8. Nocturnal haemodialysis is associated with a reduced occurrence of low triiodothyronine serum levels in haemodialysed patients

Abstract

Background End-stage renal disease (ESRD) is associated with a broad spectrum of morphological and functional thyroid disorders. Recent studies have shown that low free triiodothyronine (fT3) levels are related to inflammatory status and endothelial activation in ESRD patients on haemodialysis (HD). Limited data exist about a possible relationship between dialysis regimen, namely long nocturnal haemodialysis (LNHD), and thyroid function parameters. The aim of this study was to evaluate the relationship between dialysis regimen and thyroid function, and consequently with the main patient outcomes. Methods To this purpose, we performed a retrospective, single-centre cohort study including 220 incident chronic HD patients treated during an 8-year period (from January 2010 to December 2017). The main clinical and haematochemical parameters, including thyroid function, were evaluated and related to the main patient outcomes. Results Patients with low fT3 levels (3.05 ng/mL) (P 3.05 ng/mL was significantly higher in LNHD patients than in patients treated with diurnal dialysis. Conclusions Our data suggest that the application of alternative dialysis regimens, also reducing the frequency of low T3, could ameliorate outcomes and therefore reduce the incidence of cardiovascular events in HD patients.

Abstract

Acute aortic syndromes are life-threatening medical conditions that include classic acute aortic dissection (AAD), aortic intramural hematoma, penetrating aortic ulcer, and even aortic pseudoaneurysm and traumatic aortic injury. The European Society of Cardiology has designed a multiparametric diagnostic algorithm to provide stepwise diagnosis. All patients with AAD should receive aggressive medical therapy to control blood pressure and heart rate. Urgent surgical repair is recommended for type A AAD. Uncomplicated type B AAD requires aggressive medical therapy. In contrast thoracic endovascular repair is recommended for complicated type B. AAD should be considered a lifelong disease that affects the entire aorta.

Abstract

Background: The identification of relationship between genes and emotional distress mediated by personality traits among high level athletes may help to implement specific clinical psychology programs. Findings suggest that 5-HTTLPR genetic polymorphisms may be associated with Harm Avoidance (HA). The present research is aimed at evaluating the relationship between HA, the serotonin transporter’s (5-HTTLPR) polymorphisms and cognitive anxiety (CA) in high level athletes during championship. Methods: 133 athletes completed the Temperament and Character Inventory (TCI) test. Sport Performance Psychological Inventory (IPPS-48) was used to assess athlete’s cognitive and emotional aspects. Genotypes at the 5-HTTLPR polymorphisms were identified through a polymerase chain reaction. Results: An association has been found between the 5-HTTLPR s/s genotype and both cognitive anxiety (p0.05) and HA (p0.05). Significant correlations were proved between HA and CA (p0.01), Emotional Arousal Control (p0.001) and Concentration Disruption (p0.05). HA has been proven to mediate the association between the 5-HTTLPR polymorphisms and CA symptoms (p0.05). Conclusions: Such findings clearly suggest, there is a substantial interaction between 5-HTTLPR polymorphisms, HA and competition-related stress that predicts adverse psychological outcomes in high level athletes. Abstract

Aims Mortality and left ventricular mass (LVM) recovery/regression after aortic valve replacement in patients with prosthesis-patient mismatch (PPM) is controversial. This study evaluated the impact of different values of indexed effective orifice area (EOAi) in male patients on mortality and indexed LVM (ILVM) recovery/regression. Method The study recruited 376 male patients with and without PPM after aortic valve replacement with different EOAi cut-off values. Results At EOAi 0.85 cm2/m2 or less, 295 patients had PPM (78.5%). ILVM recovery occurred in 60.5% of no-PPM patients versus 46.1% of patients with PPM (P = 0.003), and ILVM regression was 35 versus 25% (P < 0.001). Time for ILVM regression was shorter in no-PPM group. At EOAi 0.75 cm2/m2 or less, 201 patients had PPM (53.4%). ILVM recovery occurred in 55.4% of no-PPM patients versus 45.2% of patients with PPM (P = 0.06), regression was 32 versus 29% (P = 0.09). Time for ILVM regression was similar between groups. Regardless the cut-off value for PPM definition, mortality was similar. Conclusion LVM recovery/regression, but not mortality, was different at different EOAi. The cut-off value at EOAi 0.75 cm2/m2 or less guaranteed a more balanced patient distribution between groups and the best compromise between specificity and sensitivity

Abstract

OBJECTIVE: An increased cardiovascular mortality and morbidity has been widely reported in patients with atrial fibrillation (AF). In this study, a subanalysis of the AntiThrombotic Agents Atrial Fibrillation (ATA‐AF) is performed with the aim to evaluate estimated glomerular filtration rate (eGFR) as an independent prognostic marker of cardiovascular mortality and morbidity in patients with AF. METHODS AND RESULTS: The ATA‐AF study enrolled 7148 patients with AF, in 360 Italian centers. The eGFR was calculated from data reported in patient notes or hospital database. This post‐hoc analysis included 1097 AF patients with eGFR data available and 1‐year clinical follow‐up. 20. DELIRIUM IN ICU PATIENTS FOLLOWING CARDIAC SURGERY: AN OBSERVATIONAL STUDY

Abstract

Aims and objectives. To observe the clinical and structural factors that can be associated with the post-operative onset of delirium in patients who have undergone heart surgery. Background. Several risk factors could contribute to the development of delirium, such as the use of some sedative drugs and a patient’s history with certain types of acute chronic disease. However, in the literature, there is little knowledge about the association between delirium in patients who have undergone cardio-surgical intervention and their clinical and environmental predictors. Design. We used an observational design. Methods. We enrolled 89 hospitalised patients in the ICU. Patients were first evaluated using the Richmond Agitation Sedation Scale (RASS) and subsequently using the Confusion Assessment Method for the ICU (CAM-ICU). A linear model of regression was used to identify the predictors of delirium in patients. Results. The patients had an average age of 89 years (SD=6.9), were predominantly male (84.3%) and were mostly married (79.8%). The majority of patients had been subjected to bypass (80.9%), while 19.1% had undergone the intervention of endo prosthesis. The logistic regression model showed that patient age, the duration of VAM, continuous exposure to artificial light and the presence of sleep disorders were predictors of the onset of delirium. Conclusion. This study further confirms that clinical aspects such as insomnia and one’s circadian rhythm as well as structural elements such as exposure to artificial light are variables that should be monitored in order to prevent and treat the onset of severe post-operative delirium. Relevance to clinical practice. Identifying the possible factors that predispose a patient to the onset of delirium during intensive therapy following cardiac surgery, it is fundamental to implement interventions to prevent this syndrome.

This report documents the user input and output data requirements for the FEMNAS finite element Navier-Stokes code for real-gas simulations of external aerodynamics flowfields. This code was developed for the configuration aerodynamics branch of NASA ARC, under SBIR Phase 2 contract NAS2-124568 by Computational Mechanics Corporation (COMCO). This report is in two volumes. Volume 1 contains the theory for the derived finite element algorithm and describes the test cases used to validate the computer program described in the Volume 2 user guide.

Abstract

A generalized curvilinear coordinate Taylor weak statement implicit finite element algorithm is developed for the two-dimensional and axisymmetric compressible Navier-Stokes equations for ideal and reacting gases. For accurate hypersonic simulation, air is modeled as a mixture of five perfect gases, i.e., molecular and atomic oxygen and nitrogen as well as nitric oxide. The associated pressure is then determined via Newton solution of the classical chemical equilibrium equation system. The directional semidiscretization is achieved using an optimal metric data Galerkin finite element weak statement, on a developed 'companion conservation law system', permitting classical test and trial space definitions. Utilizing an implicit Runge-Kutta scheme, the terminal algorithm is then nonlinearly stable, and second-order accurate in space and time on arbitrary curvilinear coordinates. Subsequently, a matrix tensor product factorization procedure permits an efficient numerical linear algebra handling for large Courant numbers. For ideal- and real-gas hypersonic flows, the algorithm generates essentially nonoscillatory numerical solutions in the presence of strong detached shocks and boundary layer-inviscid flow interactions.

32. Conservative-variable average states for equilibrium gas multi-dimensional fluxes

Abstract

Modern split component evaluations of the flux vector Jacobians are thoroughly analyzed for equilibrium-gas average-state determinations. It is shown that all such derivations satisfy a fundamental eigenvalue consistency theorem. A conservative-variable average state is then developed for arbitrary equilibrium-gas equations of state and curvilinear-coordinate fluxes. Original expressions for eigenvalues, sound speed, Mach number, and eigenvectors are then determined for a general average Jacobian, and it is shown that the average eigenvalues, Mach number, and eigenvectors may not coincide with their classical pointwise counterparts. A general equilibrium-gas equation of state is then discussed for conservative-variable computational fluid dynamics (CFD) Euler formulations. The associated derivations lead to unique compatibility relations that constrain the pressure Jacobian derivatives. Thereafter, alternative forms for the pressure variation and average sound speed are developed in terms of two average pressure Jacobian derivatives. Significantly, no additional degree of freedom exists in the determination of these two average partial derivatives of pressure. Therefore, they are simultaneously computed exactly without any auxiliary relation, hence without any geometric solution projection or arbitrary scale factors. Several alternative formulations are then compared and key differences highlighted with emphasis on the determination of the pressure variation and average sound speed. The relevant underlying assumptions are identified, including some subtle approximations that are inherently employed in published average-state procedures. Finally, a representative test case is discussed for which an intrinsically exact average state is determined. This exact state is then compared with the predictions of recent methods, and their inherent approximations are appropriately quantified.

A finite element CFD algorithm is developed for Euler and Navier-Stokes aerodynamic applications. For the linear basis, the resultant approximation is at least second-order-accurate in time and space for synergistic use of three procedures: (1) a Taylor weak statement, which provides for derivation of companion conservation law systems with embedded dispersion-error control mechanisms; (2) a stiffly stable second-order-accurate implicit Rosenbrock-Runge-Kutta temporal algorithm; and (3) a matrix tensor product factorization that permits efficient numerical linear algebra handling of the terminal large-matrix statement. Thorough analyses are presented regarding well-posed boundary conditions for inviscid and viscous flow specifications. Numerical solutions are generated and compared for critical evaluation of quasi-one- and two-dimensional Euler and Navier-Stokes benchmark test problems.

Abstract

A generalized flux-vector splitting implicit Galerkin finite-element algorithm for the Euler equations in curvilinear coordinates for ideal and reacting gases is derived. For an arbitrary equation of state, the curvilinear-coordinate flux vector is split in kinematic and kinetic components, and the associated jacobian matrix eigenvalues explicitly depend on the metric data. After directional semidiscretization, the terminal ordinary differential-equation system is solved via a nonlinearly stable implicit Runge-Kutta scheme in concert with an accurate tensor matrix product factorization. The results for selected two-dimensional supersonic and axisymmetric hypersonic flows validate the algorithm and verify its robustness for curvilinear-coordinate computations. The evolution towards a steady state is achieved for large Courant numbers without indication of numerical instabilities.

Abstract

Thoracic aortic emergencies (TAE) - fixured thoracic aortic aneurysms (TAA), acute type b dissection - are associated with high risk of mortality and morbility. Endovasular surgery (ES) of TAE has been considered as an alternative procedure. From March 2001 to July 2003, 26 patients with TAE were admitted to our department and submitted to ES: 8 pts (30.7%) for fixured TAA, 18 pts (69.3%) for acute type b dissection evolving to rupture. AngioCT scan was diagnostic in all pts. A stented-graft was placed by femoral arteriotomy. TAA and type B pts were submitted to a multiple stent-graft procedure to cover the entire descending thoracic aorta (2.2 ± 1.2 grafts/pt). There were no perioperative deaths and open surgical conversions. The intraoperative angiography and Ctscan at the discharge showed no significant endoleaks and good seating of the aortic dissection. Average ICU and hospital stay was 1.7 ± 0.8 and 5.9 ± 3.0 days. Follow up ranged between 4 and 23 months, included clinical examination and serial CT scan at 3, 6 and 12 months, and every 6 months successively. An 84 years old TAA pt died of septic shock from pneumonia 78 days after endovascular surgery. Endovascular surgery is a safe technique for treating TAE with encouraging early and mid-term results. This therapeutic approach seem to be particulary efficacious in high risk subsets, as those represented by elderly patients.

Abstract

A persistent distal false lumen perfusion--residual type A aortic dissection--is found in 50% to 100% of patients following ascending aorta replacement. Complications of persistent or newly developed dissection of the distal aorta are often fatal and require reoperation, leading to a higher morbidity and mortality rate for these patients than with primary surgery. Thus, many efforts have been done to treat this aortic disease by an endovascular approach. Unfortunately, patency of the supra-aortic vessel still remains an unresolved issue. The present report describes the case of a patient with a residual type A aortic dissection persisting after ascending aorta replacement, successfully treated by implantation of a new endovascular device, the Djumbodis® Dissection System, a bare stent with sufficiently large mesh to join the dissected layers without occluding main vital branches such as supra-aortic arteries.
