Tumor endothelial cells express vascular endothelial growth factor receptor 2 (VEGFR-2). VEGF can direct toxins to tumor vessels through VEGFR-2 for antiangiogenic therapy. This study aimed to selectively damage the VEGFR-2-overexpressing vasculature of pancreatic cancer by SLT-VEGF fusion protein comprising VEGF and the A subunit of Shiga-like toxin which inhibits protein synthesis of cells with high VEGFR-2 expression.Expression of VEGF and VEGF receptors was evaluated in human pancreatic cancer cells (AsPC-1, HPAF-2) and in normal human endothelial cells (HUVEC) by reverse transcription-polymerase chain reaction. Cells were treated with SLT-VEGF (0.1-10 nM), and cell viability, proliferation, and endothelial tube formation were assessed. Orthotopic pancreatic cancer (AsPC-1, HPAF-2) was induced in nude mice. Animals were treated with SLT-VEGF fusion protein alone or in combination with gemcitabine. Treatment began 3 days or 6 weeks after tumor induction. Primary tumor volume and dissemination were determined after 14 weeks. Microvessel density and expression of VEGF and VEGF receptors were analyzed by immunohistochemistry.SLT-VEGF did not influence proliferation of pancreatic cancer cells; HUVECs (low-level VEGFR-2) reduced their proliferation rate and tube formation but not their viability. SLT-VEGF fusion protein reduced tumor growth and dissemination, increasing 14-week survival (AsPC-1, up to 75%; HPAF-2, up to 83%). Results of gemcitabine were comparable with SLT-VEGF monotherapy. Combination partly increased the therapeutic effects in comparison to the respective monotherapies. Microvessel density was reduced in all groups. Intratumoral VEGFR-2 expression was found in endothelial but not in tumor cells.SLT-VEGF is toxic for tumor vasculature rather than for normal endothelial or pancreatic cancer cells. SLT-VEGF treatment in combination with gemcitabine may provide a novel approach for pancreatic cancer.

Titelblatt und Inhaltsverszeichnis Hintergrund und Fragestellung Material und Methoden Ergebnisse Diskussion Zusammenfassung und Ausblick Literatur, Die vorliegende Habilitationsschrift beschreibt im ersten Teil die Entwicklung von verbesserten, orthotopen Tiermodellen für das duktale Adenokarzinom des Pankreas in Nacktmaus und Ratte. Diese Modelle generieren eine dem humanen Krankheitsverlauf ähnliche, klinische relevante lokale und systemische Tumorerkrankung und eignen sich daher zur präklinischen Evaluierung neuer Therapiestrategien. Weiterhin steht mit der Nacktmaus ein Modellsystem zur Verfügung, welches die Etablierung unterschiedlich differenzierter humaner Tumore erlaubt. Alternativ kann im immunkompetenten Lewis-Rattenmodell die Tumorentwicklung in einem nicht immunsupprimierten Tier beobachtet werden. Für beide Tiermodelle wird neben der Erfassung des Primärtumorwachstums auch ein Score-System beschrieben, welches die exakte Quantifizierung von lokaler und systemischer Tumoraussaat erlaubt. Im zweiten Teil der Arbeit wird die therapeutische Potenz von verschiedenen Therapieansätzen evaluiert, welche auf eine Inhibierung der Tumorangiogenese zielen. Neben Faktor-unspezifischen Substanzen wie Suramin und TNP-470, werden insbesondere Ansätze zur Inhibierung des pro-angiogenen Schlüsselfaktors VEGF (Vascular Endothelial Growth Factor) mit einem spezifischen Antikörper sowie einem Antisense-Molekül untersucht. Eine weitere Strategie ist eine selektive Schädigung von Tumorendothelien mit einem VEGF-Toxin-Fusionsprotein. Schließlich erfolgt die Evaluierung einer Kombinationstherapie mit einem Anti-VEGF-Antikörper sowie einem Matrixmetalloproteinasen-Inhibitor. Zusammenfassend zeigt die Arbeit, daß sich die etablierten Tiermodelle zur präklinischen Evaluierung von neuen Therapiestrategien für das Pankreaskarzinom eignen. Eine komplette Inhibierung des Tumorwachstums ist jedoch mit den eingesetzten Therapien selbst nach prophylaktischer Gabe nicht möglich, lediglich eine Verzögerung der Tumorprogression, so daß künftige Studien die Effizienz von Kombinationstherapien ermitteln müssen., This work describes in its first part the development of improved, orthotopic animal models of ductal pancreatic cancer in nude mice and Lewis rats. The models mimic human disease and are therefore suitable for the preclinical evaluation of novel therapeutic strategies. The nude mouse allows to harbor human tumors of distinct differentiation. In contrast, Lewis rats allow the follow up of tumor growth in a fully immunocompetent animal. Beside the assessment of primary tumor growth, both models are characterized exactly with regard to local and systemic tumor spread, which is quantified with a scoring system. The second aim of this work is to evaluate novel anti-angiogenic treatment strategies for pancreatic cancer. The therapeutic effects of non- specific inhibitors (Suramin, TNP-470) are assessed; moreover, antibody and antisense strategies against VEGF (Vascular Endothelial Growth Factor), a key factor for tumor angiogenesis. A selective endothelial cell targeting is achieved with an VEGF-toxin fusion protein. Finally, a combination therapy with an anti-VEGF antibody and a broad spectrum inhibitor of matrixmetalloproteinases is tested. In summary, the evaluated animal models are suitable to preclinically evaluate novel treatment strategies for pancreatic cancer. Monotherapies with the tested substances result in reduced tumor growth and spread, but are not able to eradicate tumor progression even in a prophylactic setting. Therefore, future studies may have to evaluate, whether combination therapies are more efficient.
