The purpose of this study is to evaluate the effects of the authors' oral health care program on the stimulated whole salivary flow rate and buffer capacity before and after a 6-month intervention. The authors conducted the intervention study among 25 participants with diabetes. The salivary flow rate and buffer capacity were evaluated before and after this intervention. Overall, the results showed a significant increase in salivary flow rate and no significant change in buffer capacity. Also, it was likely that salivary flow rate significantly increased among patients with more than 20 remaining teeth and patients with well-controlled diabetes. The findings suggest that this program for type 2 diabetes led to an increase in the stimulated whole salivary flow rate. [ABSTRACT FROM AUTHOR]

Immunoregulatory function of peripheral blood monocytes was studied in patients with hepatocellular carcinoma (HCC) and liver cirrhosis (LC), by assaying interleukin I (IL- I) and prostaglandin E2 (PGE2) in the culture supernatant of lipopolysaccharide-stimulated monocytes. IL-1 activity of the monocyte culture supernatant without indomethacin was decreased in patients with HCC and LC, compared with that of controls. The activity was lower in patients with HCC than that in those with LC. The PGE2 content of the culture supernatant of monocytes from patients with LC and HCC was increased, compared to normal controls. To avoid the effect of PGE2 on the IL-1 assay, we cultured the monocytes with addition of indomethacin and assayed IL-1 activity in the culture supernatant. As a result, monocyte IL-1 production was increased in patients with HCC and LC, compared with normal controls. The decrease in IL-1 activity of the supernatant without indomethacin of patients with LC and HCC was considered to be due to increased secretion of PGE2 by the monocytes. Therefore, monocytes from patients with HCC and LC had an increased capacity of secreting both IL-1 and PGE2 over normal controls, but the effect of the suppressor function (PGE2 secretion) dominated in these patients. A 28-year-old Japanese woman positive for TSH receptor antibody and anti-nuclear antibody complained of difficulty seeing nearby objects, severe throbbing retro-orbital pain, diplopia, blepharoptosis and upward gaze palsy when she became hypothyroid during treatment with 30 mg methylmercaptoimidazole for Graves' hyperthyroidism. Brain magnetic resonance imaging revealed slightly swollen bilateral inferior rectus muscles, suggesting the external ophthalmoplegia due to the muscle pathology commonly encountered in Graves' disease. The retro-orbital pain was associated with marked accommodation failure and the pupillary abnormalities. The left and/or right eye showed intermittent, asymmetric and fluctuating mydriasis, being unresponsive to ordinary light but slowly responsive to strong sunlight and slowly responsive in a dark room. During the 5-year period, mydriasis was observed 9 times on both sides, 11 times only on the right side and 4 times only on the left side. Internal ophthalmoplegia with tonic pupils and accommodation failure affecting both the pupillary sphincter muscle and ciliary muscle due to damage to the parasympathetic outflow to these muscles was suggested. Autoimmune mechanism and/or the mechanism underlying channelopathy affecting the ciliary ganglion or short ciliary nerves might be responsible for this fluctuating complication. This very rare panophthalmopathy affecting both external and internal muscles occurred when the patient was suffering from iatrogenic hypothyroidism during the 30 mg methylmercaptimidazole treatment for Graves' disease.

We introduce dynamically warping grids for adaptive liquid simulation. Our primary contributions are a strategy for dynamically deforming regular grids over the course of a simulation and a method for efficiently utilizing these deforming grids for liquid simulation. Prior work has shown that unstructured grids are very effective for adaptive fluid simulations. However, unstructured grids often lead to complicated implementations and a poor cache hit rate due to inconsistent memory access. Regular grids, on the other hand, provide a fast, fixed memory access pattern and straightforward implementation. Our method combines the advantages of both: we leverage the simplicity of regular grids while still achieving practical and controllable spatial adaptivity. We demonstrate that our method enables adaptive simulations that are fast, flexible, and robust to null-space issues. At the same time, our method is simple to implement and takes advantage of existing highly-tuned algorithms.

Agricultural systems using advanced information and communication (ICT) technology can produce high-quality crops in a stable environment while decreasing the need for manual labor. The system collects a wide variety of environmental data and provides the precise cultivation control needed to produce high value-added crops; however, there are the problems of packet transmission errors in wireless sensor networks or system failure due to having the equipment in a hot and humid environment. In this paper, we propose a reliable wireless control system for hydroponic tomato cultivation using the 400 MHz wireless band and the IEEE 802.15.6 standard. The 400 MHz band, which is lower than the 2.4 GHz band, has good obstacle diffraction, and zero-data-loss communication is realized using the guaranteed time-slot method supported by the IEEE 802.15.6 standard. In addition, this system has fault tolerance and a self-healing function to recover from faults such as packet transmission failures due to deterioration of the wireless communication quality. In our basic experiments, the 400 MHz band wireless communication was not affected by the plants' growth, and the packet error rate was less than that of the 2.4 GHz band. In summary, we achieved a real-time hydroponic liquid supply control with no data loss by applying a 400 MHz band WSN to hydroponic tomato cultivation.

Background: To better understand the picture of premature death from oral cancer, we estimated years of life lost (YLL) and average years of life lost (AYLL) of this cancer for the years 1995 and 2005 in Japan., Methods: We obtained the mortality data for 5-year age groups from the Vital Statistics of Japan for the years 1995 and 2005, an interval of 10 years. Age-standardized rates (ASRs) per 100,000 persons were calculated for each subset of oral cancer. We estimated YLL and AYLL according to life tables in Japan to reflect premature mortality., Results: For both men and women combined, 4099 and 5679 deaths due to oral cancer were recorded for the years 1995 and 2005. In men, cancer of tongue, oropharynx, and hypopharynx were the most frequently observed anatomic sites. We observed a total of 51,339.1 years of life lost in 1995 and 68,630.4 years in 2005, corresponding to an overall AYLL for all oral cancer deaths combined of 17.2 and 16.5 years earlier than life expectancy, respectively. The greatest AYLL was seen for deaths from nasopharyngeal cancer, with AYLL of 21.1 years in 1995 and 20.3 years in 2005. In women, cancer of the tongue and gum were the most affected anatomic sites. Total numbers of YLL were 18,884.8 years in 1995 and 24,765.7 in 2005, corresponding to an overall AYLL of 16.9 and 16.2 years earlier than life expectancy. The greatest AYLL was seen for deaths from nasopharyngeal cancer, with AYLL of 22.4 years and 20.4 years in 1995 and 2005, respectively., Conclusion: The present study shows that cancer of pharynx, tongue, and gum were the most frequent oral cancers in both sexes in both 1995 and 2005, and responsible for a remarkable number of years of life expectancy lost. In Japan, one of the duties of the long-term care insurance system is the prevention of oral function degradation. Although various exercise programs for oral function have been developed and practiced in Japan, to date, no study has reported their effects. In the present study, we examined the effects of an exercise program on oral functions in healthy elderly people. Healthy elderly people participated in the study and were randomly divided into intervention and control groups (39 subjects/group). The exercise program consisted of four exercises: exercise for expression muscles, tongue, salivary glands, and swallowing. Before and after a six-month intervention period, we evaluated changes in oral functions, namely bite force, swallowing ability, and salivary flow rate. Fifty-four subjects completed the study protocol: 26 and 28 subjects from intervention and control groups, respectively. A significant improvement in all oral functions, including bite force, swallowing ability, as well as unstimulated and stimulated salivary flow rate, was observed in the intervention group after six months, whereas no improvement was observed in the control group. Further, among the intervention group, a significant improvement in oral functions was observed in 17 subjects with 20 or more remaining teeth, whereas no improvement was observed in the other nine with less than 20 teeth. Our results suggest that oral functions significantly improve after conducting a six-month exercise program for oral function. Further, this improvement appears to be influenced by the number of remaining teeth.

In order to prepare a new prevention program under the long term care insurance from the fiscal year 2006, we have organized an oral health care program for the elderly in a town of Kagoshima prefecture. We conducted a series of oral health education, i.e., instruction on brushing and flossing, and oral function exercises. Before and after the three months program, we evaluated the unstimulated and stimulated salivary flow rate, the counts of S. mutans, the counts of repetitive saliva swallowing test (RSST), bilateral bite force and the number of permanent teeth and artificial teeth. According to the results, a statistically significant improvement was observed in RSST and the unstimulated and stimulated salivary flow rate, but not in the S. mutans level and bite force. The present research suggests the effectiveness of the oral health care program for the aged. A larger sized and longer period intervention will be necessary in order to validate our findings.

Under the coming amendment of long term care insurance (LTCI), the preventive activities will be more strengthened in order to ameliorate the quality of life (QOL) levels of the slightly frail elderly and then to rationalize the LTCI expenditures. The functional training of the musculoskeletal system is regarded as especially important. In order to validate its effectiveness and availability in a rural setting, we organized a physical fitness program for the slightly frail elderly in an isolated island of Kagoshima prefecture. According to the results of our preliminary study, statistically significant improvement was observed for some items of the physical fitness level, but not for life style, activities of daily living (ADL), instrumental activities of daily living (IADL) levels or Frenchay Activities Index. In addition to the relatively small number of participants, it is thought that physical fitness training alone cannot increase the elderly activity volume. Further studies are necessary to clarify the usefulness of physical training on the ADL/IADL level of the elderly.
