Background and Aims Early detection of fibrosis is important in identifying individuals at risk for advanced liver disease in non-alcoholic fatty liver disease (NAFLD). We tested whether second-harmonic generation (SHG) and coherent anti-Stokes Raman scattering (CARS) microscopy, detecting fibrillar collagen and fat in a label-free manner, might allow automated and sensitive quantification of early fibrosis in NAFLD. Methods We analyzed 32 surgical biopsies from patients covering histological fibrosis stages 0-4, using multimodal label-free microscopy. Native samples were visualized by SHG and CARS imaging for detecting fibrillar collagen and fat. Furthermore, we developed a method for quantitative assessment of early fibrosis using automated analysis of SHG signals. Results We found that the SHG mean signal intensity correlated well with fibrosis stage and the mean CARS signal intensity with liver fat. Little overlap in SHG signal intensities between fibrosis stages 0 and 1 was observed. A specific fibrillar SHG signal was detected in the liver parenchyma outside portal areas in all samples histologically classified as having no fibrosis. This signal correlated with immunohistochemical location of fibrillar collagens I and III. Conclusions This study demonstrates that label-free SHG imaging detects fibrillar collagen deposition in NAFLD more sensitively than routine histological staging and enables observer-independent quantification of early fibrosis in NAFLD with continuous grading.

The aim of this study was to set up a new total internal reflection (TIRF) microscope acquired by Biomedicum Imaging Unit, one of the core facility service units at the Meilahti medical campus of the University of Helsinki. The existing literature was examined from the point of view of potential users of the imaging system, i.e. assuming a good working knowledge of cell biology and labelling procedures. General information on fluorescence and the exploitation of this methodology in microscopy is dealt with, as are the general components of a fluorescence microscope. Different optical sectioning techniques and live cell imaging are also discussed. TIRF and the resultant evanescent wave are discussed in more detail as are the configurations of a microscope setup to utilize this phenomenon in functional fluorescence microscopy. In the experimental part of the work, the verification of the system setup was done using cultured carcinoma cells with fluorescently labelled cell-substrate adhesion sites. The effect of the TIRF angle on the penetration depth of the evanescent field in cells, and the resultant image intensity was analyzed. The suitability of the system for live cell imaging was evaluated using fluorescent VSVG3-GFP constructs. Finally, preliminary feasibility studies were performed on a sterol analog transport together with specific inhibitions to evaluate the usefulness of TIRF techniques in future studies of cholesterol transport., Työn tarkoituksena oli ottaa Biomedicumin solukuvantamisyksikköön sijoitettu uusi kokonaisheijastukseen perustuva fluoresenssimikroskooppi (TIRF mikroskooppi) käyttöön. Laite sijoitettiin Helsingin yliopiston Biomedicumin solukuvantamisyksikköön. Kirjallisuusosassa käydään läpi fluoresenssiin perustuvan mikroskopoinnin perusteet solukuvantamisyksikön oletetun käyttäjän näkökulmasta. Yleisesti käytössä olevien optisten segmentointimenetelmien perusteet käsitellään samoin kuin elävien solujen kuvantamisen erityisvaatimukset. Valon kokonaisheijastuminen ja siitä seuraava eksponentiaalisesti häviävä energia-aalto ovat kirjallisuusosan keskeinen sisältö. Tämän ilmiön käyttö fluoresoivien merkkiaineiden virittämiseen ja siitä seuraavan signaalin havaitsemiseen mikroskopialla käydään läpi. Lisäksi käsitellään erilaiset TIRF mikroskoopin kokoonpanot. Tutkimusosassa laitteiston asetukset tarkistetaan käyttäen karsinoomasolujen fluoresoivalla merkkiaineella leimattuja solun ja soluväliaineen välisiä tarttumarakenteita. TIRF heijastuskulma, sen vaikutus fluoresenssia aiheuttavan energia-aallon syvyyteen sekä tämän syvyyden vaikutus kuvan kirkkauteen tarkastellaan. Järjestelmän sopivuus elävien solujen kuvantamiseen arvioitiin fluoresoivalla VSVG3-GFP konstruktilla. Lopuksi suoritettiin alustava arvio järjestelmän sopivuudesta solunsisäisen kolesterolin kuljetuksen kuvantamisessa käyttäen steroli-analogia ja spesifejä estoaineita.
