Abstract

Aim: The SAMe-TT2R2 score helps identify patients with atrial fibrillation (AF) likely to have poor anticoagulation control during anticoagulation with vitamin K antagonists (VKA) and those with scores >2 might be better managed with a target-specific oral anticoagulant (NOAC). We hypothesized that in clinical practice, VKAs may be prescribed less frequently to patients with AF and SAMe-TT2R2 scores >2 than to patients with lower scores. Methods and results: We analyzed the Phase III dataset of the Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation (GLORIA-AF), a large, global, prospective global registry of patients with newly diagnosed AF and ≥1 stroke risk factor. We compared baseline clinical characteristics and antithrombotic prescriptions to determine the probability of the VKA prescription among anticoagulated patients with the baseline SAMe-TT2R2 score >2 and ≤ 2. Among 17,465 anticoagulated patients with AF, 4,828 (27.6%) patients were prescribed VKA and 12,637 (72.4%) patients an NOAC: 11,884 (68.0%) patients had SAMe-TT2R2 scores 0-2 and 5,581 (32.0%) patients had scores >2. The proportion of patients prescribed VKA was 28.0% among patients with SAMe-TT2R2 scores >2 and 27.5% in those with scores ≤2. Conclusions: The lack of a clear association between the SAMe-TT2R2 score and anticoagulant selection may be attributed to the relative efficacy and safety profiles between NOACs and VKAs as well as to the absence of trial evidence that an SAMe-TT2R2-guided strategy for the selection of the type of anticoagulation in NVAF patients has an impact on clinical outcomes of efficacy and safety. The latter hypothesis is currently being tested in a randomized controlled trial. Abstract

Whereas multiple national, international, and trial registries for heart failure have been created, international standards for clinical assessment and outcome measurement do not currently exist. The working group’s objective was to facilitate international comparison in heart failure care, using standardized parameters and meaningful patient-centered outcomes for research and quality of care assessments. The International Consortium for Health Outcomes Measurement recruited an international working group of clinical heart failure experts, researchers, and patient representatives to define a standard set of outcomes and risk-adjustment variables. This was designed to document, compare, and ultimately improve patient care outcomes in the heart failure population, with a focus on global feasibility and relevance. The working group employed a Delphi process, patient focus groups, online patient surveys, and multiple systematic publications searches. The process occurred over 10 months, employing 7 international teleconferences. A 17-item set has been established, addressing selected functional, psychosocial, burden of care, and survival outcome domains. These measures were designed to include all patients with heart failure, whether entered at first presentation or subsequent decompensation, excluding cardiogenic shock. Sources include clinician report, administrative data, and validated patient-reported outcome measurement tools: the Kansas City Cardiomyopathy Questionnaire; the Patient Health Questionnaire-2; and the Patient-Reported Outcomes Measurement Information System. Recommended data included those to support risk adjustment and benchmarking across providers and regions. Abstract

AMD's 6th generation “Carrizo” APU, targeted at 12–35 W mobile computing form factors, contains 3.1 billion transistors, occupies 250.04 mm $^{2}$ and is implemented in a 28 nm HKMG planar dual-oxide FET technology with 12 metal layers. The design achieves a 29% improvement in transistor density compared to the 5th generation “Kaveri” APU, also a 28 nm design, and implements several power management features resulting in area and power improvements similar to a technology shrink. Increased power density makes meeting the thermal limits required for reliability and power distribution to the APU's processors substantial design challenges. Pre-silicon thermal analysis is used to understand and take advantage of thermal gradients. Adaptive voltage-frequency scaling in the processor core as well as wordline and bitline assist techniques in the L2 cache enable lower minimum voltage requirements.

Abstract

Codenamed “Zen”, AMD's next-generation, high-performance ×86 core targets server, desktop, and mobile client applications. Utilizing Global Foundries' energy-efficient 14nm LPP FinFET process, the 44mm2 Zen core complex unit (CCX) has 1.4B transistors and contains a shared 8MB L3 cache and four cores (Fig. 3.2.7). The 7mm2 Zen core contains a dedicated 0.5MB L2 cache, 32KB L1 data cache, and 64KB L1 instruction cache. Each core has a digital low drop-out (LDO) voltage regulator and digital frequency synthesizer (DFS) to independently vary frequency and voltage across power states.

Abstract

This paper describes key circuit innovations in a new x86-64 micro-architecture AMD code-named “Bulldozer” , . It is implemented in 32 nm high-K metal gate SOI CMOS. It occupies 30.9 mm-2, contains 213 million transistors, reduces the number of F04 gates per cycle by more than 20% compared to a previous processor in the same technology , and demonstrates superior frequency scaling across voltage. The module includes two independent integer cores but shares the fetch, decode, floating-point, and L2 cache units to maximize single-threaded performance and multi-threaded throughput while significantly improving power and area efficiency compared to fully replicated CPU cores. The design includes a new soft-edged flop (SEF) family to enable high frequency and low power. Achieving power efficiency in combination with high-frequency design is a particular challenge, and this paper describes several of the unique approaches to power optimization that have been employed in the design. The gate-count reduction and power optimization enable faster frequencies in the same power envelope compared to previous designs.

Abstract

AMD's 2-core “Bulldozer” module contains 213 million transistors in an 11-metal layer 32nm HKMG SOI CMOS process and is designed to operate from 0.8 to 1.3V. This new micro-architecture [1] improves performance and frequency while reducing area and power compared to a previous AMD x86–64 CPU in the same process [2]. To achieve these goals, the design reduced the number of FO4 inverter delays/cycle by more than 20%, achieving higher frequencies in the same power envelope even with increased core counts. The 2-core CPU module area (including 2MB L2 cache) is 30.9mm2 (Fig. 4.5.7).

AP Conduction in Hypertrophied Myocardium. Introduction: Cardiac hypertrophy is associated with an increased incidence of arrhythmias that result from altered action potential configuration or propagation velocity. These variables were measured in isolated preparations of human left ventricular myocardium and correlated with the degree of hypertrophy. Methods and Results: Cardiac mass was estimated by echocardiography and cell diameter was measured from fixed isolated specimens; the two variables correlated significantly. Action potential duration was measured under field stimulation but was independent of the degree of hypertrophy; however, the duration was longer in septal preparations (405 ± 12 msec, 37°C, 1-Hz stimulation) than in papillary muscles (342 ± 11 msec). Conduction velocity decreased progressively as cell diameter increased both in septal and papillary muscle preparations. Cable analysis showed that the variation of conduction velocity could be accounted for adequately by an increase of the intracellular resistivity of the preparations. Conclusion: The data suggest that conduction defects occur in a progressive manner in human hypertrophy, which would provide an important substrate for dysrhythmias in human left ventricular hypertrophy and could result from a decrease of electrical coupling between adjacent myocardial cells.
