Objectives: Vertical root fractures (VRF) can be a major challenge in root canal treatment due to bacterial biofilms' colonization of the fractures. The aim of this study was to evaluate the histological bacterial penetration and proliferation after sealing the simulated VRF with different intracanal sealing materials using confocal laser scanning microscopy (CLSM). Materials and methods: Sixty extracted human single-rooted premolars were used, and VRFs were created after root canal preparation. The canals were filled with either mineral trioxide aggregate (MTA), gutta-percha (GP) with AH plus sealer, or GP with bioceramic (BC) sealer. Enterococcus faecalis bacterial infection was induced, and bacterial colonization was assessed in the dentinal tubules and simulated VRF. Results: In the MTA group, there were significantly more dead bacteria in the dentinal tubules than live bacteria. The AH group had significantly more live bacteria than dead bacteria, and there were significantly more bacteria in the AH group compared to the BC and MTA groups. The study also revealed that bacteria can colonize the simulated VRF, from the part of the fracture to the outer surface. Clinical relevance: Understanding the pathological process following VRF and evaluating the ability of the VRF filling to prevent bacterial migration through the filled root end is crucial. Furthermore, quantifying the exact routes of microbial colonization, at the filling-dentine interfaces, and into the dentinal tubules, can provide valuable information to clinicians for selecting the appropriate root canal filling material to minimize bacterial colonization and improve treatment outcomes.

Objectives: The aim of this study was to assess the effect of the imaging modality on the evaluation of the outcome of modern surgical endodontic treatments, based on a systematic review of the literature. Methods: Strict inclusion criteria were adopted in order to identify studies that assessed the outcome of surgical endodontic treatments. Treatment success rates were pooled. The effect of the imaging modality used for the outcome assessment, and the methodological quality of the studies (based on the risk of bias (ROB)), were evaluated. Results: Nineteen articles were included. The success rates assessed by periapical (PA) radiography were significantly higher than when cases were evaluated by cone beam computed tomography (CBCT; 90 and 35% respectively). This difference was mainly due to a significant proportion of cases that were assessed by CBCT as uncertain healing (48%) compared to only 4% using PA. The success rates ranged between 86 and 92% in low ROB studies, and between 19–100% in high ROB studies. Conclusions: Outcome assessment based on CBCT may lead to significantly lower estimates of rate of success, and higher rates of uncertain healing, thus presenting a dilemma in the decision-making following surgical endodontic treatment. The success rates of studies with lower methodological quality are more variable than for high quality studies.

Introduction: Discoloration of anterior teeth can result in cosmetic impairment in young children. The walking bleach technique stands out because of its esthetic results with minor side effects. Little information is available regarding the influence of various irrigation solutions on peroxide penetration. The aim of this study is to evaluate the influence of different irrigation protocols on peroxide penetration into dentinal tubules using confocal laser scanning microscopy (CLSM). Study design: Cavity preparations were made in 50 extracted permanent premolars. The teeth went through different irrigation sequences: A. control B. saline C. EDTA, NaOCl D. phosphoric acid E. EDTA, NaOCl, phosphoric acid. Then, mixture of fluorescent dyed sodium perborate paste was placed along the pulp chamber and the coronal access cavity, and was refilled at days 7, 14 and 21. Results: The minimal and maximal penetration depths were 324 and 3045 μm, respectively, with a mean of 1607μm. The stained areas were significantly larger in the buccal and lingual directions (P Conclusion: Bleaching agents penetrate to the extra-radicular region of teeth; however, the level of peroxide penetration is significantly higher when the irrigation sequence consists of phosphoric acid prior the bleaching agent placement.

Objective To compare observer agreement between endodontists and oral and maxillofacial radiologists (OMRs) in the detection and measurement of periapical lesions as depicted in cone beam computed tomography (CBCT) with 2 voxel sizes. Study Design In total, 256 CBCT images of maxillary molars were evaluated by 2 endodontists and 2 OMRs. Images were obtained at voxel sizes of 0.2 and 0.4 mm. Observers evaluated 64 endodontically and 64 nonendodontically treated teeth for the presence of periapical lesions using a 5-point confidence scale. Weighted κ values were calculated to determine intra- and interobserver agreement. Intraclass correlation coefficients (ICCs) were calculated to assess intra- and interobserver agreement in width and height measurements of the lesions. Results Intraobserver agreement ranged from fair to almost perfect, with κ values higher for the OMRs than for the endodontists. Interobserver agreement between endodontists ranged from fair to moderate at the 0.2mm voxel size and was slight at 0.4 mm. Agreement between OMRs was almost perfect at 0.2 mm and ranged from substantial to almost perfect at 0.4 mm. ICC was excellent for all observers in all conditions. Conclusions Intra- and interobserver reliability was affected by voxel size and specialty. Correlation for measurements exhibited no variation.

In this study, we evaluated the invasion of bacteria into the dentinal tubules of retrofilled extracted human teeth, and the influence of different fluorescently labeled retrograde filling materials on the bacterial invasion and viability, by means of confocal laser scanning microscopy (CLSM). The root apices of extracted teeth were cut, prepared, and filled retrogradely using either intermediate restorative material (IRM), mineral trioxide aggregate (MTA), or Biodentine. The roots were filled with Enterococcus faecalis bacteria from their coronal part for 21 days. Then, 3-mm-long apical segments were cut to get root axial slices, and the bacteria were fluorescently stained and evaluated by CLSM. Bacterial penetration into the dentinal tubules favored the bucco-lingual directions. The filling materials penetrated up to 957 &micro, m into the tubuli, and the bacteria, up to 1480 &micro, m (means: 130 and 167 &mu, m, respectively). Biodentine fillings penetrated less and the associated bacteria penetrated deeper into the tubuli compared to MTA or IRM (p = 0.004). Deeper filling penetration was associated with shallower penetration of both dead and live, or live alone, bacteria (p = 0.015). In conclusion, the current study enables better understanding of the microbiological&ndash, pathological course after endodontic surgical procedures. It was found that even with retrograde fillings, bacteria invade deep into the dental tubules, where deeper filling penetration prevents deeper penetration of the bacteria and adversely affects the viability of the bacteria.

The objective of this study was to evaluate potential risk factors, including the placement of dental implants, for the development of tooth cracks. A series of 212-patients, who were referred for endodontic treatment, were retrospectively screened, of which 72 (34%) patients had been diagnosed with 80-cracked teeth confirmed with an operating microscope. These patients had an average age of 53-years and were equally distributed between genders. Forty-one percent of the cracked teeth were diagnosed after the placement of dental implants, with an average of 3-implants per patient. Introduction: the study aimed to evaluate Enterococcus Faecalis colonization in the pulp chamber in pulpotomized extracted human teeth filled by different pulpotomy base materials (PBMs), using confocal laser scanning microscopy (CLSM). Study design: Cavity preparations were made in 70 extracted primary molars. The pulp chambers were filled using either Intermediate restorative material (IRM), Mineral Trioxide Aggregate (MTA) or Glass ionomer (GI). Twenty-five teeth served controls. The specimens were sterilized, and coronally filled with bacterial suspension for 21 days. The specimens were cut through the furcation area, stained using LIVE/DEAD BacLight Bacterial Viability Kit and evaluated using CLSM. Results: The extent of fluorescent staining was larger in the GI group, compared to the IRM and MTA groups, and larger in the IRM group compared to the MTA group (P0.05). The ratio of live bacteria to dead bacteria within the evaluated areas was higher in the GI group compared to the IRM and the MTA groups, and higher in the IRM group compared to the MTA group (P0.05). Conclusions: bacteria colonize the interface between the PBM and dentin and penetrate deeply into the dentinal tubules. The extent and the vitality of the colonized bacteria may be affected by the type of PBM.

Background and Objectives: Large radicular cysts of the maxilla present a clinical challenge, as they may cause recurrent infection, severe alveolar bone loss and disruption of the nasal and maxillary sinus floors. The aim of this study was to evaluate the effect of previous root canal treatment on the clinical presentation of large maxillary radicular cysts. Materials and Methods: All cases of radicular cysts treated at the Oral and Maxillofacial Surgery Department of a tertiary public hospital over a period of six years (2012–2018) were evaluated. Histologically confirmed radicular cysts of the maxilla with a maximal dimension of over 15 mm were included. Demographic data of the patients, clinical presentation and radiographic features of the lesions were analyzed. Results: A total of 211 inflammatory cysts were treated in the study period, of these 54 histologically diagnosed radicular cysts in the maxilla were found to have a maximal dimension of over 15 mm. The mean age of patients with large maxillary radicular cysts was 43.3 years, 57.6% of which were male and 42.4% female. The lateral incisor was the most common tooth affected (46.3%). The mean size of the large radicular cysts was 25 mm. Then, 83.8% of the cysts were observed in teeth with previous endodontic treatment. Teeth without endodontic treatment presented clinically with significantly fewer acute symptoms in comparison to teeth with previous endodontic treatment. Conclusions: the vast majority (83.8%) of large maxillary radicular cysts were associated with endodontically treated teeth. Previous endodontic treatment was correlated to increased frequency of clinical symptoms.

The combination of ethylenediaminetetraacetic acid (EDTA) and sodium hypochlorite (NaOCl) has been advocated as an effective irrigation methodology to remove organic and inorganic matter in root canal therapy. Yet, it was suggested that both solutions might lead to structural changes of the dentinal wall surface, depending on the order of application which might affect sealer mechanical retention. This study aims to evaluate the effect of different irrigating protocols on dentin surface roughness using quantitative 3D surface texture analysis. Data stems from 150 human root dentin sections, divided into five groups, each prepared according to one of the following protocols: Negative control; 17% EDTA; 17% EDTA followed by 5.25% NaOCl; 5.25% NaOCl; and 5.25% NaOCl followed by 17% EDTA. Each dentin sample was examined for its three-dimensional surface texture using a high-resolution confocal disc-scanning measuring system. EDTA 17% and the combined EDTA 17% with NaOCl 5.25% showed considerably higher roughness properties compared to the control and to NaOCl 5.25% alone. However, the irrigation sequence did not affect the dentin roughness properties. Therefore, mechanical retention is probably not dependent upon the selection of irrigation protocol sequence.

The aim of the present study was to evaluate the pulpal and periapical status of vital teeth within the area of a lateral sinus floor elevation (LSFE). The files of patients treated by LSFE between 2009 and 2015 were retrospectively evaluated. The radiographic bone measurements and the periapical status of the teeth adjacent to the LSFE area were evaluated preoperatively and 6 months following the surgery by CBCT. The pulpal status of the adjacent teeth was clinically evaluated using thermal pulp vitality testing. Vital teeth within the sinus floor elevation area were the main inclusion criteria for this study. A total of 158 LSFE procedures were evaluated, 18 cases fulfilled the inclusion criteria, and 20 adjacent teeth were vital. There were no cases of periapical pathology or loss of pulp vitality. There was a statistically significant difference in the ridge height between the baseline and 6 months postoperatively and in the distance from the apex to the sinus floor between pre- and 6 months postoperatively (p &lt, 0.01).The pulpal and periapical status of vital teeth was not affected following LSFE.

Introduction Increase in efficacy during root canal irrigation may contribute to better treatment outcomes. This study investigated the efficacy of ultrasonic and negative pressure irrigation systems using sodium hypochlorite (NaOCl) in the reduction of bacterial load in human teeth. One hundred thirty-one single-rooted teeth were cleaned and shaped, autoclaved, and incubated with E. faecalis. Teeth were randomly assigned to three experimental groups (n = 40) and treated in the presence of 1% sodium hypochlorite using EndoUltra cordless ultrasonic, conventional ultrasonic, and Endovac negative pressure irrigation. Syringe irrigation controls were treated with 1% sodium hypochlorite and phosphate-buffered saline via side-vented needle irrigation. All groups with NaOCl received 5% sodium thiosulfate neutralization for 5 min after treatment. Samples of root canal fluid and dentin chips were acquired from canals before and after treatment, incubated on BHI agar, and colony forming units categorized according to quantity. Wilcoxon rank-sum and Bonferroni tests were used for statistical analysis. p values less than 0.05 were considered significant. Results Endovac group was significantly better in eliminating bacteria from the root canals than 1% NaOCl (p = 0.006) and PBS syringe irrigation (p = 0.015). However, it was not significantly different from the two ultrasonic groups (p > 0.05). Both ultrasonic groups showed better performance than 1% NaOCl and PBS syringe irrigation, however, not statistically significant (p < 0.03). There was no significant difference between the two ultrasonic devices (p > 0.05). Conclusion EndoVac may be an important tool for bacterial load reduction in oval canals.

The extension of sinus floor augmentation beyond the edentulous area, apical to the adjacent teeth, has many therapeutic advantages, but the reliability and safety of the procedure has not been assessed in depth. The present study compares the gain of bone anterior and posterior to the edentulous area and evaluates potential advantages and limitations in the clinical setting. The maximum vertical bone height in the edentulous and extended maxillary sinus augmentation (EMSA) areas and the thickness of the sinus membrane of 65 patients were measured. Those measurements were analyzed using the t test and Pearson correlations. The average vertical bone gain was 11.98 ± 3.53 mm in the edentulous sinus area and 8.60 ± 3.89 mm in the EMSA area (P < .05). Minor perforations of the sinus membrane occurred in 4 patients. There were no postsurgical graft contaminations or periradicular changes during follow-up. EMSA is a reliable and safe procedure with a very low complication rate. This approach is effective and safe for patients who have lost part of their posterior dentition. It enables future implant placement while avoiding the need for sinus reentry and proximal teeth extraction.

Traditionally, when a root-originated fracture (ROF) was diagnosed in an endodontically treated tooth, the tooth was scheduled for extraction. However, modern endodontics offers new treatment options to manage and maintain certain ROF teeth. The decision of whether to extract a ROF tooth and substitute it with a dental implant, or to implement a more conservative management approach by attempting an additional endodontic treatment aimed to preserve the natural tooth, is complicated and multifactorial. The management alternatives of ROF teeth range from a traditional root amputation in multi-rooted teeth to modern endodontic surgical modalities that may enable the preservation of a fractured tooth. This required decision-making process includes prosthetic, periodontal, esthetic, endodontic, and patient value concerns.

Enterococcus faecalis is frequently found in infected root canals of teeth with persistent apical periodontitis. To review the challenges involved in the eradication of persistent Enterococcus faecalis infections and their impact on human health. In the root canal system, Enterococcus faecalis resides in biofilm communities that are able to resist a wide range of harsh conditions and treatments and persist for years. Various virulence and resistance factors provide Enterococcus faecalis capabilities in adherence, colonization, and biofilm formation, which is much more resistant to antibacterial agents than planktonic bacteria. These capabilities explain its persistence in root canal infections. To date, the available therapeutic tools to efficiently eradicate Enterococcus faecalis infections remain limited. Recently, in a model mimicking Enterococcus faecalis root canal infection, novel biofilm inhibitors and dispersing agents, such as D-Leucine, presented superior capability over sodium hypochlorite, the frequently used root-canal antibacterial irrigation solution, in the eradication of Enterococcus faecalis. These recent studies present promising treatment regimens for Enterococcus faecalis persistent infections.

An endo-perio lesion is one of the more common tooth-related problems. An association between the presence of apical and marginal periodontitis is known in the literature and has been observed in 5.7% of individuals aged 40–45 years old. The purpose of the present article is to present three case reports describing the successful retreatment of endo-perio lesions. In each of these cases, we used a biologically active bioceramic root canal sealer, GuttaFlow Bioseal, which is a bioactive root canal filling material composed of gutta percha, polydimethylsiloxane, platinum catalyzer, zirconium dioxide, and bioglass. All cases were followed up clinically and radiographically for a period of at least 11 months.

Cone beam computed tomography (CBCT) has become a common diagnostic method in endodontics. However, the current literature provides insufficient information about different aspects that are related to the use of CBCT, such as: the efficacy of CBCT to support the practitioner’s clinical decision making and to affect treatment outcomes; about the required training of the practitioner so he can efficiently examine the entire region that appears in the CBCT scan and diagnose abnormalities or possible pathologies; and on the long-term health risks associated with the use of CBCT in endodontics. In addition, CBCT has the strong potential to be used for accurate diagnosis of complete and uneventful healing. In this thematic series, we call for manuscripts that discuss aspects of the use of CBCT in endodontics.

The purpose of this study was to evaluate Enterococcus faecalis colonization at the apical part of root canals following root-end resection and filling using confocal laser scanning microscopy (CLSM).The apical 3-mm root-ends of 55 extracted single rooted human teeth were resected, and 3-mm retrograde cavities were prepared and filled using either mineral trioxide aggregate (MTA), intermediate restorative material (IRM), or Biodentine (n = 10 each); 25 teeth served as controls. The roots were placed in an experimental model, sterilized, and coronally filled with E. faecalis bacterial suspension for 21 days. Then, the apical 3-mm segments were cut to get two slabs (coronal and apical). The slabs were stained using LIVE/DEAD BacLight Bacterial Viability Kit and evaluated using CLSM.The fluorescence-stained areas were larger in the bucco-lingual directions compared with the mesio-distal directions (p 0.05). The mean and maximal depths of bacterial colonization into the dentinal tubules were 755 and 1643 μm, respectively, with no differences between the root-end filling materials (p 0.05). However, more live bacteria were found in the MTA group in comparison to IRM and Biodentine groups (p 0.05).CLSM can be used to histologically demonstrate bacterial root-end colonization following root-end filling. This colonization at the filling-dentine interfaces and deeper into the dentinal tubules may be inhomogeneous, favoring the bucco-lingual aspects of the root.Following root-end resection and filling bacterial colonization may lead to inflammatory reactions at the periapical tissues; the viability of the colonized bacteria may be affected by the type of root-end filling material.

Cone beam computed tomography (CBCT) has become a popular diagnostic method in endodontics. However, recently published data provides a glance into possible missing links regarding the efficacy of CBCT and its use to support the endodontic clinical decision-making process.

An abstract is a brief overview of a scientific, clinical or review manuscript as well as a stand-alone summary of a conference abstract. Scientists, clinician-scientists and clinicians rely on the summary information provided in the abstracts of systematic reviews to assist in subsequent clinical decision-making. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) for Abstracts checklist was developed to improve the quality, accuracy and completeness of abstracts associated with systematic reviews and meta-analyses. The PRISMA for Abstracts checklist provides a framework for authors to follow, which helps them provide in the abstract the key information from the systematic review that is required by stakeholders. The PRISMA for Abstracts checklist contains 12 items (title, objectives, eligibility criteria, information sources, risk of bias, included studies, synthesis of results, description of the effect, strength and limitations, interpretation, funding and systematic review registration) under six sections (title, background, methods, results, discussion, other). The current article highlights the relevance and importance of the items in the PRISMA for Abstracts checklist to the specialty of Endodontology, while offering explanations and specific examples to assist authors when writing abstracts for systematic reviews when reported in manuscripts or submitted to conferences. Strict adherence to the PRISMA for Abstracts checklist by authors, reviewers, and journal editors will result in the consistent publication of high-quality abstracts within Endodontology. The periodontium and the dental pulp are closely associated, sharing embryonic, functional, and anatomical interrelationships. Consequently, the relationship of endodontic and periodontal lesions has stirred confusion and debate as dental practitioners have become gradually further aware of the possible associations between the periodontium and the dental pulp.

The advance of the endodontic disease involves inflammatory reaction of the peri-radicular tissues while the periodontal involves marginal periodontium and results in the progressive loss of the supportive tissues. There are multiple routes of communication between the root canal space and marginal periodontium. By these communications the bacteria from the root canal space may contaminate and infect the marginal periodontium and vice versa. While in most cases the manifestation of the periodontal and endodontic diseases is clearly distinct, there are certain clinical scenarios when the signs and symptoms may be confusing, making the final diagnosis complicated and subsequently result in the wrong treatment choice.

When presented with a patient with a gingival swelling and/or pain, the clinician will need to determine the etiology before a diagnosis and treatment can be rendered. Depending on the age of the patient and the condition of their dentition, this may be a challenge. Appropriate treatment depends on the clinician’s understanding of endodontic sensibility testing, endodontic pathogenesis, periodontal pathogenesis, and some basic dynamics of pulp biology. This chapter will review these topics with the aid of clinical cases, and discuss the endodontic considerations in the management of these types of endodontic-periodontal lesions. Sections on Endodontic Diagnosis, Understanding the Pulpodentinal Complex, and Pathogenesis of Pulp Necrosis will be presented.

Introduction The aim of this study was to systematically review and evaluate the literature regarding the prognosis of altered sensation after extrusion of root canal filling materials and the possible factors influencing it. Methods A systematic search of the literature was performed to identify studies that reported on altered sensation after extrusion of root canal filling materials during endodontic treatments. The articles were evaluated for their relevance based on strict inclusion criteria, and the identified suitable articles were subject to data extraction and analysis. Results Initially, 109 possibly relevant articles were identified. After screening and full-text evaluations, 28 articles that met the inclusion criteria were analyzed, reporting on a total of 84 patients with altered sensation after extrusion of root canal filling materials. All the included studies, except 1 case series, were case reports. Under the limited available data, the extracted data showed that 91% of the patients had fully or partially recovered over time. Most of the cases in the lower molars as well as most of the cases in which the obturation was performed using paraformaldehyde-containing sealer or cases in which an immediate treatment was not performed did not fully recover. Conclusions The current scientific knowledge regarding the prognosis of nerve injuries caused by overextruded endodontic materials relies primarily on case reports. Within the limitations of the published data, it seems that the tooth locations, types of extruded materials and the obturation technique, and treatment after the injury may affect the nerve injury prognosis.

This study aimed to evaluate the precision of electronic apex locators (EALs) in locating the apical constriction (AC) during a root canal treatment compared with a histologic evaluation of the AC as well as the effects of possible influencing factors by means of a systematic review of the literature and meta-analysis.A systematic search of the literature was performed to identify studies that histologically evaluated the precision of EALs in human teeth. The identified studies were subject to strict inclusion criteria followed by data extraction and meta-analysis.From 247 articles, 10 articles met the inclusion criteria, with a total of 1105 EAL measurements performed by 4 types of EALs: Root ZX (J Morita, Tokyo, Japan), Justy II (HagerWerken GmbHCo, Duisburg, Germany), Endy 5000 (Loser Co, Leverkusen, Germany), and Endox (Lysis Co, Milan, Italy). Root ZX, Justy II, and Endy 5000 were found to be significantly more accurate than Endox in determining the distance between the file tip and the apical constriction (P .05). The longest mean distance was measured by Endox (1.35 ± 0.41 mm), and the shortest mean distance was measured by Justy II (0.25 ± 0.17 mm, P .05). The mean distance measured by Root ZX and Justy II in the presence of hydrogen peroxide was shorter compared with the mean distance measured by them in the presence of sodium hypochlorite (P .05). The pulp status (vital or necrotic) had no significant effect on the precision of the EALs.The precision of electronic working length measurement depends on the device used and the type of irrigation and is not influenced by the status of the pulp tissue.

Introduction The aim of this study was to evaluate the diagnostic efficacy of cone-beam computed tomographic (CBCT) imaging in endodontics based on a systematic search and analysis of the literature using an efficacy model. Methods A systematic search of the literature was performed to identify studies evaluating the use of CBCT imaging in endodontics. The identified studies were subjected to strict inclusion criteria followed by an analysis using a hierarchical model of efficacy (model) designed for appraisal of the literature on the levels of efficacy of a diagnostic imaging modality. Results Initially, 485 possible relevant articles were identified. After title and abstract screening and a full-text evaluation, 58 articles (12%) that met the inclusion criteria were analyzed and allocated to levels of efficacy. Most eligible articles (n = 52, 90%) evaluated technical characteristics or the accuracy of CBCT imaging, which was defined in this model as low levels of efficacy. Only 6 articles (10%) proclaimed to evaluate the efficacy of CBCT imaging to support the practitioner's decision making; treatment planning; and, ultimately, the treatment outcome, which was defined as higher levels of efficacy. Conclusions The expected ultimate benefit of CBCT imaging to the endodontic patient as evaluated by its level of diagnostic efficacy is unclear and is mainly limited to its technical and diagnostic accuracy efficacies. Even for these low levels of efficacy, current knowledge is limited. Therefore, a cautious and rational approach is advised when considering CBCT imaging for endodontic purposes.
