Background. 6-mercaptopurine (6-MP) is a drug that is included in the treatment protocols for children and adults with acute lymphoblastic leukemias/lymphomas (ALL/LBL). It is known that individual differences in 6-MP tolerance can be explained by the TPMT and NUDT15 polymorphisms.Aim. To determine 6-MP toxicity profile in adult patients with Ph-negative ALL/LBL treated by ALL-2016 protocol, depending on the TPMT and NUDT15 polymorphisms.Materials and methods. The study included 54 adult patients with Ph-negative ALL/LBL (40 male and 14 female). The median age was 31 (18-51) years. T-ALL/LBL was diagnosed in 29 patients, B-ALL/LBL - in 22, acute leukemia with a mixed immunophenotype - in 3. All patients received treatment according to the multicenter study ALL-2016 (ClinicalTrials.gov, NCT03462095). polymorphisms in NUDT15 (*2, *3) and TPMT (*2, *3A, *3B, *3C) genes were detected using the allele-specific real-time polymerase chain reaction. Genomic DNA was extracted from patients peripheral blood samples. On the induction and consolidation therapy by the protocol, the received and proper 6-MP doses were calculated for all the patients. Drug toxicity was evaluated based on clinical and laboratory data.Results. TPMT and NUDT15 polymorphisms were detected in 11 (20 %) patients, more often in B-ALL - 7 (32 %) of 22 (p 0.05).Conclusion. There were no differences in the received dose of 6-MP and the incidence of toxicity in adult patients between Ph-negative ALL/LBL with or without TPMT and NUDT15 polymorphisms treated according to ALL-2016 protocol (p >0.05). further studies including evaluation of 6-MP metabolites concentrations are required for a more complete understanding of the metabolism of this drug.

Introduction. CDKN2A deletion is a frequent cytogenetic abnormality in acute lymphoblastic leukemia (ALL), ranging from 18 to 46 %, associating with Т-cell ALL, high WBC counts, splenomegaly, lymphadenopathy. In pediatric group of patient’s CDKN2A deletion was associated with poor event-free survival. The prognostic impact of CDKN2A deletion in adult ALL patients appear controversial.The aim of this study was to evaluate the prognostic impact of the CDKN2A deletion in adult patients with acute lymphoblastic leukemia, which were treated by RALL-2009.Materials and methods. We present the results of the CDKN2A deletion in 110 adult patients with newly diagnosed Ph-negative (Ph‒) ALL, which were treated by RALL-2009 (NCT01193933) since June 2009 till September 2016. Patients characteristics: the median age was 26 years (range 15–54), 65 (59 %) of the 110 patients had a B-precursor phenotype, 42 (38 %) had a T-cell phenotype, 3 (2.7 %) patients – biphenotypical ALL. Interphase fluorescence in situ hybridization (FISH) was performed for detection CDKN2A deletion, MLL, с-MYC rearrangement, TP53 deletion, t (1;19) (q23; p13.3)/TCF3-PBX1, t (12;21) (p13.2; q22.1)/ETV6-RUNX1 and iAMP21. The median follow-up was 31 months (0.5 to 80 months).Results. The prevalence of the CDKN2A deletion in all studied population was 24.3 % (27 cases). Our study demonstrated that CDKN2A deletion had no significant association with age, sex and blast cells immunophenotype. The analysis for T-ALL has detected that CDKN2A deletion was strongly associated with high WBC count (the median is 86 × 109/L; p = 0.006) and with high lactate dehydrogenase level (the median is 3062 IU; p = 0.0004). But in BCP-ALL cases similar correlation was not found. CDKN2A deletion didn’t have statistically significant impact on outcome of patients. OS for patients with BCP-ALL with and without deletion was 85 and 76 % (p = 0.35); DFS was 92 and 65 % (p = 0.07), respectively. OS for T-ALL patients with and without deletion was 90 and 80 % (p = 0.63); DFS was 100 and 82 % (p = 0.24), respectively.Conclusion: CDKN2A deletion is not adverse prognostic factor in adult ALL patients treated according to protocol RALL-2009.

This article describes the main issues and problems that arise in the process of implementing long-term home care and maintaining the quality of life of elderly people who are in conditions of forced self-isolation. The aim of the work is to identify factors that have a negative impact on the provision of a decent quality of life by long-term home care professionals for the elderly in conditions of self-isolation. The research material was official statistics and the results of scientific research by Russian scientists. The main research methods used were the method of theoretical scientific analysis, the method of systematization, interpretation, and the method of generalization of the activities of medical and social services that provide long-term care for the elderly at home. The analysis showed that the forced self-isolation of the elderly, caused by the presence of acute respiratory viral infections, leads to a number of negative consequences, which together can worsen both the physical and mental state of a person, which ultimately will lead to a decrease in the quality of life. The result of this study was to identify such problems that exist in the practice of long-term care for older people who are in conditions of forced self-isolation, such as: the problem of quality control of services, imperfection of methods and technologies used to work with older people to ensure a decent quality of life in conditions of forced self-isolation and limited social contacts. Based on the results of the study, some measures have been proposed that will resolve the existing problems in a relatively short time.

Внутрішньоутробне обмеження росту плода (IUGR, FGR) означає стан, при якому плід не в змозі досягти генетично визначеного потенційного розміру за різних умов. Таке функціональне визначення має на меті відокремити популяцію плодів, перинатальні наслідки яких можна модифікувати (запобігти антенатальнiй загибелі або народженню дитини з тяжкою інвалідністю). Таким чином, завданням клініциста та експерта з ультразвукової діагностики (плодово-материнської медицини) є виявлення плодів з обмеженням росту плода (ОРП), які мають найвищий ризик антенатальної загибелі внаслідок «несприятливого внутрішньоутробного середовища» та потребують окремого алгоритму контролю й ятрогенного втручання для дострокового розродження. Також необхідно чітко виокремити плоди з малою гестацiйною вагою (МГВ) для зниження ятрогенних ризиків. Найактуальнішою є розробка такого діагностично-клінічного алгоритму в клініці екстрагенітальної патології, оскільки ризик ОРП значно зростає в жінок із тяжкою патологією, насамперед із системним червоним вовчаком, артеріальною гіпертензією, «ціанотичними» вадами серця матері (неоперованими), хворобою Аерзи, онкологічною патологією, виявленою під час вагітності, та в разі проведення поліхіміотерапії. У статті проаналiзовано данi cвiтових та власних дослiджень із вищезазначеного питання. Систематизовано дані про види, причини, термiни виникнення та особливостi показникiв залежно вiд форм ОРП. Основною причиною ОРП є недостатнє забезпечення плода киснем і поживними речовинам, порушення системи доставки кисню або пошкодження структур плацентарного бар'єру внаслідок захворювань матері. При ОРП виникає багато ускладнень, що потребують вчасної діагностики та проведення адекватних втручань для запобігання перинатальної захворюваності та смертності. Розроблено алгоритм дiагностики ОРП на пiдставi клiнiчного перебiгу вагiтностi, даних лабораторного, ультразвукового, допплерометричного дослiджень та створено акушерську стратегiю при ОРП. У разі виявлення цієї патології мультидисциплінарна команда має скласти індивідуальний план спостереження за станом плода, оцінити ефективність терапії основного захворювання вагітної з екстрагенітальною патологією. Як програма максимум розглядається збільшення терміну гестації при розродженні, мінімізація ризиків захворюваності та смертності новонароджених. Короткотривала мета — виявити плід із підозрою на ОРП/МГВ і підтвердити або виключити ОРП. Середньотривала мета — створити алгоритм частоти та набору спостережень з урахуванням екстрагенітальної патології та акушерських ускладнень вагітної. Довготривала мета — оптимізувати термін пологів, щоб мінімізувати гіпоксемію і забезпечити максимально досяжний термін гестації для розродження й поліпшення результату для матері. Автори заявляють про відсутність конфлікту інтересів.

Вроджені вади серця (ВВС) є найпоширенішими в структурі всіх аномалій розвитку та часто спричиняють летальність плодів і новонароджених першого місяця життя. Завдяки успіхам кардіохірургії, сьогодні є можливість виконувати складні реконструктивні операції при ВВС, які раніше вважалися неоперабельними. За таких умов основним завданням в організації допомоги дітям із ВВС є своєчасна діагностика і надання кваліфікованої допомоги в кардіохірургічній клініці. Більшість ВВС діагностують під час скринінгового обстеження вагітних із фетальною ехокардіографією. Але не всі ВВС можна виявити внутрішньоутробно, у таких випадках надання оптимальної кардіохірургічної допомоги новонародженим може бути несвоєчасним. Мета — обґрунтувати перинатальну тактику при ВВС у новонароджених до переведення в кардіохірургічний центр. Пацієнти та методи. Усім вагітним виконано повну ехокардіографію плода зі встановленням топічного діагнозу, функціональної оцінки шлуночків серця, а також оцінки гемодинамічних порушень у фетоплацентарній системі, виявлення супутньої патології. Найбільш оптимальними термінами для обстеження плода обрали 20–22 і 30–32 тижні вагітності. Усього за період 2015–2019 рр. у ДУ «Інститут педіатрії, акушерства і гінекології імені академіка О.М. Лук'янової НАМН України» за розробленим нами алгоритмом обстежено 8 457 вагітних. Пренатально у 144 із них діагностовано гемодинамічно значущі ВВС. З метою правильної оцінки стану дитини при народженні ретельно вивчено пренатальний анамнез і дані ультразвукового дослідження плода. Дитині проведено повне клініко-інструментальне обстеження. Пренатальне і постнатальне ультразвукове дослідження проведено на апаратах AcusonX300 (Siemens, Німеччина), MyLabTwice (Esaote, Італія). Результати та висновки. Враховуючи дані літератури та аналіз власних досліджень, розроблено пре- і постнатальну тактику ведення ВВС у плода та новонародженого для надання своєчасної спеціалізованої допомоги цій категорії хворих. Це дасть змогу знизити рівень летальності, провести профілактику ускладнень, подовжити тривалість і поліпшити якість життя дітей. Дослідження виконані відповідно до принципів Гельсінської Декларації. Протокол дослідження ухвалений Локальним етичним комітетом зазначеної в роботі установи. На проведення досліджень отримано поінформовану згоду батьків дітей. Автори заявляють про відсутність конфлікту інтересів.

Aim. To analyze the efficiency and reproducibility of the ALL-2009 protocol within the Russian prospective multicenter study based on different principles of cytostatic effects (non-intensive, but continuous cytotoxic treatment and a small number of allogeneic hematopoietic stem cells). Subjects and methods. The ALL-2009 (NCT01193933) study conducted in April 2009 to December 2016 included 194 patients (95 males and 99 females) aged 15 to 55 years (median age 28 years) with Ph-negative B-cell acute lymphoblastic leukemia (ALL). There was early pre-B-cell ALL in 54 patients, common ALL in 101, pre-B ALL in 39, initial leukocytosis in 9.4·109/l (0.4-899.0), lactate dehydrogenase in 901 IU (31-13 059), an initial central nervous system lesion in 17 (8.7%), mediastinal injury in 3 (1.5%), and splenomegaly in 111 (57.2%). The results of standard cytogenetic analysis are known in 113 (60.4%) patients. Normal karyotypes were detected in 49 (54.5%) out of the patients; t(4;11) in 9 (5.4%), t(1;19) in 2 (1.2%), and other karyotypic abnormalities in 53 (46.9%). Thirteen (7.8%) patients underwent allogeneic hematopoietic stem cell transplantation in first complete remission (CR); their proportion did not differ in the federal and regional centers. Results. The frequency of CR achievement was the same in the federal and regional centers and generally amounted to 87.5%. Early (8.8%) and CR (9.6%) mortality rates remained high despite the low aggressiveness of cytotoxic action, necessitating the improvement of auxiliary treatment. The five-year overall survival (OS) rates vary considerably in the federal and regional centers (72.6 and 43.8%), the relapse-free survival (RFS) (70.2 and 53.4%) and recurrence risk (23.1 and 36.5%) are comparable. This suggests that the non-intensive, but continuous exposure principle built in the ALL-2009 protocol makes it possible to reproduce the envisaged treatment program and to achieve satisfactory results. Conclusion. The ALL-2009 protocol allows both the federal and regional centers to obtain the long-term results comparable with those of current foreign studies: OS (54.2%), RFS (56.5%); and relapse risk (35.4%). Multivariate analysis has identified age (over 30 years), initial leukocytosis (30·109/l and more) and t(4;11) among the main clinical prognostic factors. Gene mutation detection evaluated in a small number of patients (8/36) is not a poor prognostic sign. There is a need for further investigations with centralized evaluation of the mutation status of leukemic cells and the clearance of minimal residual disease.
