Throughout modern history, there have been attempts to divide communities in the US between groups as to point at and blame others. Worse still, some groups have preference towards contemporary American cultural values preferred over the values that represent minority populations. The status quo was to translate American contemporary cultural values as being white, angelo-saxon Protestant (WASP), primarily patriarchal populations or images as ideally preferred over those counter to this narrative, especially among minority groups. One can question whether contemporary American cultural values, for higher education, were any different within African American communities. We might appreciate the diversity of values and cultural influences a true American identity and diversity of opinions brings to being a stronger nation overall. African American cultural values towards education parallel contemporary values.

This chapter presents an overview of the various challenges to overcoming racial disparities of minority groups in Arkansas. This chapter focuses on the efforts that have gone into education, lack of funding, discrimination, racial disparities, and the justice system through guidelines and reform both at the state and federal levels. Recommendations for policy reform will benefit the minority groups of Arkansas. Arkansas' minority groups are chosen for this chapter because Arkansans, often, out of all the states in the South, have traditionally failed to benefit from access to privileged groups, political elites, and the programs that would lift them from despair so that they may have access to those programs and institutions that would enable them to lead more productive lives.

Burn severity is determined by total body surface area affected, temperature of source, and duration of exposure. Patients with impaired mobility are less capable of avoiding hazards and escaping traumatic injuries. Additionally, patients with impaired mobility frequently suffer from other comorbid conditions and have specialized needs, which can complicate their acute treatment. This study was a retrospective electronic medical records review of all adult patients, aged 18 years and older with a preexisting mobility impairment, admitted as inpatients to a single burn center for treatment of burn-related injuries from January 1, 2009, to December 31, 2019. The 10-year review of 1520 adult burn admissions meeting the initial criteria of inpatient admission and burn injury revealed 174 patients with documentation supporting preexisting functional mobility impairment (11%). Surprisingly, patients’ overall lengths of stays were consistent with all burn populations at 0.81 days per % TBSA, with the average length of stay being 6.7 days. The demographic data were consistent with national burn registry data as male, Caucasian, and older, with mean age of 61.1 years. Demographic data and details of hospital course focusing on treatment, complications, and outcomes were extracted and analyzed. There is a paucity of literature describing the needs of this unique burn population. Burn-injured patients with preexisting mobility impairments suffer from similar mechanisms of injury. By identifying attributes unique to this population, we hope to develop specialized prevention education and treatment protocols.

The objective of this study was to examine the pregnancy outcomes from frozen embryo transfer (FET) cycles using different endometrial preparation regimens, compared to ovulation induction with letrozole (letrozole OI). A retrospective cohort study was conducted at a fertility centre in Sydney, Australia, on 6060 FET cycles. The cycles were stratified into one of four ways to achieve endometrial preparation. These were either a natural, letrozole OI, OI with follicle-stimulating hormone (FSH OI) or a programmed cycle. The primary outcome was live birth rate (LBR) per embryo transfer. Secondary outcomes included clinical pregnancy and biochemical pregnancy rates, adverse events including miscarriage, ectopic pregnancy, stillbirth, neonatal death and multiple births. Ovarian stimulation parameters were also analysed including the time taken to reach the luteal phase and the number of blood or urine tests required for monitoring the cycle. The results of the study showed that the LBR following letrozole OI cycles was higher when compared to natural cycles (odds ratio (OR): 1.27 (1.07–1.49)) and programmed cycles (OR: 2.36 (1.67–3.34)). There was no significant difference between letrozole OI and FSH OI LBR (OR: 0.99 (0.76–1.28)). An improved LBR with letrozole OI compared to natural cycles was maintained when only women with a normal length cycle were considered (OR: 1.44 (1.10–1.89)). There was a significant reduction in miscarriage rates when letrozole OI was compared to programmed cycles (OR: 0.46 (0.26–0.83)). It was therefore concluded that the use of letrozole OI for endometrial preparation in an FET cycle may be associated with higher LBR and lower miscarriage rate, compared to using a programmed cycle. Lay summary Couples suffering from infertility frequently try to conceive following the transfer of an embryo which has been frozen during an in vitro fertilisation cycle. Embryos will only lead to a pregnancy if the woman’s womb lining has particular characteristics that allow it to accept the embryo. Despite the thousands of frozen embryo transfer cycles carried out across the world, it is not known how best to prepare a woman’s lining so it has these particular characteristics. This study looked at the pregnancy outcomes of 6060 cycles to compare four different ways to prepare a woman’s womb lining. These included relying on a woman’s natural menstrual cycle, or using an oral medication called letrozole, or injectable medicine called follicle-stimulating hormone, or oestrogen and progesterone hormonal medications. The comparison found that using letrozole before transfer of a frozen embryo may be associated with higher rates of a live birth for some women.

It is widely assumed that long-wavelength infrared (IR) emission from cold dust (T~20-40K) is a reliable tracer of star formation even in the presence of a bright active galactic nucleus (AGN). Based on radiative transfer (RT) models of clumpy AGN tori, hot dust emission from the torus contributes negligibly to the galaxy spectral energy distribution (SED) at $\lambda\ga100$ \micron. However, these models do not include AGN heating of host-galaxy-scale diffuse dust, which may have far-IR (FIR) colors comparable to cold diffuse dust heated by stars. To quantify the contribution of AGN heating to host-galaxy-scale cold dust emission at $\lambda\ga100$ \micron, we perform dust RT calculations on a simulated galaxy merger both including and excluding the bright AGN that it hosts. By differencing the SEDs yielded by RT calculations with and without AGN that are otherwise identical, we quantify the FIR cold dust emission arising solely from re-processed AGN photons. In extreme cases, AGN-heated host-galaxy-scale dust can increase galaxy-integrated FIR flux densities by factors of 2-4; star formation rates calculated from the FIR luminosity assuming no AGN contribution can overestimate the true value by comparable factors. Because the FIR colors of such systems are similar to those of purely star-forming galaxies and redder than torus models, broadband SED decomposition may be insufficient for disentangling the contributions of stars and heavily dust-enshrouded AGN in the most IR-luminous galaxies. We report the time-resolved spectral analysis of a bright near-infrared and moderate X-ray flare of Sgr A*. We obtained light curves in the $M$-, $K$-, and $H$-bands in the mid- and near-infrared and in the $2-8~\mathrm{keV}$ and $2-70~\mathrm{keV}$ bands in the X-ray. The observed spectral slope in the near-infrared band is $\nu L_\nu\propto \nu^{0.5\pm0.2}$; the spectral slope observed in the X-ray band is $\nu L_\nu \propto \nu^{-0.7\pm0.5}$. We tested synchrotron and synchrotron self-Compton (SSC) scenarios. The observed near-infrared brightness and X-ray faintness, together with the observed spectral slopes, pose challenges for all models explored. We rule out a scenario in which the near-infrared emission is synchrotron emission and the X-ray emission is SSC. A one-zone model in which both the near-infrared and X-ray luminosity are produced by SSC and a model in which the luminosity stems from a cooled synchrotron spectrum can explain the flare. In order to describe the mean SED, both models require specific values of the maximum Lorentz factor $\gamma_{max}$, which however differ by roughly two orders of magnitude: the SSC model suggests that electrons are accelerated to $\gamma_{max}\sim 500$, while cooled synchrotron model requires acceleration up to $\gamma_{max}\sim5\times 10^{4}$. The SSC scenario requires electron densities of $10^{10}~\mathrm{cm^{-3}}$ much larger than typical ambient densities in the accretion flow, and thus require in an extraordinary accretion event. In contrast, assuming a source size of $1R_s$, the cooled synchrotron scenario can be realized with densities and magnetic fields comparable with the ambient accretion flow. For both models, the temporal evolution is regulated through the maximum acceleration factor $\gamma_{max}$, implying that sustained particle acceleration is required to explain at least a part of the temporal evolution of the flare., Comment: accepted for publication in Astronomy & Astrophysics; preview abstract shortened due to arXiv requirements

The Galaxy Evolution Probe (GEP) is a concept for a mid- and far-infrared space observatory to measure key properties of large samples of galaxies with large and unbiased surveys. GEP will attempt to achieve zodiacal light and Galactic dust emission photon background-limited observations by utilizing a 6-K, 2.0-m primary mirror and sensitive arrays of kinetic inductance detectors (KIDs). It will have two instrument modules: a 10 to 400 μm hyperspectral imager with spectral resolution R = λ / Δλ ≥ 8 (GEP-I) and a 24 to 193 μm, R = 200 grating spectrometer (GEP-S). GEP-I surveys will identify star-forming galaxies via their thermal dust emission and simultaneously measure redshifts using polycyclic aromatic hydrocarbon emission lines. Galaxy luminosities derived from star formation and nuclear supermassive black hole accretion will be measured for each source, enabling the cosmic star formation history to be measured to much greater precision than previously possible. Using optically thin far-infrared fine-structure lines, surveys with GEP-S will measure the growth of metallicity in the hearts of galaxies over cosmic time and extraplanar gas will be mapped in spiral galaxies in the local universe to investigate feedback processes. The science case and mission architecture designed to meet the science requirements is described, and the KID and readout electronics state of the art and needed developments are described. This paper supersedes the GEP concept study report cited in it by providing new content, including: a summary of recent mid-infrared KID development, a discussion of microlens array fabrication for mid-infrared KIDs, and additional context for galaxy surveys. Introduction Lower extremity amputations (LEAs) in the burn population may occur urgently due to critical loss of tissue and perfusion or may proceed more slowly if viability remains uncertain. The existing literature describes amputations following electrical injuries; however, little is published discussing burn-injured patients who underwent delayed lower extremity amputations due to other burn mechanisms. Co-morbid illness and infections, patient/family resistance and provider delay in hopes of salvage may prolong time to amputation. Examining outcomes for burn patients’ suffering LEAs will delineate relationships among demographics, burn size, surgical timing, morbidity, mortality and length of stay (LOS) supporting the importance of early amputation. Methods Single verified burn center conducted an IRB exempted retrospective medical records review of adult burn – injured patients, 18 years of age and older, admitted as inpatients from January 1, 2010 to December 31, 2020. Patients were identified using International Classification of Disease 9 & 10 and Current Procedural Terminology codes. Patients with non-burn related amputations or toe / partial foot amputations resulting from diabetes were excluded. The association between multiple variables and both day of first amputation and burn size was measured through linear regression, one sample t-test, and one-way analysis of variance (ANOVA). Results The study population was 12 with 11 males, mean age 44, and one mortality. The majority (83%) suffered flame burns and 58% with mechanism of motor vehicle collision. Hospital day of first amputation ranged from 1-30, with a mean 11. Seven patients required vasopressors ranging from 30 minutes to 500 hours. For every 24 total pressor hours, day of first amputation increased by 1 day. Three patients (25%) had positive blood cultures and two (16%) had positive sputum cultures. Five patients (42%) had positive tissue cultures from the affected extremity. The diagnosis and treatment of infections were detailed and analyzed. Patients with positive tissue cultures were found to have waited an average 10.17 days longer for amputation than those without. Hospital LOS ranged from 31-126 days with mean of 54 days. Conclusions Day of first amputation was the most impactful parameter, with longer time to first amputation associated with positive tissue cultures, mortality, vasopressor therapy and hospital length of stay. Limitations include retrospective nature, small sample size and reliance on electronic medical record and registry data. This study provided data to support earlier amputation for non-viable extremities, though additional research is clearly needed to better describe these relationships.

The focus of a cultural-contextual approach to development is to recognize the fact that considerable differences exist among human individuals, especially in terms of their diverse sociocultural backgrounds. This paper examines how a group of early childhood education students responded to the presentation of a cultural-contextual approach during a human development course. A variety of data were gathered on the reactions of 81 students enrolled in the course during their first year of a four-year undergraduate degree program in early childhood education. The data reflected a number of course-related issues, and the investigation looked for signs that the cultural approach was salient for the students. There appeared to be evidence for only a small degree of success in conveying the approach in terms of students actually mentioning such issues. In analyzing the findings we propose that to reach a larger number of students, it may be necessary to address in a deliberate way the explicit and implicit messages concerning what is important to know about human development. The underlying assumptions of the wider course of study may need to be addressed as well, both at the program level and within the individual courses themselves. Implications for teaching are presented.

Iodine supplements are recommended for women planning pregnancy, but their impact on thyroid function during controlled ovarian hyperstimulation (COH) and into pregnancy is unknown. The aim of this study was to assess the impact of iodine supplementation on thyroid function during COH.One-hundred and six euthyroid women (thyroid stimulating hormone (TSH) 0.4-2.5 mIU/L) planning their first COH cycle were subdivided according to iodine supplementation (nil,6 months, ≥6 months) and compared to levothyroxine (LT4)-treated controls. Serial TSH, free thyroxine, free triiodothyronine and thyroglobulin (Tg) levels were recorded at four time points: (i) baseline, (ii) day 7 ovarian stimulation, (iii) ovulation trigger and (iv) two weeks post oocyte retrieval. Oocyte numbers, fertilisation rates and pregnancy outcome were recorded.TSH increased during COH for those women taking iodine supplements for ≥6 months (P = 0.025). One quarter recorded a TSH level2.5 mIU/L before embryo transfer. A similar increase in TSH was demonstrated by LT4-dependent controls (P = 0.024) but not the remaining subgroups. Tg levels did not change during COH in any group but decreased significantly post oocyte retrieval if nil iodine (P 0.0001) or supplemented for ≥6 months (P 0.005). Iodine supplementation did not influence oocyte count, fertilisation or implantation rates. Women taking iodine for6 months were four times more likely to achieve a live birth than women taking iodine for longer.Women taking iodine supplements for ≥6 months are less able to adapt to the thyroidal demands of COH, with responses comparable to LT4-dependent patients.

Galaxies with nuclear bars are believed to efficiently drive gas inward, generating a nuclear starburst and possibly an active galactic nucleus. We confirm this scenario for the isolated, double-barred, luminous infrared galaxy ESO 320-G030 based on an analysis of Herschel and ALMA spectroscopic observations. Herschel/PACS and SPIRE observations of ESO 320-G030 show absorption or emission in 18 lines of H2O, which we combine with the ALMA H2O 4(23)-3(30) 448 GHz line (E-upper similar to 400 K) and continuum images to study the physical properties of the nuclear region. Radiative transfer models indicate that three nuclear components are required to account for the multi-transition H2O and continuum data. An envelope, with radius R similar to 130-150 pc, dust temperature T-dust approximate to 50 K, and N-H2 similar to 2x10(23) cm(-2), surrounds a nuclear disk with R similar to 40 pc that is optically thick in the far-infrared (tau (100 mu m)similar to 1.5-3, N-H2 similar to 2x10(24) cm(-2)). In addition, an extremely compact (R similar to 12 pc), warm (approximate to 100 K), and buried (tau (100 mu m)> 5, N-H2 greater than or similar to 5x10(24) cm(-2)) core component is required to account for the very high-lying H2O absorption lines. The three nuclear components account for 70% of the galaxy luminosity (SFR similar to 16-18 M-circle dot yr(-1)). The nucleus is fed by a molecular inflow observed in CO 2-1 with ALMA, which is associated with the nuclear bar. With decreasing radius (r=450-225 pc), the mass inflow rate increases up to M-inf similar to 20 M yr(-1), which is similar to the nuclear star formation rate (SFR), indicating that the starburst is sustained by the inflow. At lower r, similar to 100-150 pc, the inflow is best probed by the far-infrared OH ground-state doublets, with an estimated M-inf similar to 30 M yr(-1). The inferred short timescale of similar to 20 Myr for nuclear gas replenishment indicates quick secular evolution, and indicates that we are witnessing an intermediate stage (< 100 Myr) proto-pseudobulge fed by a massive inflow that is driven by a strong nuclear bar. Sagittarius A* (Sgr A*) is the variable radio, near-infrared (NIR), and X-ray source associated with accretion onto the Galactic center black hole. We have analyzed a comprehensive submillimeter (including new observations simultaneous with NIR monitoring), NIR, and 2-8 keV dataset. Submillimeter variations tend to lag those in the NIR by $\sim$30 minutes. An approximate Bayesian computation (ABC) fit to the X-ray first-order structure function shows significantly less power at short timescales in the X-rays than in the NIR. Less X-ray variability at short timescales combined with the observed NIR-X-ray correlations means the variability can be described as the result of two strictly correlated stochastic processes, the X-ray process being the low-pass-filtered version of the NIR process. The NIR--X-ray linkage suggests a simple radiative model: a compact, self-absorbed synchrotron sphere with high-frequency cutoff close to NIR frequencies plus a synchrotron self-Compton scattering component at higher frequencies. This model, with parameters fit to the submillimeter, NIR, and X-ray structure functions, reproduces the observed flux densities at all wavelengths, the statistical properties of all light curves, and the time lags between bands. The fit also gives reasonable values for physical parameters such as magnetic flux density $B\approx13$ G, source size $L \approx2.2R_{S}$, and high-energy electron density $n_{e}\approx4\times10^{7}$ cm$^{-3}$. This study uses a phenomenological approach to analyze interviews of minoritized parents about their perceptions of “respectful” and “disrespectful” experiences with school personnel. Holistic content analysis of the parental interviews revealed several themes that indicated a pervasive “deficit discourse” within public school communities serving minoritized youth. This list includes the heritage language or cultural practices of English language learners (ELLs), immigrants, and people of color. As parents recounted their experiences with school personnel, it appeared that—even when queried about their personal associations with the school—many parents calibrated their relationship based on their perception of the treatment their children were receiving from school personnel. Findings suggest that culturally affirming learning environments are not necessarily universal and that many educators lack the disposition or training to provide culturally efficacious pedagogy.

Blazars represent about 62% of the sources in the ?-ray Fermi-LAT catalog, and about 80% of the ?-ray sources associated with a lower-energy counterpart, dominating the ?-ray sky. In particular, about 42% of the ?-ray blazars are classified as Blazars Candidates of Uncertain type (BCUs) for which spectroscopic observations are mandatory to confirm their blazar nature. Here we report the spectra of 64 targets observed as part of our follow-up optical spectroscopic campaign. This sample includes 62 sources classified in Fermi-LAT catalog as BCUs, one source classified as a generic active galactic nucleus, and one source classified as a BL Lac in Fermi-LAT and as blazar of uncertain type in the Roma-BZCAT catalog. We confirm the blazar nature of all BCUs. We classify 33 of them as BL Lacs, 11 as Flat Spectrum Radio Quasars, and 18 as blazars with nonnegligible host-galaxy emission. We also classify the generic active galactic nucleus as a BL Lac. Finally, we confirm the classification of the BL Lac. We also reported a lower limit redshift for 43 sources. For 20 sources, we got redshift measurements consistent with the literature values. For the other 21 sources, we obtained their first spectroscopic redshift measurement; while, for two sources, we provide lower limits on the redshift. With the upcoming release of the sixth edition of the Roma-BZCAT Multifrequency Catalog of Blazars, we take the opportunity in the present work to present the collective results of the first phase of our optical spectroscopic follow-up campaign. © 2021. Introduction Occupational and Physical Therapist provide acute services to burn-injured patients and continue treatment well into their functional recovery for mobility, strengthening, scar management, contracture prevention and return of independence. Unfortunately, we recognized that patients encountered obstacles when attempting to access these services in the outpatient setting. It was evident that these impediments frequently coincided with social determinants of health (SDOH). According to Healthy People 2020, SDOH represent the cumulative influences of individual, cultural, community and societal attributes on health and quality-of-life. Self-pay patients accounted for 12% of clinic patients. However, patients with coverage were not always receiving adequate therapy due to lack of burn knowledge among community-based therapist. By expanding outpatient burn clinic services, we are better positioned to meet the needs of all burn-injured patients in our community. Methods A plan was outlined to increase the presence of Occupational and Physical Therapy (OT/PT) in the outpatient burn clinic. Pertinent literature was reviewed, and a presentation was developed to establish need and outline a model for integration of services. The inpatient OT/PT department managers collaborated with the Burn Director and Burn APRN on projecting staffing and equipment needs. Multidisciplinary cost analysis was completed. Guidelines for outpatient burn rehabilitative therapy staffing and a description of services were developed. Results After much collaboration and effort, and with community support, our PT/OT inpatient burn staff provide rehabilitative therapy to all patients attending outpatient burn clinic two days weekly. Additionally, educational material, hands on range of motion training and free new and donated compression garments are readily available during all four burn clinics weekly. The new outpatient burn clinic opened in 2021, equipped with two large showers and a designated therapy room, ready to wear compression garments, putty, and assistive devices designed to further support burn recovery. Attendance for burn outpatient visits increased significantly from 2015 to 2018. Conclusions Transportation, childcare, employment restrictions, individual health and wellness beliefs, lack of funding and access to technology represent examples of blockades to healthcare care. This multidisciplinary effort increased the availability and scope of rehabilitative services in the outpatient burn clinic patient, allowing patients to receive comprehensive burn wound care and rehabilitative therapy in one location to promote healing, manage pain, improved function, maximizes independence and reintegrate into their family and community.

Star formation primarily occurs in filaments where magnetic fields are expected to be dynamically important. The largest and densest filaments trace spiral structure within galaxies. Over a dozen of these dense ($\sim$10$^4$\,cm$^{-3}$) and long ($>$10\,pc) filaments have been found within the Milky Way, and they are often referred to as "bones." Until now, none of these bones have had their magnetic field resolved and mapped in their entirety. We introduce the SOFIA legacy project FIELDMAPS which has begun mapping $\sim$10 of these Milky Way bones using the HAWC+ instrument at 214\,$��$m and 18$\farcs$2 resolution. Here we present a first result from this survey on the $\sim$60\,pc long bone G47. Contrary to some studies of dense filaments in the Galactic plane, we find that the magnetic field is often not perpendicular to the spine (i.e., the center-line of the bone). Fields tend to be perpendicular in the densest areas of active star formation and more parallel or random in other areas. The average field is neither parallel or perpendicular to the Galactic plane nor the bone. The magnetic field strengths along the spine typically vary from $\sim$20 to $\sim$100\,$��$G. Magnetic fields tend to be strong enough to suppress collapse along much of the bone, but for areas that are most active in star formation, the fields are notably less able to resist gravitational collapse., Accepted to ApJL; typo in author list metadata corrected

Emission from active galactic nuclei (AGNs) is known to play an important role in the evolution of many galaxies including luminous and ultraluminous systems (U/LIRGs), as well as merging systems. However, the extent, duration, and exact effects of its influence are still imperfectly understood. To assess the impact of AGNs on interacting systems, we present a Spectral Energy Distribution (SED) analysis of a sample of 189 nearby galaxies. We gather and systematically re-reduce archival broad-band imaging mosaics from the ultraviolet to the far-infrared using data from GALEX, SDSS, 2MASS, IRAS, WISE, Spitzer and Herschel. We use spectroscopy from Spitzer/IRS to obtain fluxes from fine-structure lines that trace star formation and AGN activity. Utilizing the SED modelling and fitting tool CIGALE, we derive the physical conditions of the ISM, both in star-forming regions and in nuclear regions dominated by the AGN in these galaxies. We investigate how the star formation rates (SFRs) and the fractional AGN contributions ($f_{\rm{AGN}}$) depend on stellar mass, galaxy type, and merger stage. We find that luminous galaxies more massive than about $10^{10} \rm{M}_{*}$ are likely to deviate significantly from the conventional galaxy main-sequence relation. Interestingly, infrared AGN luminosity and stellar mass in this set of objects are much tighter than SFR and stellar mass. We find that buried AGNs may occupy a locus between bright starbursts and pure AGNs in the $f_{\rm{AGN}}$-[Ne V]/[Ne II] plane. The fourth Fermi Large Area Telescope Source Catalog (4FGL) lists over 5000 gamma-ray sources with statistical significance above 4$\sigma$. About 23% of the sources listed in this catalog are unidentified/unassociated gamma-ray sources while ~26% of the sources are classified as blazar candidates of uncertain type (BCUs), lacking optical spectroscopic information. To probe the blazar nature of candidate counterparts of UGSs and BCUs, we started our optical spectroscopic follow up campaign in 2012, which up to date account for more than 350 observed sources. In this paper, the tenth of our campaign, we report on the spectroscopic observations of 37 sources, mostly BCUs, whose observations were carried out predominantly at the Observatorio Astron\'omico Nacional San Pedro M\'artir and the Southern Astrophysical Research Observatory between August 2018 and September 2019. We confirm the BL Lac nature of 27 sources and the flat spectrum radio quasar nature of three sources. The remaining ones are classified as six BL Lacs galaxy-dominated and one normal galaxy. We were also able to measure the redshifts for 20 sources, including 10 BL Lacs. A significant fraction of all $\gamma$-ray sources detected by the Large Area Telescope aboard the \fer\ satellite is still lacking a low-energy counterpart. In addition, there is still a large population of $\gamma$-ray sources with associated low-energy counterparts that lack firm classifications. In the last 10 years we have undertaken an optical spectroscopic campaign to address the problem of unassociated/unidentified $\gamma$-ray sources (UGSs), mainly devoted to observing blazars and blazar candidates because they are the largest population of $\gamma$-ray sources associated to date. Here we describe the overall impact of our optical spectroscopic campaign on sources associated in \fer-LAT catalogs, coupled with objects found in the literature. In the literature search, we kept track of efforts by different teams that presented optical spectra of counterparts or potential counterparts of \fer-LAT catalog sources. Our summary includes an analysis of an additional 30 newly-collected optical spectra of counterparts or potential counterparts of \fer-LAT sources of previously unknown nature.New spectra were acquired at the Blanco 4-m and OAN-SPM 2.1-m telescopes, and those available in the Sloan Digital Sky Survey (data release 15) archive. All new sources with optical spectra analyzed here are classified as blazars. Thanks to our campaign, we altogether discovered and classified 394 targets with an additional 123 objects collected from a literature search. We began our optical spectroscopic campaign between the release of the second and third \fer-LAT source catalogs (2FGL and 3FGL, respectively), and classified about 25\% of the sources with uncertain nature and discovered a blazar-like potential counterpart for $\sim$10\% of UGSs listed therein. In the 4FGL catalog, about 350 \fer-LAT sources are classified to date thanks to our campaign. Summary Parathyroid-independent hypercalcaemia of pregnancy, due to biallelic loss of function of the P450 enzyme CYP24A1, the principal inactivator of 1,25(OH)2D results in hypervitaminosis D, hypercalcaemia and hypercalciuria. We report two cases of this disorder, with intractable hypercalcaemia, one occurring during gestation and into the postpartum, and the other in the postpartum period. Case 1, a 47-year-old woman with a twin pregnancy conceived by embryo transfer, presented with hypercalcaemia at 23 weeks gestation with subnormal serum parathyroid hormone (PTH) and normal serum 25-OH D levels. She was admitted to hospital at 31 weeks gestation with pregnancy-induced hypertension, gestational diabetes and increasing hypercalcaemia. Caesarean section at 34 weeks gestation delivered two healthy females weighing 2.13 kg and 2.51 kg. At delivery, the patient’s serum calcium level was 2.90 mmol/L. Postpartum severe hypercalcaemia was treated successfully with Denosumab 60 mg SCI, given on two occasions. CYP24A1 testing revealed she was compound heterozygous for pathogenic variants c.427_429delGAA, (p.Glu143del) and c.1186C>T, (p.Arg396Trp). Case 2, a 36-year-old woman presented 4 days after the delivery of healthy twins with dyspnoea, bradycardia, severe headaches, hypertension and generalized tonic-clonic seizures after an uneventful pregnancy. She was hypercalcaemic with a suppressed PTH, normal 25(OH)D, and elevated 1,25(OH)2D levels. Her symptoms partially responded to i.v. saline and corticosteroids in the short term but bisphosphonates such as Pamidronate and Zoledronic acid did not result in sustained improvement. Denosumab 120 mg SCI successfully treated the hypercalcaemia which resolved completely 2 months post-partum. CYP24A1 testing revealed she was homozygous for the pathogenic variant c.427_429delGAA, (p.Glu143del). Learning points: Hypercalcaemia in pregnancy can be associated with considerable morbidity with few options available for management. In non-PTH-related hypercalcaemia the diagnosis of CYP24A1 deficiency should be considered. Making a definitive diagnosis of CYP24A1 deficiency by genetic testing delays the diagnosis, while the availability of serum 24,25-dihydroxyvitamin D (24,25(OH)2D) will expedite a diagnosis. In pregnant women with CYP24A1 deficiency hypercalcaemia can worsen in the post-partum period and is more likely to occur with twin pregnancies but generally resolves within 2–3 months. Therapeutic alternatives are limited in pregnancy and their effectiveness is short-lived and mostly ineffective. Denosumab used in both our patients after delivery was the most effective agent normalizing calcium and may have benefit as a long-term therapeutic agent in preventing complications in patients with CYP24A1 deficiency.
