The transport sector is an integral element of southern Africa's economic development. The impact of HIV/AIDS on the region is therefore a major cause for concern. Using a method of triangulation, we used the findings of a comprehensive literature review, a stakeholder questionnaire and a United Nations regional workshop to take stock of the existing evidence linking HIV/AIDS to the transport sector. This paper outlines the strategies used to address HIV/AIDS by a range of stakeholders in the road, maritime, aviation and railway transport sectors. It highlights regional, national, and intervention-specific good practice examples in the field, suggesting key research gaps and putting forward policy recommendations for the region. As a result, national action plans will hopefully be able to benefit from this knowledge-sharing, enhanced regional cooperation and implementation of more effective responses to the epidemic in this critical sector.

Background: In the era of enhanced access to ART, many people live longer lives but with episodes of disability resulting from HIV, HIV-related conditions, and/or as side-effects of ART. It is crucial to understand the extent of disability among people living with HIV in high-prevalence settings to inform choices regarding care, policy and research. This article presents the results of the first scoping review to examine the extent, nature and range of disability among people living with HIV in HIV hyper-endemic countries. Methods: This scoping review used the World Health Organization’s International Classification of Functioning, Disability and Health (ICF) to conceptualize “disability”. A systematic search of electronic databases was conducted using specific keyword and subject heading combinations. Identified publications were screened and reviewed according to inclusion/exclusion criteria. Data were systematically extracted and reviewed for quality. Extracted data were reviewed for patterns related to methods or results. Results were aligned with the corresponding ICF code. Results: Forty-one articles were included, reporting data from 38 unique studies. Most (78%) of the studies were conducted in South Africa; five in Botswana, one in Zimbabwe and Lesotho, and none in Swaziland. Almost all studies recruited more females than males. All studies except two were in adults. The studies indicate that people living with HIV experience a variety of disabilities. Impairments in body structure/function comprise the majority of data, with particular focus on mental function. Data on activity limitations and participations restriction were limited, however, they were recorded. They indicate severe impact on people’s life and possible adherence. Conclusions: We argue that the time has come to elevate the focus holistically on health and life-related consequences of living with HIV and to integrate disability into the discussions and approaches to HIV care.

The Eastern and Southern Africa (ESA) region is the epicentre of the global HIV epidemic and also home to a large number of people with disabilities. Both HIV and Disability are significant public health issues. While they are generally viewed as distinct and unrelated phenomena data seems to suggest that they are particularly closely intertwined in ESA.For the first time in history, by using the same disability indicator consistently, the publication of the World Report on Disability in 2011 has allowed for the comparison of disability data between countries, and across regions. This has the potential to shed some light on the relationship between disability and socio-economic markers and other health conditions in a way that was not possible previously. In the absence of disability and HIV-specific population-based surveys, this paper uses global socio-economic and HIV datasets and compares them to data contained in the most recent World Report on Disability.The analysis suggests that disability prevalence may be related to HIV-prevalence in ESA (Pearson 0.87). It identifies research and policy gaps and seeks to shed light on the relationship between the two phenomena. It concludes that, more than any other region in the world, ESA needs to ensure better data collection on disability and the inclusion of disability throughout its HIV programmes in order to provide a comprehensive and appropriate response to the epidemic.

The incidence and prevalence of tuberculosis (TB) in children are increasing and becoming a particular problem in countries that are also affected by the HIV epidemic.1 Tuberculosis in children has been seen as hard to diagnose but at least in developed countries relatively easy to treat. However in children with HIV severe disease is more frequent and this has led to a reexamination of treatment regimens. (excerpt)
