Abstract

The potency of health systems to effectively respond to crises varies between high- and low-income countries. The COVID-19 pandemic has presented numerous challenges for hospitals worldwide. In this context, the resilience of health systems and the capacity of health institutions and populations play a key role in mounting an effective response to crises. We gathered data on the condition and resilience of health systems in the two main hospitals in Mashhad, Iran, during three peaks of the COVID-19 pandemic. The results highlighted the fact that health systems officials and managers need to consider the consequences of COVID-19, such as the need for more beds and trained healthcare workers. Hospitals should take into account the impact of the COVID-19 pandemic across all wards and departments and prioritize the well-being of healthcare workers since they are at the forefront of the fight against this pandemic. Abstract

Background: The study aimed to compare the prognostic accuracy of six different severity-of-illness scoring systems for predicting in-hospital mortality among patients with confirmed SARS-COV2 who presented to the emergency department (ED). The scoring systems assessed were worthing physiological score (WPS), early warning score (EWS), rapid acute physiology score (RAPS), rapid emergency medicine score (REMS), national early warning score (NEWS), and quick sequential organ failure assessment (qSOFA). Materials and methods: A cohort study was conducted using data obtained from electronic medical records of 6,429 confirmed SARS-COV2 patients presenting to the ED. Logistic regression models were fitted on the original severity-of-illness scores to assess the models' performance using the Area Under the Curve for ROC (AUC-ROC) and Precision-Recall curves (AUC-PR), Brier Score (BS), and calibration plots were used to assess the models' performance. Bootstrap samples with multiple imputations were used for internal validation. Results: The mean age of the patients was 64 years (IQR:50--76) and 57.5% were male. The WPS, REMS, and NEWS models had AUROC of 0.714, 0.705, and 0.701, respectively. The poorest performance was observed in the RAPS model, with an AUROC of 0.601. The BS for the NEWS, qSOFA, EWS, WPS, RAPS, and REMS was 0.18, 0.09, 0.03, 0.14, 0.15, and 0.11 respectively. Excellent calibration was obtained for the NEWS, while the other models had proper calibration. Conclusion: The WPS, REMS, and NEWS have a fair discriminatory performance and may assist in risk stratification for SARS-COV2 patients presenting to the ED. Generally, underlying diseases and most vital signs are positively associated with mortality and were different between the survivors and non-survivors. [ABSTRACT FROM AUTHOR]

29. Cost-utility analysis of home-based cardiac rehabilitation as compared to usual post-discharge care: systematic review and meta-analysis of randomized controlled trials

Abstract

Determining cost-utility differences between home-based cardiac rehabilitation (HBCR) on the one hand, and usual post-discharge care (UC) on the other, can improve resource-allocation in healthcare settings. In June 2019, PubMed, Web of Science, Scopus, and Cochrane library were searched for randomized controlled HBCR trials. Standardized mean differences (SMDs) of cost and quality-adjusted life years (QALYs) between HBCRs and UCs were calculated using random effect models. Heterogeneity was assessed by inconsistency index (I2) and publication bias by funnel plot and Egger���s regression test. Thirteen articles, representing 2,992 participants, were deemed representative for final analysis. In the meta-analysis, a significant difference with respect to QALYs favored HBCR, while no significant cost difference was observed between HBCR and UC. However, subgroup-analysis of trials with different follow-up durations revealed somewhat different results, and HBCR was found to be significantly better with regard to both cost and QALYs for patients with heart failure. Cost-utility analysis categorizing interventions as ���dominant���, ���effective���, ���doubtful���, and ���dominated���, found HBCRs dominant. Although HBCR tended to be superior compared to UC in this review, larger and more robust trials addressing specific patients groups are needed for definitive results.

Abstract

Coronavirus disease 2019 (Covid-19) is expanding worldwide. Although it seems to be a purely respiratory disease, occasional reports of lesions in other organs have been published. We report here an asymptomatic child with Covid-19 and with the main symptom of abdominal pain distension and without any respiratory symptoms. A 14-year-old male patient with main complaints of fever, malaise, anorexia, and severe abdominal pain was admitted to a hospital in Mashhad, Iran. Laparotomy revealed distension of the small intestine and an adhesive ileocaecal band that had produced ileum herniation without free fluid in the abdomen. Chest radiography and high-resolution computed tomography of the lungs showed bilateral and diffuse, peripheral dense areas of ground-glass appearance. A nasopharyngeal swab for diagnosis of Covid-19, was ordered due to lymphopenia together with these diffuse lung infiltrations, which showed a positive result. This led to drug treatment with lopinavir/ ritonavir, hydroxychloroquine, ribavirin/oseltamivir, and meropenem. The patient was febrile and developed tachycardia on the third day, accompanied by a respiratory rate of 44/min. At this point, tracheal intubation was done, but the patient died after 3 hours due to cardiac arrest. The case report brings forth the hypothesis that the gastrointestinal manifestation may be an atypical symptom of Covid-19 infection and highlights the importance of the diagnosis based on combined laboratory-based data and scanning imagery.
