapidly changing global demographics, adult spinal deformity (ASD) now accounts for a significant proportion of the Global Burden of Disease. Sagittal imbalance caused by age-related degenerative changes leads to back pain, neurological deficits, and deformity, which negatively affect the health-related quality of life (HRQoL) of patients. Along with the recognized regional, global, and sagittal spinopelvic parameters, poor paraspinal muscle quality has recently been acknowledged as a key determinant of the clinical outcomes of ASD. Although the Scoliosis Research Society-Schwab ASD classification system incorporates the radiological factors related to HRQoL, it cannot accurately predict the mechanical complications. With the rapid advances in surgical techniques, many surgical options for ASD have been developed, ranging from minimally invasive surgery to osteotomies. Therefore, structured patient-specific management is important in surgical decision-making, selecting the proper surgical technique, and to prevent serious complications in patients with ASD. This article analyzes the effects of three channels of technology transfer—global value chain participation (GVCP), foreign technology import (FTI), and domestic technology purchase (DTP)—on innovation efficiency by using 11 years of data from Chinese manufacturing industries. Empirical results show FTI and GVCP significantly facilitate innovation efficiency, whereas DTP decreases it. Further, absorptive capacity (AC) positively moderates the relationship between FTI and innovation efficiency. Moreover, in the high-tech industry, we find that GVCP positively affects innovation efficiency, and AC positively moderates the effect of GVCP on innovation efficiency. This finding is informative from a strategic perspective as firms can make strategic decisions in the selection of technology transfer channels. In addition, firms may also absorb and apply external technologies while developing AC to improve enterprises' ability according to their own organizational and strategic contexts in order to achieve and maintain competitive advantages. This finding provides convincing empirical evidence on the relationship between technology transfer channels and innovation efficiency and valuable lessons to other developing countries for the selection of technology transfer channels. Combined with the global value chain (GVC) and the innovation value chain, this study analyses whether and how global value chain participation (GVCPA) and global value chain position (GVCPO) affect innovation efficiency (IE) to explain the difference of IE in China's provinces from 2005 to 2016. It uses Stochastic Frontier Analysis to examine the influencing factors of IE. Results show that both GVCPA and GVCPO are important factors influencing IE, with GVCPA promoting IE and GVCPO inhibiting IE. Moreover, human capital (HC) positively affects the relationship between GVCPO and commercialisation efficiency as well as the relationship between GVCPA and R&D efficiency. The recognition performance deteriorates if degradation factors including blur, noise, and non-uniform illumination exist in the image when acquiring a finger-vein image. Especially, multiple degradation factors can occur when acquiring the finger-vein image, and they require the image restoration. However, previous flow-based model produced lower image quality than the other restoration models, and diffusion-based model had the disadvantage of slow inference speed. Therefore, this study suggests a deep learning-based generative adversarial network for multi-degraded finger-vein image restoration by non-uniform illumination and noise (MFNN-GAN). It considers multiple degradation factors such as non-uniform illumination and noise. Unlike the existing finger-vein image restoration model, MFNN-GAN is capable of adaptive restoration to multiple degradations. Therefore, even if the illumination by near-infrared (NIR) illuminator of finger-vein recognition device is weak or non-uniform, or the consequent captured image is noisy, good recognition performance can be achieved only by our method without replacing the illuminator or camera sensor. The experimental results obtained using finger-vein open datasets, session 1 images from database version 1 of the Hong Kong Polytechnic University finger-image (HKPU-DB) and finger-vein database of SDUMLA-HMT (SDUMLA-HMT-DB)-based degraded databases. This study explores the impact of temperature on silica particle agglomeration in Chemical Mechanical Planarization (CMP) slurries during storage. Slurries were stored at 15 °C, room temperature, and 45 °C, and analyzed. ATR-FTIR, RAMAN, SEM, zeta potential, and LPC were employed providing qualitative and quantitative data on slurry stability and agglomeration tendencies. Elevated temperature storage significantly increased agglomeration and large particle counts with particle counts value of ∼1.2×107 Part./ml, likely due to water molecule dehydration leading to destabilization. Low-temperature storage also induced agglomeration but to a lesser extent with particle counts the value of ∼4.0×106 Part./ml compared with the RT (∼2.0×106 Part./ml). Short-term storage yielded soft agglomerates with LPC of ∼(2.0–4.0)×106 Part./ml, while long-term high-temperature storage led to hard agglomerates with LPC of ∼3.0×107 Part./ml. The ATR-FTIR and Raman analysis indicate that the spectroscopic features of the original slurry exhibited changes upon variation of storage temperature from RT to 15 and 45 °C. The study also explored the reversibility of agglomeration post-treatment, indicating a potential mitigation pathway. The results demonstrated an inverse relationship between temperature and dissolved oxygen (DO) in silica slurries. For example. the curves depict a decrease in oxygen content from ∼10 ppm at RT to 7 ppm at 45 °C with increasing aging time. A mechanism outlining the temperature and aging-induced agglomeration was proposed, linking temperature fluctuations and thermodynamic aspects to colloidal stability. These findings offer a robust understanding of temperature-induced agglomeration in silica-based CMP slurry storage, paving the way for developing strategies to alleviate these issues and ensuring consistent CMP performance in semiconductor manufacturing. Tether breakage is a common mechanical event that sometimes leads to loss of correction. No data has been published that evaluates the outcome of re-tethering in patients who underwent revision surgery for failed VBT, which was the purpose of this study.This is an analysis of a prospectively collected single center database of 290 patients who have had VBT. Patients for this study were included if they have had re-tethering after failed VBT and a minimum follow up of 24 months after index surgery as well as a minimum follow up of 12 months after revision surgery. Revision surgeries included tether exchange, tether reinforcement and/or mono- and bisegmental lateral fusion. Main outcome of interest was curve magnitude at latest follow up.11 patients were identified who received VBT for 16 curves of which 13 curves have had failed index surgery. Mean follow up from index surgery was 40 months, time between index and revision surgery was 22 months and latest follow up after revision surgery 19 months. Re-tethering resulted in an additional correction of 42% for thoracic and 63% for thoracolumbar curves. These results remained clinically stable with only minor loss of correction at final follow up. Patients for this study were included if they have had re-tethering after failed VBT and a minimum follow up of 24 months after index surgery as well as a minimum follow up of 12 months after revision surgery. Revision surgeries included tether exchange, tether reinforcement and/or mono- and bisegmental lateral fusion. Amorphous oxide semiconductors are widely used in the backplane thin film transistors (TFTs) of displays and are extensively researched as materials for next-generation memory devices. However, they face reliability degradation due to stress factors such as thermal, bias, and light exposure. To mitigate these issues, extensive research is focused on improvements, including doping, enhancing crystallinity, and applying pre-and post-treatments. This study investigates the significantly improved reliability and electrical properties of amorphous indium gallium tin oxide (a-IGTO) TFTs by adopting a dual-channel configuration through simultaneous co-sputtering of HfO 2 and IGTO targets. The device fabrication involved co-sputtering HfO 2 and IGTO at the gate oxide interface, followed by depositing an IGTO-only layer, rather than doping the entire channel material. The effect of varying Hf concentration was evaluated by applying different HfO 2 sputtering powers. Optimal hafnium doping reduced the threshold voltage of the a-IGTO TFT from −0.85 V to 0.15 V compared to the pristine a-IGTO TFT, while enhancing the field-effect mobility (μ F E). Furthermore, the Hf-doped device exhibited improved stability, with a reduced threshold voltage shift (Δ V t h ) under positive (+15 V) and negative(-15 V) bias stress at 3000s, decreasing from 11.5 V to 4.8 V , and 4.7 V –1.1 V , respectively. Textile electronics are ideal for novel electronic devices owing to their flexibility, light weight, and wearability. In this work, wearable organic field-effect transistors (OFETs) with all-graphene electrodes, fabricated using hot pressing, are described. First, highly conductive and flexible electrodes consisting of a cotton textile substrate and electrochemically exfoliated graphene (EEG) were prepared via hot pressing. The EEG/textile electrodes exhibited a low sheet resistance of 1.3 Ω sq−1 and high flexibility; these were used as gate electrodes in the wearable OFETs. In addition, spray-coated EEG was also used as the source/drain (S/D) electrodes of the wearable OFETs, which recorded a sheet resistance of 14.8 Ω sq−1 after hot pressing. The wearable OFETs exhibited stable electrical performance, a field-effect mobility of 13.8 cm2 V−1 s−1, and an on–off current ratio of ~103 during 1000 cycles of bending. Consequently, the fabrication method for wearable transistors developed using textiles and hot-pressed graphene electrodes has potential applications in next-generation wearable devices. Proximal junctional problems are among the potential complications of surgery for adult spinal deformity (ASD) and are associated with higher morbidity and increased rates of revision surgery. The diverse manifestations of proximal junctional problems range from proximal junctional kyphosis (PJK) to proximal junctional failure (PJF). Although there is no universally accepted definition for PJK, the most common is a proximal junctional angle greater than 10° that is at least 10° greater than the preoperative measurement. PJF represents a progression from PJK and is characterized by pain, gait disturbances, and neurological deficits. The risk factors for PJK can be classified according to patient-related, radiological, and surgical factors. Background: A single-stage correction for congenital scoliosis through a posterior-only approach is a commonly used surgical technique. However, there are few studies on the surgical treatment effect of posterior single-stage correction in patients with neglected congenital scoliosis. Methods: Patients who underwent a single-stage posterior correction for congenital scoliosis with a minimum follow-up of 2 years were divided into three groups based on age: Group A (7–11 years), B (12–18 years) and C (>18 years). A comparison of surgical, radiological, and clinical outcomes was performed for three groups. Results: The Cobb angle changed form 75 ± 18° to 37 ± 18° with a correction rate of 53%. Group A showed a significantly higher correction rate than Group B and C (all p < 0.001). The amount of blood loss in Groups B and C was significantly larger than that of Group A (p = 0.015). Pulmonary complications were significantly higher in Group C (p = 0.007). Conclusions: A single-stage correction with pedicle screws through a posterior-only approach achieved a significant correction with improved outcomes, even in neglected cases. The authors investigated the interbasin interactions between the Pacific and Atlantic Oceans depending on the phase relationship of Pacific decadal oscillation (PDO)/Atlantic multidecadal oscillation (AMO) based on observations and idealized model experiments. When the AMO and the PDO are in phase (i.e., +PDO/+AMO or −PDO/−AMO), the Pacific Ocean regulates the SST anomalies in the equatorial Atlantic Ocean with altering of the Walker circulation. During this period, there is a negative SST–precipitation relationship in the equatorial Atlantic Ocean where the atmosphere forces the ocean. In contrast, when they are out of phase (i.e., either +PDO/−AMO or −PDO/+AMO), the Atlantic Ocean influences the equatorial Pacific Ocean by modifying the Walker circulation, resulting in a westward shift of a center of convective forcing in the equatorial Pacific Ocean compared to that during an in-phase relationship of PDO/AMO. During this period, a positive SST–precipitation relationship is dominant in the equatorial Atlantic Ocean where the ocean forces the atmosphere. To verify this result, we conducted pacemaker experiments using the Nanjing University of Information Science and Technology Earth System Model version 3 (NESM3). Model results supported our findings obtained from the observations. Background: It would be expected that local recurrence could be reduced by performing precutting (with sufficient margins) prior to endoscopic piecemeal mucosal resection (EPMR). We explored the clinical outcomes and local recurrence after precutting EPMR of large colorectal neoplasias. Methods: Between January 2005 and December 2015, in total, 223 patients with colorectal neoplasias ≥ 2 cm in diameter removed via EPMR in four tertiary hospitals were enrolled. The patients were divided into a precut EPMR group (n = 62) and a non-precut EPMR group (n = 161). We retrospectively evaluated clinical outcomes and factors associated with local recurrence. Results: The mean total procedure time was significantly shorter in the non-precut EPMR group than in the precut EPMR group. However, the number of pieces, and the complete resection and recurrence rates, did not differ significantly [for the latter, precut 8.1% vs. non-precut 9.9%, P = 0.668]. The complete resection rate, number of pieces, and use of argon plasma coagulation (APC) were significantly associated with the local recurrence rate on univariate analysis. In the Cox's proportional hazards model, prophylactic APC [hazard ratio 0.307, 95% confidence interval (CI) 0.114–0.823; P = 0.019] and complete resection rate (odds ratio 0.083, 95% CI 0.011–0.655; P = 0.018) were significantly associated with the local recurrence rate. Conclusion: Precutting prior to EPMR did not significantly reduce the local recurrence rate or the number of resected pieces. Histologically complete resection, reducing the number of pieces, and prophylactic APC seem to be important in terms of reducing local recurrence. [ABSTRACT FROM AUTHOR]

Background/Aims Food retention, which is a characteristic observed in patients with achalasia, can interfere with peroral endoscopic myotomy (POEM). However, there is no established guideline for esophageal preparation for POEM. A previous study has shown that drinking warm water may reduce the lower esophageal sphincter pressure in patients with achalasia. This study aims to evaluate the possibility of proper preparation of POEM by instructing the patient to drink warm water. Methods The warm water preparation was performed in 29 patients with achalasia who underwent POEM. The patients drank 1 L of warm water (60oC) the night before POEM. We evaluated the esophageal clearness and determined the preparation quality. Twentynine patients were prospectively recruited and compared to control group. The control cohort comprised achalasia patients whose endoscopic image was available from the achalasia database of our institution. A 1:2 propensity score-matched control cohort was established from the database of achalasia subjects (n = 155) to compare the outcome of the preparation. Results In the warm water preparation group, only 1 patient (3.4%) had some solid retention, but it did not interfere with the POEM procedure. The grade of clearness (P = 0.016) and quality of preparation (P < 0.001) were significantly better in the warm water preparation group than in the matched control group. There was no any adverse event at all related to warm water preparation protocol. Conclusions Drinking warm water dramatically reduces esophageal food retention and significantly improves the quality of esophageal preparation. Spinal cord injury (SCI) leads to disruption of the blood–spinal cord barrier, hemorrhage, and tissue edema, which impair blood circulation and induce ischemia. Angiogenesis after SCI is an important step in the repair of damaged tissues, and the extent of angiogenesis strongly correlates with the neural regeneration. Various biomaterials have been developed to promote angiogenesis signaling pathways, and angiogenic self-assembling peptides are useful for producing diverse supramolecular structures with tunable functionality. RADA16 (Ac-RARADADARARADADA-NH2), which forms nanofiber networks under physiological conditions, is a self-assembling peptide that can provide mechanical support for tissue regeneration and reportedly has diverse roles in wound healing. In this study, we applied an injectable form of RADA16 with or without the neuropeptide substance P to the contused spinal cords of rats and examined angiogenesis within the damaged spinal cord and subsequent functional improvement. Histological and immunohistochemical analyses revealed that the inflammatory cell population in the lesion cavity was decreased, the vessel number and density around the damaged spinal cord were increased, and the levels of neurofilaments within the lesion cavity were increased in SCI rats that received RADA16 and RADA16 with substance P (rats in the RADA16/SP group). Moreover, real-time PCR analysis of damaged spinal cord tissues showed that IL-10 expression was increased and that locomotor function (as assessed by the Basso, Beattie, and Bresnahan (BBB) scale and the horizontal ladder test) was significantly improved in the RADA16/SP group compared to the control group. Our findings indicate that RADA16 modified with substance P effectively stimulates angiogenesis within the damaged spinal cord and is a candidate agent for promoting functional recovery post-SCI.
