2. Management in Patients with Simultaneous Acute Myocardial and Cerebral Infarction – Clinical Cases.

Intravenous thrombolysis and endovascular treatment are the gold standard in the treatment of patients with acute ischemic strokes. We present two rare cases of patients with ischemic stroke in different vascular basins with laboratory and ECG findings of acute coronary syndrome (ACS) in a therapeutic window for intravenous thrombolysis. According to a national consensus, in patients with concurrent acute stroke and myocardial infarction, intravenous thrombolysis could be administered after further risk assessment, despite the current trend towards endovascular treatment. Introduction: Ischemic stroke (IS) is an acute disorder of cerebral circulation. Intravenous thrombolysis and endovascular treatment are the gold standard in the treatment of patients with acute ischemic strokes. Concomitant recent myocardial infarction (MI) is a relative contraindication for alteplase therapy because of the increased risk of cardiac rupture. The therapeutic decision in these cases is usually individualized. When both diseases coexist, the condition worsens rapidly. Due to the different order of onset of the two diseases, the optimal treatment plan and choice of reperfusion method are still uncertain. Therefore, it is a great challenge for clinicians. We share two cases to provide relevant evidence for clinical behavior, decision-making and also review some literature data. Objective: To present two cases of simultaneous ischemic stroke and myocardial infarction in which an intravenous thrombolysis approach was chosen and to discuss recommendations for possible therapeutic options in such cases. Clinical cases: Two cases of young men with ischemic stroke are presented. During the hospitalization, myocardial infarction was also established. Both men were admitted in a time window for differential treatment, which was conducted. Patients were carefully screened for risk factors for vascular events in young adults. Conclusion: Intravenous thrombolysis is a treatment option in patients with clinical evidence of acute myocardial infarction and acute cerebral infarction. 3. Airtraq® is the preferred device for difficultintubation by residents?

Background: The Airtraq® optical laryngoscope is an intubation device designed to provide a view of the glottis without alignment of the oro-pharyngeal and laryngeal axes. Recent literature shows that, given its two significant features: time effectiveness and short learning curve, Airtraq® is the most favorable option when it comes to difficult intubation. Objectives: The goal was to analyze Airtraq® effectiveness when used by inexperienced physicians in anticipated difficult intubation in adult patients. Materials and methods: We conducted a prospective evaluation in ten medical residents using the Airtraq® device for the first time. All of them were experienced in using Macintosh. Each resident conducted laryngoscopy and intubation with the Airtraq® device after short didactic guidance. Eighteen patients were included, over a period of seven months. The patients showed four difficult intubation predictors: history of difficult intubation, thyromental distance less than 60 mm, mouth opening less than 35 mm and Mallampati class 3 or 4. All of them were clinically examined for difficult airway by an ENT specialist. Results: Before induction of anaesthesia all residents received a short demonstration on the use of the Airtraq®. Every participant was supervised by an Airtraq® handling specialist for each intubation maneuver. In sixteen patients, Airtraq® insertion, glottis visualization and subsequent intubation were easy and rapid, without arterial oxygen desaturation. In two patients the trachea was intubated from the second and third attempt. There were two tracheal intubation failures, associated with extended tracheal intubation and an Airtraq® specialist had to continue with intubation. The Airtraq® reduced the duration of intubation attempts in all cases, reduced the number of optimization maneuvers required, and reduced the potential for dental trauma. However, the two intubation failures emphasize the fact that Airtraq® laryngoscopy requires a clinical training process, especially in the event of anticipated difficult airway management situations. Conclusion: The residents participating the study, found the Airtraq® easier to use in all scenarios compared to the Macintosh laryngoscope. BACKGROUND: Data are lacking on the long-term effect on cardiovascular events of adding sitagliptin, a dipeptidyl peptidase 4 inhibitor, to usual care in patients with type 2 diabetes and cardiovascular disease. METHODS: In this randomized, double-blind study, we assigned 14,671 patients to add either sitagliptin or placebo to their existing therapy. Open-label use of antihyperglycemic therapy was encouraged as required, aimed at reaching individually appropriate glycemic targets in all patients. To determine whether sitagliptin was noninferior to placebo, we used a relative risk of 1.3 as the marginal upper boundary. The primary cardiovascular outcome was a composite of cardiovascular death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for unstable angina. RESULTS: During a median follow-up of 3.0 years, there was a small difference in glycated hemoglobin levels (least-squares mean difference for sitagliptin vs. placebo, -0.29 percentage points; 95% confidence interval [CI], -0.32 to-0.27). Overall, the primary outcome occurred in 839 patients in the sitagliptin group (11.4%; 4.06 per 100 person-years) and 851 patients in the placebo group (11.6%; 4.17 per 100 person-years). BACKGROUND: The cardiovascular effects of adding once-weekly treatment with exenatide to usual care in patients with type 2 diabetes are unknown. METHODS: We randomly assigned patients with type 2 diabetes, with or without previous cardiovascular disease, to receive subcutaneous injections of extended-release exenatide at a dose of 2 mg or matching placebo once weekly. The primary composite outcome was the first occurrence of death from cardiovascular causes, nonfatal myocardial infarction, or nonfatal stroke. The coprimary hypotheses were that exenatide, administered once weekly, would be noninferior to placebo with respect to safety and superior to placebo with respect to efficacy. RESULTS: In all, 14,752 patients (of whom 10,782 [73.1%] had previous cardiovascular disease) were followed for a median of 3.2 years (interquartile range, 2.2 to 4.4). A primary composite outcome event occurred in 839 of 7356 patients (11.4%; 3.7 events per 100 person-years) in the exenatide group and in 905 of 7396 patients (12.2%; 4.0 events per 100 person-years) in the placebo group (hazard ratio, 0.91; 95% confidence interval [CI], 0.83 to 1.00), with the intention-to-treat analysis indicating that exenatide, administered once weekly, was noninferior to placebo with respect to safety (P

The European Physical and Rehabilitation Medicine Journal Network (EPRMJN), a joint initiative between the European Society Physical and Rehabilitation Medicine (ESPRM) and the European Journal of Physical and Rehabilitation Medicine (EJPRM), has the aim to increase scientific knowledge among PRM specialists and foster collaboration among physical and rehabilitation medicine (PRM) journals. This article reports the results of a survey of national PRM journals in Europe we conducted to obtain an overview of the current state of PRM research in the European setting.Every year 682 PRM papers are published in a total of 66 issues per 3 294 page in the 16 national journals that responded to our survey, out of the 21 published in 15 countries; 12 countries have no PRM journals and two did not respond. Some 94% of the journals responding to the survey have a research aim and 88% an educational aim; all journals use a peer-review process (75% blinded); on average, 58.8 submissions are received per year, of which 6.7% are invited papers; the rejection rate is 21.4%. The majority of papers report original research, and main topics are musculoskeletal and neurological rehabilitation.Since the national journals published in Europe have a good peer review process and publish mainly original articles, it is possible that good research can be found. A major problem is the English language barrier to wider readerships, as many researchers publish only in their native language. The EPRMJN aims to discover this research and make it accessible to international audiences through systematic collection of articles appearing in the national journals of the EPRMJN and publication of content summaries on the ESPRM website.

The incidence of dental caries /Is/ on deciduous molars in 1216 children aged 3 to 8 from Plovdiv has been studied. The results showed highest Is in the fissures of m1 and m2 and lowest Is on the smooth surfaces. The incidence of dental caries is getting higher with statistically significant differences in the lower jaw up to the 6th year and in the upper jaw up to 7th year of age. Is in the lower jaw is higher than that of the upper jaw only in the fissures.

El procés biològic bàsic subjacent de l'envelliment va ésser avançat per la teoria de l'envelliment basada en els radicals lliures l'any 1954: la reacció dels radicals lliures actius, produïts fisiològicament en l'organisme, amb els constituents cel·lulars inicia els canvis associats a l'envelliment. La implicació dels radicals lliures en l'envelliment està relacionada amb el seu paper clau en l'origen i l'evolució de la vida. La informació disponible avui en dia ens mostra que la composició específica de les macromolècules cel·lulars (proteïnes, àcids nucleics, lípids i carbohidrats) en les espècies animals longeves tenen intrínsicament una resistència elevada a la modificació oxidativa, la qual cosa probablement contribueix a la longevitat superior d'aquestes espècies. Les espècies longeves també mostren unes taxes reduïdes de producció de radicals lliures i de lesió oxidativa. D'altra banda, la restricció dietària disminueix la producció de radicals lliures i la lesió molecular oxidativa. Aquests canvis estan directament associats a la reducció de la ingesta de proteïnes dels animals sotmesos a restricció, que alhora sembla que són deguts específicament a la reducció de la ingesta de metionina. En aquesta revisió s'emfatitza que una taxa baixa de generació de lesió endògena i una resistència intrínsecament elevada a la modificació de les macromolècules cel·lulars són trets clau de la longevitat de les espècies animals., The basic chemical process underlying aging was first put forward by the free radical theory of aging in 1956; the reaction of active free radicals (normally produced within an organism itself) with cellular constituents initiates the changes associated with aging. The involvement of free radicals in aging is related to their key role in the origin and evolution of life. The specific composition of tissue macromolecules (proteins, nucleic acids, lipids and carbohydrates) in long-lived animal species gives them an intrinsically high resistance to modification that probably contributes to the superior longevity of these species. Long-lived species also show low rates of reactive oxygen species (ROS) generation and oxidative damage to their mitochondria. Dietary restriction further decreases mitochondrial ROS production and oxidative molecular damage due to the decreased intake of dietary proteins. These effects of protein restriction seem to be specifically due to the lowered methionine intake of protein and dietary restricted animals. Both a low rate of generation of endogenous damage and an intrinsically high resistance to the modification of tissue macromolecules are key traits of animal longevity.

Aim. In the last 40 years, physical and rehabilitation medicine (PRM) has made significant steps forward in Europe with the foundation of the European Federation of Physical Medicine and Rehabilitation (EFPMR) (1963) which gave rise to the European Society of Physical and Rehabilitation Medicine (ESPRM) (2004) the European Academy of Rehabilitation Medicine (1970), the PRM Section of the European Union of Medical Specialists (1974), and the European Board of PRM (1991). Our journal, formerly Europa Medico-physica (1964), the official journal of the EFPMR, now European Journal of Physical and Rehabilitation Medicine (EJPRM) and official journal of the ESPRM since 2008, is distinct for its steadfast European vocation, long-standing Mediterranean interests and connections with various national scientific societies. Jointly with the ESPRM, efforts are under way to set up the European Physical and Rehabilitation Medicine Journal Network (EPRMJN). The aim of this article is to present a profile of the national journals in the EPRMJN so as to give a better overview of how the scientific part of PRM in Europe has developed within a national perspective. Methods. A profile of the following national journals is presented: Annals of Physical and Rehabilitation Medicine (France), Fizikalna i rehabilitacijska medicina (Physical and Rehabilitation Medicine) (Croatia), Neurorehabilitation (Bulgaria), Physical and Rehabilitation Medicine Portuguese Society Journal (Portugal), Physical Medicine, Rebabilitaton, Health (Bulgaria), Physikalische Medizin - Rehabilitationsmedizin - Kurortmedizin/Journal of Physical and Rehabilitation Medicine (Germany and Austria) Prevention and Rehabilitation (Bulgaria), Rehabilitacija (Rehabilitation) (Slovenia), Rehabilitacion (Madr) (Spain), Turkish Journal of Physical Medicine and Rehabilitation (Turkey). Conclusion. Some national journals in Europe have a very long history and tradition of research and education. Having a better knowledge of these realities, usually hidden to the international readership owing to the English language barrier, could promote science in our specialty.

The behaviour of Bulgarian children dental patients at the age of 4-12 has been studied by application of Venham's 6--grade behaviour scale. A sample of 274 clinically healthy children, divided in 3 age groups, were the object of the study. It has been found that the number of children whose behaviour has been assessed as one within the norm, is much greater that the number of the rest of patients-amounting to 77.74% and their number increasing with age. The negative behaviour, regardless of its rate, is not influenced by age.

Human lung cancers including squamous cell carcinoma (SCC) are a leading cause of death, and while evidence suggests that basal stem cells drive SCC initiation and progression, themechanisms regulating these processes remain unknown. In this study we show that β-catenin signalling regulates basal stem cell fate and subsequent SCC progression. In a cohort of preinvasive SCCs we established that elevated basal stem cell β-catenin signalling is positively associated with increased disease severity, epithelial proliferation, and reduced intercellular adhesiveness. We demonstrate that transgene-mediated β-catenin inhibition within keratin 14-expressing basal stem cells delayed normal airway repair while basal cell-specific β-catenin activation increased cell proliferation, directed differentiation, and promoted an epithelial-to-mesenchymal transition (EMT) that included increased Snail transcription and reduced E-cadherin-mediated adhesiveness. These effects were recapitulated in normal human bronchial epithelial cells in vitro following both pharmacological β-catenin activation and E-cadherin inhibition, and mirrored our findings in preinvasive SCCs. Overall this data shows that airway stem cell β-cateninmodulates cell adhesiveness to determine cell fate and its mis-expression is a key step in the development of human lung cancer. during its fourth year of existence, cochrane rehabilitation went on to promote evidence-informed health decision-making in rehabilitation. in 2020, the outbreak of the coVid-19 pandemic has made it necessary to alter priorities. in these challenging times, cochrane rehabilitation has firstly changed its internal organisation and established a new relevant project in line with pandemic needs: the REH-COVER (Rehabilitation – coVid-19 evidence-based response) action. the aim was to focus on the timely collection, review and dissemination of summarised and synthesised evidence relating to COVID-19 and rehabilitation. Cochrane Rehabilitation REH-COVER action has included in 2020 five main initiatives: 1) rapid living systematic reviews on rehabilitation and coVid-19; 2) interactive living evidence map on rehabilitation and coVid-19; 3) definition of the research topics on “rehabilitation and COVID-19” in collaboration with the World Health Organization (WHO) rehabilitation programme; 4) Cochrane Library special collection on Coronavirus (COVID-19) rehabilitation; and 5) collaboration with COVID-END for the topics “rehabilitation” and “disability.” Furthermore, we are still carrying on five different special projects: Be4rehab; RCTRACK; definition of rehabilitation for research purposes; ebook project; and a prioritization exercise for Cochrane Reviews production. The Review Working Area continued to identify and “tag” the rehabilitation-relevant reviews published in the cochrane library; the publication Working area went on to publish Cochrane Corners, working more closely with the Cochrane Review Groups (CRGs) and Cochrane Networks, particularly with Cochrane Musculoskeletal, oral, skin and sensory Network; the Education Working area, the most damaged in 2020, tried to continue performing educational activities such as workshops in different online meetings; the Methodology Working area organized the third and fourth cochrane Rehabilitation Methodological (CRM) meetings respectively in Milan and Orlando; the Communication Working Area spread rehabilitation evidences through different channels and translated the contents in different languages.

During its third year of existence, Cochrane Rehabilitation goals included to point out the main methodological issues in rehabilitation research, and to increase the Knowledge Translation activities. This has been performed through its committees and specific projects. In 2019, Cochrane Rehabilitation worked on five different special projects at different stages of development: 1) a collaboration with the World Health Organization to extract the best evidence for Rehabilitation (Be4rehab); 2) the development of a reporting checklist for Randomised Controlled Trials in rehabilitation (RCTRACK); 3) the definition of what is the rehabilitation for research purposes; 4) the ebook project; and 5) a prioritization exercise for Cochrane Reviews production. The Review Committee finalized the screening and "tagging" of all rehabilitation reviews in the Cochrane library; the Publication Committee increased the number of international journals with which publish Cochrane Corners; the Education Committee continued performing educational activities such as workshops in different meetings; the Methodology Committee performed the second Cochrane Rehabilitation Methodological Meeting and published many papers; the Communication Committee spread the rehabilitation evidence through different channels and translated the contents in different languages. The collaboration with several National and International Rehabilitation Scientific Societies, Universities, Hospitals, Research Centers and other organizations keeps on growing.

During its second year of existence, Cochrane Rehabilitation worked hard to accomplish new and old goals. The Review Committee completed the massive task of identifying and "tagging" all rehabilitation reviews in the Cochrane library. The Publication Committee signed agreements with several international journals and started the publication of Cochrane Corners. The Education Committee performed educational activities such as workshops in International Meetings. The Methodology Committee has completed a two days Cochrane Rehabilitation Methodological Meeting in Paris of which the results will soon be published. The Communication Committee reaches almost 5,000 rehabilitation professionals through social media, and is working on the translation of contents in Italian, Spanish, French, Dutch, Croatian and Japanese. Memoranda of Understanding have been signed with several National and International Rehabilitation Scientific Societies, Universities, Hospitals, Research Centres and other organizations. The be4rehab (best evidence for rehabilitation) project has been started with the World Health Organisation (WHO) to extract from Cochrane reviews and clinical guidelines the best currently available evidence to produce the WHO Minimum Package of Rehabilitation Interventions. The Cochrane Rehabilitation ebook is under development as well as a priority setting exercise with 39 countries from all continents.

44. Complicated Pneumonia in a Child: Hydropneumothorax Associated with MIS-C and GAS Superinfection.

- Abstract
A hydropneumothorax is an uncommon complication of pneumonia, particularly in pediatric patients, and typically arises secondary to conditions such as malignancies, esophageal-pleural fistula, thoracic trauma, or thoracocentesis. While pneumothorax is rarely reported in adults with COVID-19 and is even less common in children, isolated cases have been noted in those with Multisystem Inflammatory Syndrome in Children (MIS-C). A recent alert has also been issued about increased Group A Streptococcus (GAS) infections in Europe. Against this background, the primary aim of this case report is to describe a rare and severe complication of pneumonia in a previously healthy child with MIS-C and a positive throat culture for GAS.

- Abstract
Background: Many stroke survivors are both sedentary and physically inactive, even those who have adequate mobility. This increases cardiometabolic risk and has impacts on physical and other functions., Objective: The aim of this Cochrane Review summary is to discuss from a rehabilitation perspective the results of the Cochrane review investigating the effects of the interventions designed to reduce sedentary behavior after stroke on mortality, secondary vascular events, cardiovascular risk, adverse events and sedentary behavior., Methods: The review authors searched for randomized controlled trials that had been published up to December 2019, comparing the effects of the interventions aimed to reduce sedentary behavior in patients after stroke with usual care, no intervention, sham intervention., Results: The results of the review showed that the interventions included did not affect the number of deaths or the incidence of recurrent cardiovascular or cerebrovascular events, falls or other adverse events in stroke patients. Evidence for their impact on sedentary behavior is currently inconclusive., Conclusions: The evidence about reducing sedentary behavior in patients after stroke is incomplete, since research in this field is relatively new. Practitioners may also encourage reduction in sitting during daytime by considering interventions for other therapeutic targets (e.g. increasing physical activity and mobility), besides the studied interventions that proved to be safe and harmless.

Abstract: Although the physiatric community increasingly embraces evidence-based medicine (EBM), the current state of EBM training for trainees in physiatry is unclear. The purposes of this article are to report the results of the Association of Academic Physiatrists' surveys of physiatry residency programs in the United States, to discuss the implications of their findings, and to better delineate the "baseline" upon which sound and clear recommendations for systematic EBM training can be made. The two Association of Academic Physiatrists surveys of US physiatry residency programs reveal that most survey respondents report that they include EBM training in their programs that covers the five recommended steps of EBM core competencies. However, although most respondents reported using traditional pedagogic methods of training such as journal club, very few reported that their EBM training used a structured and systematic approach. Future work is needed to support and facilitate physiatry residency programs interested in adopting structured EBM training curricula that include recommended EBM core competencies and the evaluation of their impact., (Copyright © 2021 Wolters Kluwer Health, Inc. Study Objectives: Insomnia and snoring are common sleep disorders. The aim was to investigate the association of having a combination of insomnia symptoms and snoring with comorbidity and daytime sleepiness., Methods: The study population comprised 25,901 participants (16-75 years, 54.4% women) from 4 Swedish cities, who answered a postal questionnaire that contained questions on snoring, insomnia symptoms (difficulties initiating and/or maintaining sleep and/or early morning awakening), smoking, educational level, and respiratory and nonrespiratory disorders., Results: Snoring was reported by 4,221 (16.2%), while 9,872 (38.1%) reported ≥ 1 insomnia symptom. A total of 2,150 (8.3%) participants reported both insomnia symptoms and snoring. The association with hypertension (adjusted odds ratio [OR], 1.4; 95% confidence interval [CI], 1.2-1.6), chronic obstructive pulmonary disease (adjusted OR, 1.8; 95% CI, 1.3-2.4), asthma (adjusted OR, 1.9; 95% CI, 1.6-2.3), daytime sleepiness (adjusted OR, 7.9; 95% CI, 7.1-8.8), and the use of hypnotics (adjusted OR, 7.5; 95% CI, 6.1-9.1) was highest for the group with both insomnia symptoms and snoring., Conclusions: Participants with both snoring and insomnia run an increased risk of hypertension, chronic obstructive pulmonary disease, asthma, daytime sleepiness, and use of hypnotics. - Abstract
Cerebral palsy (CP) is a group of the most common developmental disorders affecting movement and posture of the body, causing activity limitations and participation restrictions. The motor disorders of persons with CP are often accompanied by disturbances of sensation, cognition, communication and perception. The symptoms of CP are very diverse and persons with CP are usually presented with a mixed type of symptoms. The non-progressive disturbances can be attributed to disorders that were developed during pregnancy, birth and/or infant stage. The aim of this study was to improve physicians' professional practice of Physical and Rehabilitation Medicine for persons with cerebral palsy in order to improve their functionality, social and community integration, and to reduce activity limitations and/or participation restrictions. A systematic review of the literature including an 18-year period and consensus procedure by means of a Delphi process was performed and involved the delegates of all European countries represented in the Union of European Medical Specialists Physical and Rehabilitation Medicine (UEMS PRM) Section. As the result of a Consensus Delphi procedure, 74 recommendations are presented together with the systematic literature review. The PRM physician's role for persons with cerebral palsy is to lead and coordinate the multiprofessional team, working in an interdisciplinary way. They should propose and manage the complex but individual PRM program developed in conjunction with other health professionals, medical specialists and importantly in agreement with the patient, their family and care giver. This should be, according to the specific medical diagnosis to improve patients' health, functioning, social and education status, considering all impairments, comorbidities and complications, activity limitations and participation restrictions. This evidence-based position paper is representing the official position of The European Union through the UEMS PRM Section and designates the professional role of PRM physicians in persons with cerebral palsy.
