運動イメージには，一人称イメージと三人称イメージがある．実際の治療場面では，鏡に映っ た被験者自身や隣坐する療法士または被験者自身の足を見つつ治療を行う場合が多い．我々の研究では，被験者自身の右下肢の三人称イメージ映像に合わせて右下肢の模倣運動を行った場合，被験者自身の右下肢の一人称イメージ映像に合わせて右下肢の模倣運動を行った場合と比較して，右の舌状回が賦活した．今回，左右の被験者自身の一人称イメージ映像と三人称イメージ映像を組み合わせ舌状回の左右差に着目し検討を行った．結果，被験者自身の右下肢の三人称映像の模倣運動の場合に特異的に右舌状回が賦活する傾向を認めた．右舌状回は自己の右下肢三人称映像に反応する特有の部位と考える．

運動イメージには，一人称イメージと三人称イメージがある．今回我々は，歩行に関するリズ ム生成を検討するために，対座する治療者を第三者の三人称イメージと想定し，「一定でないリズム」 と「一定なリズム」での第三者の三人称映像の足趾模倣運動を行い，fMRI にて脳画像を比較した． 結 果，第三者の三人称映像を用いた「一定でないリズム」課題は，固視課題と比較して，左上前頭回の BA6・前運動野: 補足運動野及びその近傍部，左中前頭回のBA6 近傍部の活動が増加した．「一定なリ ズム」課題は，固視課題と比較して，左内側前頭回のBA6・前運動野: 補足運動野及びその近傍部の活 動が増加した．第三者の三人称映像による「一定でないリズム」課題と「一定なリズム」課題の比較に おいて，「一定でないリズム」課題を見ながら運動課題を行った場合，「一定なリズム」課題を行った 場合と比較して，右下頭頂小葉のBA40・縁上回の活動が増加した．この研究の結果、リズム形成には BA40 が関与し，リズム形成を補足する課題として「一定でないリズム」課題と「一定なリズム」課題 を組みあわせることで縁上回を賦活することにより効果的な歩行のリハビリテーションの準備を行うこ とが可能と考える．

運動イメージには，一人称イメージと三人称イメージがある．今回我々は，隣に座る第三者の下肢映像を治療者（セラピスト）の一人称イメージ課題と想定し，そして向かいに座る第三者の下肢映像を治療者の三人称イメージ課題と想定した．この二つの第三者の下肢映像とあらかじめ撮影した被験者自身の一人称イメージ課題（手前から前方に突き出た下肢）の下肢映像を組み合わせて，隣座する治療者と対座する治療者の下肢映像を利き足趾（右）で摸倣した場合の脳活動の相違をfMRI にて比較した．結果，第三者の三人称課題群は，第三者の一人称課題群と比較して，左吻側前帯状皮質と右角回近傍部が賦活した．第三者の一人称課題群は，第三者の三人称課題群と比較して，左背側前帯状皮質近傍部と左縁上回近傍が賦活した．吻側前帯状皮質は，情動に関与する．背側前帯状皮質は，認知に関与する．治療者が対座するか隣坐するかによって脳の賦活部位が異なるという実験結果は，運動模倣を促す時の治療者の位置の違いが患者様の情動や認知に異なる影響をもたらす可能性を示している．

運動イメージ課題をリハビリテーションに効果的に応用するために，一人称イメージと三人称イメージの運動イメージ想起の方法の違いに着目し，機能的磁気共鳴画像法（functional magnetic resonance imaging ，以下f-MRI）を用いて脳活動部位の比較を行った。対象は，一人称イメージ課題群19名，三人称イメージ群17名である．結果，一人称イメージ課題群では，被験者自身の動く足趾映像を見ながら運動イメージ課題を行った場合，第三者の動く足趾映像を見ながら運動実行課題を行った場合と比較して，左角回と右紡錘状回の活動が増加した．三人称イメージ課題群では，第三者の動く足趾映像を見ながら運動イメージ課題を行った場合，被験者自身の動く足趾映像を見ながら運動実行課題を行った場合と比較して，左中前頭回ブロードマンの9野の後部領域の活動が増加した．一人称イメージ課題群と三人称イメージ課題群の群間比較では，三人称イメージとしての被験者自身の動く足趾映像を見ながら運動イメージ課題を行った場合，一人称イメージとしての第三者の動く足趾映像を見ながら運動イメージ課題を行った場合と比較して，左右の舌状回と右前帯状皮質の活動が増加した．これは，一人称イメージ課題と三人称イメージ課題に被験者自身と第三者を組み合わせることで相補的な制御で課題の遂行を行った可能性があることを示す．以上の結果より，一人称イメージを運動イメージ課題として用いる場合は被験者自身の足趾を見つめさせつつセラピストが他動的に足趾を動かすこと，三人称イメージ課題を用いる場合は向かいに座ったセラピストの足の動作を模倣しつつ同時にセラピストが他動的に足趾を動かすことが自己を認識しつつ効果的にリハビリテーションを行う可能性があると考える．

脳皮質における右半球と左半球の認知機能に関する左右差を比較検討するために，認知的葛藤を含む後出し負けじゃんけんを用いてfMRI 測定を行った．対象は右利き群9 名，左利き群9 名である．足趾を用いた後出し負けじゃんけんの場合，利き足趾では動作を行う運動野と同側の腹側運動前野が賦活したが，上肢の右利き右手指を用いた後出し負けじゃんけんの場合，動作を行う運動野とは対側の右脳の腹側運動前野が賦活した．これは日頃課題に慣れた手指はスクリーン上の課題を他者として見て右の頭頂葉- 腹側運動野が賦活するのに対し，日頃課題に慣れない足趾は自己の課題遂行を目的として動作を行うため右利きでは左の（左利きでは右の）運動野と同側の頭頂葉- 腹側運動野系を賦活するためと考える．このことにより，上肢では右利き群は自己と他者の判断を左右の頭頂葉- 腹側運動野で分業するのに対し，左利き群は頭頂葉- 腹側運動野を両側性に使用していることが推察される．また，背外側運動前野と前頭極は，課題の相違により分業し活動していることが示唆された．, The purpose of this study is to determine the differences cortex representations during movements of toes and fingers of dominant hands and feet using functional Magnetic Resonance Imaging (fMRI). Subjects and METHODS: We enrolled 9 right dominant hand-foot and 9 left-dominant hand-foot healthy students, age-matched controls, and they signed consent forms. fMRI of 1.5T was used to map cortical representations associated with motor tasks of the fingers and toes in the two groups. In these tasks, subjects watched a videoclip depicting “performance of rock- paper-scissors” of fingers or toes and were required to purposely lose. We used SPM 2 for analysis. RESULTS: We compared the right-dominant hand-foot group with the left-dominant hand-foot group in fMRI maps. During toe movements, the right-dominant hand-foot group showed increased representations of activation in the left primary motor area and left ventral premotor area, and the left-dominant hand-foot group showed increased representations of activation in the right primary motor area and right ventral premotor area. But during finger movements, the right-dominant hand-foot group showed increased representations of activation in the left primary motor area and right ventral premotor area. Flow is the holistic experience felt when an individual acts with total involvement. Although flow is likely associated with many functions of the prefrontal cortex (PFC), such as attention, emotion, and reward processing, no study has directly investigated the activity of the PFC during flow. The objective of this study was to examine activity in the PFC during the flow state using functional near-infrared spectroscopy (fNIRS). Twenty right-handed university students performed a video game task under conditions designed to induce psychological states of flow and boredom. During each task and when completing the flow state scale for occupational tasks, change in oxygenated hemoglobin (oxy-Hb) concentration in frontal brain regions was measured using fNIRS. During the flow condition, oxy-Hb concentration was significantly increased in the right and left ventrolateral prefrontal cortex. Oxy-Hb concentration tended to decrease in the boredom condition. There was a significant increase in oxy-Hb concentration in the right and left dorsolateral prefrontal cortex, right and left frontal pole areas, and left ventrolateral PFC when participants were completing the flow state scale after performing the task in the flow condition. In conclusion, flow is associated with activity of the PFC, and may therefore be associated with functions such as cognition, emotion, maintenance of internal goals, and reward processing. 運動イメージは大別して，一人称イメージと三人称イメージがある．今回，我々は一人称イメージと「横から突き出た足」と「前から突き出た足」の二パターンの三人称イメージを用いて足趾を動かそうとイメージする課題を行い，fMRI にて脳画像を比較した． 【方法】対象は，健常者20 名（年齢20 － 28 歳）．課題は，スクリーンに映し出された一人称イメージ と二パターンの三人称イメージの映像に合わせて右足趾を動かそうとイメージする運動とした．一連の動作をf － MRI にて解析し比較した． 【結果】映像を見ながら足趾の運動をイメージする場合，三人称イメージを見ながら運動した場合，二パターン共に著名な変化はなかった．一人称イメージを見ながら運動を行うほうが右の側頭極の活動が増加した． 【考察】三人称イメージは方向に関わらず活動増加しなかった．側頭極は「心の理論」と関係する．他 者の一人称イメージ映像での運動課題の方が他者の意図を理解する脳活動を増加させる可能性がある．, There are two main modes of motor imagery: a first-person perspective imagery (1PPI: inesthetic motor imagery) and a third-person perspective imagery (3PPI: Visual motor imagery). 1PPI involves imagining the feeling of performing a certain movement. 3PPI involves imagining seeing yourself performing a certain movement. But 3PPI involves two patterns; they are a horizontal pattern and a front pattern. The purpose of this study is to determine the cortical representations in 1PPI and two patterns’ 3PPI during toes’ movements in healthy persons using f-MRI. Methods:We enrolled 20 healthy subjects with right-handed leg, and they signed consent forms. f-MRI of 1.5T was used to map cortical representations associated with motor tasks of the right toes. In these tasks, subjects watched video-clip depicting simple 1PPI and two patterns’ 3PPI actions of toes. And they were required to image the models. Results:We compared 1PPI tasks with two patterns’ 3PPI tasks at f-MRI maps. During imaging toes’ movements, subjects in 1PPI tasks showed an increased representation of activation in the right temporal pole. However in two patterns’ 3PPI tasks increased activation was not found there. Conclusions:In a recent study, the temporal pole was active with regards to Theory of Mind. Objective: The aim was to determine whether mini mental state examination (MMSE) scores improved in older participants of a Functioning Improvement Tool (FIT) home-visit program. Methods: Two hundred fifty-two participants aged 65 years or older living at home and receiving preventive services or a community long-term care prevention project according to the Japanese social long-term care insurance system were enrolled and randomly assigned to an intervention group (n = 128) or a control group (n = 124). Intervention group subjects received a 60-min FIT home-visit program for 3 months, which included guidance, assistance, and help in writing and teaching calculation in order to complete the FIT. Control subjects did not receive any home visits. Cognitive function was evaluated by MMSE. Analysis of covariance was used to examine the effects of the FIT adjusting for baseline MMSE scores, age, and sex. Results: Fifty-three subjects were excluded because of withdrawal, hospitalization, death, relocation, or missing data of MMSE; 199 subjects (60 men, 139 women; age 78.6 ± 7.4 years) were analyzed. The baseline MMSE scores did not differ between the intervention and control groups (24.2 ± 4.3 vs. 24.1 ± 4.7, p = 0.90). After the study period, the change in the MMSE score was significantly better in the intervention group than in the control group (0.8 ± 0.3 vs. −0.1 ± 0.2, p = 0.04). Stratified analyses showed that the intervention strategy was most effective in subjects with mild cognitive decline, with baseline MMSE scores from 18 to 23 points (1.9 ± 0.5 vs. −0.1 ± 2.8, p = 0.04). Conclusions: Our FIT home-visit program improved MMSE scores in older participants with mild cognitive decline.

運動イメージは大別して、一人称イメージと三人称イメージがある。今回、我々は一人称イメージと三人称イメージを用いて足趾の運動を行い、fMRI にて脳画像を比較したので報告する。【方法】対象は、健常者20 名（年齢20-22 歳）。スクリーンに映し出された一人称イメージ（下から突き出た足）と三人称イメージ（今回は横から突き出た足を用いた）の映像に合わせて右足趾を握る動作を、実際に足趾を動かす運動、足趾を動かそうとイメージすること、その動作を見るだけの3 課題を行った。運動は、一定でないリズムの運動とした。一連の動作をf-MRI にて撮影した後、SPM2 を用いて画像を解析し比較した。 【結果】映像を見ながら足趾の運動を行う場合、一人称イメージを見ながら運動を行うほうが左の頭頂間溝前部領域が賦活する傾向があった。また、イメージする場合は一人称イメージで左の腹側運動前野後部が、見る場合は三人称イメージで左の腹側運動前野後部が賦活する傾向にあった。 【考察】模倣運動を行ったとき、下頭頂葉小葉が活性化されることが知られている。今回の研究では、 一人称イメージで、左の頭頂間溝前部領域がより賦活する傾向にあることがわかった。頭頂間溝前部領 域は、一般的に上肢での把持動作に関連することが報告されている。今回、足趾運動にも関与すること が示唆された。また、腹側運動前野後部は足趾のミラーニューロンに関与することが示唆された。, There are two main modes of motor imagery: a first-person perspective image (1PPI or kinesthetic motor image) and a third-person perspective image (3PPI or visual motor image). We selected 20 healthy persons with written consents and used f-MRI of 1.5T to map cortical representations associated with motor tasks of the subjects’ right toes. In these tasks subjects watched video-clip depicting simple 1PPI and complex 3PPI actions of toes, and are required to imitate the model images. During imitative toe movements, we found, the subjects of 1PPI tasks showed an increased representation of activation in the left anterior AIP. During image toe movements, we found, the subjects of 1PPI tasks showed an increased representation of activation in the left vPM. And while they were watching toe movements, we found, the subjects of 3PPI tasks showed an increased representation of activation in the left vPM. 背景・目的 : 高次脳機能障害のため, 社会復帰を果たせない患者の問題が社会的に注目され, 行政的な対応や疫学調査などが行われているが, これらの調査は慢性期の症状が安定した患者を母集団とした調査が中心であった。救急医療施設からの転院や退院の後に, 高次脳機能障害を呈するも外来経過観察から脱落し社会的に困窮している患者群が存在しているとの仮説を立て, そのような状況に至った問題点を明らかにすることを, 本研究の目的とした。対象 : 2000年1月から2003年12月の4年間に北海道大学病院救急部に, 受傷現場から直接搬入となり生存退院もしくは転科となった鈍的外傷患者204症例。方法・結果 : 2回のスクリーニング用紙による調査や外来診察, 入院精査の結果より高次脳機能障害患者の発生数を調査し, その受傷時の状態, 検査の施行状況, 退院までの経過などに関して検討を行った。対象患者204症例のうち, 79症例から1次スクリーニング調査に回答があった。最終的に6症例, 全体の約3%が高次脳機能障害と診断された。本調査により新たに診断された症例は3症例であり, いずれも高次脳機能障害とは関わりの少ない診療科からの退院であった。頭部外傷に対する診断に関しては, 急性期治療の必要性を判断するために頭部CTが82% (79症例中, 65症例) で施行されていた。頭部CTや意識レベルなど臨床的に問題を認めない症例に対し, 微細な脳損傷を否定するためにMRIなどの検査を施行している症例は認められなかった。考察・結論 : 救急領域における高次脳機能障害患者の見逃しは, 頭部外傷患者における脳損傷に対する検査・診断方法の感度の問題と, 高次脳機能障害の疾患自体の認知不足にあると考えられた。
