Abstract

Dual-energy X-ray absorptiometry lean soft tissue hydration: independent contributions of intra- and extracellular water. Am J Physiol Endocrinol Metab 287: E842-E847, 2004. First published July 6, 2004; doi:10.1152/ajpendo.00361. 2003.--Dual-energy X-ray absorptiometry (DEXA) provides a measure of lean soft tissue (LST). LST hydration, often assumed to be constant, is relevant to several aspects of DEXA body composition estimates. The aims of this study were to develop a theoretical model of LST total body water (TBW) content and to examine hydration effects with empirically derived model coefficients and then to experimentally test the model's prediction that, in healthy adults, LST hydration is not constant but varies as a function of extra- and intracellular water distribution (E/I). The initial phase involved TBW/ LST model development and application with empirically derived model coefficients. Model predictions were then tested in a cross-sectional study of 215 healthy adults. LST was measured by DEXA, extracellular water (ECW) by NaBr dilution, intracellular water (ICW) by whole body [sup.40]K counting, and TBW by [sup.2][H.sub.2]O dilution. TBW estimates, calculated as ECW + ICW, were highly correlated with (r = 0.97, SEE = 2.1 kg, P < 0.001) and showed no significant bias compared with TBW measured by [sup.2][H.sub.2]O. Model-predicted TBW/ LST was almost identical to experimentally derived values (means [+ or -] SD) in the total group (0.767 vs. 0.764 [+ or -] 0.028). LST hydration was significantly correlated with E/I (total group, r = 0.30, SEE = 0.027, P < 0.001). Although E/I increased with age (men, r = 0.48; women, r = 0.37; both P < 0.001), the association between TBW/LST and age was nonsignificant. Hydration of the DEXA-derived LST compartment is thus not constant but varies predictably with ECW and ICW distribution. This observation has implications for the accuracy of body fat measurements by DEXA and the use of TBW as a means of checking DEXA system calibration. whole body counting; body composition; water distribution

Abstract

Objective. Pediatricians are encountering body composition information more frequently, with percentage of body fat (%BF) measurement receiving particular attention as a result of the obesity epidemic. One confounding issue is that different methods may yield different %BF results in the same person. The objective of this study was to compare dual-energy X-ray absorptiometry (DXA) with the criterion 4-compartment model (4-CM) for measurement of %BF in a large pediatric cohort and to assist pediatricians in appropriate interpretation of body composition information by recognizing differences between techniques. Methods. Height, weight, anthropometrics, body density by underwater weighing, total body water by deuterium dilution, and bone mineral content and %BF by DXA (Lunar DPX/DPX-L) were measured in 411 healthy subjects, aged 6 to 18 years. Values for %BF by 4-CM and DXA were compared using regression analysis. Results. The mean [+ or -] standard deviation values for %BF by DXA (22.73% [+ or -] 11.23%) and by 4-CM (21.72% [+ or -] 9.42%) were different, but there was a strong relationship between the 2 methods ([R.sup.2] = 0.85). DXA underestimated %BF in subjects with lower %BF and overestimated it in those with higher %BF. The relationship between the 2 methods was not affected by gender, age, ethnicity, pubertal stage, height, weight, or body mass index. The standard error of the estimate was 3.66%. Conclusion. This analysis demonstrates a predictable relationship between DXA and 4-CM for %BF measurement. Because of its ease of use, consistent relationship with 4-CM, and availability, we propose that DXA has the capacity for clinical application including prediction of metabolic abnormalities associated with excess %BF in pediatrics. Abstract

OBJECTIVE: To examine the validity of body mass index z-score (zBMI) as a measure of percent body fat in prepubertal children. METHODS: 111 multi-ethnic, healthy, Tanner 1 children aged 6–12 years had fat percent and fat mass measured by the 4 compartment method as part of the Pediatric Rosetta Body Composition Cohort. Multiple regression models were developed with fat percent as the dependent variable and zBMI, age, sex, and ethnicity as independent variables. RESULTS: zBMI predicted fat percent, adjusted for age in both girls (p0.05). CONCLUSIONS: Given the relatively large error in the models, BMI z-scores are not a useful indicator of fat mass in healthy, Tanner 1 children. zBMI(2) scores are associated with significantly lower absolute percent errors in girls and boys.

Abstract

Mutation in the DAX1 gene which causes adrenal hypoplasia congenita (AHC) and hypogonadotropic hypogonadism (HH) may be detected by sophisticated molecular testing. AHC is incomplete development of the adrenal gland that can lead to HH, which is a dysfunction of the sex glands. Males with AHC from four families underwent molecular genetic testing. DNA was examined using the polymerase chain reaction (PCR), and males were diagnosed by analyzing PCR amplification. Mothers carrying the X-linked gene may be diagnosed by fluorescence in situ hybridization analysis. Early diagnosis of this disorder is important so that adrenal insufficiency can be treated. Treatments include glucocorticoid and mineralocorticoid replacement, and testosterone and pulsatile gonadotropin-releasing hormone.

Abstract

Background: Children have a high resting energy expenditure (REE) relative to their body weight. The decline in REE during growth may be due to changes in body composition or to changes in the metabolic rate of individual organs and tissues. Objectives: The goals were to quantify body-composition components in children at the organ-tissue level in vivo and to determine whether the observed masses 1) account for the elevated REE in children and 2) account, when combined with specific organ-tissue metabolic constants, for children's REE. Design: This was a cross-sectional evaluation of 15 children (aged 9.3 [+ or -] 1.7 y) and 13 young adults (aged 26.0 [+ or -] 1.8 y) with body mass indexes (in kg/[m.sup.2]) < 30. Magnetic resonance imaging-derived in vivo measures of brain, liver, kidney, heart, skeletal muscle, and adipose tissue were acquired. REE was measured by indirect calorimetry ([REE.sub.m]). Previously published organ-tissue metabolic rate constants were used to calculate whole-body REE ([REE.sub.c]). Results: The proportion of adipose-tissue-free mass as liver (3.7 [+ or -]0.5% compared with 3.1 [+ or -] 0.5%; P < 0.01) and brain (6.2 [+ or -] 1.2% compared with 3.3 [+ or -] 0.9%; P < 0.001) was significantly greater in children than in young adults. The addition of brain and liver mass significantly improved the model but did not eliminate the role of age. [REE.sub.c] with published metabolic coefficients underestimated [REE.sub.m] ([REE.sub.c] = 3869 [+ or -] 615 kJ/d; [REE.sub.m] = 5119 + 769 kJ/d; P < 0.001) in children. Conclusion: The decline in REE during growth is likely due to both a decrease in the proportion of some of the more metabolically active organs and tissues and changes in the metabolic rate of individual organs and tissues. Abstract

Background: Waist circumference (WC) is now accepted as a practical measure of adipose tissue distribution. Four body sites for WC measurements are commonly used, as follows: immediately below the lowest ribs (WC1), the narrowest waist (WC2), the midpoint between the lowest rib and the iliac crest (WC3), and immediately above the iliac crest (WC4). Objective: We sought to compare the magnitude and reliability of WC measured at these 4 sites in males and females. Design: WC was measured at each site 1 time in all subjects [49 males and 62 females, aged 7-83 y, with a body mass index (in kg/ [m.sup.2]) of 9-43] and 3 times in a subgroup (n = 93) by one experienced observer using a heavy-duty inelastic tape. Body fat was measured in a subgroup (n = 74) with the use of dual-energy X-ray absorptiometry. Results: The mean values of WC were WC2 < WC1 < WC3 < WC4 (P < 0.01) in females and WC2 < WC1, WC3, and WC4 (P < 0.01 ) in males. For all 4 sites, measurement reproducibility was high, with intraclass correlation (r) values > 0.99. WC values were significantly correlated with fatness; correlations with trunk fat were higher than correlations with total body fat in both sexes. Conclusions: WC values at the 4 commonly used anatomic sites differ in magnitude depending on sex, are highly reproducible, and are correlated with total body and trunk adiposity in a sex-dependent manner. These observations have implications for the use of WC measurements in clinical practice and patient-oriented research. Abstract

Background: Bioelectrical impedance analysis (BIA) is an attractive method of measuring pediatric body composition in the field, but the applicability of existing equations to diverse populations has been questioned. Objective: The objectives were to evaluate the performance of 13 published pediatric BIA-based predictive equations for total body water (TBW) and fat-free mass (FFM) and to refit the best-performing models. Design: We used TBW by deuterium dilution, FFM by dual-energy X-ray absorptiometry, and BIA-derived variables to evaluate BIA models in a cross-sectional study of 1291 pediatric subjects aged 4-18 y, from several ethnic backgrounds, including 54 children with HIV infection and 627 females. The best-performing models were refitted according to criterion values from this population, cross-validated, and assessed for performance. Additional variables were added to improve the predictive accuracy of the equations. Results: The correlation between predicted and criterion values was high for all models tested, but bias and precision improved with the refitted models. The 95% limits of agreement between predicted and criterion values were 16% and 11% for TBW and FFM, respectively. Bias was significant for some subgroups, and there was greater loss of precision in specific age groups and pubertal stages. The models with additional variables eliminated bias, but the limits of agreement and the loss of precision persisted. Conclusion: This study confirms that BIA prediction models may not be appropriate for individual evaluation but are suitable for population studies.
