Background: Colonoscopy is the endoscopic examination of the large bowel and the distal part of the small bowel with a CCD camera or a fiber-optic camera on a flexible tube passed through the anus. It can provide a visual diagnosis. Methods: In this study, clemastine plus fentanyl was compared with fentanyl plus midazolam in analgesia during colonoscopy. Patients were monitored for pain and hemodynamic changes. The mean age in the group treated with clemastine was 52 years and in the group treated with midazolam was 54 years. There were 56 males and 35 females in the study. There was no significant difference in gender in the two groups. Systolic blood pressure was 109.9 mmHg before and 109.35 mmHg after clemastine administration, which showed no significant difference. Results: Diastolic blood pressure was 72.2 mmHg before and 73.7 mmHg after clemastine administration, which showed no significant difference. No change in mean pulse rate (PR) was seen (84/min before and after clemastine administration). In the group receiving midazolam, systolic and diastolic blood pressure changes were not significant, but changes in PR and arterial oxygen saturation were significantly different. Meanwhile, the two groups had a significant lower pain level after colonoscopy in the group receiving clemastine compared to midazolam. Conclusion: Based on the results of the present study, clemastine can be used as a main drug for sedation during colonoscopy. [ABSTRACT FROM AUTHOR]

Background: 10%-15% of the world's population suffers from kidney stones. Nearly 50% increase was observed in diagnosing and treating nephrolithiasis in the last decades. Effective medical treatment for the disease is not yet well established. Moreover, there is an increasing global demand to manage diseases using complementary and alternative medicine. This study aimed to formulate and assess the safety and efficacy of a multi-ingredient formulation from traditional Persian medicine (TPM) known as Mofatet powder in patients suffering from calcium kidney stones. Materials and Methods: The aqueous extract of Mofatet powder was prepared, freeze-dried, and formulated as capsules. 26 patients in the drug group and 25 patients in the placebo group used 500 mg capsules of the drug/placebo twice daily for 5 weeks. Ultrasonography/kidney, ureter and bladder imaging, urine analysis, and biochemical parameters were evaluated before and after the intervention. Results: The imaging results showed a 60.73% decrease (P < 0.001) in stone size in the drug group. Moreover, the urinary calcium decreased (P = 0.02) and the urinary magnesium increased (P < 0.001) in the drug group. No remarkable changes were observed in the placebo group in these parameters. No significant effect was observed in aspartate transaminase, alanine transaminase, serum creatinine, and blood urea nitrogen levels in none of the groups. Conclusion: This study suggests that Mofatet powder was effective in reducing calcium kidney stones size with no potential nephro/hepatotoxicity. After confirming these results in larger clinical trials with longer duration, this formulation can be considered a treatment for nephrolithiasis. [ABSTRACT FROM AUTHOR]

The purpose of this experiment was to study the effects of silage type (ST) and levels of silage particle size (SPS) on feed intake, digestibility, rumen parameters, and feed intake behavior in Kermani sheep. Corn and millet crops were cultivated in May and harvested into coarse and fine forages in mid-September. Four rams (two years old, BW 39.2±3.1 kg) were randomly assigned to four treatments in a 2×2 factorial arrangements in a Latin square design. The experimental diets were coarse corn silage diet; short corn silage diet; coarse millet silage diet and short millet silage diet. Diets were iso-nitrogenous and isoenergetic, and the ratio of forage to concentrate was 50:50. The potential of gas production was higher in corn silage (CS) than millet silage (MS) (P<0.03) and coarse SPS than short SPS (P<0.02). The dry matter (DM), organic matter (OM) and crude protein (CP) intakes were higher in short SPS diets while DM, OM and neutral detergent fiber (NDF) digestibility were higher in CS diets (76.26, 73.43 and 58.71%, respectively), while the CP digestibility was higher in short SPS diets (74.98%). The mean ruminal pH value was lower in CS diets (P<0.05). The mean ruminal ammonia nitrogen (NH3-N) concentration was higher in CS and short SPS diets. Microbial protein (MP) synthesis was higher in short SPS diets (P<0.05). Chewing activity was higher in MS and short SPS diets (P<0.05). In this paper, a numerical method for solving three-asset Black–Scholes partial differential equation is presented. The model is based on the Crank–Nicolson and the radial basis function methods. Also, the convergency of the proposed method is proved. Implementation of the proposed method is specially studied on cash-or-nothing option. This research study presents a novel highly accurate matrix approach for numerical treatments of a strongly nonlinear boundary value problem occurring in the modeling of the corneal shape of the human eye. Using the technique of quasilinearization, the nonlinear model is reduced into a sequence of linearized problems. Then, a spectral collocation procedure based on novel shifted Vieta‐Fibonacci (SVF) is applied to transform each subproblem into a linear algebraic system of equations. An upper bound for the error is provided, and convergence analysis of the SVF series solution in the weighted L2$$ {L}_2 $$ norm is discussed. The technique of error correction is used to improve the SVF polynomial solutions by means of the residual error function. Various numerical tests are provided to demonstrate the accuracy and efficiency of the presented collocation algorithm.
