Abstract

In this work, the spatiotemporal pressure field of MHz-focused ultrasound is measured using a background-oriented schlieren technique combined with fast checkerboard demodulation and vector tomography (VT-BOS). Hydrophones have been commonly employed to directly measure the local pressure in underwater ultrasound. However, their limitations include that they disturb the acoustic field and affect the measured pressure through the spatial averaging effect. To overcome such limitations, we propose VT-BOS as a non-contact technique for acoustic field measurements using only a background image and a camera. In our experiments, VT-BOS measures focused acoustic fields with a focal width of 1.0 mm and a frequency of 4.55 MHz, capturing traveling, reflected, and standing waves. We discuss three key features of this approach:(1) the temporal evolution of pressure measured by VT-BOS and hydrophones, (2) the differences in computational cost and spatial resolution between VT-BOS and other techniques, and (3) the measurement range of VT-BOS. The results demonstrate that VT-BOS successfully quantifies spatiotemporal acoustic fields and can estimate the hydrophones' spatial averaging effect over a finite area. VT-BOS measures pressure fields of several MPa with high spatiotemporal resolution, requiring less computational and measurement time. It is used to measure pressure amplitudes from 0.4 to 6.4 MPa, with the potential to extend the range to 0.3 ~ 201.6 MPa by adjusting the background-to-target distance. Abstract

The PHENIX experiment measured the centrality dependence of two-pion Bose-Einstein correlation functions in $\sqrt{s_{_{NN}}}=200$~GeV Au$+$Au collisions at the Relativistic Heavy Ion Collider at Brookhaven National Laboratory. The data are well represented by L\'evy-stable source distributions. The extracted source parameters are the correlation-strength parameter $\lambda$, the L\'evy index of stability $\alpha$, and the L\'evy-scale parameter $R$ as a function of transverse mass $m_T$ and centrality. The $\lambda(m_T)$ parameter is constant at larger values of $m_T$, but decreases as $m_T$ decreases. The L\'evy scale parameter $R(m_T)$ decreases with $m_T$ and exhibits proportionality to the length scale of the nuclear overlap region. The L\'evy exponent $\alpha(m_T)$ is independent of $m_T$ within uncertainties in each investigated centrality bin, but shows a clear centrality dependence. At all centralities, the L\'evy exponent $\alpha$ is significantly different from that of Gaussian ($\alpha=2$) or Cauchy ($\alpha=1$) source distributions. Comparisons to the predictions of Monte-Carlo simulations of resonance-decay chains show that in all but the most peripheral centrality class (50%-60%), the obtained results are inconsistent with the measurements, unless a significant reduction of the in-medium mass of the $\eta'$ meson is included. In each centrality class, the best value of the in-medium $\eta'$ mass is compared to the mass of the $\eta$ meson, as well as to several theoretical predictions that consider restoration of $U_A(1)$ symmetry in hot hadronic matter., Comment: 401 authors from 75 institutions, 20 pages, 15 figures, 2 tables. v1 is version submitted to Physical Review C. Abstract

High-momentum two-particle correlations are a useful tool for studying jet-quenching effects in the quark-gluon plasma. Angular correlations between neutral-pion triggers and charged hadrons with transverse momenta in the range 4--12~GeV/$c$ and 0.5--7~GeV/$c$, respectively, have been measured by the PHENIX experiment in 2014 for Au$+$Au collisions at $\sqrt{s_{_{NN}}}=200$~GeV. Suppression is observed in the yield of high-momentum jet fragments opposite the trigger particle, which indicates jet suppression stemming from in-medium partonic energy loss, while enhancement is observed for low-momentum particles. The ratio and differences between the yield in Au$+$Au collisions and $p$$+$$p$ collisions, $I_{AA}$ and $\Delta_{AA}$, as a function of the trigger-hadron azimuthal separation, $\Delta\phi$, are measured for the first time at the Relativistic Heavy Ion Collider. Abstract

BACKGROUND: Primary Health Care (PHC) is essential for effective, efficient, and more equitable health systems for all people, including those living with HIV/AIDS. This study evaluated the impact of the exposure to one of the largest community-based PHC programs in the world, the Brazilian Family Health Strategy (FHS), on AIDS incidence and mortality. METHODS AND FINDINGS: A retrospective cohort study carried out in Brazil from January 1, 2007 to December 31, 2015. We conducted an impact evaluation using a cohort of 3,435,068 ≥13 years low-income individuals who were members of the 100 Million Brazilians Cohort, linked to AIDS diagnoses and deaths registries. We evaluated the impact of FHS on AIDS incidence and mortality and compared outcomes between residents of municipalities with low or no FHS coverage (unexposed) with those in municipalities with 100% FHS coverage (exposed). We used multivariable Poisson regressions adjusted for all relevant municipal and individual-level demographic, socioeconomic, and contextual variables, and weighted with inverse probability of treatment weighting (IPTW). We also estimated the FHS impact by sex and age and performed a wide range of sensitivity and triangulation analyses; 100% FHS coverage was associated with lower AIDS incidence (rate ratio [RR]: 0.76, 95% CI: 0.68 to 0.84) and mortality (RR: 0.68, 95%CI: 0.56 to 0.82). FHS impact was similar between men and women, but was larger in people aged ≥35 years old both for incidence (RR: 0.62, 95% CI: 0.53 to 0.72) and mortality (RR: 0.56, 95% CI: 0.43 to 0.72). The absence of important confounding variables (e.g., sexual behavior) is a key limitation of this study. CONCLUSIONS: AIDS should be an avoidable outcome for most people living with HIV today and our study shows that FHS coverage could significantly reduce AIDS incidence and mortality among low-income populations in Brazil. Universal access to comprehensive healthcare through community-based PHC programs should be promoted to achieve the Sustainable Development Goals of ending AIDS by 2030.

Abstract

The blockchain ecosystem, particularly with the rise of Web3 and Non-Fungible Tokens (NFTs), has experienced a significant increase in users and applications. However, this expansion is challenged by the need to connect early adopters with a wider user base. A notable difficulty in this process is the complex interfaces of blockchain wallets, which can be daunting for those familiar with traditional payment methods. To address this issue, the category of "embedded wallets" has emerged as a promising solution. These wallets are seamlessly integrated into the front-end of decentralized applications (Dapps), simplifying the onboarding process for users and making access more widely available. However, our insights indicate that this simplification introduces a trade-off between ease of use and security. Embedded wallets lack transparency and auditability, leading to obscured transactions by the front end and a pronounced risk of fraud and phishing attacks. This paper proposes a new protocol to enhance the security of embedded wallets. Our VELLET protocol introduces a wallet verifier that can match the audit trail of embedded wallets on smart contracts, incorporating a process to verify authenticity and integrity. In the implementation architecture of the VELLET protocol, we suggest using the Text Record feature of the Ethereum Name Service (ENS), known as a decentralized domain name service, to serve as a repository for managing the audit trails of smart contracts. This approach has been demonstrated to reduce the necessity for new smart contract development and operational costs, proving cost-effective through a proof-of-concept. Abstract

We study a primal-dual (PD) reinforcement learning (RL) algorithm for online constrained Markov decision processes (CMDPs). Despite its widespread practical use, the existing theoretical literature on PD-RL algorithms for this problem only provides sublinear regret guarantees and fails to ensure convergence to optimal policies. In this paper, we introduce a novel policy gradient PD algorithm with uniform probably approximate correctness (Uniform-PAC) guarantees, simultaneously ensuring convergence to optimal policies, sublinear regret, and polynomial sample complexity for any target accuracy. Notably, this represents the first Uniform-PAC algorithm for the online CMDP problem. In addition to the theoretical guarantees, we empirically demonstrate in a simple CMDP that our algorithm converges to optimal policies, while baseline algorithms exhibit oscillatory performance and constraint violation.

It is conjectured that, on a non-trivial knot in the 3-sphere, no pair of Dehn surgeries along distinct slopes are purely cosmetic, that is, none of them yield 3-manifolds those are orientation-preservingly homeomorphic. In this paper, we show that alternating knots having reduced alternating diagram with the twist number at least 7, Montesinos knots of length at least 5, and alternating Montesinos knots of length at least 4 do not admit purely cosmetic surgeries. Abstract

The PHENIX experiment has performed a systematic study of identified charged-hadron ($\pi^\pm$, $K^\pm$, $p$, $\bar{p}$) production at midrapidity in $p$$+$Al, $^3$He$+$Au, Cu$+$Au collisions at $\sqrt{s_{_{NN}}}=200$ GeV and U$+$U collisions at $\sqrt{s_{_{NN}}}=193$ GeV. Identified charged-hadron invariant transverse-momentum ($p_T$) and transverse-mass ($m_T$) spectra are presented and interpreted in terms of radially expanding thermalized systems. The particle ratios of $K/\pi$ and $p/\pi$ have been measured in different centrality ranges of large (Cu$+$Au, U$+$U) and small ($p$$+$Al, $^3$He$+$Au) collision systems. The values of $K/\pi$ ratios measured in all considered collision systems were found to be consistent with those measured in $p$$+$$p$ collisions. However the values of $p/\pi$ ratios measured in large collision systems reach the values of $\approx0.6$, which is $\approx2$ times larger than in $p$$+$$p$ collisions. These results can be qualitatively understood in terms of the baryon enhancement expected from hadronization by recombination. Identified charged-hadron nuclear-modification factors ($R_{AB}$) are also presented. Enhancement of proton $R_{AB}$ values over meson $R_{AB}$ values was observed in central $^3$He$+$Au, Cu$+$Au, and U$+$U collisions. The proton $R_{AB}$ values measured in $p$$+$Al collision system were found to be consistent with $R_{AB}$ values of $\phi$, $\pi^\pm$, $K^\pm$, and $\pi^0$ mesons, which may indicate that the size of the system produced in $p$$+$Al collisions is too small for recombination to cause a noticeable increase in proton production., Comment: 480 authors from 78 institutions, 18 pages, 6 tables, 16 figures. v2 is version accepted for publication in Physical Review C. Abstract

This study presents a novel approach for reconstructing all stress components of the dynamic axisymmetric fields of a soft material using photoelastic tomography (PT) and a high-speed polarization camera. This study focuses on static and dynamic Hertzian contact as an example of transient stress field reconstructions. For the static Hertzian contact (a solid sphere pressed against a gel block), all stress components in the urethane gel, which has an elastic modulus of 47.4 kPa, were reconstructed by PT using the measured photoelastic parameters. The results were compared with theoretical solutions and showed good agreement. For the dynamic Hertzian contact (a sphere impacting gel), a high-speed polarization camera was used to reconstruct the transient stress field within the gel. PT was used to quantitatively measure the shear and axial stress waves and showed different propagation speeds on the substrate. The technique allowed the simultaneous measurement of stress fields ranging from $O(10^{-1})$ to $O(10^1)$ kPa during large deformations, demonstrating its accuracy in capturing rapidly changing stress tensor components in dynamic scenarios. 34. Correlations Between Subduction of Linear Oceanic Features and Arc Volcanism Volume Around the Pacific Basin

Arc volcanoes, created by magma generated from the dehydration of subducting slabs, show great variability in their sizes and along-arc spatial distributions. In this study, we address a fundamental question, namely, how do subduction zones and volcanic arcs respond to the subduction of ``atypical'' oceanic lithosphere. We investigate the correlation between the geographical location and volume of arc volcanoes and the subduction of linear oceanic features, including hotspot tracks, oceanic plateaus, volcanic ridges, mid-oceanic ridges, arc volcano chains, and fracture zones, around the Pacific basin. We use multidisciplinary and complementary data sets (topography and bathymetry, seismology and volcano morphometry), and design new analytical and data processing methods. We analyze 35 oceanic linear features. The subduction of three oceanic plateaus and five hotspot chains are clearly associated with volcanism increase, whereas four hotspot chains are related to volcanic gaps. We propose that the patterns of volcanism increase or decrease related to these oceanic features depend on the interplay between chemical (potentially enhancing melting) and thermo-mechanical (inhibiting melting) effects, and/or by the variations of the chemical signatures along hotspot chains. The subduction of volcanic ridges is generally associated with small increases in arc volcanism, which may be accounted for by the fact that these features are highly hydrated and therefore promote melt. The subduction of active mid-oceanic ridges is generally associated with slab windows and arc volcano gaps. No clear inference is found for the subduction of inactive arc ridges.

Abstract

For a hyperbolic knot in $S^3$, Dehn surgery along slope $r \in \Q \cup \{\frac10\}$ is {\em exceptional} if it results in a non-hyperbolic manifold. We say meridional surgery, $r = \frac10$, is {\em trivial} as it recovers the manifold $S^3$. We provide evidence in support of two conjectures. The first (inspired by a question of Professor Motegi) states that there are boundary slopes $b_1 < b_2$ such that all non-trivial exceptional surgeries occur, as rational numbers, in the interval $[b_1,b_2]$. We say a boundary slope is {\em NIT} if it is non-integral or toroidal. Second, when there are non-trivial exceptional surgeries, we conjecture there are NIT boundary slopes $b_1 \leq b_2$ so that the exceptional surgeries lie in $[\floor{b_1},\ceil{b_2}]$. Abstract

Living with extremely low-income is an important risk factor for HIV/AIDS and can be mitigated by conditional cash transfers. Using a cohort of 22.7 million low-income individuals during 9 years, we evaluated the effects of the worlds largest conditional cash transfer, the Programa Bolsa Família, on HIV/AIDS-related outcomes. Exposure to Programa Bolsa Família was associated with reduced AIDS incidence by 41% (RR:0.59; 95%CI:0.57-0.61), mortality by 39% (RR:0.61; 95%CI:0.57-0.64), and case fatality rates by 25% (RR:0.75; 95%CI:0.66-0.85) in the cohort, and Programa Bolsa Família effects were considerably stronger among individuals of extremely low-income [reduction of 55% for incidence (RR:0.45, 95% CI:0.42-0.47), 54% mortality (RR:0.46, 95% CI:0.42-0.49), and 37% case-fatality (RR:0.63, 95% CI:0.51 -0.76)], decreasing gradually until having no effect in individuals with higher incomes. Similar effects were observed on HIV notification. Programa Bolsa Família impact was also stronger among women and adolescents. Several sensitivity and triangulation analyses demonstrated the robustness of the results. Conditional cash transfers can significantly reduce AIDS morbidity and mortality in extremely vulnerable populations and should be considered an essential intervention to achieve AIDS-related sustainable development goals by 2030.

Study Design Biomechanical study. Purpose To investigate the mechanical characteristics of bone models created from medical images. Overview of Literature Recent advancements in three-dimensional (3D) printing technology have affected its application in surgery. However, a notable gap exists in the analyses of how patient’s dimorphism and variations in vertebral body anatomy influence the maximum insertional torque (MIT) and pullout strength (POS) of pedicle screws (PS) in osteoporotic vertebral bone models derived from medical images. Methods Male and female patients with computed tomography data were selected. Dimensions of the first thoracic (T1), fourth lumbar (L4), and fifth lumbar (L5) vertebrae were measured, and bone models consisting of the cancellous and cortical bones made from polyurethane foam were created. PS with diameters of 4.5 mm, 5.5 mm, and 6.5 mm were used. T1 PS were 25 mm long, and L4 and L5 PS were 40 mm long. The bone models were secured with cement, and the MIT was measured using a calibrated torque wrench. After MIT testing, the PS head was attached to the machine’s crosshead. POS was then calculated at a crosshead speed of 5 mm/min until failure. Results The L4 and L5 were notably larger in female bone models, whereas the T1 vertebra was larger in male bone models. Consequently, the MIT and POS for L4 and L5 were higher in female bone models across all PS diameters than in male bone models. Conversely, the MIT for T1 was higher in male bone models across all PS; however, no significant differences were observed in the POS values for T1 between sexes. Conclusions The mechanical properties of the proposed bone models can vary based on the vertebral structure and size. For accurate 3D surgical and mechanical simulations in the creation of custom-made medical devices, bone models must be constructed from patient-specific medical images.

Abstract

In Japan, pharmaceutical approval for the use of the da Vinci SP (dV SP) Surgical System in colorectal cancer surgery was obtained in September 2022. This system has an operating arm with three instruments and one scope to be manipulated through a single incision in colorectal cancer surgery. An 88-year-old female presented to our hospital with melena and was diagnosed with cStage IIa descending colon cancer (cT3N0M0). The patient underwent left hemicolectomy with the dV SP using an Access Port kit. The Access Port kit was inserted into a 3-cm vertical skin incision at the umbilicus. With only this surgical wound, mobilization from the rectum to the colon, and lymph node dissection were performed. Herein, we report the world's first descending colon cancer resection with the dV SP using an Access Port kit.

Abstract

Abstract Background Hemostatic devices are now frequently used in femoral artery punctures, and the Angio-Seal (Terumo, Tokyo, Japan) is one of the most commonly used devices for closure of the femoral artery because it provides rapid hemostasis. Although device failure rarely occurs, if the collagen falls into the femoral artery, it may lead to severe limb ischemia. Herein, we describe a case of a novel endovascular technique for the treatment of Angio-Seal arterial closure device failure. Case presentation The patient in Case 1 was a 75-year-old man with severe left limb claudication. We used a contralateral antegrade approach and used the Angio-Seal for hemostasis. However, the Angio-Seal collagen and footplate dropped and stopped at the bifurcation of the superficial femoral artery and deep femoral artery. The collagen with the footplate was caught with myocardial biotome forceps (MBF) and pulled into the external iliac artery (EIA). The distal common femoral artery (CFA) was punctured, and we delivered a 10.0- × 80-mm stent (SMART®; Cordis, USA) to the EIA from the ipsilateral sheath. The stent was deployed at the EIA and crushed the collagen. The patient in Case 2 was an 88-year-old man with rest pain in the right limb. The right CFA was punctured using an ipsilateral approach and the Angio-Seal was used for hemostasis. The Angio-Seal collagen with the footplate dropped into the bifurcation of the deep femoral artery. The collagen and footplate were caught with MBF and pulled up to the EIA. The right CFA was punctured and a 10.0- × 60-mm stent (SMART®; Cordis) was delivered from the ipsilateral sheath. The stent was deployed at the EIA and crushed the collagen with the footplate. Conclusions MBF were used to grasp the dislodged collagen with the anchor and cover it with a stent at the iliac artery. This may be a useful bailout technique for Angio-Seal dislodgement.

Abstract

Kubernetes is a container management system that has many automated functionalities. Those functionalities are managed by configuring objects and resources in the control plane. Since most objects change their state depending on other objects' states, a change propagates to other objects in a chain. As cluster availability is influenced by the time required for these cascading changes, it is essential to make the propagations measurable and shed light on the behavior of the Kubernetes control plane. However, it is not easy because each object constantly monitors other objects' status and acts autonomously in response to their changes to play its role. In this paper, we propose a measurement system that outputs objects' change logs published from the API server in the control plane and assists in analyzing the time of cascading changes between objects by utilizing the relationships among resources. With a practical change scenario, our system is confirmed that it can measure change propagation times within a cascading change. Also, measurements on the system itself showed it has a small CPU and memory footprint.

Abstract

An implantable ventricular assist device became smaller and has been used for small body size patients. However, it is still challenging to determine whether it is implantable for pediatric patients. The preoperative computed tomography virtual simulation provided spatial information among the pump, intracardiac structures, and extracardiac structures, which was very useful to assess the implantability for borderline children. We present a 9-year-old girl with dilated cardiomyopathy (weight, 20.0 kg; body surface area, 0.86 m2) and an 8-year-old boy with Marfan syndrome with severe ventricular dysfunction (weight, 25.0 kg; body surface area, 1.05 m2), who had preoperative virtual simulation and subsequent successful HeartMate 3 (Abbott Cardiovascular) implantation.
