Introduction. The geopolitical and economic agenda for Russia’s economic sovereignty has required significant changes in the modes of production and, consequently, in the economic environment. The construction market is an extremely important and integral part of it in the conditions of increasing sanctions pressure. The analysis of the SMO period has convincingly proved the viability of market rules of business turnover in the investment and construction sector. None of its segments turned out to be in crisis and depression.Materials and methods. Business activity grew frontally. Meanwhile, the observed time compression of the emergence of new growing external challenges, which revealed and made it possible to identify latent processes slowing down the modernization of the construction industry, prompted the authors to change the generally accepted perspective of studying the construction market from the use of the method of decomposition of its segments to the implementation of the methodological approach of “time management in restructured space” in order to complement the megapolis concept of settlement with the strategy of accelerated development of territories in the points and corridors of economic.Results. The synthesized objectives of total acceleration of territorial, project, subject and resource maneuvering allowed to identify goals and objectives of transformation of commodity-money relations emerging within the construction market in the context of the selected types of capital construction. In this context, the authors have identified and disclosed the content of the following tasks of construction market development: providing spatial and temporal maneuverability of production capacities of market entities; synchronization and scaling of investment and construction processes; reshoring and technological import substitution; unfinished construction liquidation; supporting development rates of socially-oriented segment of construction market. Solving these tasks will require territorial, project, subject and resource front-end maneuvering. As practice has shown, this is impossible without institutionalization of a new toolkit of spatial-territorial development. Its goal is to fully realize the country’s geospatial advantage by mitigating the risks of the megacity concept of settlement, while simultaneously developing territories not only at points, but also along major spatial corridors of economic growth.Conclusions. The article took into account approaches to accelerate the commissioning of various types of capital construction projects, which allowed the study to identify the directions of improving market mechanisms across different ССP. The unity of different directions is ensured by preventive methods of avoiding the conflict of economic interests, and the resolution of inevitably arising contradictions of development is proposed using methods of extrajudicial resolution of economic disputes, based on the use of spatial-territorial, legal, judicial, cost, construction and technical, environmental expertise.

The quality of planted forest crops depends significantly on the qualities of the planting stock. Numerous studies have been carried out to find clear seedling quality criteria and work out methods for their quantitative assessment to optimize the cultivation regime. Indicators of planting stock that are available for measurement under nursery conditions are needed to predict the success of forest crop establishment. This paper provides a review of the studies that applied different morphological criteria to evaluate coniferous planting stock quality and different determination methods. The suitability of planting stock quality indicators is determined by the effectiveness of predicting the outplanting survival rate (%) and growth. The key morphometric attributes of planting stock are the linear dimensions and biomass of whole seedlings and their individual organs: root collar diameter (RCD), seedling height (SH), dry mass of needles (NDM), stem (SBDM), roots (RDM), shoot/aboveground part of the seedling (SDM), seedling’s total dry matter (TDM), number of needles (NN). Effective indicators of seedling quality are morphometric indicators of roots: total length (TRL), volume (TRV), dry mass (RDM), number of first- (FOLR N), second- (SOLR N), and third-order (TOLR N) lateral roots, root surface area (TRS). TRL is a fairly reliable indicator of absorbent surface area. To minimize errors in the use of one or two traits, integrated seedling quality indices based on two or more indices have been developed. Widely used indices include: SQ = SH(cm)/RCD(mm); SDM/RDM ratio; Dickson quality index DQI = TDM/ ((SH / RCD) + (SDM / RDM)). RCD was the most suitable parameter to indicate seedling quality for many species due to its higher correlation level with the Dickson quality index. The SH index proved to be an effective indicator for analysis only when used together with RCD. When studying the formation of frost resistance, a number of morphological parameters are used: initiation of needle primordia, size of primordial shoots, cell volume, mitotic index (MI) of terminal primordia. Despite advances in testing planting stock quality and predicting field performance, not a single test is universally applicable for all woody plant species and environmental conditions. According to the target seedling concept, the application of morphometric criteria for assessing seedling quality in forest nurseries is a prerequisite for predicting outplanting success and maximizing the predictability of the outcomes of highproductivity forest crop planting. Aim. To study the prevalence of alexithymia among adolescents in the central (Krasnoyarsk city/Caucasians) and southern (Abakan, the Republic of Khakassia, Caucasians/Khakasses) regions of Siberia, taking into account ethnicity and gender differences. Materials and methods. Interviewing schoolchildren of various ethnic backgrounds aged 15-18 years is carried out using the Toronto Alexithymic Scale (TAS-26). Results. Mong Caucasian adolescents from different regions of residence (Krasnoyarsk and Abakan, the Republic of Khakassia), the prevalence rates of alexithymia were comparable (30,0% and 34,2%). Between 1975 and 2005, 566patients (pts) with primary Hodgkin s lymphoma were treated at N.N. Blokhin Cancer Research Center. We analyzed an influence of treatment programs execution on long-term results based on data of treatment efficacy. Median follow-up period was 58 months (range 5 months — 25 years). Deviations of treatment program were registered in 28,6% of pts in cases of execution of full treatment program at Cancer Research Center. The main part of treatment program was performed at Cancer Research Center and the minority of chemotherapy courses was accomplished domiciliary (40,4% pts). In cases of pts examination, choice of therapy and start of treatment at Cancer Research Center, execution of the main part of course was accomplished domiciliary (63,5% pts).Even increase for more than 6 weeks of the only one interval significantly resulted to reduction of long-term treatment effect. We observed statistically significant decreased of long-term treatment effect in case of reduction the chemotherapy dimension (dose reduction) to 1/3 from predesigned regimen though after achievement of complete remission as well as cancellation of radiotherapy in combined treatment after achievement of complete remission. All deviations indicated above were revealed in 38,8% pts. An exact adherence to modern treatment programs for pts with Hodgkin's lymphoma facilitate to increase effectiveness of therapy.

Aim. To study the level of alexithymia and anxiety in adolescents depending on age and gender, to identify relationships. Materials and methods. A survey of clinically healthy adolescents was carried out using the Toronto alexithymic scale (TAS-26) and the scale of situational and personal anxiety Ch.D. Spielberger – Yu. L. Khanin. Results. Among practically healthy adolescents, over 60% have signs of alexithymia, and half of them are adolescents with a pronounced level of alexithymia. “Vulnerable” for alexithymia are adolescent girls, and by age - younger adolescents. For practically healthy students, the normal level of situational and personal anxiety is natural. With an increase in the symptoms of alexithymia, the presence of high situational and personal anxiety in adolescents is characteristic. September, 2021 marks the 175th anniversary of the opening of the Faculty Therapeutic Clinic (FTC) of the Imperial Moscow University, which is a successor of the First (1805) and Second (1820) Clinical Institutes with their therapeutic beds and chairs, and thus is the oldest therapeutic clinic in Russia. From 1930 the Clinic became part of the newly formed I.M. Sechenov First Moscow Medical Institute (now Sechenov University) and retains continuity to the present day. This publication is one of three research articles prepared by the staff of the Clinic for the anniversary and devoted to little-known pages of biography and scientific activities of the three heads of the FTC at the crucial stages of its history – A.I. Over, G.A. Zakharyin and the last prerevolutionary director – N.F. Golubov.

Aim. To study autonomic homeostasis and reactivity of erythrocyte membranes to bioregulators of the autonomic nervous system in adolescents with somatoform dysfunction of the autonomic nervous system to predict the course of the disease and assess adaptive capabilities. Materials and methods. Clinical, instrumental and laboratory examination of adolescents with somatoform dysfunction of the autonomic nervous system and healthy adolescents was carried out. Results. In adolescents with somatoform dysfunction of the autonomic nervous system, dysregulatory types of autonomic response were revealed at the level of erythrocyte membranes. Significant biophysical risk factors for the paroxysmal course of the disease have been identified: low peak and rate fluorescence values with acetylcholine, as well as low peak fluorescence values with adrenaline. Conclusion. Identification of autonomic dysregulation at the level of cell membranes in adolescents with somatoform dysfunction of the autonomic nervous system is an important factor determining the course of the disease, as well as the adaptive capabilities of the adolescent’s body. Establishment of significant membrane risk factors for a complicated course of the disease is necessary for a personalized approach to the implementation of therapeutic and prophylactic measures.

Цель. Изучить анамнез и физико-химические свойства эритроцитарных мембран у школьников 7-11 лет для установления риска развития синдрома вегетативной дисфункции у конкретного ребенка. Материалы и методы. Проводилось клинико-анамнестическое и лабораторное обследование школьников с синдромом вегетативной дисфункции и практически здоровых детей. Физико-химические свойства эритроцитарных мембран изучали методом флуоресцентной спектроскопии. Результаты. У детей с синдромом вегетативной дисфункции методом флуоресцентной спектроскопии выявлены структурные преобразования и изменения функций мембран эритроцитов дестабилизирующего характера. Существенными биофизическими факторами риска развития синдрома вегетативной дисфункции у детей у детей 7-11 лет оказались: низкая кальций-аккумулирующая способность мембран, высокие показатели мембраносвязанной воды и текучести поверхностных слоев мембраны, а также повышенный коэффициент асимметрии текучести липидного бислоя. Заключение. Выявление у конкретного ребенка значимых анамнестических и биофизических (на уровне мембран) факторов риска развития синдрома необходимо для включения его в группу риска по психосоматическим заболеваниям для динамического наблюдения и своевременного проведения профилактических мероприятий.

Aim. To establish the prevalence of myocarditis and primary (genetic) cardiomyopathies (CMP) among patients in a cardiology hospital.Material and methods. Medical records of 671 patients of the cardiology department were analyzed. The diagnosis at admission and at discharge was recorded. The diagnoses were divided into 7 following categories: hypertension, coronary artery disease, heart disease, idiopathic arrhythmias, cardiomyopathy, myocarditis and others. Types of myocarditis and cardiomyopathy, the presence of arrhythmias and heart failure were also recorded.Results. Myocarditis was diagnosed in 194 (28,9%) patients, cardiomyopathy — in 76 (11,3%) patients, combination of cardiomyopathy and myocarditis — in 26 (3,9%) patients. Myocarditis with the development of arrhythmia and heart dilatation prevailed as follows: 47,4 and 41,2%, respectively. The most numerous CMPs were left ventricular noncompaction (n=30), non-inflammatory dilated CMP (n=13), hypertrophic CMP (n=10) and arrhythmogenic CMP of the right ventricle (n=9). In the group with idiopathic arrhythmias, 64,3% of patients were diagnosed with myocarditis, and 19,4% — with cardiomyopathy.Conclusion. The prevalence of non-coronary myocardial diseases among patients in a cardiology hospital is high and amounts to 40,2%. The presence of arrhythmias, heart failure or dilated cardiomyopathy may be a manifestation of non-coronary myocardial diseases, and requires a comprehensive examination aimed, in particular, at ruling out or verifying the diagnosis of myocarditis.

Цель. Повысить эффективность специализированной персонифицированной медицинской помощи детскому слабослышащему населению республики Тыва с помощью программного модуля «Живой звук». Результаты. У слабослышащих детей при проведении реабилитационных мероприятий неречевыми и речевыми стимулами посредством программного модуля «Живой звук» получена положительная динамика слуха в зависимости от возраста, формы и степени тугоухости. Заключение. Использование полученных в работе результатов предполагает наличие экономического эффекта: сокращения времени обследования и улучшения качества диагностических и лечебных мероприятий среди детского слабослышащего населения республики Тыва.

The aim of this study was to investigate the prevalence of Internet addiction (IА) and the structure of consumed content among adolescents in Central Siberia.Materials and methods. From January to May 2019, 3,012 adolescents (45.8% of boys and 54.2% of girls) aged 12–18 years were examined (average age 14.5 ± 1.3 years). Younger adolescents (12–14 years old) accounted for 52.5%, and seniors (15–18 years old) accounted for 47.5%. Peculiarities of online behavior were evaluated according to Chen’s Internet Addiction Scale (CIAS) adapted by V.L. Malygin and K.A. Feklisov; a total CIAS score of ≥ 65 indicated Internet addiction. Gambling addiction was rated according to the Game Addiction Scale for Adolescents, and addiction to social networks was rated according to the Social Media Disorder Scale. The structure of the consumed content was estimated in the overall sample and in two age groups. The data obtained were processed by non-parametric statistical methods using the Statistica 12.0 software. Quantitative characteristics are presented as the median and the interquartile range Me (Q25 – Q75), binary signs are represented as a share (%) and the confidence interval. The significance of the differences (p) for quantitative indicators was evaluated by Mann – Whitney U-test and for binary characters by Pearson’s χ2 criterion. The differences between the groups were considered statistically significant at p ≤ 0.05.Results. The overall prevalence of IА was 6.9%, adaptive use of the Internet was observed in 49.4% of adolescents, non-adaptive use was registered in 43.6% of cases. The frequency of Internet addiction increases with age: from 6.0% in 12–14 year-old adolescents to 8.0% in 15–18 year-old adolescents (p = 0.0324). Content consumed by adolescents included gambling addiction (11%), addiction to social networks (8.0%), mixed IA (2.6%), and undifferentiated Internet addiction (2.8%). Younger adolescents are more often dependent on online games (12.2%), social networks (9.3%), and mixed IА (3.3%), while in older adolescents, undifferentiated IА is more often observed (39.5%).Conclusion. High level of IA prevalence in adolescents in Central Siberia confirms the relevance of this problem and indicates the need to develop preventive measures aimed at maintaining child and adolescent health.

Abstract. The COVID-19 pandemic necessitated the urgent regulation and renewal of financial and banking services in Azerbaijan in the era of global digitalization. A number of important steps have been taken in the country, which are highlighted in the present article. First and foremost, numerous adopted legal acts, measures taken to increase the volume of non-cash payments and minimize the amount of cash payments have been analysed in the present article. It has been emphasized that strengthening of the role of banks in business financing also stimulates the economic growth as a result of the application of opportunities created by the digitalization of banks, legal entities and population. The importance to carry out reforms in the field of payments, to develop the legal framework, to strengthen institutional capacity has been examined. Next, the use of information technology and the enhancement of digital literacy have been identified as crucial issues. Moreover, digitalization is the main goal of improving risk management in banks and is defined as a strategic approach for the long term. The development of information technology and the acceleration of digital transformation in the international arena have contributed to the formation of modern and different approaches in the financial system, as well as in the operating part of banks. Furthermore, a new prudential system for responding to new challenges has been developed and the existing risk management tools based on international experience have been applied. The widespread use of mobile services over the Internet to facilitate consumer access to these services also plays a significant role in reducing costs and wasted time by creating additional capabilities. Finally, the article shows how the use of modern technological channels has increased the ability to collect and analyse more extensive and necessary information about current and potential customers. Given several nonnegative matrices with a single pattern of allocation among their zero/nonzero elements, the average matrix should have the same pattern as well. This is the first tenet of the pattern-multiplicative average (PMA) concept, while the second one suggests the multiplicative nature of averaging. The concept of PMA was motivated in a number of application fields, of which we consider the matrix population models and illustrate solving the PMA problem with several sets of model matrices calibrated in particular botanic case studies. The patterns of those matrices are typically nontrivial (they contain both zero and nonzero elements), the PMA problem thus having no exact solution for a fundamental reason (an overdetermined system of algebraic equations). Therefore, searching for the approximate solution reduces to a constrained minimization problem for the approximation error, the loss function in optimization terms. We consider two alternative types of the loss function and present a general algorithm of searching the optimal solution: basin-hopping global search, then local descents by the method of conjugate gradients or that of penalty functions. Theoretical disadvantages and practical limitations of both loss functions are discussed and illustrated with a number of practical examples.

Background. The progressive increase of the number of schoolchildren with adaptation disorders and low level of health in recent years determines the need to assess the characteristics of the psychosomatic status in children and adolescents with various types of somatic pathology, including diseases of ENT organs, accompanied by nasal breathing disorders and hypoxia. It is also necessary to determine the severity of stress in children, for timely prevention and correction of these disorders.Aim: to assess the level of stress in schoolchildren with nasal breathing disorders against the background of nasal diseases and concomitant psychosomatic disorders.Materials and methods. 481 schoolchildren aged 12–17 (boys and girls) were examined. Nasal breathing disorders were assessed by a comprehensive ENT examination, including rhinoscopy, radiography of the sinuses, and olfactometry. The presence and severity of stress were determined by the questionnaire “Social factors and stress”.Results. We found that the high level of stress in children with nasal breathing disorders on the background of ENT pathology is more often associated with the high frequency and severity of psychosomatic symptoms: the presence of dorsalgia, asthenic syndrome, chronic headache and frequent episodes of abdominal pain. Moderately and significantly increased levels of stress associated with the presence of children with hypertension, asthenic syndrome, frequent abdominal pain, frequent pain in the cervical spine, panic disorders.Conclusions. Thus, the presence of certain psychosomatic complaints in children with nasal breathing disorders is directly related to the level of stress, which is important to take into account when planning preventive and corrective measures aimed at increasing the adaptive capacity and stress resistance of children. The studies illustrate the need to assess the psychosomatic status and the level of stress in schoolchildren with nasal breathing disorders taking into account their existing comorbid disorders of the psychosomatic spectrum.

Free arginine (Arg) content was observed to multiply when the level of nitrogen (N) nutrition was high, and additional fertilization with boron (B) potentiated this effect. Owing to this feature, conifers can be suggested for use as bioproducers of Arg. Concentrations of Arg in relation to N and B fertilization needed to be better understood. The effect of soil fertilization with N and B on accumulation of these elements and free Arg in one-year-old needles of 16-year-old Scots pine (Pinus sylvestris L.) trees was determined in this study. Plantations were fertilized with doses of N from 0 to 1000 kg ha−1 and B from 0 to 6 kg ha−1. Fertilization with 3 kg ha−1 B at N doses of 200–500 kg ha−1 stimulated the accumulation of N in needles of up to 3.1–3.6% dry weight (DW). The level of Arg in needles increased from 74.7 to 175.9 μmol g−1 DW at these levels of N and B.

Despite the fact that small fiber neuropathy (SFN) has already been studied for 30 years, it remains one of the most mysterious diseases that are extremely difficult to diagnose and cure. The prevalence of SFN is 52.95 per 100,000 population; diabetes mellitus is considered to be the most frequent cause of this disease. Chronic neuropathic pain syndrome, temperature sensation abnormalities, and vegetative disorders develop as a result of damage to fine myelinated А- and unmyelinated C fibers. The disease mainly spreads in the upward direction: from feet to the proximal body parts and arms; primary axonal damage takes place. Although SFN is believed to be one of the most benign neuropathy types as it does not affect the large sensory and motor fibers, it significantly decreases patients quality of life.

Subject of Research.The paper deals with questions of operation safety for continuous welded rail track under dynamic loads. We study the gap value formed in the rail bar in view of its possible break under the conditions of dynamic loads caused by the moving train and varying temperatures. Occurring strains are linked to a variable stiffness of the rail track base (ballast). Methods. We studied longitudinal oscillations in a semi-infinite rod under dynamic loads and in the process of thermal expansion of the finite length rod on the elastic-damped foundation. Oscillatory processes in a semi-infinite rod on an elastic foundation was modeled using the extended equation of longitudinal vibrations, taking into account the reaction of the foundation, performing the role of a damper. To determine the state of the model after the damping of oscillations we used the time limiting transition. In solving the problem of thermal expansion of the finite length rod on the elastic foundation we used equilibrium equation for small fragment, which takes into account the thermal load and the elastic force of both the rod and the foundation. Main Results. We have calculated the maximum value of the dynamic displacement of the rod end at different values of parameters. We have obtained the distributions of stresses and displacements along the rod length for different stiffness distribution in the elastic foundation. In the event of accidental break in the rail, the gap value after the damping of oscillations is set at the level of 5-7 cm. We have shown that the amplitude of these oscillations can reach 10-12 cm. The availability of limited stiffness change zones in the rail base causes no significant deformation of rail bars, but can lead to the growth of the gap in the rail bar break. Practical Relevance. The proposed model can be used in the calculation and design of continuous welded rail tracks.

Background. In the Soviet period, people came to work in the Far North of the Krasnoyarsk Territory. Now they are retired. Remaining retired in the North is not economically feasible. Resettlement of unemployed elderly people to the more favorable southern climatic zones of central Siberia is connected with a demographic situation. For families 60+ years old, the “From the Extreme North to the South” program was created. The article presents the results of studies on the psychoemotional state of migrants 60+ years old. Objectives . To study the psychoemotional state of patients 60+ years old with chronic cerebrovascular insufficiency when adapting from the North to the South. Material and methods. 98 migrants from the North to the South were examined. These were people with a chronic insufficiency of brain blood circulation at the age of 60+ years old. Patients were tested utilizing the G. Eysenck and A.S. Zigmond, R.P. Snaith. Hospital Anxiety and Depression Scale. Results . Various types of personality were revealed: extroverts – 60.2% (59 people), introverts – 39.8% (39 people). Emotionally stable migrants 60+ years old – 25.5% (28 people). Emotionally unstable – 74.5% (70 people). The sociability combination with impulsiveness was characteristic with a high level of emotional instability (17.27 ± 0.26 points). At very high level (19.5 ± 0.31 points), low self-esteem and an emotional block were revealed. Conclusions . 1. Emotionally stable migrants 60+ years old readapt fairly. 2. High and very high levels of neuroticism demonstrate the formation of anxiety disorders and restriction of adaptive opportunities in migrants 60+ years old. Treatment and prophylactic actions are provided to them.

Toll-like receptors (TLR) are involved into innate immune recognition of microorganisms, including pathogenic bacteria. The aim of this work is to assess relative levels of TLR2 and TLR9 expression in cervical epithelial cells of women with inflammatory diseases of pelvic organs. The total group of patients consisted of 47 persons, including 20 women with urogenital infections, and 27 women comprised a control group. Using real-time PCR, we identified the opportunistic pathogens and gene expression levels of TLR2, TLR9 in the cervix epithelial cells. It was shown that expression of TLR9 in the group with urogenital infections was 13.7-fold higher than in control group. We have revealed that a significant increase of TLR9 expression in the mucosal epithelium from cervical canal correlated with detection of infectious pathogens in the samples.

Research objective: Morphological study of tissue necrosis stages in experimental organ-preserving tuberculosis pharmacotherapy using Quercetin and Polyvinylpyrrolidone (QP). Background and methods: 32 laboratory mice of C57BL/6JLacSto strain were used in the experiment. The animals were divided into five groups, six to seven mice in each: group 1- Mycobacterium tuberculosis (MBT) uninfected mice; group 2- MBT infected mice; group 3- MBT infected and treated with antituberculosis preparation (ATP); group 4- MBT infected and QP treated; group 5- MBT infected and treated with ATP and QP. The mice were infected through caudal vein injection with MTB H37Rv strain. The preparation QP, which belongs to the capillary-stabilizing-remedy group, was used for the research. The ATP were izoniazid and streptomycin. Results: QP produced a strict delineation of caseous necrosis from the unaffected parts of the connective tissue with fibrosis in the center and a large number of Langerhans cells, which was not observed in the control groups without QP. The combination of QP and ATP had more pronounced effects. In MBT-infected mice, where QP was not used, unlike the group where QP was used, adipose dystrophy of hepatocytes was observed. Thus, the hepatoprotective effect of QP against TB can be suggested. Conclusion: QP produces a clear delineation of caseous necrosis from an uninfected tissue by connective-tissue formation, and by forming fibrotic tissue in the center of epithelioid cells that prevents further TB dissemination by enhancing TB pharmacotherapy.

Aim: to characterize the markers of endothelial dysfunction and the effect of antihypertensive drugs on them in patients with chronic urate tubulointestinal nephritis (UTIN) and articular gout. Subjects and methods. The study enrolled 81 patients aged 39 to 59 years with gout and urate nephropathy. All the patients were diagnosed as having grade 1-2 arterial hypertension. A lower glomerular filtration rate (GFR) was noted in 49.9%; microalbuminuria (MAU) and elevated serum endothelin-1 (ET-1) levels were recorded in all the patients. Combination antihypertensive therapy based on the use of angiotensin-converting enzyme (ACE) inhibitors and/or an angiotensin II receptor blocker (ARB) in combination with calcium channel blockers was performed during 12 months. The time course of changes in blood pressure, the parameters of target organ lesion, and the markers of endothelial dysfunction were monitored. Results. Before the study, all the examinees were found to have MAU and increased serum ET-1, which are regarded simultaneously as signs of renal lesion and as markers of endothelial function. A combination of an ACE inhibitor or an ARB could diminish albuminuria and reduce ET-1 concentrations, lower systolic and diastolic blood pressure significantly, and increase GFR. Conclusion. The patients with articular gout and chronic UTIN are observed to have signs of endothelial dysfunction eliminated by combination antihypertensive therapy.
