Objectives: To investigate compliance rates with the recommendations published in 2003 by the French National Authority for Health (HAS), in SLE, on the prevention and treatment of glucocorticoid-induced osteoporosis (GIOP) in the Bordeaux University Hospital. Methods: A cross-sectional study was performed on 109 patients with SLE, from May 2014 to November 2014, in the Departments of Rheumatology and Internal Medicine of the Bordeaux University Hospital. Results: Of the hundred and nine patients identified with SLE, 21 (19%) patients had indication for anti-osteoporotic treatment. These 21 patients were older (54±19 years) than the patients without indication to be treated and more women were menopaused (77%).They had a long-term use of corticosteroid (103.7±87.7 months) and a higher dose (9.9±9.6 mg/d), as well as greater use of immunosuppressant (81%). Among these 21 patients, 12 patients were received an anti-osteoporotic treatment. The recommendations were followed for 57% of patients, 71% received a supplementation of vitamins D and calcium and 86% underwent a bone mineral densitometry. Doing the same analyses with the new recommendations published in 2014, 30 (27%) patients should have been treated and 12 patients were received an anti-osteoporotic treatment. The recommendations were followed for 40% patients. Conclusion: The prevention and treatment of osteoporosis in SLE is still suboptimal. It is important that practitioners become aware of the importance of managing complications induced by the disease itself and also the treatments used, in particular musculoskeletal complications and osteoporosis., Objectif : Évaluer l’adhésion aux recommandations sur la prévention et le traitement de l'ostéoporose cortico-induite, publiées en 2003 par l’AFSSAPS, dans le lupus érythémateux systémique (LES), au CHU de Bordeaux. Méthodes : Une étude transversale a été réalisée, de mai 2014 à novembre 2014, dans les services de rhumatologie et de médecine interne du CHU de Bordeaux. 109 patients atteints de LES ont été inclus. Résultats : Sur les 109 patients inclus dans l’étude, 21 (19%) patients avaient une indication à un traitement anti-ostéoporotique. Ces 21 patients étaient plus âgés (54 ± 19 ans) que les patients sans indication à un traitement anti-ostéoporotique. Ils avaient également une durée d’utilisation plus longue (103,7 ± 87,7 mois) et une posologie plus élevée de corticoïdes (9,9 ± 9,6 mg / j), ainsi qu’une plus grande utilisation d’immunosuppresseurs (81%). Parmi ces 21 patients, 12 ont reçu un traitement anti-ostéoporotique, soit 57%. Concernant les mesures générales de prévention 71% des patients ont reçu une supplémentation en vitamines D et en calcium et une ostéodensitométrie a été réalisée dans 86% des cas. Les mêmes analyses ont été réalisées en utilisant les nouvelles recommandations sur l’ostéoporose cortico-induite publiées en novembre 2014 par l’HAS. Avec ces recommandations, 30 (27%) patients avaient une indication à recevoir un traitement anti-ostéoporotique et 12 patients ont finalement reçu un tel traitement. Les recommandations ont donc été suivies pour 40% des patients. Conclusion : La prévention et le traitement de l'ostéoporose dans le LES est encore sous-optimale. Un nouveau travail pourra être réalisé afin de comprendre les raisons de la non prescription d’un traitement anti-ostéoporotique alors que la prise en charge de l’ostéoporose est initiée par la réalisation quasi-systématique d’une ostéodensitométrie.
