Background: Although the Korean Society for Cytopathology has developed educational goals as guidelines for cytopathology education in Korea, there is still no systematic approach to cytopathology education status for pathology residents. Furthermore, satisfaction with cytopathology education and with the outcome of the current training/educational program has not been investigated in Korea. This study aimed to obtain comprehensive data on the current state of cytopathology education for residents and evaluate education outcomes. Methods: An online survey was conducted in December 2020 for the board-certified pathologists and training residents registered as members of the Korean Society for Cytopathology. The questionnaire comprised questions that investigated the current status of cytopathology at each training institution, the degree of satisfaction with the work and education related to cytopathology, outcomes of cytopathology training, and educational accomplishments. Results: Of the participants surveyed, 12.3% (132/1,075) completed the questionnaire, and 36.8% (32/87) of cytopathology residents participated. The mean overall satisfaction with cytopathology education was 3.1 points (on a 1- to 5-point scale, 5: very satisfied). The most frequent suggestion among the free description format responses was to expand educational opportunities, such as online education opportunities, outside of the individual institutions. Conclusions: Our results showed that cytopathology training in Korea needs further improvement. We expect that this study will inform systematic training of competent medical personnel armed with broad cytopathology knowledge and strong problem-solving abilities.

In numerical models, the amount of clouds affects atmospheric temperature through interaction with radiation. In the Korean Integrated Model (KIM), which has been in operation at the Korea Meteorological Administration since April 2020, the amount of clouds is determined from prognostic equation consisting of source and sink terms by physical processes such as planetary boundary layer (PBL) mixing, convection, advection, condensation, and evaporation. In the control KIM version, the temperature forcing used for calculating the rate of changes in time of saturation specific humidity which determines formation of cloud area due to condensation is calculated by considering those from all physical processes, such as radiation, cumulus convection, and turbulence, as well as cloud microphysical processes. However, we found the inconsistency between the cloud fraction and mixing ratio by using the methodology, so in this study, we modify the temperature forcing from all physical processes into that only due to the microphysical process. It is confirmed that the change in the amount of clouds changes the temperature and humidity of the atmosphere through the interaction between physical processes such as radiation, which also affects precipitation. In this study, to examine the effect of changes in cloud cover on precipitation in the Korean Peninsula, we perform one case study July 4, 2021 when precipitation in Gangwon occurs due to the convergence of air currents caused by east wind of high pressure in the eastern of Korea. Up to 172.5 mm of daily maximum precipitation was reported in the Gangwon region. In the 3-day forecast of the case, the control KIM underestimates inland precipitation. But the trend of under-estimation is improved by increasing the amount of precipitation when the cloud amount is modified. The increase in precipitation mainly occurs in the large-scale precipitation due to the microphysical process. This is because the cloud amount generally increases in the Asian area including the Korean Peninsula, which makes the environment favor to the precipitation, by decreasing the temperature through the radiative cooling, in turn resulting the decrease in saturation vapor pressure. For the statistical evaluation of the precipitation performance, precipitation verification is also performed for one month in July, and it is found that ETS (Equivalent Threat Score) performance against the reanalysis data on the Korean Peninsula is also improved.

Purpose: The purpose of this study was to examine the prognostic and oxaliplatin predictive value of mismatch repair (MMR) status and common hot spot mutations, which we previously identified in stage II and III colon cancer.Experimental Design: Mutations in BRAF, KRAS, NRAS, MET, and PIK3CA were profiled in 2,299 stage II and III colon tumors from National Surgical Adjuvant Breast and Bowel Project (NSABP) clinical trials C-07 (n = 1,836) and C-08 (n = 463) with Type Plex chemistry and mass spectrometry. C-07 tested the worth of adding oxaliplatin to 5-fluorouracil plus leucovorin, and C-08 tested the worth of adding bevacizumab to FOLFOX. Cox proportional hazard models were used to assess prognostic or oxaliplatin predictive value of mutations for tumor recurrence, overall survival (OS), and survival after recurrence (SAR).Results:BRAF mutations were associated with MMR-deficient tumors (P < 0.0001), poor OS [HR, 1.46; 95% confidence interval (CI), 1.20–1.79; P ≤ 0.0002], and poor SAR (HR, 2.31; 95% CI, 1.83–2.95; P < 0.0001). Mutations in KRAS, NRAS, MET, and PIK3CA were not associated with recurrence, OS, or SAR. MMR-deficient tumors were associated with an improved prognosis based on recurrence (HR, 0.48; 95% CI, 0.33–0.70; P < 0.0001). Mutations and MMR status were not predictive for oxaliplatin benefit.Conclusions: This study shows that BRAF mutations profiled from stage II and III colon cancer tumors were associated with poor SAR and validates and explains, at least in part, previous observations associating it with poor OS. Profiling of all of these mutations is warranted for future clinical trials testing new targeted therapies that block relevant signaling pathways. Such clinical trials are under development at NSABP. Purpose To evaluate the accuracy of MRI in predicting the pathological complete response (pCR) and the residual tumor size of breast cancer after neoadjucant chemotherapy (NAC), and to determine the factors affecting the accuarcy. Materials and Methods Eighty-eight breast cancer patients who underwent surgery after NAC at our center between 2010 and 2017 were included in this study. pCR was defined as the absence of invasive cancer on pathological evaluation. The maximum diameter of the residual tumor on post-NAC MRI was compared with the tumor size of the surgical specimen measured pathologically. Statistical analysis was performed to elucidate the factors affecting pCR and the residual tumor size-discrepancy between the MRI and the pathological measurements. Results The pCR rate was 10%. The diagnostic accuracy of MRI and the area under the curve for predicting pCR were 90.91% and 0.8017, respectively. The residual tumor sizes obtained using MRI and pathological measurements showed a strong correlation (r = 0.9, p < 0.001), especially in patients with a single mass lesion (p = 0.047). The size discrepancy between MRI and the pathological measurements was significantly greater in patients with the luminal type (p = 0.023) and multifocal tumors/non-mass enhancement on pre-NAC MRI (p = 0.047). Conclusion MRI is an accurate tool for evaluating pCR and residual tumor size in breast cancer patients who receive NAC. Tumor subtype and initial MRI features affect the accuracy of MRI.

The effects of a nonlocal planetary boundary layer (PBL) scheme that considers scale dependency in the parameterized turbulent vertical transport are investigated for a case of wintertime lake-effect precipitation over Korea at gray-zone resolutions using a mesoscale model. An experiment using the scale-aware PBL scheme is compared with that using a conventional PBL scheme, which shows that the simulated precipitation amount at a resolution of less than 1 km is smaller with the scale-aware PBL scheme. The role of turbulent processes in simulating lake-effect precipitation is understood through interaction with microphysical processes. When the scale-aware PBL scheme is used, liquid water content is increased while ice water content is reduced. The higher cloud water content is because of enhanced condensation with stronger updrafts, attributed to the suppression of parameterized turbulent mixing. This results in higher rainwater content by enhancing autoconversion and accretion from cloud water to rainwater. The cloud ice content is reduced mainly because of the suppressed deposition and enhanced sublimation centered near the PBL top, and the snow content is reduced mainly because of the enhanced sublimation below and near the PBL top and suppressed growth of cloud ice to snow. The lower ice water content is mainly due to the drier PBL, attributed to the enhanced resolved (suppressed parameterized) turbulent moisture transport and enhanced condensation. The melting of a smaller amount of snow under dominant cold rain processes is responsible for the reduced surface precipitation with the scale-aware PBL scheme.

The parametrization of orographic drag processes is a major source of circulation uncertainty in models. The COnstraining ORographic Drag Effects (COORDE) project makes a coordinated effort to narrow this uncertainty by bringing together the modeling community to: explore the variety of orographic drag parametrizations employed in current operational models; assess the resolution sensitivity of resolved and parametrized orographic drag across models; and to validate the parametrized orographic drag in low‐resolution simulations using explicitly resolved orographic drag from high‐resolution simulations. Eleven models from eight major modeling centers are used to estimate resolved orographic drag from high‐resolution (km‐scale) simulations and parametrized orographic drag from low‐resolution simulations, typically used for seasonal forecasting (∼40 km) and climate projections (∼100 km). In most models, at both seasonal and climate resolutions, the total (resolved plus parametrized) orographic gravity wave drag over land is shown to be underestimated by a considerable amount (up to 50%) over the Northern and Southern Hemisphere and by more than 60% over the Middle East region, with respect to the resolved gravity wave drag estimated from km‐scale simulations. The km‐scale simulations also provide evidence that the parametrized surface stress and the parametrized low‐level orographic drag throughout the troposphere are overestimated in most models over the Middle East region, particularly at climate resolutions. Through this process‐based evaluation, COORDE provides model developers new valuable information on the current representation of orographic drag at seasonal and climate resolutions and the vertical partitioning of orographic low‐level and gravity wave drag.

BACKGROUND Human leukocyte antigen class I (HLA-I) molecules play important roles in regulating immune responses. Loss or reduction of HLA-I expression has been shown to be associated with prognosis in several cancers. Regulatory T-cells (Tregs) also play critical functions in immune response regulation. Evaluation of HLA-I expression status by the EMR8-5 antibody and its clinical impact in breast cancer have not been well studied, and its relationship with Tregs remains unclear. METHODS We evaluated HLA-I expression and Treg infiltration by immunohistochemistry in 465 surgically resected breast cancer samples. We examined the correlation between HLA-I expression and Treg infiltration and clinicopathologic characteristics and survival analyses were performed. RESULTS Total loss of HLA-I expression was found in 84 breast cancer samples (18.1%). Univariate survival analysis revealed that loss of HLA-I expression was significantly associated with worse disease-specific survival (DSS) (p = .029). HLA-I was not an independent prognostic factor in the entire patient group, but it was an adverse independent prognostic factor for DSS in patients with advanced disease (stage II-IV) (p = .031). Treg numbers were significantly higher in the intratumoral stroma of HLA-I-positive tumors than in HLA-I-negative tumors (median 6.3 cells/high power field vs 2.1 cells/high power field, p < .001). However, Tregs were not an independent prognostic factor in our cohort. CONCLUSIONS Our findings suggest that the loss of HLA-I expression is associated with poor prognosis in breast cancer patients, highlighting the role of HLA-I alterations in immune evasion mechanisms of breast cancer. HLA-I could be a promising marker that enables the application of more effective and precise immunotherapies for patients with advanced breast cancer.

This study implements the parameterizations of convective and frontal gravity wave drag (GWD) with wide phase speed spectra into a global forecast model with a model top near 0.3 hPa. The new convective GWD scheme replaces the existing one that considers only a stationary convective GW, and the frontal GWD scheme is newly introduced. When the new GWD schemes are used, the Rayleigh friction, applied above 2 hPa to mimic the effects of missing GWD, is removed. The convective (frontal) GWs are generated mainly in the Intertropical Convergence Zone and winter extratropical storm track regions (extratropics where strong baroclinicity exists). The convective and frontal GWD derived from the new schemes are significant near the model top, with maxima of ~2-4 and ~26-58 m s−1 day−1, respectively. The differences in convective GWD between the stationary and non-stationary schemes appear mainly in the tropics and summer hemisphere, where stationary GWs cannot propagate upward. The new schemes improve the seasonal representation of stratospheric wind, through changes in both the GWD and the resolved wave forcing, which is modulated by the changed large-scale wind due to the GWD. The downward influence, in response to the changed GWD, is also positive in the tropospheric fields, such as subtropical jet and planetary-scale disturbances. For the medium-range forecasts, improved skill scores on wind speed are achieved globally with the new schemes. The improvements mostly appear only in the stratosphere during the early forecast period (~3 days) but expand to the troposphere as forecast time increases.

Blood coagulation tests are a critical part in diagnosis of patients with bleeding disorders and cardiovascular diseases. However, most conventional blood coagulation tests require large and expensive instruments and skilled personnel. Here, we present a simple and low-cost approach to evaluate blood coagulation using Lamb wave-based device. To assess coagulation, we monitored the acoustic streaming of 5 μm fluorescent particles suspended in citrated plasma. Using a Lamb wave, citrated blood plasma and coagulation reagents can be mixed in a few seconds and the viscosity of the droplet increases due to blood coagulation. Then, the acoustic streaming of particles eventually stops, at which the “coagulation time” is defined. As a preliminary test, coagulation time was reduced with increasing calcium concentration from 0 to 25 mM. Prothrombin time results from the Lamb wave-based device were validated by comparing with those of commercial instruments with the correlation coefficient (R2) as 0.9432. Using the Lamb wave for blood coagulation test, the temperature can be controlled as 37 °C simultaneously by modulating experimental conditions. Our device enables lithography-free, simple and low-cost measurement of blood coagulation, which can be applied to clinical settings and developing countries.

Targeted application of next-generation sequencing (NGS) technology allows detection of specific mutations that can provide treatment opportunities for cancer patients. We evaluated the applicability of the Ion AmpliSeq Cancer Hotspot Panel V2 (CHV2) using formalin-fixed, paraffin-embedded (FFPE) tissue of clinical specimens. Thirty-five FFPE tumour samples with known mutational status were collected from four different hospitals and sequenced with CHV2 using an Ion Chef System and Ion S5 XL system. Out of 35 cases, seven were sequenced with Oncomine focus Assay Panel for comparison. For the limit of detection test, we used an FFPE reference standard, a cell line that included an engineered 50% EGFR T790 M in an RKO cell line background. Coverage analysis results including number of mapped reads, on target percent, mean depth, and uniformity were not different according to hospitals. Sensitivity for mutation detection down to 3% was demonstrated. NGS results showed 100% concordance with the results from single molecular pathology tests Assay in 30 cases with 24 known positive mutations and 14 known negative mutations, and another NGS panel of the Oncomine focus in seven cases. The CHV2 NGS test for solid tumours using Ion chef system and S5 XL system in clinical molecular pathology laboratories for analysis of solid tumours could be routinely used and could replace some single molecular pathology tests after a stringent and thorough validation process.

SNS users are continuously increasing and SNS is emerging as a major PR channel of government. In particular, SNS can be a useful tool, but also the challenges for risk communication. This study is examined how nuclear regulatory organizations'SNS management and visitors’ responses to the facebook pages in Korea and U.S..Results indicated that both organizations frequently used information-oriented posting. Concerning the types of PR, NSSC tended to mainly use 'Relationship-Management PR', NRC mainly use 'Image PR.' In addition, our analyses on visitors' responses showed that visitors usually in response to participational and visual(ex. video, card news) typed contents. Based on its’ results, this study provides practical suggestions for government’s future SNS management strategies.
