Abstract

Unter Qualitatsmanagement versteht man die umfassende Anwendung von strukturierten und festgeschriebenen Masnahmen zur Betrachtung eines Tatigkeits-oder Organisationsfeldes mit dem Ziel, zuvor definierte Ergebnisse moglichst weitgehend und konstant zu erreichen. Qualitatsmanagement zielt also darauf ab, im betreffenden Tatigkeitsbereich eine zuvor beschriebene Leistung mit einer moglichst hohen und konstanten Effizienz und Qualitat erbringen zu konnen. Qualitat ist dabei nach DIN ISO 8402 definiert als die Gesamtheit von Eigenschaften und Merkmalen eines Produktes oder einer Dienstleistung bezuglich ihrer Eignung festgelegte oder vorausgesetzte Erfordernisse zu erfullen (Baumgartl u. Forterre 1996).

Abstract

1 It can be stated in regard to the metabolism of nomifensine that the metabolites and also the starting compound are present in alkaline urine almost completely in conjugated form. 2 The metabolites M I-M III represent about 30% of the radioactivity excreted with urine. Two-thirds are N-glucuronides and one-third O-glucuronides. 3 Nearly the entire remainder of the renally eliminated radioactivity in nomifensine which is present in conjugated form as N-glucuronide. 4 Thus over 90% of the metabolites formed in man and excreted via urine have been identified.

Abstract

Some years ago we found that α-halogenoorganolithium compounds (carbenoids) previously postulated as transient intermediates in organolithium-initiated α-eliminations can be obtained in a stable form. The present paper is a review of the methods for their preparation and of their reactivity. They possess both nucleophilic and electrophilic properties, which can be utilized singly or combained for the synthesis of substances of very different types. Their thermolability, which is considerably reduced (with only one known exception) by the solvent tetrahydrofuran, is due to electrophilic secondary reactions, in which carbenes evidently do not occur as intermediates. A mechanism is proposed which fits the experimental data for various carbenoid reactions.

Abstract

Die Moglichkeiten zum Einsatz von Verdunnungsautomaten in der Flammenphotometrie von Na, K und Ca wurden untersucht und der Zeitaufwand und die Genauigkeit bei Verwendung dieser Gerate mit der herkommlichen Arbeitsweise verglichen. Ein weiterer Automationsschritt fur die Messung stellt der Einsatz eines Digitalvoltmeters mit gekoppeltem Drucker dar, der das Ergebnis in Konzentrationseinheiten ausdruckt. Auserdem wird auf die Fehlermoglichkeiten bei der Verwendung eines Probenwechslers hingewiesen. Schlieslich wird die Integration mehrerer Gerate zu einem halbautomatisch ablaufenden Prozes bei der Sulfonamidbestimmung gezeigt und die dabei erhaltenen Ergebnisse im Vergleich zur manuellen Methode diskutiert.

Abstract

1. Following the dermal application of the carbon-14 labelled broad spectrum antimycotic 6-cyclohexyl-1-hydroxy-4-methyl-2(1H)-pyridone, 2-aminoethanol salt (ciclopiroxolamine, Hoe 296, Batrafen) in the form of a 1% aqueous cream to healthy human dorsal skin (penetration time: 6 h; occlusive dressing for 5 h), percutaneous absorption accounted on average for 1.3% of the dose applied. Excretion occurred via the kidney, with biological half-lives of 1.7 h. As can be seen from penetration studies of cadaverous skin, the horny layer contained the highest concentrations, with values of 2300-4500 microgram/cm3. The levels determined in the corium were still above the minimum inhibitory concentrations. These concentrations were already obtained at the first test stage (1.5 h after application) and did not change virtually at all over the longer penetration period. According to studies using histoautoradiography, ciclopirox can penetrate the skin via the epidermis and the hair follicles. When ciclopirox-14C-olamine aqueous cream was spread on the surface of fingernails, the radioactive-labelled compound penetrated right through the nail. The percutaneous absorption in dogs was higher, at 5-15% of the dose, than it was in humans. 2. After vaginal application (1 mg/kg) of ciclopirox-14C-olamine in the form of a 1% aqueous cream to bitches, between 42 and 97% of the dose (depending on the animal) was recovered in the urine and faeces, the remainder having penetrated into the tampon used to close the vagina. 3. Ciclopirox is excreted by dogs and man in the urine, primarily as a glucuronide. In humans another glucuronide with properties similar to those of the original substance was detected. Two conjugated, relatively non-polar metabolites were also present in small amounts. The metabolite patterns after oral and dermal application were similar. The binding of ciclopirox to serum proteins in humans was 96 +/- 2% in a concentration range of 0.01-11.0 microgram/ml. 4. Placental transfer was low in the rats studied. Though there was good absorption by the mother animal, the radioactivity in the foetal tissues was always lower than that of the maternal blood.

Abstract

Studies on absorption and distribution in both skin and organism, as well as on elimination and biotransformation were performed in rats, pigs, and rabbits following topical application of the corticoid prednisolone-17-ethyl carbonate-21-propionate (prednicarbate; test name: Hoe 777), which had been labeled with 14C in position 4 for this purpose. After allowing the 0.25% greasy ointment to take effect for 6 hours, about half of the dose applied to rats and three quarters of that applied to pigs could be removed from the application area (rejection rate). Measurable blood levels, which could only be continuously determined in rats, initially occurred 0.5 to three hours after application; they remained very low throughout the experiment and finally reached concentrations between 0.007 and 0.012 micrograms equivalents of prednicarbate per ml 24 hours after administration. The excretion rate was 5 to 6% of the dose applied to rats and maximally 1.9% in pigs. In rats, the absorption after dermal application on an average amounted to 14% in healthy skin and to 22% in abraded skin. In pigs, we received values of about 1% and 4%, respectively. Our studies demonstrated that the horny layer serves both as a reservoir and as a barrier for prednicarbate. Experimental impairment of the barrier function resulted in an increased amount of active substance penetrating through the skin. During its passage through the organism, prednicarbate is almost completely metabolized, its metabolic fate resembling largely the biotransformation pathway of prednisolone. Among the numerous biotransformation products (about 20-30), 20 beta F-20-dihydroprednisolone and 6 beta-hydroxy-20 beta F-20-dihydroprednisolone were identified as the quantitatively most important metabolites in rats. On the basis of our results, we assume that at the site of action - the skin - highly potent prednicarbate becomes more and more changed by biotransformation on its way into the organism; this process results in formation of numerous metabolites, probably accounting for the very low systemic effect of the compound.

1 The absorption, excretion, distribution and metabolism of a new antidepressant agent, nomifensine, was examined in rats, dogs and monkeys. After oral administration of 14C-nomifensine 1 mg/kg body weight, the compound was absorbed rapidly, and as shown in rats and dogs, virtually completely. Nomifensine was predominantly found in a conjugated form in the plasma of rats, dogs and monkeys. Nomifensine was bound to serum proteins (approximately 60%) and the degree of binding was not species specific. 2 The maximum levels of total radioactivity (unchanged drug and metabolites) in the blood were 0.50 ± 0.14 μg equiv./ml (n = 5) in dogs and 0.34 ± 0.17 μg equiv./ml (n = 6) in monkeys. These leves were markedly higher than those in rats, with values of 0.034 ± 0.014 μg equiv./ml at 0.5-0.75 h and 0.048 ± 0.018 μg equiv./ml at 2-8 h after dosing (second peak; n = 19). 3 The ratio of radioactivity in organs and tissues relative to plasma was small and similar to that in blood. The highest concentrations of radioactivity were measured in the excretory organs; the lowest concentrations were found in the brain. When multiple doses of 14C-nomifensine were administered accumulation was minimal. 4 After single oral doses, the plasma levels of radioactivity were higher than those of blood. Accumulation of unconjugated nomifensine in plasma could not be demonstrated in dog, and multiple doses of the drug had no influence on the elimination of either conjugated or unconjugated nomifensine. 5 Rats excreted about 50% and dogs excreted about 70% of the compound in urine, predominantly as conjugates. The metabolites found in man were also found in monkey, these being 4'-hydroxy nomifensine, 4'-hydroxy-3'-methoxy nomifensine and 3'-hydroxy-4'-methoxy nomifensine.

Plasma levels of nomifensine-N-glucuronide are up to 100-fold higher than those of nomifensine, obviously because of a smaller volume of distribution. As nomifensine-N-glucuronide is extremely unstable and cleaved to nomifensine, determinations of nomifensine are easily falsified. It is therefore recommended only to determine the sum of nomifensine and its N-glucuronide (total nomifensine) in clinical trials. Kinetics of total nomifensine can best be described by the open two-compartment model: Maximum plasma levels are obtained 1-2 hours postadministration; mean elimination half-life is 2 hours. Excretion is almost entirely by the kidneys, with approximately 88% of an oral dose excreted within 24 hours.

Abstract

Background: The UN Convention on the Rights of Persons with Disabilities, and the reformed guardianship law in Germany, require that persons with a disability, including people with dementia in Alzheimer's disease (PwAD), are supported in making self-determined decisions. This support is achieved through communication. While content-related communication is a deficit of PwAD, relational aspects of communication are a resource. Research in supported decision-making (SDM) has investigated the effectiveness of different content-related support strategies for PwAD but has only succeeded in improving understanding, which, although one criterion of capacity to consent, is not sufficient to ensure overall capacity to consent. The aim of the 'spatial intervention study' of the DECIDE project is to examine an innovative resource-oriented SDM approach that focuses on relational aspects. We hypothesise that talking to PwAD in their familiar home setting (as opposed to a clinical setting) will reduce the complexity of the decision-making process and enhance overall capacity to consent., Methods: People with a suspected or confirmed diagnosis of dementia in Alzheimer's disease will be recruited from two memory clinics (N = 80). We will use a randomised crossover design to investigate the intervention effect of the decision-making place on capacity to consent. Besides reasoning capacity, which is part of overall capacity to consent and will be the primary outcome, various secondary outcomes (e.g., other aspects of capacity to consent, subjective task complexity, decisional conflict) and suspected moderating or mediating variables (e.g., meaning of home, demographic characteristics) will be assessed., Discussion: The results of the study will be used to develop a new SDM strategy that is based on relational resources for PwAD. Abstract

Background: Everybody has the right to decide whether to receive specific medical treatment or not and to provide their free, prior and informed consent to do so. As dementia progresses, people with Alzheimer's dementia (PwAD) can lose their capacity to provide informed consent to complex medical treatment. When the capacity to consent is lost, the autonomy of the affected person can only be guaranteed when an interpretable and valid advance directive exists. Advance directives are not yet common in Germany, and their validity is often questionable. Once the dementia diagnosis has been made, it is assumed to be too late to write an advance directive. One approach used to support the completion of advance directives is 'Respecting Choices' ® -an internationally recognised, evidence-based model of Advance Care Planning (ACP), which, until now, has not been evaluated for the target group of PwAD. This study's aims include (a) to investigate the proportion of valid advance directives in a memory clinic population of persons with suspected AD, (b) to determine the predictors of valid advance directives, and (c) to examine whether the offer of ACP can increase the proportion of valid advance directives in PwAD., Method: We intend to recruit at least N = 250 participants from two memory clinics in 50 consecutive weeks. Of these, the first 25 weeks constitute the baseline phase (no offer of ACP), the following 25 weeks constitute the intervention phase (offer of ACP). The existence and validity of an advance directive will be assessed twice (before and after the memory clinic appointment). Moreover, potential predictors of valid advance directives are assessed., Discussion: The results of this study will enhance the development of consent procedures for advance directives of PwAD based on the ACP/Respecting Choices (R) approach. Therefore, this project contributes towards increasing the autonomy and inclusion of PwAD and the widespread acceptance of valid advance directives in PwAD. Abstract

1. Following the dermal application of the carbon-14 labelled broad spectrum antimycotic 6-cyclohexyl-1-hydroxy-4-methyl-2(1H)-pyridone, 2-aminoethanol salt (ciclopiroxolamine, Hoe 296, Batrafen) in the form of a 1% aqueous cream to healthy human dorsal skin (penetration time: 6 h; occlusive dressing for 5 h), percutaneous absorption accounted on average for 1.3% of the dose applied. Excretion occurred via the kidney, with biological half-lives of 1.7 h. As can be seen from penetration studies of cadaverous skin, the horny layer contained the highest concentrations, with values of 2300-4500 microgram/cm3. The levels determined in the corium were still above the minimum inhibitory concentrations. These concentrations were already obtained at the first test stage (1.5 h after application) and did not change virtually at all over the longer penetration period. According to studies using histoautoradiography, ciclopirox can penetrate the skin via the epidermis and the hair follicles. When ciclopirox-14C-olamine aqueous cream was spread on the surface of fingernails, the radioactive-labelled compound penetrated right through the nail. The percutaneous absorption in dogs was higher, at 5-15% of the dose, than it was in humans. 2. After vaginal application (1 mg/kg) of ciclopirox-14C-olamine in the form of a 1% aqueous cream to bitches, between 42 and 97% of the dose (depending on the animal) was recovered in the urine and faeces, the remainder having penetrated into the tampon used to close the vagina. 3. Ciclopirox is excreted by dogs and man in the urine, primarily as a glucuronide. In humans another glucuronide with properties similar to those of the original substance was detected. Two conjugated, relatively non-polar metabolites were also present in small amounts. The metabolite patterns after oral and dermal application were similar. The binding of ciclopirox to serum proteins in humans was 96 +/- 2% in a concentration range of 0.01-11.0 microgram/ml. 4. Placental transfer was low in the rats studied. Though there was good absorption by the mother animal, the radioactivity in the foetal tissues was always lower than that of the maternal blood.

Studies on absorption and distribution in both skin and organism, as well as on elimination and biotransformation were performed in rats, pigs, and rabbits following topical application of the corticoid prednisolone-17-ethyl carbonate-21-propionate (prednicarbate; test name: Hoe 777), which had been labeled with 14C in position 4 for this purpose. After allowing the 0.25% greasy ointment to take effect for 6 hours, about half of the dose applied to rats and three quarters of that applied to pigs could be removed from the application area (rejection rate). Measurable blood levels, which could only be continuously determined in rats, initially occurred 0.5 to three hours after application; they remained very low throughout the experiment and finally reached concentrations between 0.007 and 0.012 micrograms equivalents of prednicarbate per ml 24 hours after administration. The excretion rate was 5 to 6% of the dose applied to rats and maximally 1.9% in pigs. In rats, the absorption after dermal application on an average amounted to 14% in healthy skin and to 22% in abraded skin. In pigs, we received values of about 1% and 4%, respectively. 1 It can be stated in regard to the metabolism of nomifensine that the metabolites and also the starting compound are present in alkaline urine almost completely in conjugated form. 2 The metabolites M I-M III represent about 30% of the radioactivity excreted with urine. Two-thirds are N-glucuronides and one-third O-glucuronides. 3 Nearly the entire remainder of the renally eliminated radioactivity in nomifensine which is present in conjugated form as N-glucuronide. 4 Thus over 90% of the metabolites formed in man and excreted via urine have been identified.
